Squamous cell carcinomas of the head and neck are appearing with increased frequency in both marrow transplanted and non-transplanted Fanconi anemia (FA) patients. FA patients commonly display radiosensitivity of epithelial tissues, complicating effective radiotherapy. Fancd2(−/−) mice (C57BL/6J and 129/Sv background) demonstrate epithelial tissue sensitivity to single-fraction or fractionated irradiation to the head and neck and distant marrow suppression (abscopal effect), both ameliorated by intraoral administration of the mitochondrial-targeted antioxidant, GS-nitroxide, JP4-039. We now report that mice of two other FA genotypes, Fancg(−/−) (B6) and the most prevalent human genotype Fanca(−/−) (129/Sv), also demonstrate: 1. reduced longevity of hematopoiesis in long-term bone marrow cultures; 2. radiosensitivity of bone marrow stromal cell lines; and 3. head and neck radiation-induced severe mucositis and abscopal suppression of distant marrow hematopoiesis. Intraoral administration of JP4-039/F15, but not non-mitochondrial-targeted 4-amino-Tempo/F15 or F15 alone, prior to each radiation treatment ameliorated both local and abscopal radiation effects. Head and neck irradiated TGF-β-resistant SMAD3(−/−) (129/Sv) mice and double-knockout SMAD3(−/−) Fancd2(−/−) (129/Sv) mice treated daily with TGF-β receptor antagonist, LY364947, still displayed abscopal bone marrow suppression, implicating a non-TGF-β mechanism. Thus, amelioration of both local normal tissue radiosensitivity and distant marrow suppression by intraoral administration of JP4-039 in Fancg(−/−) and Fanca(−/−) mice supports a clinical trial of this locally administered normal tissue radioprotector and mitigator during head and neck irradiation in FA patients.

The drilling practices, well designs, and experiences from drilling 179 unconventional horizontal wells drilled from 2012 through late 2017, in the New Mexico Delaware Basin, are presented.Geological conditions addressed include high-pressure flows, lost circulation, salt and anhydrite, and drilling effects. Successful practices covered include casing design and setting depths, bits, rotary steerable systems (RSS), rig specifications, pad layout, and others. Mud systems discussed include brine, mixed-metal hydroxide, invert and direct emulsions. The benefit of rotary steerable drilling assemblies is discussed. Techniques to predict, prevent, and mitigate the effect of downhole vibrations are addressed, including bit and bottom-hole-assembly design, and operating practices.Overall drilling speed was increased by 149% (measured as feet-per-day) during the period from 2012 to 2017 through the practices that are covered. During the same period, average lateral lengths increased from approximately 3,000 ft in 2016 to approximately 7,000 ft in 2017, with 10,000 ft laterals common. Rig capabilities, pad layout, casing designs, drilling fluids, drilling dynamics practices, and other designs improved significantly in the past 3 years. A methodical approach to vibrations has proven to be critical to consistent performance. Direct emulsion mud has shown promising results as a low-density fluid with salt-saturated water phase. Casing designs were optimized to eliminate strings in some cases, and to reduce hole sizes in other cases. Rig modifications were required to support long laterals and larger drill pipe sizes.The economic success of New Mexico unconventional developments is heavily dependent on minimizing well cost. It is a difficult area requiring complex optimization.

206. GHG Methodologies for Sewer CH4, Methanol-Use CO2, and Biogas-Combustion CH4 and their Significance for Centralized Wastewater Treatment

Greenhouse gas (GHG) arising from the treatment of domestic sewage in the developed world is known to contribute to GHG emissions in the atmosphere – principally carbon dioxide (CO2), methane (CH4) and nitrous oxide (N2O). Accounting protocols exist that allow development of emissions inventories on a geographic, utility, or facility-specific basis using uniform methodologies and assumptions. This thesis develops new, improved methodologies for estimating direct GHG emissions sources from: 1) Sewer CH4, 2) Anthropogenic CO2 from methanol use for nutrient removal, and 3) CH4 from incomplete digester gas combustion. The significance of these GHG sources is estimated herein on a national scale for centralized wastewater treatment in the United States (US). Current GHG-emissions-inventorying protocols consistently ignore sewer CH4.However, by the methods in this thesis, sewer CH4 is estimated to represent between 26% and 35% of DC Water’s and more than 54% of centralized-wastewater-related direct GHG emissions in the US. A new sewer CH4 methodology developed in this thesis uses sewer hydraulic models at average flow rates to classify pipe segments as either gravity or surcharged sewers. CH4 generation in surcharged segments is derived using a prior forcemain algorithm while gravity sewers are modelled using a new algorithm. After method verification using exhaust-CH4 mass fluxes from an odour-control fan evacuating air from a regional sewer, the methodology is applied to the entire DC Water (Washington, DC - US) collection system to estimate utility-wide sewer CH4 emissions, which are found to be substantial.Similarly, until recently, GHG-accounting protocols also ignored CO2 emissions attributable to methanol addition for nutrient removal. As most methanol is derived from natural gas, its carbon represents an anthropogenic CO2 emission when released. In this thesis, a site-specific method is developed for DC Water and used to determine that methanol CO2 represents 40% and 60% of DC Water’s direct GHG emissions, before and after anaerobic-digestion upgrades, respectively. When applied to the US treated wastewater flows, considering likely effluent-nitrogen requirements and presence of and/or lack of digestion that would influence methanol use, methanol CO2 is estimated to account for nearly 12% of the country’s centralized-wastewater-treatment GHG emissions.A new method is also developed to account for fugitive CH4 from digester-gas combustion based on a specific end uses of the biogas. This method contrasts with current protocols which uniformly suggest that 1 to 2% of all produced digester gas is vented to the atmosphere (whether through incomplete combustion or simply leakage is unclear). Use of the new methodology could, depending on the biogas-use technologies in operation, more accurately replace a 1% protocol-based estimation of 4,200 MT CO2e/year for DC Water with emission rates ranging from 60 to over 21,000 MT CO2e/year. Accordingly, current protocol-based simplifications are shown to result in an actual 70-time overestimation for DC Water or a possible 5-times underestimation for the poorest-efficiency biogas-combustion devices: conventional, candle-stick flares.As the final subject of this thesis, electricity use, which comprises the largest portion of centralized-treatment GHG emissions inventories, is reviewed. GHG emissions from electricity consumption occur at the power plant, and their make-up and carbon intensity is a function of how that plant produces electricity. While local electricity carbon intensity is normally beyond the control of the wastewater utility, how much is purchased is largely under its control. Aeration-blower and total-plant power consumption over one or two years for 13 large to very-large facilities in New York City (NY - US) are correlated to average flow and total-oxygen-demand loading and removal to evaluate electricity-use metrics. Additionally, the local carbon intensity of power is shown to dramatically impact the magnitude of GHG emissions and to have surprising effects on an example “sustainability-enhancing” upgrade.This research discusses these improved GHG-accounting methodologies and evaluates their significance using large-scale utility process and GHG data. When combined, application of new sewer CH4 and methanol CO2 methods herein would roughly triple direct GHG emissions attributed to an industry that already represents a significant GHG source and energy consumer. An improved understanding of emissions intensity and the significance of various emissions sources would allow an already proactive wastewater industry to better target or incorporate intervention strategies that mitigate GHG emissions. This thesis offers these improved methodologies while providing evidence that: 1) GHG accounting science is far from fully understood; 2) additional research is warranted; so that 3) the industry’s understanding and GHG emissions baselines are accurate and thereby support almost-certain eventual regulation.

Background: \ud Glucagon-like peptide 1 receptor agonists differ in chemical structure, duration of action, and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. We aimed to determine the safety and efficacy of albiglutide in preventing cardiovascular death, myocardial infarction, or stroke.\ud \ud Methods: \ud We did a double-blind, randomised, placebo-controlled trial in 610 sites across 28 countries. We randomly assigned patients aged 40 years and older with type 2 diabetes and cardiovascular disease (at a 1:1 ratio) to groups that either received a subcutaneous injection of albiglutide (30–50 mg, based on glycaemic response and tolerability) or of a matched volume of placebo once a week, in addition to their standard care. Investigators used an interactive voice or web response system to obtain treatment assignment, and patients and all study investigators were masked to their treatment allocation. We hypothesised that albiglutide would be non-inferior to placebo for the primary outcome of the first occurrence of cardiovascular death, myocardial infarction, or stroke, which was assessed in the intention-to-treat population. If non-inferiority was confirmed by an upper limit of the 95% CI for a hazard ratio of less than 1·30, closed testing for superiority was prespecified. This study is registered with ClinicalTrials.gov, number NCT02465515.\ud \ud Findings: \ud Patients were screened between July 1, 2015, and Nov 24, 2016. 10 793 patients were screened and 9463 participants were enrolled and randomly assigned to groups: 4731 patients were assigned to receive albiglutide and 4732 patients to receive placebo. On Nov 8, 2017, it was determined that 611 primary endpoints and a median follow-up of at least 1·5 years had accrued, and participants returned for a final visit and discontinuation from study treatment; the last patient visit was on March 12, 2018. These 9463 patients, the intention-to-treat population, were evaluated for a median duration of 1·6 years and were assessed for the primary outcome. The primary composite outcome occurred in 338 (7%) of 4731 patients at an incidence rate of 4·6 events per 100 person-years in the albiglutide group and in 428 (9%) of 4732 patients at an incidence rate of 5·9 events per 100 person-years in the placebo group (hazard ratio 0·78, 95% CI 0·68–0·90), which indicated that albiglutide was superior to placebo (p

To assess the prevalence and severity of dysphonia in patients with cystic fibrosis sinusitis. We hypothesized that patients with CF sinusitis, compared with 2 control groups, would have higher self-reported prevalence of dysphonia and greater severity of dysphonia, according to patient-reported outcome measures as well as auditory-perceptual evaluation by expert listeners.Cross-sectional comparative pilot study.Academic tertiary care clinic.Analysis included 37 study participants: 17 patients with CF sinusitis, 10 healthy individuals, and 10 patients with non-CF sinusitis. All participants completed the 10-item Voice Handicap Index (VHI-10) questionnaire and provided voice samples. On all samples, 6 blinded speech-language pathologists independently performed auditory-perceptual evaluation, using Consensus Auditory-Perceptual Evaluation of Voice. To assess severity of sinonasal symptoms, we used the 20-item Sinonasal Outcome Test (SNOT-20). Standard parametric and nonparametric statistical analysis was performed.The differences between the 3 groups in prevalence of abnormal VHI-10 scores were not statistically significant. SNOT-20 scores were similar in the 2 sinusitis patient groups. VHI-10 scores were highest in patients with CF sinusitis, intermediate in patients with non-CF sinusitis, and lowest in healthy individuals (P = .005). Auditory-perceptual evaluation demonstrated greater overall severity of dysphonia in patients with CF sinusitis compared with the 2 control groups (P = .0005).Cystic fibrosis sinusitis appeared to be associated with worse vocal function as measured by patient self-report as well as auditory-perceptual evaluation of voice compared with patients with non-CF sinusitis and healthy controls. Further investigation in this area is warranted.

In the absence of canopy-opening disturbances, upland oak forests in the eastern United States are shifting to shade-tolerant, fire-sensitive tree species (i.e. mesophytes) via a hypothesized positive feedback loop of less flammable, self-promoting conditions, termed mesophication. To evaluate species-specific impacts on mesophication, I quantified canopy, bark, and leaf litter traits of five hypothesized mesophytes [red maple ( Acer rubrum ), sugar maple ( A. saccharum ), American beech ( Fagus grandifolia ), hickory ( Carya spp.), and tulip poplar ( Liriodendron tulipifera )] and four upland oaks [black oak ( Quercus velutina ), chestnut oak ( Q. montana ), scarlet oak ( Q. coccinea ), and white oak ( Q. alba )] in central Kentucky. Red maple, sugar maple, and American beech had increased canopy depth with stem size, smoother bark, and small, thin leaves when compared to oaks. My findings suggest that some mesophytes, such as red maple, sugar maple, and American beech, may decrease future forest flammability by reducing understory light and increasing fuel moisture.

Nanofluids often exhibit superior heat transfer characteristics when compared with conventional heat transfer fluids. The increase in thermal conductivity due to the presence of various nanoparticles was experimentally examined using commercially available equipment that utilizes the two thickness method. The thermal conductivity of 10 and 30 nm aluminum oxide nanoparticles suspended in distilled water at concentrations of 2% and 5% was measured for a temperature range of 15°C to 70°C in increments of 5°C. For a 2% concentration of 10 nm aluminum oxide the experimentally derived thermal conductivity deviated from the theoretical thermal conductivity predicted by Maxwell by an average of 1.55%. The average percent increase in the thermal conductivity of the base fluid due to the presence of 10 nm aluminum oxide nanoparticles was found to be 4.17 and 4.90% for concentrations of 2 and 5% respectively. The presence of 30 nm nanoparticles resulted in a greater discrepancy with the theoretical model developed by Maxwell, regardless of concentration. In addition, the presence of 10 nm aluminum oxide nanoparticles resulted in a greater increase in thermal conductivity when compared with 30 nm aluminum oxide nanoparticles. In addition, the thermal conductivity of a base fluid dispersed with multi-walled carbon nanotubes (MWNTs) with an outer diameter ranging from 13–18 nm and a length ranging from 3–30 micrometers (μm) was examined. The presence of a 0.2% concentration of MWNTs resulted in an average increase in thermal conductivity of 0.31%. Unfortunately, there was a large standard deviation in the results for the MWNTs and significant fluctuations with temperature. While this experimental methodology may be sufficient for metal based nanofluid particles it may be undesirable for fluids enhanced by MWNTs.

Serpins are a superfamily of serine protease inhibitors that regulate proteolytic pathways by utilizing a conformational change to bind to and inhibit their target peptidases. Currently 36 human serpins have been identified, but the biological roles of most are not fully understood (Silverman et al., J Biol Chem. 285(32):24299-305, 2010). Serpinb3a (the mouse orthologue of human SERPINB3 and -B4) is an intracellular serpin and inhibits both serine and cysteine peptidases invitro. We now report radioresistance of Serpinb3a-/- mice and their hematopoietic progenitor cells. Serpinb3a-/- and control Balb/c mice were irradiated to a dose of 8.0 Gy total body irradiation and followed for the development of the hematopoietic syndrome. Long Term Bone Marrow Cultures (LTBMCs) were established from the bone marrow flushed from the femurs and tibias of Serpinb3a-/- and Balb/c mice. Irradiation survival curves were performed on bone marrow stromal cell lines established from the LTBMCs and on fresh bone marrow isolated from the femurs of sacrificed Serpinb3a-/- and Balb/c mice. Colony forming unit granulocyte-macrophage (CFU-GM) colonies of 50 cells or greater were counted. The data from 3 experiments were analyzed using linear quadratic and single-hit, multi-target models. Serpinb3a-/- mice showed increased survival following 8.0 Gy TBI compared to Balb/c mice (p < 0.0001). CFU-GM from Serpinb3a-/- were radioresistant compared to Balb/c marrow (ň = 4.9 ± 1.4 and 1.4 ± 0.1, p = 0.0451, respectively). Serpinb3a-/- bone marrow stromal cells were radioresistant with an increased Do (1.97 + 0.11 Gy) compared to bone marrow stromal cells from Balb/c mice (Do = 1.45 + 0.03 Gy, p = 0.0113). Loss of Serpinb3a provides a radiation survival advantage, increases radioresistance of both hematopoietic progenitor cells and marrow stromal cells, and may lead to identification of new targets for design of radiation mitigator drugs. This research finds the optimum period of a basic coprime sensor array (CSA) needed to reduce the PSL height to -13 dB for both minimum and product processing methods. Our design does not have the constraint that the subarray lengths be one integer apart unlike in [Adhikari et al., 2014]. We apply MUSIC algorithm to the optimized CSA and compare the PSD estimate obtained by Vaidyanathan and Pal’s covariance matrix estimation using a single sensor pair versus our covariance matrix estimated using all available sensor pairs. We show that minimum processing requires fewer periods than product processing to reach the PSL height of -13 dB. Larger coprime pairs require fewer periods and cause an increase in variance and a decrease in the number of lags available between the two subarrays. This decrease lowers the accuracy of the covariance matrix estimation. However, using all available sensor pairs to generate the covariance matrix greatly increases the accuracy compared to using a single sensor pair. When only one signal is present, both covariance estimates produce a similar, accurate DOA, but as the number of signals increases, our covariance estimate performs better.
