With the advent of advanced trauma critical care, and precise methods of assessing renal trauma with computed tomography, most patients with high-grade renal trauma can be managed conservatively. Some patients, however, do not do well with conservative management. This article evaluates specific radiographic characteristics that have recently been associated with intervention for renal hemorrhage after trauma.

Investigators from Parkland Hospital proposed substratification of the AAST (American Association for the Surgery of Trauma) grading scale based on 3 risk factors, including active vascular extravasation, a medial laceration and a perinephric hematoma of greater than 3.5 cm. We hypothesized that these characteristics would also be associated with intervention for renal hemorrhage in our large trauma series.From January 2005 to January 2011 we retrospectively reviewed the renal trauma records at adult level 1 trauma centers in Utah. AAST grade 3 and 4 injuries were characterized based on the mentioned 3 risk factors. Our primary outcome was intervention to control renal hemorrhage.AAST grade 3 or greater injury was identified in 147 patients, including 115 who had grade 3 and 4 injuries as well as imaging available for review. There were 63 grade 3 (53%) and 52 grade 4 (43%) renal injuries. Eight patients (7%) underwent intervention for renal hemorrhage. Vascular extravasation (OR 16.4, 95% CI 2.6-179.8, p0.001) and perinephric hematoma greater than 3.5 cm (OR 8.4, 95% CI 1.4-52.5, p = 0.0099) were associated with intervention, while a medial laceration was not (p = 0.454). Patients with 1 or fewer, 2 and 3 risk factors had an intervention rate of less than 2.9%, 18% and 50%, respectively (p0.001).Vascular extravasation, a perinephric hematoma greater than 3.5 cm and the number of risk factors (0 to 3) were associated with intervention for renal hemorrhage. Our findings are similar to those at Parkland Hospital. These imaging features may serve as useful prognostic indicators for renal trauma.

Background: This study aimed to analyze characteristics and outcomes of gunshot wounds to the lower urinary tract at our Level I trauma center. Our hypothesis is that gunshot wounds to the lower urinary tract have characteristic bullet trajectories, injury patterns, and associated injuries. Methods: Our prospective trauma database was composed of reviewed gunshot wounds to the lower urinary tract including the pelvic ureter, bladder, or urethra from 1989 through 2011. Results: We identified 50 patients (median age, 25 years; range, 3-53 years) with lower urinary tract injury. There was a mean of 2.3 bullets per patient (range, 1-8), with 26 patients injured from a single bullet. Urologic injury involving only the bladder occurred in 72% (36 of 50) of the patients. Ureteral injury was diagnosed in 20% (10 of 50) of the patients. Bullet trajectory was known in the majority of multiple bullet injuries and all cases involving a single bullet. All patients but one were managed operatively. During exploration, 90% (34 of 38) with transmural bladder injury had recognized bladder entry and exit wounds. Overall, 80% (40 of 50) had concurrent gastrointestinal injury. In patients with a single gunshot wound to the lower urinary tract, 58% (15 of 26) sustained concomitant intestinal injury, and 23% (6 of 26) sustained rectal injury. Of 20 posteroanterior gunshot wounds, 80% had buttock entry. All 10 single-bullet buttock-entry gunshot wounds injured the bladder. Isolated ureteral injury was associated with lower abdominal entry and anteroposterior trajectory. Urethral injury occurred in 4, with 75% upper-thigh entry. Conclusion: Penetrating injuries to the lower urinary tract most commonly involve the bladder. During exploration for gunshot wounds to the bladder, two injury sites should be expected because failure to close may lead to complications. Gunshot wounds to the lower urinary tract often occur with concomitant bowel injury, with buttock entry. A multidisciplinary approach involving general surgery is imperative. Purpose: Male urethral stricture disease accounts for a significant number of hospital admissions and health care expenditures. Although much research has been completed on treatment for urethral strictures, fewer studies have addressed the treatment of strictures in men with recurrent stricture disease after failed prior urethroplasty. We examined outcome results for repeat urethroplasty. Materials and Methods: A prospectively collected, single surgeon urethroplasty database was queried from 1977 to 2011 for patients treated with repeat urethroplasty after failed prior urethral reconstruction. Stricture length and location, and repeat urethroplasty intervention and failure were evaluated with descriptive statistics, and univariate and multivariate logistic regression. Results: Of 1,156 cases 168 patients underwent repeat urethroplasty after at least 1 failed prior urethroplasty. Of these patients 130 had a followup of 6 months or more and were included in analysis. Median patient age was 44 years (range 11 to 75). Median followup was 55 months (range 6 months to 20.75 years). Overall, 102 of 130 patients (78%) were successfully treated. For patients with failure median time to failure was 17 months (range 7 months to 16.8 years). Two or more failed prior urethroplasties and comorbidities associated with urethral stricture disease were associated with an increased risk of repeat urethroplasty failure. Conclusions: Repeat urethroplasty is a successful treatment option. Patients in whom treatment failed had longer strictures and more complex repairs. © 2012 American Urological Association Education and Research, Inc.

To characterize anterior urinary fistulae following radiotherapy for prostate cancer. Over 10 years, 31 men were identified to have an anterior urinary fistula. A retrospective database was created to evaluate patient demographics, presentation, diagnostic procedures, operative interventions, outcomes, and complications. Comparisons between men who underwent cystectomy versus bladder-sparing surgeries were performed. At a median age of 73 (interquartile range (IQR) 68.5, 79) years, presenting symptoms included as follows: pubic pain (19/31, 61%), urine drainage via fistula (10/31, 32%), or a superficial wound infection (6/31, 19%). Recent instrumentation prior to diagnosis of anterior urinary fistula was reported by 18/31 (58%) at a median of 14.9 months (IQR 7.9, 103.8) after manipulation. Anterior fistula formation was either isolated to the pubic symphysis (19/31, 61%) or the thigh (12/31, 38%). Nineteen men underwent a cystectomy, whereas 12 men underwent a fistula repair. Excluding grades 1 and 2, 30- and 90-day postoperative complications were limited to four and two men, respectively, all of whom had a grade 3 complication. At 6-month follow-up, 26/31 (84%) men reported their pain had resolved. There was one fistula recurrence managed with subsequent cystectomy. Complex anterior urinary fistulae to the pubic symphysis and thigh are devastating yet treatable conditions. Universally, these men have a history of radiotherapy and repeated endoscopic interventions. Surgical intervention with either cystectomy or primary repair was highly successful.

Objective To describe the management of Radiation Therapy Oncology Group (RTOG) grade 4 urinary adverse events (UAEs) after radiotherapy (RT) for prostate cancer (PCa). Methods We conducted a single-centre retrospective review, over a 6-year period (2010–2015), to identify men with RTOG grade 4 UAEs after RT for PCa. RT was classified as combined therapy (radical prostatectomy [RP] followed by external beam radiotherapy [EBRT], EBRT + low-dose-rate [LDR] brachytherapy, EBRT + high-dose-rate [HDR] brachytherapy or other combinations of RT) or monotherapy RT. UAEs were classified as outlet (urethral stricture, bladder neck contracture, prostate necrosis, or recto-urethral fistula) or bladder (contraction, necrosis, fistula, ureteric stricture or haemorrhage) UAEs. Results We identified 73 men with a mean age of 73 years. Of these, 44 (60%) received combined therapy, consisting of RP + EBRT (n = 19), HDR brachytherapy + EBRT (n = 19), LDR brachytherapy + EBRT (n = 5), and other combined RT (n = 1). Twenty-nine (40%) patients had monotherapy consisting of EBRT (n = 4), HDR brachytherapy (n = 11), LDR brachytherapy (n = 12), or proton beam therapy (n = 2). UAEs were isolated to the bladder in six men (8%), the outlet in 52 men (71%), and to both in 15 men (21%). UAE management included: conservative in 21 (29%), indwelling catheters in 12 (16%), reconstructive in 19 (26%), and urinary diversion (UD) in 23 men (32%). Reconstruction included: ureteric (n = 4), recto-urethral fistula repair (n = 2), and posterior urethroplasty (n =13), of which 14/16 surgeries (88%) with follow-up >90 days were successful. Conclusions Although the incidence of RTOG grade 4 UAEs after PCa radiation treatment is not well defined, their associated morbidity is significant, and approximately one third of patients with these high-grade complications require UD. Conversely, only about a quarter of patients can be managed with conservative strategies or local surgeries. Reconstruction is successful in selected patients.

OBJECTIVE To determine the safety profile of clomiphene citrate (CC) in men being treated for hypogonadism or infertility by measuring prostate-specific antigen (PSA), haematocrit (Hct), and testosterone levels. PATIENTS AND METHODS We identified patients presenting to our institution who were placed on CC, 50 mg every day or every other day, for male infertility and/or symptomatic hypoandrogenism between September 2013 and April 2016. Patients with documented exogenous testosterone, human chorionic gonadotrophin, or anastrozole use within 2 weeks of baseline evaluations were excluded. Our primary outcomes were the effects of CC on PSA, Hct, and total testosterone levels evaluated at the 3, 6, 9, or 12 months of follow-up. Outcomes were averaged within patients across visits and summarised by mean, median, range, standard deviation (SD) and the 95% confidence interval (CI) for the mean. RESULTS A total of 77 patients had recorded PSA, Hct, and/or testosterone values. The mean (SD, range) age and body mass index was 34 (6, 22-51) years and 31 (6, 22-52) kg/m2 , respectively. The mean (SD) follow-up was 358 (29) days. Within this group, CC concentration was changed in 24 patients (31%) and was discontinued in 24 patients (31%). The median (range) duration of CC therapy before discontinuation was 127 (44-161) days. The use of CC significantly raised both mean total and bioavailable testosterone levels by 200 ng/dL and 126 ng/dL, respectively (P < 0.001). This increase in testosterone had significant clinical effects with improvements in Androgen Deficiency in Aging Male questionnaire scores (P < 0.01) but not Sexual Health Inventory for Men scores. CC had no effect on mean PSA (1 ng/dL, 95% CI 0.8-1.1) or Hct (49%, 95% CI 41-53) levels, which were within normal ranges. CONCLUSIONS As more men are placed on CC for infertility or hypogonadism, characterising the safety effect profile becomes important. Our study found that CC significantly increased testosterone levels without changing PSA or Hct values. Because the biochemical response to CC can vary, we suggest scheduling laboratory evaluation at regular intervals; however, ordering routine assessment of PSA and Hct may not be necessary.

INTRODUCTION AND OBJECTIVES: Grade 4 RTOG complications after radiotherapy (XRT) for prostate cancer (CaP) are purportedly rare; however, the literature is limited by short follow up and lack of surgical management outcomes. We hypothesized that grade 4 complications require complex surgical management with high morbidity. METHODS: A retrospective/prospective review identified men with RTOG grade 4 urinary complications after XRT for CaP referred in a 5-year period to a tertiary care center. We excluded patients with progression to grade 4 complications after treatment for incontinence or erectile dysfunction. XRT was classified as dual therapy (radical prostatectomy (RRP) followed by external beam radiotherapy (EBRT), or various combinations of XRT) or monotherapy (single modality XRT). Complications were classified as outlet (urethral stricture, bladder neck contracture, prostate necrosis, and recto-urethral fistula) or bladder (contraction, necrosis, fistula, ureteral stricture, hemorrhage). RESULTS: We identified 67 men with a median age of 74 years. 42 (63%) had dual therapy, consisting of RRP + EBRT (18), high dose rate brachytherapy (HDR) + EBRT (18), low dose rate brachytherapy (LDR) + EBRT (4), and other dual XRT (2). 25 (37%) patients had monotherapy consisting of EBRT (4), HDR (9), LDR (10), or proton beam (2). Complications were isolated to the bladder in 4 (6%), the outlet in 46 (67%), and both in 17 (25%). The majority of outlet problems were identified as urethral obstruction (n1⁄448, 72%). Complications were managed conservatively in 17 (25%) cases (no intervention besides intermittent catheterization or local treatment), with indwelling catheters in 14 (21%), by urinary diversion (UD) in 21 (31%) (conduit or catheterizable pouch), and with reconstruction in 17 (25%). Reconstruction consisted of ureteral (4), recto-urethral fistula repair (2), and posterior urethroplasty (11). Success was achieved in 15/17 (88%) cases. Two failures were treated by UD and suprapubic tube. Hyperbaric oxygen (HBO2) was used in 25 (37%), gracilis flap in 14 (21%), and increased to 65% for both in the reconstructive group. CONCLUSIONS: RTOG grade 4 complications were most common in those patients that underwent dual therapy, but there was significant morbidity regardless of treatment modality. Reconstruction is successful in highly selected patients. Outcomes are improved with HBO2 and gracilis flaps. Grade 4 complications necessitated UD in one out of three patients. Further series with longer follow up are necessary to define and predict the outcomes in these patients.

Objective To validate the use of the International Prostate Symptom Score (IPSS) as a stand-alone tool to detect urethral stricture recurrence following urethroplasty. Materials and Methods This study included 393 men who had undergone anterior urethroplasty and were enrolled in a multi-institutional outcomes study. Data analyzed included pre- and post-operative answers to the IPSS in addition to findings from a same- day cystoscopy. IPSS from men found to have cystoscopic recurrence were then compared to scores from those with successful repairs, and receiver operating characteristic curves were plotted to illustrate the predictive ability of these questions to screen for cystoscopic recurrence. Results Mean postoperative scores were lower (fewer symptoms) in successful repairs; IPSS improved from preoperative values regardless of recurrence. Successful repairs had significantly better degree of improvement in question #5 (assessing weak stream) compared to recurrences. Receiver operating characteristic curves demonstrated the highest area under the curve for the IPSS quality of life question (0.66) that alone outperformed the complete IPSS questionnaire (0.56). Conclusion The IPSS had inadequate sensitivity and specificity to be used as a stand-alone screening tool for stricture recurrence in this large cohort of men, highlighting the need to continue development of a disease-specific, validated patient-reported outcome measure.

To determine urodynamic studies (UDS) findings in adult cerebral palsy (CP) patients. CP patients may suffer from voiding dysfunction. UDS in children with CP has consistently shown an upper motor neuron bladder with detrusor-sphincter dyssynergia.We included adult CP patients seen at Gillette Transitional Urology Clinic who underwent UDS for voiding dysfunction between 2011 and 2014. Descriptive statistics were used to characterize findings.There were 49/211 CP patients who underwent UDS. Average age was 30 years; 55% were men. Ninety-eight percent had moderate to severe CP. UDS was initiated for irritative symptoms in 55%, obstructive voiding symptoms in 25%, hydronephrosis in 18%, and other reasons in 2%. Incontinence was reported in 57%. Detrusor-sphincter dyssynergia was seen in 12%, detrusor overactivity in 30%, and detrusor leak point pressure (DLPP)40 cmH2O in 51%. Median compliance was 18 mL/cmH2O (0.78-365). Maximum cystometric capacity (MCC) was 80-1400 mL and was300 mL in 27%. Sixteen percent had an MCC300 mL and a compliance20 mL/cmH2O. Twelve percent had an MCC300 mL and a DLPP40 cmH2O.UDS findings in symptomatic adult CP patients are varied. Fifty-one percent had upper motor neuron bladder findings, similar to that seen in the pediatric literature, but 6% had large flaccid bladders. Half of the patients had concerning findings, such as compliance20 or DLPP40 cmH2O. Our results emphasize the need to thoroughly investigate voiding dysfunction in those with CP. Further characterization of this population is needed to correlate these UDS findings with clinical outcomes.

Neurogenic bladder is a very broad disease definition that encompasses varied disease and injury states affecting the bladder. The majority of patients with neurogenic bladder dysfunction do not have concomitant intrinsic sphincteric deficiency (ISD), but when this occurs the challenges of management of urinary incontinence from neurogenic bladder are compounded. There are no guidelines for surgical correction of ISD in adults and most of the literature on treatment of the problem comes from treatment of children with congenital diseases, such as myelomeningocele. Our goal, in this review, is to present some of the common surgical options for ISD [including artificial urinary sphincters, bladder slings, bladder neck reconstruction (BNR) and urethral bulking agents] and the evidence underlying these treatments in adults with neurogenic bladder.

The past decade has seen significant changes in the training for all surgical residents.1 Residents are working fewer hours and spending less time in the operating rooms. Of the hours spent operating, a greater percentage of them are being focused on learning robotic and laparoscopic skills than ever before, especially in urology, as these are the skills often coveted by the private practice jobs being pursued by nearly 90% of graduates.2,3 This means that by default, less time will be spent during training performing complex open procedures. It is perhaps not surprising, that the decade has also seen an increase in fellowship training of all surgical subspecialties. Proposed reasons for this trend are many,4 but it has been postulated that with less operating room exposure, graduating residents are more commonly feeling they need more training before starting their careers.5 Until recently, few residents pursued formal postgraduate training in adult reconstructive urology, a specialty that performs many complex open procedures, but there are now 15 single-year positions available for postresidency training,6 prompting the Genitourinary Reconstructive Society to begin a formal matching process for reconstructive fellows beginning in the year 2013. As this fellowship becomes more formalized and learning objectives are outlined, it is important to better understand the scope of practice required of potential fellows upon fellowship graduation. The purpose of this study was to examine the early practice patterns of recently trained reconstructive fellows. We hypothesized that the practice patterns will vary widely depending on the respective fellows’ home institution, but that reconstructive procedures will make up a large percentage of their practice. Additionally, we hypothesized that with increased years in practice, the number and complexity of reconstructive cases will increase.

Purpose: Adults with complications from previous hypospadias surgery experience various problems, including urethral stricture, persistent hypospadias and urethrocutaneous fistula. Innate deficiencies of the corpus spongiosum and multiple failed operations makes further management challenging. Materials and Methods: We reviewed our prospective urethroplasty database of men who presented with complications of previous hypospadias surgery. Patients were included in study if they had greater than 6 months of followup. Our surgical management was defined as an initial success if there were no urethral complications. The overall success rate included men with the same result after additional treatment. Results: A total of 50 men had followup greater than 6 months (median 89) and were included in study. These 50 patients presented with urethral stricture (36), urethrocutaneous fistula (12), persistent hypospadias (7), hair in the urethra (6) and severe penile chordee (7). Patients underwent a total of 74 urethroplasties, including stage 1 urethroplasty in 19, a penile skin flap in 11, stage 2 urethroplasty in 11, urethrocutaneous fistula closure in 9, permanent perineal urethrostomy in 6, excision and primary anastomosis in 6, a 1-stage buccal mucosa onlay in 4, tubularized plate urethroplasty in 3, combined techniques in 3 and chordee correction in 1. In 25 men (50%) treatment was initially successfully. Of the 25 men in whom surgery failed 18 underwent additional procedures, including 13 who were ultimately treated successfully for an overall 76% success rate (38 of 50). Conclusions: Managing problems from previous hypospadias surgery is difficult with a high initial failure rate. OBJECTIVES To describe the surgical technique and outcomes for perineal urethrostomy. We sought to identify factors that predicted surgery failure within our patient cohort and to describe key aspects of the operation aimed at preservation of the dorsal urethral plate and longitudinal blood supply within the urethra. METHODS We reviewed our prospectively collected database and identified 45 men who underwent definitive perineal urethrostomy from 1989 to 2009. Primary success was defined as no need for additional treatment. If 1 urethral dilation was performed, outcome was defined as a secondary success. Statistical analyses were performed to determine variables associated with failure. RESULTS Urethral pathology was varied and included idiopathic strictures (20%), lichen sclerosis (20%), infection (16%), radiation for prostate cancer (13%), prior hypospadias repair (11%), instrumentation or catheter trauma (11%), and penile cancer or condyloma (9%). Of these men, 21 (48%) had prior urethroplasty. Forty patients had follow-up greater than 3 months (median, 31 months). Postoperative stenosis occurred in 7 (18%) patients. Proton-pump inhibitors (PPIs) are among the most widely used drugs worldwide. PPI use has recently been linked to adverse changes in semen quality in healthy men; however, the effects of PPI use on semen parameters remain largely unknown specifically in cases with male factor infertility. We examined whether PPI use was associated with detrimental effects on semen parameters in a large population of subfertile men. We retrospectively reviewed data from 12 257 subfertile men who had visited our fertility clinic from 2003 to 2013. Patients who reported using any PPIs for >3 months before semen sample collection were included; 7698 subfertile men taking no medication served as controls. Data were gathered on patient age, medication use, and conventional semen parameters; patients taking any known spermatotoxic medication were excluded. Linear mixed-effect regression models were used to test the effect of PPI use on semen parameters adjusting for age. A total of 248 patients (258 samples) used PPIs for at least 3 months before semen collection. In regression models, PPI use (either as the only medication or when used in combination with other nonspermatotoxic medications) was not associated with statistically significant changes in semen parameters. To our knowledge, this is the largest study to compare PPI use with semen parameters in subfertile men. Using PPIs was not associated with detrimental effects on semen quality in this retrospective study.

We evaluated the effect of partial bladder outlet obstruction on bladder weight, protein synthesis, mitotic markers and the mitogen activated protein kinase pathway in a mouse model.Mice were divided into 3 groups, including control, sham treated and partially obstructed. Bladders were harvested from the mice in the partially obstructed group 12, 24, 48, 72 and 168 hours after surgical partial outlet obstruction, respectively. Partially obstructed bladders were compared to bladders in the control and sham treated groups by weight, protein content, and expression of proliferating cellular nuclear antigen, cyclin D3, HsP 70, c-jun and phosphorylated c-jun. Bladders were examined histologically for changes occurring with partial obstruction.We tested 3 groups of mice, including control, sham treated and partially obstructed mice, to understand the pathophysiology of the bladder response to partial obstruction. We found no statistical difference in body weight among the groups. Furthermore, there was a significant increase in bladder weight and protein content in partially obstructed mice compared to those in controls and sham operated mice. There was up-regulation of proliferating cellular nuclear antigen, cyclin D3, HsP70, c-jun and phosphorylated c-jun with partial obstruction. Fibrosis was prominent at 168 hours compared to that in controls.Bladder weight and protein content increase with partial bladder outlet obstruction in mice. Cell cycle proteins and elements of the mitogen activated protein kinase pathway are up-regulated during this process.

Although the male sling procedure is becoming increasingly popular, the artificial urinary sphincter (AUS) remains the gold standard due to its favorable outcome, long-term durability, and ability to treat mild, moderate, and severe urinary incontinence caused by intrinsic sphincter deficiency. However, AUS mechanical failure and the small but predictable occurrence of infection, erosion, and sub-cuff atrophy make it imperative that prosthetic urologists be familiar with salvage techniques. Secondary techniques to manage common complications are Mulcahy AUS salvage for infection, Webster’s transcorporal AUS cuff placement to prevent urethral erosion, and tandem cuff placement for urethral atrophy. Recent design modifications have introduced antibiotic-impregnated urinary sphincters that may decrease infections in primary and secondary AUS implants. Innovations in AUS placement, such as Mulcahy immediate AUS salvage and Webster’s transcorporal cuff placement, have allowed restoration of continence in men who previously had conditions considered untreatable.

Lichen sclerosus is a chronic, inflammatory skin condition of the genitalia of unknown origin that accounts for nearly 10% of urethral stricture disease. In this study we determine systemic comorbidities associated with lichen sclerosus in men.We analyzed data from 1,151 men who were enrolled in a multi-institutional prospective urethroplasty outcomes database. Individuals were grouped by stricture etiology, and baseline demographics, medical histories and patient reported outcome measures were retrospectively compared across groups.Of the 1,151 men in the database 81 (7.0%) were noted to have lichen sclerosus related urethral stricture disease. Average patient age was 46.06 ± 16.52 years, with those with lichen sclerosus being significantly older than those without lichen sclerosus (51.26 ± 13.84 vs 45.68 ± 16.64, p = 0.0011). Men with lichen sclerosus were more likely to have hypertension, hyperlipidemia and diabetes, and to use tobacco products. Controlling for age, men with lichen sclerosus related urethral stricture disease had a higher body mass index (aOR 1.089, 95% CI 1.050-1.130), and were more likely to have hypertension (aOR 2.028, 1.21-3.41) and be active tobacco users (aOR 2.0, 1.36-3.40). Mean preoperative patient reported outcome measures scores for urinary and sexual function were similar. Controlling for stricture length and location, the adjusted odds of surgical failure were higher for lichen sclerosus related urethral stricture disease (aOR 1.9, 95% CI 0.9-4.2).Lichen sclerosus related urethral stricture disease is associated with chronic systemic diseases. This association may implicate a systemic inflammatory and/or autoimmune pathophysiology. A 2-hit mechanism implicating local and systemic factors for lichen sclerosus related urethral stricture disease development and progression is hypothesized.

Objective To evaluate the urethroplasty learning curve. Published success rates of urethral reconstruction for urethral stricture disease are high even though these procedures can be technically demanding. It is likely that success rates improve with time although a learning curve for urethral reconstruction has never been established. Materials and Methods We retrospectively reviewed anterior urethroplasties from a prospectively maintained multi-institutional database. Success was analyzed at the 18-month mark in all patients and defined as freedom from secondary operation for stricture recurrence. A multivariate logistic regression was performed for outcomes vs time from fellowship and case number. Results A total of 613 consecutive cases from 6 surgeons were analyzed, with a functional success rate of 87.3%. The success rate for bulbar urethroplasties was higher than that for penile urethroplasties (88.2% vs 78.3%, P = .0116). The success rate of anastomotic repairs was higher than that for substitution repairs (95.0% vs 82.4%, P = .0001). There was a statistically significant trend toward improved outcomes with increasing number of cases (P = .0422), which was most pronounced with bulbar repairs. There was no statistical improvement in penile repairs over time. The case number to reach proficiency (>90% success) was approximately 100 cases for all types of reconstruction and 70 cases for bulbar urethroplasty. There were statistical differences in success rates among the participating surgeons (P = .0014). Complications decreased with time (P = .0053). Conclusion This study shows that success rates of anterior urethral reconstruction improve significantly with surgeon experience. Proficiency occurs after approximately 100 cases.

Introduction: Most high-grade renal injuries (American Association for Surgery of Trauma (AAST) grades III-V) result from motor vehicle collisions associated with numerous concomitant injuries. Sports-related blunt renal injury tends to have a different mechanism, a solitary blow to the flank. We hypothesized that high-grade renal injury is often isolated in sports-related renal trauma. Material and methods: We identified patients with AAST grades III-V blunt renal injuries from four level 1 trauma centres across the United States between 1/2005 and 1/2014. Patients were divided into "Sport" or "Non-sport" related groups. Outcomes included rates of hypotension (systolic blood pressure 110 bpm), concomitant abdominal injury, and procedural/surgical intervention between sports and non-sports related injury. Results: 320 patients met study criteria. 18% (59) were sports-related injuries with the most common mechanisms being skiing, snowboarding and contact sports (25%, 25%, and 24%, respectively). Median age was 24 years for sports and 30 years for non-sports related renal injuries (p = 0.049). Males were more commonly involved in sports related injuries (85% vs. 72%, p = 0.011). Median injury severity score was lower for sports related injuries (10 vs. 27, p < 0.001). There was no difference in renal abbreviated injury scale scores. Sports related trauma was more likely to be isolated without other significant injury (69% vs. 39% (p < 0.001)). Haemodynamic instability was present in 40% and 51% of sports and non-sports renal injuries (p = 0.30). Sports injuries had lower transfusion (7% vs. 47%, p < 0.001) and lower mortality rates (0% vs. 6%, p = 0.004). There was no difference in renal-specific procedural interventions between the two groups (17% sports vs. 18% non-sports, p = 0.95). Conclusions: High-grade sports-related blunt renal trauma is more likely to occur in isolation without other abdominal or thoracic injuries and clinicians must have a high suspicion of renal injury with significant blows to the flank during sports activities.

INTRODUCTION AND OBJECTIVES: Lower urinary tract symptoms (LUTS) are common in elderly men. We assessed symptom scores and voiding parameters in a group of community-dwelling elderly men over a period of 5 years with emphasis on the natural history of men with elevated postvoid residual urine at baseline. METHODS: Community dwelling men aged 70 years and over were enrolled to participate in the Concord Health and Ageing in Men Project (CHAMP), a population based study of men living in a defined geographical area in metropolitan Sydney, Australia. Men were required to complete a survey including International Prostate Symptom Score (IPSS) and medical, medication and urological history, then attend a clinical assessment where uroflowmetry and post void residual volume were measured. Subjects were re-assessed after two years and five years with repeat survey and clinical assessment. We report results at baseline and five year follow-up. RESULTS: A total of 1705 men aged 70-97 years participated; 1367 presented for follow-up assessment 2 years later and 940 at 5 years (382 (23%) had died). 275 men with prostate cancer were omitted from analyses. Mean IPSS was 7.4 (mild 63%, moderate 29%, severe 8%) at baseline and 7.3 (mild 64%, moderate 30%, severe 6%) at 5 years. Mean peak flow rate at baseline was 12.7 mls/sec and at 5 years was 12.0 mls/sec (P 85 years) compared to younger men (70-80 years) both at baseline (P 200mls at baseline the residual volume did not change significantly over 5 years (P1⁄40.51). Ten men were using a catheter at baseline and 9 at 5 year assessment. CONCLUSIONS: Both urinary symptom scores and voiding parameters of uroflowmetry and post void residual volume remain remarkably stable over a five year period in elderly, community-dwelling men. Men with elevated postvoid residual urine did not deteriorate significantly and the number of men requiring surgical intervention was low.

Introduction It is generally accepted that there is a risk of hypertension after renal trauma, particularly in high-grade and devascularizing injuries. Hypertension following renal trauma is estimated to occur in five percent of adults, however, the incidence is unknown in the pediatric population. Materials and methods We performed a retrospective review of all pediatric trauma patients at Primary Children's Hospital in Salt Lake City, Utah between 2002 and 2012. We included all children age ≤17years old with American Association for Surgery of Trauma (AAST) grade 3–5 renal injury. Hypertension was defined as persistent hypertension that required anti-hypertensive medications. Our primary outcomes were incidence of hypertension during the acute trauma and in long-term follow. Results A total of 62 children were identified with AAST grade 3–5 renal injuries during our study period. Follow up blood pressures were recorded in 36 (58%) of these children with a median follow of 4.1years (IQR 2.1–5.1years) after trauma. Four children (6.5%) were identified to have some degree of hypertension while hospitalized after trauma and started on anti-hypertensive medication. Two out of these four children remained on hypertensive medication at follow up, while the remaining two children's hypertension resolved. No children who were normotensive in the immediate post-trauma period, developed delayed hypertension during long-term follow up. Conclusions There is a low risk of developing hypertension following severe renal trauma in the pediatric population. Patients who develop long-term problems with hypertension after renal trauma manifest it during the initial hospitalization, rather than subsequently during the long-term.

Objective To evaluate sexual function after staged penile urethroplasty with oral mucosal graft (OMG). Methods We identified men with completed staged penile urethroplasty with OMG from the Trauma and Urologic Reconstruction Network of Surgeons database between January 1, 2010 and May 1, 2014. Our primary outcome was change in total Sexual Health Inventory for Men (SHIM) and total Male Sexual Health Questionnaire Ejaculatory Domain (MSHQ-EjD) Short Form at baseline vs after the second stage of the procedure. Second, we assessed subjective changes in penile curvature, length, and sensation. Results Thirty-three patients were included with a mean age of 45 years and mean body mass index of 27.6 kg/m2. Urethral strictures arose from failed hypospadias repair in 52% and lichen sclerosus in 27%. Fifty-two percent of patients reported a previous urethroplasty. The median follow-up time between the second stage procedure and postoperative questionnaires was 6.3 months (interquartile range: 3.5-13.3). There was no significant change in the total SHIM (Δ0.64, 95% confidence interval [CI]: −3.00∼1.72) and MSHQ-EjD (Δ1.55, 95% CI: −1.53∼4.63) scores preoperatively vs postoperatively. In addition, 32% reported improved and 52% no change in satisfaction with sexual intercourse (SHIM Q5). Forty percent of patients reported reduced and 45% no change in bother with ejaculation after surgery (MSHQ-EjD Q4). Men reported new penile curvature (23%), loss of penile length (55%), and altered penile sensitivity (45%) after surgery. Conclusion Patients undergoing staged penile urethroplasty with OMG are likely to have minimal changes in erectile and ejaculatory function postoperatively, although many may experience new penile curvature, reduced penile length, and/or reduced penile sensitivity.

The optimal approach to treating urinary tract infections patients with reconstructed and neurogenic bladders is a complex and evolving topic. This chapter aims to provide an outline of considerations which can be utilized by medical practitioners to diagnose a UTI and select an appropriate workup for determination of an underlying etiology. Also delineated is an overview of available evidence regarding the efficacy of various strategies for management and prevention of UTIs in this challenging population.

Angiomyolipoma (AML) is a benign renal lesion with the ‘pathognomic’finding of fat density contained within the tumor, usually identified on CT exam. In rare cases, especially when there is minimal fat density or the presence of calcifications within the mass, tumors may be misdiagnosed as AML and most often represent renal cell carcinoma. We present a case of a 59-year-old man diagnosed with AML who underwent subsequent partial nephrectomy and was found to have a large papillary renal cell carcinoma.

Injection of mitomycin C may increase the success of transurethral incision of the bladder neck for the treatment of bladder neck contracture. We evaluated the efficacy of mitomycin C injection across multiple institutions.Data on all patients who underwent transurethral incision of the bladder neck with mitomycin C from 2009 to 2014 were retrospectively reviewed from 6 centers in the TURNS. Patients with at least 3 months of cystoscopic followup were included in the analysis.A total of 66 patients underwent transurethral incision of the bladder neck with mitomycin C and 55 meeting the study inclusion criteria were analyzed. Mean ± SD patient age was 64 ± 7.6 years. Dilation or prior transurethral incision of the bladder neck failed in 80% (44 of 55) of patients. Overall 58% (32 of 55) of patients achieved resolution of bladder neck contracture after 1 transurethral incision of the bladder neck with mitomycin C at a median followup of 9.2 months (IQR 11.7). There were 23 patients who had recurrence at a median of 3.7 months (IQR 4.2), 15 who underwent repeat transurethral incision of the bladder neck with mitomycin C and 9 of 15 (60%) who were free of another recurrence at a median of 8.6 months (IQR 8.8), for an overall success rate of 75% (41 of 55). Incision with electrocautery (Collins knife) was predictive of success compared with cold knife incision (63% vs 50%, p=0.03). Four patients experienced serious adverse events related to mitomycin C and 3 needed or are planning cystectomy.The efficacy of intralesional injection of mitomycin C at transurethral incision of the bladder neck was lower than previously reported and was associated with a 7% rate of serious adverse events.

To describe existing bladder and bowel specific quality of life (QoL) measurement tools, QoL in patients with multiple sclerosis (MS), spinal cord injury (SCI), Parkinson's Disease (PD), stroke, or spina bifida (SB) affected by bladder or bowel dysfunction, and the impact of specific bladder and bowel management on QoL.We performed a systematic review in PubMed/Medline databases in accordance with the PRISMA statement for English publications between January 1, 2000 and January 1, 2014. Articles were first screened based on their abstract and select full-text articles were then reviewed for eligibility. Articles with no QoL or PROM assessing urinary or bowel dysfunction were excluded. Risk of bias assessment included randomization, incomplete outcomes data, selective outcomes reporting, and other biases. All articles were graded using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) system as per the Cochrane Handbook for Systematic Reviews of Interventions.The most common QoL measurement tool for urinary and bowel dysfunction was the Medical Outcomes Study SF-36. Twelve (24%) studies used only non-validated QoL questionnaires. Only three urinary or bowel specific QoL measures were found: the Qualiveen questionnaire, the FICQoL, and the QoL-BM. Several studies identified instances were clinical and patient-reported outcomes were inconsistent particularly with indwelling urinary catheter usage and reconstructive surgery. Additionally, certain clinical outcomes surrogates commonly used as primary outcomes measures may not correlate with the patient reported outcomes (PRO).Current PRO measures (PROM) and QoL assessments are heterogeneous and several inconsistencies in clinical and PRO for various management options exist. Standardized PROM will help identify optimal bladder and bowel management for patients with neurologic conditions.

Penile implants hold a major position in the treatment algorithm for patients with erectile dysfunction who find medications and vacuum erection devices ineffective or unsatisfactory. As with any surgical procedure, adverse events may occur. The infection rate associated with implant placement has been lowered to the range of 1 % or less due to multifactorial improvements including no-touch techniques, the use of antibiotic-coated devices, and improved quality measures in the operating room. Urologists have been proactive in employing techniques and procedures which minimize loss of erectile length, hence enhancing patient satisfaction. Flat reservoirs have been developed and techniques of placing these to avoid problems in the space of Retzius have reduced complication rates as well. Device reliability has improved to the point that penile implants are among the most durable mechanical surgical products that contribute to patient and partner satisfaction, which is by far the greatest among all the treatments of erectile dysfunction.
