Background and Purpose. Job-related repetitive strain injuries (RSIs) are increasing, and current treatment strategies often fail to return injured people to work. This study documented the neural consequences of using two different movement strategies for active, repetitive hand closing and opening. Methods. Two owl monkeys were trained for 20 weeks to repetitively close a handpiece against an 80-g force (3--400 trials per day, training at 80%--90% accuracy). One monkey used a highly articulated hand-squeezing strategy, and the other monkey used a proximal armpulling strategy. Changes in motor performance were analyzed, and the electrophysiological maps of the hand representation on the trained primary sensory cortex (area 3b) were compared with those of untrained control animals and the untrained sides of the trained monkeys. Results. The monkey using the articulated hand-squeezing strategy showed motor deterioration and dedifferentiation of the normally sharply segregated areas of the hand representation in area 3b. Mild degradation of the hand representation was measured in the monkey using the proximal armpulling strategy, but there was no motor dysfunction. Conclusion and Discussion. Attended, highly articulated, repetitive finger squeezing degrades the hand representation and interferes with motor control. A proximal, more variable repetitive strategy minimized the sensory degradation and preserved motor control. Abstract Background Tobacco use continues to be a global public health problem. Helping patients to quit is part of the preventive role of all health professionals. There is now increasing interest in the role that the dental team can play in helping their patients to quit smoking. The aim of this study was to determine the feasibility of undertaking a randomised controlled smoking cessation intervention, utilising dental hygienists to deliver tobacco cessation advice to a cohort of periodontal patients. Methods One hundred and eighteen patients who attended consultant clinics in an outpatient dental hospital department (Periodontology) were recruited into a trial. Data were available for 116 participants, 59 intervention and 57 control, and were analysed on an intention-to-treat basis. The intervention group received smoking cessation advice based on the 5As (ask, advise, assess, assist, arrange follow-up) and were offered nicotine replacement therapy (NRT), whereas the control group received 'usual care'. Outcome measures included self-reported smoking cessation, verified by salivary cotinine measurement and CO measurements. Self-reported measures in those trial participants who did not quit included number and length of quit attempts and reduction in smoking. Results At 3 months, 9/59 (15%) of the intervention group had quit compared to 5/57 (9%) of the controls. At 6 months, 6/59 (10%) of the intervention group quit compared to 3/57 (5%) of the controls. At one year, there were 4/59 (7%) intervention quitters, compared to 2/59 (4%) control quitters. In participants who described themselves as smokers, at 3 and 6 months, a statistically higher percentage of intervention participants reported that they had had a quit attempt of at least one week in the preceding 3 months (37% and 47%, for the intervention group respectively, compared with 18% and 16% for the control group). Conclusion This study has shown the potential that trained dental hygienists could have in delivering smoking cessation advice. While success may be modest, public health gain would indicate that the dental team should participate in this activity. However, to add to the knowledge-base, a multi-centred randomised controlled trial, utilising biochemical verification would be required to be undertaken.

The definitive version was published in Geophysical Research Letters 45 (2018): 4163-4170, doi:10.1029/2018GL077000., The Greenland Ice Sheet has been, and will continue, losing mass at an accelerating rate. The influence of this anomalous meltwater discharge on the regional and large‐scale ocean could be considerable but remains poorly understood. This uncertainty is in part a consequence of challenges in observing water mass transformation and meltwater spreading in coastal Greenland. Here we use tracer observations that enable unprecedented quantification of the export, mixing, and vertical distribution of meltwaters leaving one of Greenland's major glacial fjords. We find that the primarily subsurface meltwater input results in the upwelling of the deep fjord waters and an export of a meltwater/deepwater mixture that is 30 times larger than the initial meltwater release. Using these tracer data, the vertical structure of Greenland's summer meltwater export is defined for the first time showing that half the meltwater export occurs below 65 m., National Science Foundation Grant Number: OCE-1536856, 2018-11-05

The definitive version was published in Nature Communications 9 (2018): 2431, doi:10.1038/s41467-018-04421-3., Tectonic landforms reveal that the West Antarctic Ice Sheet (WAIS) lies atop a major volcanic rift system. However, identifying subglacial volcanism is challenging. Here we show geochemical evidence of a volcanic heat source upstream of the fast-melting Pine Island Ice Shelf, documented by seawater helium isotope ratios at the front of the Ice Shelf cavity. The localization of mantle helium to glacial meltwater reveals that volcanic heat induces melt beneath the grounded glacier and feeds the subglacial hydrological network crossing the grounding line. The observed transport of mantle helium out of the Ice Shelf cavity indicates that volcanic heat is supplied to the grounded glacier at a rate of ~ 2500 ± 1700 MW, which is ca. half as large as the active Grimsvötn volcano on Iceland. Our finding of a substantial volcanic heat source beneath a major WAIS glacier highlights the need to understand subglacial volcanism, its hydrologic interaction with the marine margins, and its potential role in the future stability of the WAIS., This research was supported by the NSF Antarctic program through Award #1341630.

The importance of the role that parasites play in ecological communities is becoming increasingly apparent. However much about their impact on hosts and thus populations and communities remains poorly understood. A common observation in wild populations is high variation in levels of parasite infestation among hosts. While high variation could be due to chance encounter, there is increasing evidence to suggest that such patterns are due to a combination of environmental, host, and parasite factors. In order to examine the role of host condition on parasite infection, rates of Gnathia marleyi infestation were compared between experimentally injured and uninjured fish hosts. Experimental injuries were similar to the minor wounds commonly observed in nature. The presence of the injury significantly increased the probability of infestation by gnathiids. However, the level of infestation (i.e., total number of gnathiid parasites) for individual hosts, appeared to be unaffected by the treatment. The results from this study indicate that injuries obtained by fish in nature may carry the additional cost of increased parasite burden along with the costs typically associated with physical injury. These results suggest that host condition may be an important factor in determining the likelihood of infestation by a common coral reef fish ectoparasite, G. marleyi

BackgroundCombined positron emission tomography (PET) and computed tomography (CT) can assess both anatomy and biology of carotid atherosclerosis. We sought to assess whether 18F-fluoride or 18F-fluorodeoxyglucose can identify culprit and high-risk carotid plaque. Methods and ResultsWe performed 18F-fluoride and 18F-fluorodeoxygluose PET/CT in 26 patients following recent TIA or minor ischemic stroke: 18 patients with culprit carotid stenosis awaiting carotid endarterectomy and 8 controls without culprit carotid atheroma. We compared standardized uptake values (SUVs) in the clinically adjudicated culprit to the contralateral asymptomatic artery, and assessed the relationship between radiotracer uptake and plaque phenotype or predicted cardiovascular risk (ASSIGN score). We also performed micro PET/CT and histological analysis of excised plaque.On histological and microPET/CT analysis, 18F-fluoride selectively highlighted micro-calcification. Carotid 18F-fluoride uptake was increased in clinically adjudicated culprit plaques compared to asymptomatic contralateral plaques (log10SUVmean 0.29±0.10 versus 0.23±0.11, p=0.001) and compared to control patients (log10SUVmean 0.29±0.10 versus 0.12±0.11, p=0.001). 18F-Fluoride uptake correlated with high-risk plaque features (remodeling index [r=0.53, p=0.003]; plaque burden [r=0.51, p=0.004]) and predicted cardiovascular risk [r=0.65, p=0.002]). Carotid 18F-FDG uptake appeared to be increased in 7/16 culprit plaques but no overall differences in uptake were observed in culprit versus contralateral plaques or control patients. However, 18F-FDG did correlate with predicted cardiovascular risk (r=0.53, p=0.019) but not with plaque phenotype. Conclusions18F-Fluoride PET/CT highlights culprit and phenotypically high-risk carotid plaque. This has the potential to improve risk-stratification and selection of patients who may benefit from intervention.

A study was conducted to analyze the ventilation of the eastern North Atlantic subtropical gyre. Multivariate regression analysis was utilized to determine the large-scale spatiotemporal structure from a 15-year-old historical data set of tritium and 3H measurements. Results indicated that the tritium-3He age was directly proportional over time with the structure of the change supporting smooth changes with depth and position.

Using a recently compiled global marine data set of dissolved helium isotopes and helium and neon concentrations, we make an estimate of the inventory of hydrothermal 3He in the Southern Ocean to be 4.9 ± 0.6 × 104 moles. Under the assumption that the bulk of the hydrothermally sourced 3He is upwelled there, we use recent estimates of the global hydrothermal 3He flux to determine an e‐folding residence time of 99 ± 18 years, depending on assumptions of water mass and upwelling boundaries. Our estimate is within the broad range of values obtained from recent Southern Ocean circulation models. Plain Language Summary: Seafloor hydrothermal systems inject significant amounts of dissolved iron, but it remains unknown what fraction of that trace element survives in situ chemical scavenging to upwell in the Southern Ocean to arrive at the sea surface. Since iron is a necessary trace‐level nutrient for marine biological productivity, knowledge of the time it takes for deep water to rise to the surface is important. Along with dissolved iron, helium‐3, a rare, inert isotope, is also injected into the deep water. Since the global flux of this isotope has been independently determined by large‐scale ocean models, we can estimate the upwelling time scale by dividing it into the inventory of this isotope in the Southern Ocean. Objective: First-phase ejection fraction (EF1) is a novel measure of early left ventricular systolic dysfunction. We investigated determinants of EF1 and its prognostic value in aortic stenosis.Methods: EF1 was measured retrospectively in participants of an echocardiography/cardiovascular magnetic resonance cohort study which recruited patients with aortic stenosis (peak aortic velocity of ≥2 m/s) between 2012 and 2014. Linear regression models were constructed to examine variables associated with EF1. Cox proportional hazards were used to determine the prognostic power of EF1 for aortic valve replacement (AVR, performed as part of clinical care in accordance with international guidelines) or death.Results: Total follow-up of the 149 participants (69.8% male, 70 (65-76) years, mean gradient 33 (21-42) mm Hg) was 238 029 person-days. Sixty-seven participants (45%) had a low baseline EF1 (<25%) despite normal ejection fraction (67% (62%-71%)). Patients with low EF1 had more severe aortic stenosis (mean gradient 39 (34-45) mm Hg vs 24 (16-35) mm Hg, p<0.001) and more myocardial fibrosis (indexed extracellular volume (iECV) (24.2 (19.6-28.7) mL/m2 vs 20.6 (16.8-24.3) mL/m2, p=0.002; late gadolinium enhancement (LGE) prevalence 52% vs 20%, p<0.001). Zva, iECV and infarct LGE were independent predictors of EF1. EF1 improved post-AVR (n=57 with post-AVR EF1 available, baseline 16 (12-24) vs follow-up 27% (22%-31%); p<0.001). Low baseline EF1 was an independent predictor of AVR/death (HR 5.6, 95% CI 3.4 to 9.4), driven by AVR.Conclusion: EF1 quantifies early, potentially reversible systolic dysfunction in aortic stenosis, is associated with global afterload and myocardial fibrosis, and is an independent predictor of AVR.
