OBJECTIVE Increases in serum calcitonin, a tumor marker for medullary thyroid carcinoma (MTC), have been associated with glucagon-like peptide 1 receptor agonist use in some preclinical studies. We report calcitonin changes in exenatide-treated and placebo-administered participants and MTC incidence in the EXenatide Study of Cardiovascular Event Lowering (EXSCEL) and consider the impact of within-trial calcitonin monitoring. RESEARCH DESIGN AND METHODS EXSCEL participants were randomized 1:1 to once-weekly exenatide 2 mg or placebo. Serum calcitonin was measured at baseline (with trial medication discontinued if >40 ng/L) and annually thereafter (with trial medication discontinued if ‡50 ng/L). Median calcitonin concentrations were calculated at each time point, and thyroid malignancies were collected prospectively. Data regarding follow-up after an elevated calcitonin were collected retrospectively. RESULTS At baseline, 52 (30 exenatide and 22 placebo) participants had calcitonin >40 ng/L, and during follow-up an additional 23 participants (15 exenatide and 8 placebo) had calcitonin ‡50 ng/L in the intention-to-treat population. Median calcitonin concentrations were similar between treatment groups at baseline with no increase over time. Confirmed MTC occurred in three participants (2 exenatide and 1 placebo), all of whom had significantly elevated baseline calcitonin values (413, 422, and 655 ng/L). CONCLUSIONS During a median 3.2 years’ follow-up, no change in serum calcitonin was seen with exenatide therapy. The three confirmed cases of MTC all occurred in participants with markedly elevated baseline calcitonin levels, measured prior to trial medication administration. Regular calcitonin monitoring identified no additional cases of MTC, suggesting no benefit of routine calcitonin monitoring during exenatide treatment.

Background: Recent trials have identified anti-diabetes mellitus agents that lower major adverse cardiovascular event (MACE) rates, although some increase rates of lower-extremity amputation (LEA). Patients with peripheral artery disease (PAD) have greater incidence of diabetes mellitus and risk for LEA, prompting this investigation of clinical outcomes in patients with diabetes mellitus and PAD in the EXSCEL trial (Exenatide Study of Cardiovascular Event Lowering). Methods: EXSCEL evaluated the effects of once-weekly exenatide (a GLP-1 [glucagon-like peptide-1] receptor agonist) versus placebo on the rates of the primary composite MACE end point (cardiovascular death, myocardial infarction, or stroke) among patients with type 2 diabetes mellitus. In this post hoc analysis, we assessed the association of baseline PAD with rates of MACE, LEA, and the effects of exenatide versus placebo in patients with and without PAD. Results: EXSCEL included 2800 patients with PAD (19% of the trial population). These individuals had higher unadjusted and adjusted rates of MACE compared with patients without PAD (13.6% versus 11.4%, respectively) as well as a higher adjusted hazard ratio (adjusted hazard ratio, 1.13 [95% CI, 1.00-1.27]; P=0.047). - Abstract
Tissue engineering and regenerative medicine are emerging as future approaches for the treatment of acute and chronic diseases. Numerous clinical conditions exist today and include congenital disorders, trauma, infection, inflammation, and cancer, in which hard and soft tissue damage, organ failure and loss are still not treated effectively. Researchers are constantly developing new biomaterials and tissue-engineered technologies to stimulate tissue regeneration. Various emerging approaches according to organ, tissue, disease and disorder are identified. Irrespective, engineered biomaterials are required to regenerate and ultimately reproduce the original physiological, biological, chemical, and mechanical properties. Biodegradable materials have been used extensively as regenerative therapies. The selection, design, biological and physicochemical properties of these materials are important and must be considered for stimulating tissue growth. In this review, we critique recently developed biodegradable materials for tissue regeneration of some targeted organs e.g., skin, nerves, blood vessels, heart, cornea, trachea, dental/oral structure and bones.

Aim The anastomosis technique used following right-sided colonic resection is widely variable and may affect patient outcome. This study aimed to assess the association between leak and anastomosis technique (stapled vs handsewn)., Method This was a prospective, multicentre, international audit including patients undergoing elective or emergency right hemicolectomy or ileo-caecal resection operations over a 2-month period in early 2015. The primary outcome measure was the presence of anastomotic leak within 30 days of surgery, determined using a prespecified definition. Mixed effects logistic regression models were used to assess the association between leak and anastomosis method, adjusting for patient, disease and operative cofactors, with centre included as a random-effect variable., Results This study included 3208 patients, of whom 78.4% (n = 2515) underwent surgery for malignancy and 11.7% (n = 375) underwent surgery for Crohn's disease. An anastomosis was performed in 94.8% (n = 3041) of patients, which was handsewn in 38.9% (n = 1183) and stapled in 61.1% (n = 1858). Patients undergoing hand-sewn anastomosis were more likely to be emergency admissions (20.5% handsewn vs 12.9% stapled) and to undergo open surgery (54.7% handsewn vs 36.6% stapled). The overall anastomotic leak rate was 8.1% (245/3041), which was similar following handsewn (7.4%) and stapled (8.5%) techniques (P = 0.3). After adjustment for cofactors, the odds of a leak were higher for stapled anastomosis (adjusted OR = 1.43; 95% CI: 1.04-1.95; P = 0.03)., Conclusion Despite being used in lower-risk patients, stapled anastomosis was associated with an increased anastomotic leak rate in this observational study. Further research is needed to define patient groups in whom a stapled anastomosis is safe.

- Abstract
Background: The optimal treatment for poor prognosis mCRPC is undefined and includes either taxane chemotherapy or androgen receptor (AR) targeted therapy, emphasizing the need for predictive biomarkers. Method(s): Patients (pts) with poor prognosis (liver metastases, early CRPC (<12 months from ADT start), and/or>3 of 6 poor prognostic criteria (Chi et al, Annals of Oncol, 2016)) were randomized to receive CAB (Arm A) or AR targeted therapy (Arm B, ABI or ENZ by investigator choice) with cross over at progression. No prior ABI or ENZ was permitted, but prior docetaxel allowed. Primary objective was to determine the clinical benefit rate (CBR) (PSA decline >= 50% (PSA50), objective response (OR), or stable disease (SD) >= 12 weeks). Other endpoints included time to PSA progression (TTPP), time to progression (TTP), and overall survival (OS). Serial plasma was sampled for circulating tumour DNA (ctDNA). Result(s): 95 pts were randomized (Arm A: 45, Arm B: 50). Poor prognosis was based on liver mets in 18%, early CRPC in 88%, and 30% by prognostic criteria. Other baseline factors: median age 68 years, elevated LDH in 41%, elevated ALK PHOS in 52%, ECOG PS 0-1 in 94%, 52% had prior docetaxel (half for castration sensitive disease). Median duration of therapy was 5.8 months (m) for Arm A and 4.5mfor Arm B. Treatment discontinuation reasons included disease progression (A vs B: 40% vs 46%) and toxicity (11% vs 4%). Outcomes are summarized in table. In 58 pts with available results, baseline ctDNA fraction>2% correlated with TTP (median 3.4mvs 10.8 m, p=0.011) and OS (median 15.5mvs not reached (NR), p=0.002). Genomic alterations in AR, RB1, TP53, PI3K pathway, and DNA repair were present in 69%, 36%, 51%, 40%, and 15%. (Table Presented) Conclusion(s): Treatment with CAB vs ABI/ENZA resulted in similar outcomes. There was a trend in favour of CAB for survival. Genomic correlations will be presented.

Maize (Zea mays L.) is an important cereal, which is affected adversely by nutrients deficiency. With the objective to combat nutrients deficiency in maize through bio-liquid (earthworm wash), experiments were conducted at University of Agriculture, Faisalabad, Pakistan. First experiment was executed for the characterization of bio-liquids from three different plant sources. In the second experiment, the concentrations of extracted bio-liquids were optimized on morpho-physiological characteristics of hybrid maize (P-1543). The experiment was conducted under Completely Randomized Design (CRD) with factorial arrangement replicating four times. The treatments comprised of three sources of bio-liquids i.e. alligator weed (Alternanthera philoxeroides), alfalfa (Medicago sativa) and rice (Oryza sativa) straw in first experiment and four concentrations of bioliquids (Zero, 10%, 15% and 20%) were tested on maize in second experiment. Foliar spray of bio-liquids was applied at 10, 15 and 20 days after the crop emergence. The evaluation was done 45 days after emergence on the basis of various morphological (plant height, number of leaf per plant, fresh weight of shoot, fresh weight of leaf) and physiological attributes (water potential, osmotic potential, chlorophyll fluorescence, cell membrane stability index and relative water contents). The results of first experiment showed that bio-liquid extracted from alligator weed proved to be best source of micro and macro nutrients as compared to alfalfa and rice straw bio-liquids. The results of second experiment indicated that foliar spray of bioliquids from all sources improved plant height (16.55 cm), number of leaves per plant (7.80), leaf area per plant (187.41 cm-2), total fresh weight per plant (5.74 g), water potential (-0.79 MPa), osmotic potential (-0.17MPa), chlorophyll fluorescence (0.84), cell membrane stability (40.27) and relative water content (91.80%) as compared to control. It can be concluded that growth of maize plants was improved when bio-liquids were sprayed on plants. Foliar application of alligator weed bio-liquid @ 15% showed maximum morpho-physiological performance of maize. [ABSTRACT FROM AUTHOR]

We present a simple analytic gravitational solution which describes the holographic dual of a 2+1-dimensional conductor which goes beyond the usual linear response. In particular it includes Joule heating. We find that the nonlinear frequency-dependent conductivity is a constant. Surprisingly, the pressure remains isotropic. We also apply an electric field to a holographic insulator and show that there is a maximum electric field below which it can remain an insulator. Above this critical value, we argue that it becomes a conductor due to pair creation of charged particles. Finally, we study 1+1 and 3+1 dimensional conductors at the nonlinear level; here exact solutions are not available and a perturbative analysis shows that the current becomes time dependent, but in a way that is captured by a time-dependent effective temperature.

This paper studies the uses, of waste paper as an additional material in concrete mixes. Papercrete is a term as the name seems, to imply a mixture of paper and concrete. It is a new, composite material using waste paper, as a partial addition of Portland cement, and is a sustainable, building material due to, reduced amount of waste paper being put to use. It gains, latent strength due to presence of hydrogen bonds in microstructure of paper. Papercrete has been, reported to be a low cost alternative, building construction, material and has, good sound absorption, and thermal insulation; to be a lightweight and fire-resistant material. The percent of waste paper used (after treating) namely (5%, 10%, 15% and 20%) by weight of cement to explore the mechanical properties of the mixes (compressive strength, splitting tensile strength, flexural strength, density), as compared with references mixes, it was found that fresh properties affected significantly by increasing the waste paper content. The compressive strength, splitting tensile strength, flexural strength and density got decreased with increase in the percentage of paper.

Background The ability to accurately predict operative duration has the potential to optimise theatre efficiency and utilisation, thus reducing costs and increasing staff and patient satisfaction. With laparoscopic cholecystectomy being one of the most commonly performed procedures worldwide, a tool to predict operative duration could be extremely beneficial to healthcare organisations. Methods Data collected from the CholeS study on patients undergoing cholecystectomy in UK and Irish hospitals between 04/2014 and 05/2014 were used to study operative duration. A multivariable binary logistic regression model was produced in order to identify significant independent predictors of long (> 90 min) operations. The resulting model was converted to a risk score, which was subsequently validated on second cohort of patients using ROC curves. Results After exclusions, data were available for 7227 patients in the derivation (CholeS) cohort. The median operative duration was 60 min (interquartile range 45–85), with 17.7% of operations lasting longer than 90 min. Ten factors were found to be significant independent predictors of operative durations > 90 min, including ASA, age, previous surgical admissions, BMI, gallbladder wall thickness and CBD diameter. A risk score was then produced from these factors, and applied to a cohort of 2405 patients from a tertiary centre for external validation. This returned an area under the ROC curve of 0.708 (SE = 0.013, p 90 min increasing more than eightfold from 5.1 to 41.8% in the extremes of the score. Conclusion The scoring tool produced in this study was found to be significantly predictive of long operative durations on validation in an external cohort. As such, the tool may have the potential to enable organisations to better organise theatre lists and deliver greater efficiencies in care.

The scanning electron microscopy (SEM) was used to describe the morphology and condition of hydrate crystals. This work is designed to investigate the simultaneous effect of both polymers to cement ratio (p/c) and water to cement ratio (w/c) ratios on flexural strength of type I ordinary Portland cement mortar containing PVA. The obtained results confirmed the effect of the added polymer and an optimum value for p/c equal to 1.6% at optimum w/c of 0.30 was recommended due to highest obtained flexural strength, which is two times of the strength of unmodified mortar. SEM images present an abundant crystal of cement mortar growth noticed at the air void surfaces of the modified mortars where the presence of water-soluble polymers is expected. Possible film formation on the surface of cement grains due to PVC addition was studied by examining the influence of the curing conditions on the flexural strength.

This paper presents an experimental investigation on the flexural behavior of reinforced concrete beams made from a combination of two types of concrete as a (built-up or composite) member. The experimental program consists of casting and testing of eleven rectangular cross-sections of simply supported reinforced concrete beams. Distribution of the specimens was three beams used as a reference which were fully poured from conventional concrete (CC) and reactive powder concrete (RPC). The remaining were eight (built-up) beams made by incorporation of two types of concrete as an element, about those beams, the RPC is poured as layers different in depth and the effective regions (compression and tension), where they are compatible with CC to complete the beam depth. Experimental results show that a stiffer behavior of load-deflection appears, especially when the RPC is in tension region (the bottom of the beam) with respect to increase of the layer depth and the volumetric ratio of steel fibers (vf). The load-carrying capacity of (built-up) beams increase when the RPC is used in the compression region affected by the increase of the layer depth and the volumetric ratio of steel fibers, with less stiffer behavior compared to the beams where the RPC used in the tension regions. The results can be summarized that RPC utilizing in compression regions is more influential than utilizing it in tension regions.

Present study aims to observe the consequence of varying the intensity of the accelerating impressed electrical current and situations of wetting-drying as hybrid accelerated corrosion regime on synthetically corroded concrete specimens, especially the loss of steel mass. Small-scaled reinforced concrete columns were fabricated and synthetically corroded by altered accelerating corrosion situations by applying an impressed current with intensity ranging between (50 and 500 μA/cm2). Moreover, the impressed current joined with two dissimilar durations of wetting-drying cycles for assessment. The steel mass losses during and at the end of the accelerated corrosion process were calculated, also, cracking configuration and damage shape were observed for accelerating corrosion. The outcomes designated that the hybrid method (impressed current joined with cycles of saline solution wetting and drying series) can be employed adequately to simulate the regular corrosion process in the reinforced concrete structure. The steel mass loss is mostly influenced by the intensities of the impressed accelerating current, while the crack appearance and the pattern of surface cracks, is influenced by wetting and drying cycles as well as the impressed accelerating current. Yet, increasing the current intensity produce a considerable growth in the cracking owing to the reinforcing steel corrosion in shorter stage.
