201. Abnormal striatal plasticity in a DYT11/SGCE myoclonus dystonia mouse model is reversed by adenosine A2A receptor inhibition

Abstract

Striatal dysfunction is implicated in many movement disorders. However, the precise nature of defects often remains uncharacterized, which hinders therapy development. Here we examined striatal function in a mouse model of the incurable movement disorder, myoclonus dystonia, caused by SGCE mutations. Using RNAseq we found surprisingly normal gene expression, including normal levels of neuronal subclass markers to strongly suggest that striatal microcircuitry is spared by the disease insult. We then functionally characterized Sgce mutant medium spiny projection neurons (MSNs) and cholinergic interneurons (ChIs). This revealed normal intrinsic electrophysiological properties and normal responses to basic excitatory and inhibitory neurotransmission. Nevertheless, high-frequency stimulation in Sgce mutants failed to induce normal long-term depression (LTD) at corticostriatal glutamatergic synapses. We also found that pharmacological manipulation of MSNs by inhibiting adenosine 2A receptors (A2AR) restores LTD, again pointing to structurally intact striatal circuitry. The fact that Sgce loss specifically inhibits LTD implicates this neurophysiological defect in myoclonus dystonia, and emphasizes that neurophysiological changes can occur in the absence of broad striatal dysfunction. Further, the positive effect of A2AR antagonists indicates that this drug class be tested in DYT11/SGCE dystonia.

Abstract

This study assessed the short-term effects of conventional (i.e., human-composed) and algorithmic music on the relaxation level. It also investigated whether algorithmic compositions are perceived as music and are distinguishable from human-composed music. Three hundred twenty healthy volunteers were recruited and randomly allocated to two groups where they listened to either their preferred music or algorithmic music. Another 179 healthy subjects were allocated to four listening groups that respectively listened to: music composed and performed by a human, music composed by a human and performed by a machine; music composed by a machine and performed by a human, music composed and performed by a machine. In the first experiment, participants underwent one of the two music listening conditions—preferred or algorithmic music—in a comfortable state. In the second one, participants were asked to evaluate, through an online questionnaire, the musical excerpts they listened to. The Visual Analogue Scale was used to evaluate their relaxation levels before and after the music listening experience. Other outcomes were evaluated through the responses to the questionnaire. The relaxation level obtained with the music created by the algorithms is comparable to the one achieved with preferred music. Statistical analysis shows that the relaxation level is not affected by the composer, the performer, or the existence of musical training. On the other hand, the perceived effect is related to the performer. Finally, music composed by an algorithm and performed by a human is not distinguishable from that composed by a human.

Abstract

The 12C(p,γ)13N reaction is the onset process of both the CNO and Hot CNO cycles that drive massive star, Red and Asymptotic Giant Branch star and novae nucleosynthesis. The 12C(p,γ)13N rate affects the final abundances of the stable 12,13C nuclides, with ramifications for meteoritic carbon isotopic abundances and the s-process neutron source strength. Here, a new underground measurement of the 12C(p,γ)13N cross-section is reported. The present data, obtained at the Felsenkeller shallow-underground laboratory in Dresden (Germany), encompass the 320-620 keV center of mass energy range to include the wide and poorly constrained E = 422 keV resonance that dominates the rate at high temperatures. This work S-factor results, lower than literature by 25%, are included in a new comprehensive R-matrix fit, and the energy of the 1 + first excited state of 13N is found to be 2369.6(4) keV, with radiative and proton width of 0.49(3) eV and 34.9(2) keV respectively. A new reaction rate, based on present R-matrix fit and extrapolation, is suggested.

Abstract

Background: Fibromyalgia patients can benefit from music approaches as complementary treatments. In the literature, it was shown that these interventions managed pain conditions as well as reduced complaints, increased relaxation, and improved moods. Objective: This study aimed at evaluating music therapy, in the form of therapeutic music listening, specifically for patients with fibromyalgia, to treat chronic pain by reducing pain perception, increasing well-being, and improving quality of life. Methods: Twenty-four patients with fibromyalgia were recruited to take part in this feasibility pilot study that adopted a between-subject and within-subject design. Participants were randomised into three groups: (1) standard care, (2) standard care plus preferred music listening, (3) standard care plus Melomics-Health music listening, composed by an algorithm. Participants in experimental groups listened to 30 min of music at home, twice a day for a month. Patients’ perceptions of changes following the listening, the intensity of pain and its interference in their lives, physical and mental well-being, and reported attitudes towards listening to music were evaluated respectively through the patients’ global impression of change, the brief pain inventory, the Short Form Healthy Survey-12, and the cognitive behavioural assessment-outcome evaluation. Results: The study showed that music listening can significantly affect mental well-being compared to no music. Moreover, the effects in the Melomics-Health group are maintained at follow-up. No significant effect on pain perception was noted. Conclusions: The study provides information supporting a possible role of music listening in improving well-being of patients with fibromyalgia.

Abstract

Hepatocellular carcinoma (HCC) is the most frequent liver neoplasm, and its incidence rates are constantly increasing. Despite the availability of potentially curative treatments (liver transplantation, surgical resection, thermal ablation), long-term outcomes are affected by a high recurrence rate (up to 70% of cases 5 years after treatment). HCC recurrence within 2 years of treatment is defined as "early" and is generally caused by the occult intrahepatic spread of the primary neoplasm and related to the tumor burden. A recurrence that occurs after 2 years of treatment is defined as "late" and is related to de novo HCC, independent of the primary neoplasm. Early HCC recurrence has a significantly poorer prognosis and outcome than late recurrence. Different pathogenesis corresponds to different predictors of the risk of early or late recurrence. An adequate knowledge of predictive factors and recurrence risk stratification guides the therapeutic strategy and post-treatment surveillance. Patients at high risk of HCC recurrence should be referred to treatments with the lowest recurrence rate and when standardized to combined or adjuvant therapy regimens. This review aimed to expose the recurrence predictors and examine the differences between predictors of early and late recurrence.

The questionnaire, called Maugeri Stress Index – reduced form (MASI-R), aims to assess the impact of job strain on a team or on a single worker by considering four domains: wellness, resilience, perception of social support, and reactions to stressful situations. Material and methods: The reliability of a first longer version (47 items) of the questionnaire was evaluated by an internal consistency analysis and a confirmatory factor analysis. An item reduction procedure was implemented to obtain a short form of the instrument, and the psychometric properties of the resulting instrument were evaluated using the Rasch measurement model. Results: A total of 14 items from the initial pool were deleted because they were not productive for measurement. The analysis of internal consistency led to the exclusion of eight items, while the analysis performed using structural equation models led to the exclusion of another six items. According to the Rasch model, item properties and the reliability of the instruments appear good, especially for the scales for wellness and resilience. In contrast, the scales for perception of social support and negative coping styles show a lower internal consistency. Conclusions: The Maugeri Stress Index – reduced form provides a reliable and valid measure, useful for early identification of stress levels in workers or in a team along the eustress–vadistress continuum. Abstract

For more than three decades, accelerators are in use in the underground laboratories of the Laboratori Nazionali del Gran Sasso (LNGS), located in central Italy. The LUNA Collaboration has exploited the potential of the site’s low cosmic ray background to achieve important and often groundbreaking results in the field of nuclear astrophysics. This long success story stimulated the installation of accelerators in deep underground laboratories also in other countries, including the USA and China. Recently, LNGS took a major step forward with the activation of the Bellotti Ion Beam Facility, which will provide ion beams to the scientific community for research not only in nuclear astrophysics, but in all fields that can benefit from the low cosmic ray background conditions of the underground site.

Abstract

Studies of charged-particle reactions for low-energy nuclear astrophysics require high sensitivity, which can be achieved by means of detection setups with high efficiency and low backgrounds, to obtain precise measurements in the energy region of interest for stellar scenarios. High-efficiency total absorption spectroscopy is an established and powerful tool for studying radiative capture reactions, particularly if combined with the cosmic background reduction by several orders of magnitude obtained at the Laboratory for Underground Nuclear Astrophysics (LUNA). We present recent improvements in the detection setup with the Bismuth Germanium Oxide (BGO) detector at LUNA, aiming to reduce high-energy backgrounds and to increase the summing detection efficiency. The new design results in enhanced sensitivity of the BGO setup, as we demonstrate and discuss in the context of the first direct measurement of the 65 keV resonance (Ex = 5672 keV) of the 17O(p,gamma)18F reaction. Moreover, we show two applications of the BGO detector, which exploit its segmentation. In case of complex gamma-ray cascades, e.g. the de-excitation of Ex = 5672 keV in 18F, the BGO segmentation allows to identify and suppress the beam-induced background signals that mimic the sum peak of interest. We demonstrate another new application for such a detector in form of in-site activation measurements of a reaction with beta+ unstable product nuclei, e.g., the 14N(p,gamma)15O reaction.

Abstract

COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March–April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients’ management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p < 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p < 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p < 0.0001) or urgent (20.4% vs. 38.5%; p < 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons’ personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p < 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon–patient/family communication. Abstract

The observation of oxygen isotopes in giant stars sheds light on mixing processes operating in their interiors. Due to the very strong correlation between nuclear burning and mixing processes it is very important to reduce the uncertainty on the cross sections of the nuclear reactions that are involved. In this paper we focus our attention on the reaction O18(p,γ)19F. While the O18(p,α)15N channel is thought to be dominant, the (p,γ) channel can still be an important component in stellar burning in giants, depending on the low energy cross section. So far only extrapolations from higher-energy measurements exist and recent estimates vary by orders of magnitude. These large uncertainties call for an experimental reinvestigation of this reaction. We present a direct measurement of the O18(p,γ)19F cross section using a high-efficiency 4π BGO summing detector at the Laboratory for Underground Nuclear Astrophysics (LUNA). The reaction cross section has been directly determined for the first time from 140 keV down to 85 keV and the different cross section components have been obtained individually. The previously highly uncertain strength of the 90 keV resonance was found to be 0.53 ± 0.07 neV, three orders of magnitude lower than an indirect estimate based on nuclear properties of the resonant state and a factor of 20 lower than a recently established upper limit, excluding the possibility that the 90 keV resonance can contribute significantly to the stellar reaction rate. Abstract

Effective disease-modifying treatments are an urgent need for Parkinson's disease (PD). A putative successful strategy is to counteract oxidative stress, not only with synthetic compounds, but also with natural agents or dietary choices. Vitamin E, in particular, is a powerful antioxidant, commonly found in vegetables and other components of the diet. In this work, we performed a questionnaire based case-control study on 100 PD patients and 100 healthy controls. The analysis showed that a higher dietary intake of Vitamin E was inversely associated with PD occurrence independently from age and gender (OR = 1.022; 95% CI = 0.999–1.045; p < 0.05), though unrelated to clinical severity. Then, in order to provide a mechanistic explanation for such observation, we tested the effects of Vitamin E and other alimentary antioxidants in vitro, by utilizing the homozygous PTEN-induced kinase 1 knockout (PINK1−/−) mouse model of PD. PINK1−/− mice exhibit peculiar alterations of synaptic plasticity at corticostriatal synapses, consisting in the loss of both long-term potentiation (LTP) and long-term depression (LTD), in the absence of overt neurodegeneration. Chronic administration of Vitamin E (alpha-tocopherol and the water-soluble analog trolox) fully restored corticostriatal synaptic plasticity in PINK1−/− mice, suggestive of a specific protective action. Vitamin E might indeed compensate PINK1 haploinsufficiency and mitochondrial impairment, reverting some central steps of the pathogenic process. Altogether, both clinical and experimental findings suggest that Vitamin E could be a potential, useful agent for PD patients. These data, although preliminary, may encourage future confirmatory trials.

Abstract

This paper explores the need for a competitive game for sighted and blind players alike, its effectiveness in fighting prejudices and how it can contribute and facilitate the inclusion of people with visual disability, when played by heterogeneous groups of players. The first part presents the state of the art about competitive games for blind and low vision users. Focusing on the typologies of games already on the market, and on the apparent lack of fast-paced multiplayer games, it shows a current space for research and development. Then, we present WaTa Fight!, a competitive casual party game that can be used by sighted and blind players alike, outlining its development, from its concept to its design and testing – conducted via focus groups and survey. Finally, we discuss the results obtained in terms of the game’s effectiveness in addressing the visual impairment topic and in fostering integration.

Abstract

O presente artigo objetiva refletir sobre o uso do Team-Based Learning (TBL) no ensino superior de música no sentido de superar algumas dificuldades que foram verificadas e descritas em pesquisa realizada em 2019. Uma delas se refere a falta de leitura e preparação prévia para as aulas por parte de muitos estudantes do Ensino Superior em música. Dessa forma, o engajamento, a autonomia, a organização de tempo para leitura prévia e disposição para aprender coletivamente, que são essenciais para que este tipo de abordagem seja significativa, prescinde que o estudante se aproprie dos conteúdos abordados nas disciplinas. Por meio de pesquisa bibliográfica, será apresentado o TBL como proposta interessante ao contexto do Ensino Superior incluindo o atual contexto de afastamento social e propor o acréscimo de etapas ao modelo proposto pelo TBL no intuito de superar a defasagem de leitura inicial. Assim, sugerimos a inclusão de alguns passos para além das 3 etapas já existentes: consulta ao conteúdo, acréscimo de teste individual ao preparo prévio e nova distribuição do peso valorativo das etapas individuais e coletivas. Esperamos possibilitar a reflexão sobre os limites e alcances da utilização do TBL no Ensino Superior de música contribuindo, dessa forma, com propostas mais inovadoras aos estudantes que tem chegado ao espaço universitário principalmente nesse período o qual o ensino remoto emergencial ainda é ofertado.

The neuronal protein alpha-synuclein (α-Syn) is crucially involved in Parkinson's disease pathophysiology. Intriguingly, torsinA (TA), the protein causative of DYT1 dystonia, has been found to accumulate in Lewy bodies and to interact with α-Syn. Both proteins act as molecular chaperones and control synaptic machinery. Despite such evidence, the role of α-Syn in dystonia has never been investigated.We explored whether α-Syn and N-ethylmaleimide sensitive fusion attachment protein receptor proteins (SNAREs), that are known to be modulated by α-Syn, may be involved in DYT1 dystonia synaptic dysfunction.We used electrophysiological and biochemical techniques to study synaptic alterations in the dorsal striatum of the Tor1aIn the Tor1aOur data demonstrate an unprecedented relationship between TA and α-Syn, and reveal that α-Syn and SNAREs alterations characterize the synaptic dysfunction underlying DYT1 dystonia. © 2022 The Authors. Abstract

Cardiovascular mortality is a major cause of death among in type 2 diabetes (T2DM). Endothelial dysfunction (ED) is a well-known important risk factor for the development of diabetes cardiovascular complications. Therefore, the prevention of diabetic macroangiopathies by preserving endothelial function represents a major therapeutic concern for all National Health Systems. Several complex mechanisms support ED in diabetic patients, frequently cross-talking each other: uncoupling of eNOS with impaired endothelium-dependent vascular response, increased ROS production, mitochondrial dysfunction, activation of polyol pathway, generation of advanced glycation end-products (AGEs), activation of protein kinase C (PKC), endothelial inflammation, endothelial apoptosis and senescence, and dysregulation of microRNAs (miRNAs). Metformin is a milestone in T2DM treatment. To date, according to most recent EASD/ADA guidelines, it still represents the first-choice drug in these patients. Intriguingly, several extraglycemic effects of metformin have been recently observed, among which large preclinical and clinical evidence support metformin’s efficacy against ED in T2DM. Metformin seems effective thanks to its favorable action on all the aforementioned pathophysiological ED mechanisms. AMPK pharmacological activation plays a key role, with metformin inhibiting inflammation and improving ED. Therefore, aim of this review is to assess metformin’s beneficial effects on endothelial dysfunction in T2DM, which could preempt development of atherosclerosis.

Objectives: Occupational medicine is a discipline continually evolving in response to technological advances, changes in workplaces and production processes, emergence of new occupational risks and diseases and modifications in regulatory framework for occupational health and safety. Therefore, the recurrent revaluation of professional activity, information demands and education and training needs of occupational physicians is essential in order to identify methodologies and tools that may contribute to improvement of their professional knowledge and competency. In this regard, we conducted the first large-scale national survey of Italian occupational medicine physicians to define their demographic and professional activity and to assess their information demands, training and updating needs. Material and Methods: A random sample of occupational physicians, listed in the national register of the Italian Ministry of Health, was selected to complete a voluntary survey. Subjects recruited in this study were asked to complete 3 different sections (personal and professional information, training and updating needs, professional activity and practice characteristics) of a questionnaire for a total of 35 questions. Results: Most of participants were specialized in occupational medicine, worked for a large number of companies and carried out health surveillance on a total number of workers that exceeds 1500. Occupational physicians would like to have a higher training offer towards practical aspects of health surveillance, risks assessment, manual handling of loads, chemical substances and upper limb biomechanical overload. Interestingly, statistically significant differences were observed subdividing the sample into different groups according to the legal requirements to perform the professional activity of occupational physicians in Italy or according to particular aspects of their professional activity. Conclusions: This study has provided interesting findings that may help to guide future discussion on alternative and additional instruments and/or methodologies that may be adopted to implement the quality and effectiveness of occupational medicine practice. 249. Pleural mesothelioma: Case-report of uncommon occupational asbestos exposure in a small furniture industry

Abstract

The relationship between asbestos exposure and malignant mesothelioma is no longer disputed, although it is not always easy to trace past occupational exposure. This report describes a case of uncommon asbestos exposure of a small furniture industry worker, who subsequently died of pleural malignant mesothelioma, to stress the crucial importance of a full reconstruction of the occupational history, both for legal and compensation purposes. Sarcomatoid pleural mesothelioma was diagnosed in a 70-year-old man, who was previously employed as a carpenter in a small furniture industry. He worked for about 6 years in the small factory, was exposed to asbestos during the assembly of the furniture inspired by classical architecture, in which asbestos cement tubes were used to reproduce classical columns. During this production process no specific work safety measures were applied, nor masks or local aspirators. No extra-professional exposure to asbestos was identified. This mesothelioma case was investigated by the Public Prosecutor’s assignment that commissioned expert evidence on the legal accountability for the disease. Despite its uncommon expositive circumstance, the length of latency (about 30 years), the duration of exposure, the clinical and histochemical features are all consistent with literature evidence, accounting for the occupational origin of this malignancy.
