Abstract

Background Clostridium difficile infection (CDI) recurs after initial treatment in approximately one in four patients. A single-centre pilot study suggested that this could be reduced using ‘follow-on’ rifaximin treatment. We aimed to assess the efficacy of rifaximin treatment in preventing recurrence. Methods A multisite, parallel group, randomised, placebo controlled trial recruiting patients aged ≥18 years immediately after resolution of CDI through treatment with metronidazole or vancomycin. Participants received either rifaximin 400 mg three times a day for 2 weeks, reduced to 200 mg three times a day for a further 2 weeks or identical placebo. The primary endpoint was recurrence of CDI within 12 weeks of trial entry. Results Between December 2012 and March 2016, 151 participants were randomised to either rifaximin or placebo. Primary outcome data were available on 130. Mean age was 71.9 years (SD 15.3). Recurrence within 12 weeks was 29.5% (18/61) among participants allocated to placebo compared with 15.9% (11/69) among those allocated to rifaximin, a difference between groups of 13.7% (95% CI −28.1% to 0.7%, p=0.06). The risk ratio was 0.54 (95% CI 0.28 to 1.05, p=0.07). During 6-month safety follow-up, nine participants died in each group (12%). Adverse event rates were similar between groups. Conclusion While ‘follow-on’ rifaximin after CDI appeared to halve recurrence rate, we failed to reach our recruitment target in this group of frail elderly patients, so the estimated effect of rifaximin lacks precision. A meta-analysis including a previous trial suggests that rifaximin may be effective; however, further, larger confirmatory studies are needed.

Abstract

Through Pechini method, a single phase shuttle-shaped perovskite oxide was successfully synthesised at 1000 °C. It was combined with active carbon, forming a composite electrode to be used as cathode in a room temperature ammonia fuel cell based on an alkaline membrane electrolyte and Pt/C anode. Reasonable OCV and power density were observed for an ammonia fuel cell using /C composite cathode. Although the power density is not high enough for conventional portable or transport applications, it has the potential for stationary application in removal of ammonia from wastewater because the requirements on power density is relatively low. When a dilute 0.02 M ammonia solution (340 ppm) was used as the fuel, the fuel cell using this perovskite oxide can obtain an open circuit voltage of 0.35 V and a power density of 0.03 mW/cm2. In order to obtain higher OCV, NaOH is necessary to be added in the fuel, especially when the fuel contains a low concentration of ammonia. This study indicates that perovskite oxides are potential good cathode for low temperature direct ammonia or alkaline membrane fuel cells.

The study reports the results of in-depth interviews undertaken in 2003 with 57 International Medical Graduates (IMGs) practicing in rural Victoria with the aim of identifying factors, which facilitate or inhibit their integration into rural communities and consequently affect their intention to stay in rural practice. Recognition of such a topology is useful in assisting to better target support and incentives designed to increase IMG rural retention rates towards those doctors most likely to remain in rural practice on completion of their mandatory period.

Abstract

Aim: Variation in the relationship between variables across space, known as spatial non‐stationarity, is a common phenomenon in ecology. Species distribution models (SDMs), however, typically assume stationarity in species–environment relationships across space. Spatial non‐stationarity may be particularly apparent in populations along range boundaries where species are actively adapting to changes in climate, and geographically isolated areas where isolation can facilitate adaptation. We used data collected from the southern range boundary of a winter‐adapted mammal to evaluate spatially varying environmental relationships in a region with a geographically isolated population. Taxon: Lepus americanus. Location: Laurentian Great Lakes region, USA. Methods: We surveyed 320 sites for snowshoe hares near their regional southern range boundary. We related snowshoe hare occurrence to short‐term climate variables (snow cover duration, maximum temperature and their interaction) with a spatially explicit model that allowed the effect of climate variables on snowshoe hare occurrence to vary spatially. Results: We found highest support for an SDM that incorporated geographic barriers when estimating spatial effects, in addition to spatially varying effects of maximum temperate, snow cover duration and their interaction. Hares were more likely to occupy sites with longer snow cover duration but the influence of temperature was more variable. Increasing temperature in northern portions of our study area had a positive influence on hare occurrence, but exhibited a negative relationship along the southern range boundary. Main Conclusions: We provide a robust evaluation of stationarity assumptions in bioclimatic relationships and highlight spatially variable relationships between a cold‐adapted species and short‐term climate variables across a range boundary. However, spatial non‐stationarity was not most apparent across dispersal barriers but coincided with differences in the relationship between climate variables across space. Our study provides evidence that the distribution of a winter‐adapted species is strongly constrained by snow cover, but prevailing temperature modulates the strength of this relationship across space. 237. Beyond Implementation: The Role of Chinese Board Secretaries in ESG Strategy Escalation.

Abstract

Studies of corporations' environmental, social, and governance (ESG) sustainability efforts typically focus on the central responsibilities of CEOs and boards of directors in strategic decision-making. However, given the unique secretarial political culture in China, we argue that corporate (i.e., board) secretaries also play an important role in ESG strategic decision-making that goes beyond merely the implementation and communication of firms' strategic decisions. Specifically, we posit that when firms face positive market performance, they are more likely to escalate their ESG strategic response (i.e., adopting Global Reporting Initiative standards) based on previously disclosed ESG information. In addition, we postulate that overconfidence, diversified perspectives, and diligence of board secretaries will distort the secretary's understanding of positive market value performance information and ESG decision-making, which in turn will affect the firm's ESG response decision. Accordingly, we examined personal attributes of Chinese A-share listed firms' board secretaries and their role in market capitalization performance feedback and ESG strategic response decisions. We found empirical evidence consistent with our premise that decision implementers can also play an important role in the performance feedback decision-making process. Abstract

A computer-based system has been developed for production of braille music without the use of a skilled braillist. The system Is designed to be used by an operator having no knowledge of braille or computers. Some ability to read printed music Is helpful, but not essential, and no expert musicianship is required. The input to the system is a coded representation of music notation typed on the keyboard of a graphical display unit by an operator reading printed music. Syntax checking is performed by the program during input to help reduce the number of errors made by the operator. The music is displayed graphically on the screen as it is typed, in a form similar to the printed copy, giving he operator immediate feedback and an opportunity to correct any mistakes. The music, once stored, may be edited until correct using the keyboard of the terminal in conjunction with a joystick or light pen. Any part of the stored music may be rapidly accessed and displayed on the screen or a digital plotter for checking purposes. The stored music may be archived temporarily or permanently on magnetic tape for subsequent retrieval, in both print and braille forms. The translation algorithm performs automatic translation to a close approximation to the international braille music code. Although the system is intended primarily to be used without the aid of a braillist, it does allow access to, and editing of, the braille before final output. The output from the system is the braille music notation corresponding to the input, suitable for embossing directly on an braille terminal connected on-line to the computer. The quality of samples of music of various types produced in braille using this system has been evaluated with the assistance of a number of braille music readers. The system is suitable for implementation at a braille printing house, and incorporates a general-purpose editor for music notation which may also be useful for other musicological applications.

Abstract

The purpose of this investigation was to study the relationship between selected pulmonary measurements, anthropometric indices, distance run in sixteen minutes and cardiovascular fitness potential (as measured by maximum oxygen uptake), maximum oxygen debt and certain factors closely associated with these measurements. The study involved eighty-seven male subjects who ranged between twenty and twenty-five years of age. In the 1973 study the sample involved thirty-seven participants from the sports of rugby, hockey, cross country, and soccer. In the 1974 study the sample involved thirty-nine soccer players. In 1975 physiological measurements were taken from eleven soccer players and applied to several of the 1974 regression formulae in order to predict the distance the players could run in sixteen minutes.
