A few previous studies reported varying health impacts of population ageing, but they focused only on limited countries and diseases. We comprehensively quantify the impact of population ageing on mortality for 195 countries/territories and 169 causes of death. Methods and findings Using data from the Global Burden of Disease Study 2017 (GBD 2017), this study derived the total number of deaths and population size for each year from 1990 to 2017. A decomposition method was used to attribute changes in total deaths to population growth, population ageing, and mortality change between 1990 and each subsequent year from 1991 through 2017, for 195 countries/territories and for countries grouped by World Bank economic development level. For countries with increases in deaths related to population ageing, we calculated the ratio of deaths attributed to mortality change to those attributed to population ageing. The proportion of people aged 65 years and older increased globally from 6.1% to 8.8%, and the number of global deaths increased by 9 million, between 1990 and 2017. Compared to 1990, 12 million additional global deaths in 2017 were associated with population ageing, corresponding to 27.9% of total global deaths. Population ageing was associated with increases in deaths in high-, upper-middle-, and lower-middle-income countries but not in low-income countries. The proportions of deaths attributed to population ageing in 195 countries/territories ranged from -43.9% to 117.4% for males and -30.1% to 153.5% for females. The 2 largest contributions of population ageing to disease-specific deaths globally between 1990 and 2017 were for ischemic heart disease (3.2 million) and stroke (2.2 million). Population ageing was related to increases in deaths in 152 countries for males and 159 countries for females, and decreases in deaths in 43 countries for males and 36 countries for females, between 1990 and 2017. The decreases in deaths attributed to mortality change from 1990 to 2017 were more than the increases in deaths related to population ageing for the whole world, as well as in 55.3% (84/152) of countries for males and 47.8% (76/159) of countries for females where population ageing was associated with increased death burden. As the GBD 2017 does not provide variances in the estimated death numbers, we were not able to quantify uncertainty in our attribution estimates. Conclusions In this study, we found that population ageing was associated with substantial changes in numbers of deaths between 1990 and 2017, but the attributed proportion of deaths varied widely across country income levels, countries, and causes of death. Specific preventive and therapeutic techniques should be implemented in different countries and territories to address the growing health needs related to population ageing, especially targeting the diseases associated with the largest increase in number of deaths in the elderly.

- Abstract
Paecilomyces variotii extract “ZhiNengCong” (ZNC) has been confirmed to be a high activity plant immune inducer, but its application effect and appropriate concentration in different crops and different environmental conditions need to be further verified. This study set up six treatments, including three ZNC concentration levels (0, 5, and 20 ng/mL) and two Ralstonia solanacearum (Rs) inoculation levels (with and without), to explore the effect of ZNC application on the growth and disease resistance of peppers. The results showed that ZNC application (particularly 5 ng/mL ZNC) increased the basal biomass of the pepper plant and promoted root growth. 5 ng/mL ZNC can improve photosynthetic performance (net photosynthetic rate, transpiration rate and chlorophyll content averagely increased 20.2%, 33.7% and 16.0%, respectively) and antioxidant enzyme activities (particularly, catalase activity averagely increased 38.9%) of peppers more efficient than 20 ng/mL. Moreover, 5 ng/mL ZNC significantly increased Rs resistance of pepper, while 20 ng/mL ZNC did not show this effect. In conclusion, 5 ng/mL ZNC had the most significant effects on the growth-promoting and disease resistance of peppers, indicating good application prospects in agricultural practice.

- Abstract
Concentration heterogeneity of diffusible reactants is a prevalent phenomenon in biochemical processes, requiring the generation of concentration gradients for the relevant experiments. In this study, we present a high-density pyramid array microfluidic network for the effective and precise generation of multiple concentration gradients. The complex gradient distribution in the 2D array can be adaptively adjusted by modulating the reactant velocities and concentrations at the inlets. In addition, the unique design of each reaction chamber and mixing block in the array ensures uniform concentrations within each chamber during dynamic changes, enabling large-scale reactions with low reactant volumes. Through detailed numerical simulation of mass transport within the complex microchannel networks, the proposed method allows researchers to determine the desired number of reaction chambers within a given concentration range based on experimental requirements and to quickly obtain the operating conditions with the help of machine learning-based prediction. The effectiveness in generating a multiple concentration gradient environment was further demonstrated by concentration-dependent calcium carbonate crystallization experiments. This device provides a highly efficient mixing and adaptable concentration platform that is well suited for high-throughput and multiplexed reactions.

IntroductionThe COVID-19 pandemic significantly changed society. We aimed to examine the systematic impact of the COVID-19 on injury burden in the United States.MethodsWe extracted mortality and morbidity data from CDC WONDER and WISQARS. We estimated age-standardized injury mortality rate ratio and morbidity rate ratio (MtRR and MbRR) with 95% confidence interval (95% CI) for all injuries, all unintentional injuries, homicide/assault by all methods, suicide/self-harm by all methods, as well as other 11 specific unintentional or intentional injury categories. Injury rate ratios were compared for 2020 vs. 2019 to those of 2019 vs. 2018 to demonstrate the influence of the COVID-19 pandemic on fatal and nonfatal injury burden. The ratio of MtRRs (RMtRR) and the ratio of MbRRs (RMbRR) with 95% CI between 2020 vs. 2019 and 2019 vs. 2018 were calculated separately.ResultsThe COVID-19 pandemic was associated with an increase in injury mortality (RMtRR = 1.12, 95% CI: 1.11, 1.13) but injury morbidity decreased (RMbRR = 0.88, 95% CI: 0.88, 0.89) when the changes of these rates from 2019 to 2020 were compared to those from 2018 to 2019. Mortality disparities between the two time periods were primarily driven by greater mortality during the COVID-influenced 2020 vs. 2019 from road traffic crashes (particularly motorcyclist mortality), drug poisoning, and homicide by firearm. Similar patterns were not present from 2019 vs. 2018. There were morbidity reductions from road traffic crashes (particularly occupant and pedestrian morbidity from motor vehicle crashes), unintentional falls, and self-harm by suffocation from 2019 to 2020 compared to the previous period. Change patterns in sexes and age groups were generally similar, but exceptions were observed for some injury types.ConclusionsThe COVID-19 pandemic significantly changed specific injury burden in the United States. Some discrepancies also existed across sex and age groups, meriting attention of injury researchers and policymakers to tailor injury prevention strategies to particular populations and the environmental contexts citizens face.

- Abstract
Context: Urban-rural disparity is an important issue for injury control in China. Details of the urban-rural disparities in fatal injuries have not been analyzed. Purpose: To target key injury causes that most contribute to the urban-rural disparity, we decomposed total urban-rural differences in 2006 injury mortality by gender, age, and cause. Methods: Mortality data came from the Chinese Vital Registration data, covering a sample of about 10% of the total population. The chi-square test was used to test the significance of urban-rural disparities. Findings: For all ages combined, the injury death rate for males was 60.1/100,000 in rural areas compared with 40.9 in urban areas; for females, the respective rates were 31.5 and 23.6/100,000. The greatest disparity was at age less than 1 year for both sexes, where the rate from unintentional suffocation in rural areas was more than twice the urban rate. The higher mortality from drowning among males of all ages and among females ages 1-24 and 35+ contributed substantially to the age-specific urban-rural disparities. For both sexes, transportation incidents and suicide were the most important contributors to higher rates among rural residents ages 15+. Conclusions: Unintentional suffocation, drowning, transportation incidents, and suicide not only are the major causes of injury death, but also play a key role in explaining the urban-rural disparities in fatal injuries. Further research is needed to identify factors leading to higher rural death rates and to explore economical and feasible interventions for reducing injuries and narrowing the urban-rural gap in injury mortality.

241. Brief Report: Development of a Brief Scale to Assess Family Support Function among Chinese High School Students

- Abstract
Purpose: To develop a brief family support function scale for Chinese high school students (FSFS-CHSS). Methods: Standard procedures for scale development (identification of conceptual model, literature review, item selection and modification using consumer and expert panels and item reduction techniques) were used to develop a 17-item scale. Families of students in 96 high school classes in Hunan were used for item selection and validation. A subset of 4 out of 96 classes was selected as a test-retest sample. Results: A brief CHSS-FSFS with 17 items was found to have good test-retest and internal consistency reliability (Pearson correlation coefficient: 0.89, P less than 0.05; Cronbach's alpha coefficient was 0.87). Three factors were extracted using factor analysis, accounting for 48% of total variation. Comparison of CHSS-FSFS score between healthy and unhealthy families was statistically significant, t=7.58, P less than 0.01. Conclusions: CHSS-FSFS is a reliable and valid tool for family support function assessment.

Introduction Child injuries are a public health problem in China. In 2010 --according to Global Burden of Disease estimates--approximately 86000 individuals aged 0-19 years suffered fatal injuries in China. (1) [...], Objective To examine the extent to which effective interventions to prevent unintentional child injury are reflected in the laws and regulations of China. Methods We focused on the six common causes of fatal child injuries--drowning, road traffic injury, falls, poisoning, burns and suffocation. We investigated 27 interventions recommended by the United Nations Children's Fund, the World Health Organization or the European Child Safety Alliance. We searched China National Knowledge Infrastructure and Lawyee for Chinese legislations using keywords and synonyms for the 27 interventions. We reviewed the identified legislations for statements specifying the responsible implementation department. Findings Seven national laws, nine regulations of the State Council and 46 departmental regulations were found to relate to at least one of the interventions. Although seven of the 27 internationally recommended interventions were covered by Chinese laws, 10 were not covered by any current Chinese law or regulation. None of the interventions against drowning and falls that we investigated was covered by national laws. The implementation responsibilities for effective interventions were either not specified or were assigned to multiple governmental departments in 11 or 20 legislative documents, respectively. Conclusion In Chinese laws and regulations, interventions proven to prevent major causes of unintentional child injuries are underrepresented and the associated implementation responsibilities are often poorly defined. China should include all such interventions in laws and regulations, and assign implementation responsibility for each to a single department of the national government. A continuacion, se realizaron busquedas de legislaciones de China en las bases de datos China National Knowledge Infrastructure y Lawyee usando palabras claves y sinonimos de las 27 intervenciones. Por ultimo, se revisaron las legislaciones identificadas a fin de buscar declaraciones que especificaran el departamento responsable de la aplicacion. Resultados Se hallo que siete leyes nacionales, nueve normativas del Consejo de Estado y 46 normativas departamentales estaban relacionadas con al menos una de las intervenciones. Aunque las leyes de China abarcaban siete de las 27 intervenciones que se recomiendan a nivel internacional, diez de ellas no estaban incluidas en ninguna normativa o ley actual de China. Las leyes nacionales no abarcaban ninguna de las intervenciones contra ahogamientos y caidas investigadas. Las responsabilidades de aplicacion de intervenciones efectivas no estaban especificadas o estaban asignadas a varios departamentos gubernamentales en 11 o 20 documentos legislativos, respectivamente. Conclusion En las leyes y normativas de China, las intervenciones que han demostrado la prevencion de lesiones infantiles no intencionadas estan subrepresentadas y las responsabilidades de aplicacion asociadas a menudo tienen una definicion vaga. China debe incluir todas estas intervenciones en las leyes y normativas, asi como asignar la responsabilidad de aplicacion de cada una a un unico departamento del gobierno nacional.
