- Abstract
The evolution of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus leads to new variants that warrant timely epidemiological characterization. Here we use the dense genomic surveillance data generated by the COVID-19 Genomics UK Consortium to reconstruct the dynamics of 71 different lineages in each of 315 English local authorities between September 2020 and June 2021. This analysis reveals a series of subepidemics that peaked in early autumn 2020, followed by a jump in transmissibility of the B.1.1.7/Alpha lineage. The Alpha variant grew when other lineages declined during the second national lockdown and regionally tiered restrictions between November and December 2020. A third more stringent national lockdown suppressed the Alpha variant and eliminated nearly all other lineages in early 2021. Yet a series of variants (most of which contained the spike E484K mutation) defied these trends and persisted at moderately increasing proportions. However, by accounting for sustained introductions, we found that the transmissibility of these variants is unlikely to have exceeded the transmissibility of the Alpha variant. Finally, B.1.617.2/Delta was repeatedly introduced in England and grew rapidly in early summer 2021, constituting approximately 98% of sampled SARS-CoV-2 genomes on 26 June 2021.

- Abstract
The SARS-CoV-2 lineage B.1.1.7, designated variant of concern (VOC) 202012/01 by Public Health England1, was first identified in the UK in late summer to early autumn 20202. Whole-genome SARS-CoV-2 sequence data collected from community-based diagnostic testing for COVID-19 show an extremely rapid expansion of the B.1.1.7 lineage during autumn 2020, suggesting that it has a selective advantage. Here we show that changes in VOC frequency inferred from genetic data correspond closely to changes inferred by S gene target failures (SGTF) in community-based diagnostic PCR testing. Analysis of trends in SGTF and non-SGTF case numbers in local areas across England shows that B.1.1.7 has higher transmissibility than non-VOC lineages, even if it has a different latent period or generation time. The SGTF data indicate a transient shift in the age composition of reported cases, with cases of B.1.1.7 including a larger share of under 20-year-olds than non-VOC cases. We estimated time-varying reproduction numbers for B.1.1.7 and co-circulating lineages using SGTF and genomic data. The best-supported models did not indicate a substantial difference in VOC transmissibility among different age groups, but all analyses agreed that B.1.1.7 has a substantial transmission advantage over other lineages, with a 50% to 100% higher reproduction number.

Transmission of SARS-CoV-2 is uncontrolled in many parts of the world; control is compounded in some areas by the higher transmission potential of the B.1.1.7 variant1, which has now been reported in 94 countries. It is unclear whether the response of the virus to vaccines against SARS-CoV-2 on the basis of the prototypic strain will be affected by the mutations found in B.1.1.7. Here we assess the immune responses of individuals after vaccination with the mRNA-based vaccine BNT162b22. We measured neutralizing antibody responses after the first and second immunizations using pseudoviruses that expressed the wild-type spike protein or a mutated spike protein that contained the eight amino acid changes found in the B.1.1.7 variant. The sera from individuals who received the vaccine exhibited a broad range of neutralizing titres against the wild-type pseudoviruses that were modestly reduced against the B.1.1.7 variant. This reduction was also evident in sera from some patients who had recovered from COVID-19. Decreased neutralization of the B.1.1.7 variant was also observed for monoclonal antibodies that target the N-terminal domain (9 out of 10) and the receptor-binding motif (5 out of 31), but not for monoclonal antibodies that recognize the receptor-binding domain that bind outside the receptor-binding motif. Introduction of the mutation that encodes the E484K substitution in the B.1.1.7 background to reflect a newly emerged variant of concern (VOC 202102/02) led to a more-substantial loss of neutralizing activity by vaccine-elicited antibodies and monoclonal antibodies (19 out of 31) compared with the loss of neutralizing activity conferred by the mutations in B.1.1.7 alone. The emergence of the E484K substitution in a B.1.1.7 background represents a threat to the efficacy of the BNT162b2 vaccine.

- Abstract
The SARS-CoV-2 lineage B.1.1.7, now designated Variant of Concern 202012/01 (VOC) by Public Health England, originated in the UK in late Summer to early Autumn 2020. We examine epidemiological evidence for this VOC having a transmission advantage from several perspectives. First, whole genome sequence data collected from community-based diagnostic testing provides an indication of changing prevalence of different genetic variants through time. Phylodynamic modelling additionally indicates that genetic diversity of this lineage has changed in a manner consistent with exponential growth. Second, we find that changes in VOC frequency inferred from genetic data correspond closely to changes inferred by S-gene target failures (SGTF) in community-based diagnostic PCR testing. Third, we examine growth trends in SGTF and non-SGTF case numbers at local area level across England, and show that the VOC has higher transmissibility than non-VOC lineages, even if the VOC has a different latent period or generation time. Available SGTF data indicate a shift in the age composition of reported cases, with a larger share of under 20 year olds among reported VOC than non-VOC cases. Fourth, we assess the association of VOC frequency with independent estimates of the overall SARS-CoV-2 reproduction number through time. Finally, we fit a semi-mechanistic model directly to local VOC and non-VOC case incidence to estimate the reproduction numbers over time for each. There is a consensus among all analyses that the VOC has a substantial transmission advantage, with the estimated difference in reproduction numbers between VOC and non-VOC ranging between 0.4 and 0.7, and the ratio of reproduction numbers varying between 1.4 and 1.8. - Abstract
Late in 2020, two genetically-distinct clusters of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) with mutations of biological concern were reported, one in the United Kingdom and one in South Africa. Using a combination of data from routine surveillance, genomic sequencing and international travel we track the international dispersal of lineages B.1.1.7 and B.1.351 (variant 501Y-V2). We account for potential biases in genomic surveillance efforts by including passenger volumes from location of where the lineage was first reported, London and South Africa respectively. Using the software tool grinch (global report investigating novel coronavirus haplotypes), we track the international spread of lineages of concern with automated daily reports, Further, we have built a custom tracking website (cov-lineages.org/global_report.html) which hosts this daily report and will continue to include novel SARS-CoV-2 lineages of concern as they are detected.

We present an advanced geometric multigrid strategy for finite-element (FE) meshes of nonuniform refinement levels. The proposed method exploits the fact that levels of high resolution usually extend over small subdomains only. By restricting the smoothing operations at the finer levels to the corresponding partial meshes, both memory consumption and operation count are kept at a minimum. Our computer implementation is based on a hierarchy of perfectly nested FE spaces and employs hanging variables to interface between regions of unequal refinement levels. To validate the effectiveness of the suggested approach, several numerical examples are given. Our Galaxy is known to contain a central boxy/peanut-shaped bulge, yet the importance of a classical, pressure-supported component within the central part of the Milky Way is still being debated. It should be most visible at low metallicity, a regime that has not yet been studied in detail. Using metallicity-sensitive narrow-band photometry, the Pristine Inner Galaxy Survey (PIGS) has collected a large sample of metal-poor ([Fe/H] < -1.0) stars in the inner Galaxy to address this open question. We use PIGS to trace the metal-poor inner Galaxy kinematics as function of metallicity for the first time. We find that the rotational signal decreases with decreasing [Fe/H], until it becomes negligible for the most metal-poor stars. Additionally, the velocity dispersion increases with decreasing metallicity for -3.0 < [Fe/II] < -0.5, with a gradient of -44 +/- 41un s(-1)dex(-1). These observations may signal a transition between Galactic components of different metallicities and kinematics, a different mapping on to the boxy/peanut-shaped bulge for former disc stars of different metallicities and/or the secular dynamical and gravitational influence of the bar on the pressure-supported component. Our results provide strong constraints on models that attempt to explain the properties of the inner Galaxy.

- Abstract
Background: Hospital-associated venous thromboembolism (HA-VTE) morbidity and mortality has potentially been underestimated to date. Our centre is a tertiary, multi-campus health service which manages 265,000 admissions a year. Our VTE committee, comprising haematologists and other physicians, quality service representatives and pharmacists, routinely audits inpatient VTE (IP-VTE) for potential preventability against VTE prophylaxis guidelines. Over the study period, 81 IP-VTE were reviewed with 23% deemed potentially preventable however post-discharge VTE (PD-VTE) is not routinely captured. This is where VTE, presumably because of the index admission, is diagnosed after discharge, leading to re-admission. Aim(s): To compare the incidence of PD-VTE against IP-VTE and review PD-VTE characteristics, adherence to VTE prophylaxis guidelines and potential preventability. Method(s): A retrospective audit of ICD-10 Australian Modification discharge coding for lower limb deep vein thrombosis (LLDVT) and pulmonary embolism (PE) in adults between January 2018 and May 2019 at Monash Health. Cases were filtered for criteria such as presence of new, objectively confirmed, symptomatic VTE within 90 days of an index admission greater than 48 hours (Figure 1). Our VTE committee reviewed all eligible cases. Result(s): 948 encounters had VTE discharge coding. Of these, 891 were excluded with 57 eligible patients (Figure 1 and Table 1). Median age was 71 years (range, 26 to 92) and 32% with active cancer. Most index admissions were medical (61%) with a median of 19 days between admissions (range 2-86). VTE prophylaxis during the index admission was concordant with guidelines in 51%. Non-adherence included delayed prescription (5%), interruptions (23%), none (14%) and inadequate prescriptions (e.g. inappropriately dose-reduced enoxaparin 7%). Overall, VTE prophylaxis adherence was 68% with 35% VTE events deemed potentially preventable. There were no deaths from PD-VTE. Conclusion(s): O
