226. Oxygen availability during orthotopic liver transplantation

Abstract

Studies have stressed the role of adequate tissue oxygenation in the light of an optimal patient outcome and allograft viability in liver transplantation. The practice of monitoring conventional hemodynamic parameters during liver transplantation could be complemented by parameters assessing real oxygen availability. In the present prospective study, real arterial available oxygen content (CavlO(2)) and its extraction ratio (O(2)ERavl) were calculated. These parameters include the effect of changes in oxyhemoglobin dissociation curve (ODC; expressed by P-50) on oxygen availability, under the different circumstances occurring during liver transplantation. Sixteen adult cirrhotic patients were studied during orthotopic liver transplantation with the use of venovenous bypass. Classic hemodynamic measurements using a Swan-Ganz thermodilution catheter and arterial and mixed venous blood gas analysis were performed, and P-50, oxygen delivery index (DO2 (ind)), oxygen consumption index (VO2ind), oxygen extraction ratio (O2ER), CavlO(2), and O(2)ERavl were calculated. Statistical analysis was performed using ANOVA for repeated measures and Spearman correlation coefficient matrix among the six variables (DO2 (ind), VO2 (ind), O2ER, P-50, CaV1O(2), and O(2)ERavl) taken two at a time at every phase. Parameter P-50 changed from 25.98 +/- 1.10 to 23.15 +/- 2.24 (at the end of operation). A leftward shift of the ODC was observed. The results showed positive association between P-50 and CavlO(2) after the removal of the native liver, and a weak and inconsistent relation of DO2 i.d with any of the other study variables. The intraoperative changes in P-50 values, which represent a shift of the ODC to the left, may reflect a more accurate estimation of O-2 release to the tissues, than the hemoglobin, Pao(2) and SaO(2) alone. Besides conventional hemodynamic Parameters, P-50 which includes the effect of alterations in ODC on oxygen availability, could be of value in monitoring the systemic oxygenation during liver transplantation.

Abstract

The majority of transplant centers around the world face an ethical debate whether to retransplant a young non-adherent patient. Non-adherence to lifelong immunosuppressants presents a significant risk for graft loss, yet rates remain consistently high. Despite a number of these patients presenting for retransplantation, there is little evidence to guide professionals in their decision-making. This paper aims to provide such guidance, by systematically reviewing the existing outcome data for retransplantation in patients who are known to be non-adherent to their immunosuppressants. This review searched for original papers that addressed retransplantation of a solid organ and included quantitative data on adherence or graft function. Only one original research paper was found to meet the inclusion criteria. This paper is reviewed, and details of the protocol to determine eligibility for retransplantation are summarized. The findings are discussed within the ethical context that transplant professionals work within, and the arguments for and against retransplantation are considered. The need for effective integration of adherence management into routine practice is highlighted, with an emphasis on reliable measurement of adherence throughout the patient's life. Examples of good practice are discussed, favoring prevention over cure.

247. Neonatal hemochromatosis - Medical treatment vs. transplantation: The King's experience

Abstract

The aim of our study was to compare the outcome of medical treatment vs. liver transplantation in infants with neonatal hemochromatosis (NH) referred to King's College Hospital from 1990-2002. We conducted a retrospective review of 19 children from 14 families. Fifteen children presented at birth and 4 during the first week of life. One child was diagnosed by cordocentesis at 30 weeks of gestation. NH recurred in 7 of 9 families with further children. In one family, 2 children from different fathers were affected. All patients had elevated ferritin levels, hypoalbuminemia, and coagulopathy. Liver histology showed parenchymal collapse, diffuse fibrosis, and moderate to severe hepatocyte hemosiderin deposition. Extrahepatic siderosis was demonstrated by magnetic resonance in 2 patients, lip biopsy in 3, and autopsy in 10. Ten patients received a chelation-antioxidant cocktail: 1 survived, 4 died, and 5 required liver transplantation, of whom 2 died. One of the 9 infants who did not receive the cocktail survived with medical support, 3 died, and 5 required transplantation, of whom 3 died. Seven children are alive, 5 after transplantation, at a median follow-up of 5.6 years, with excellent quality of life and no recurrence of the disease. In conclusion, chelation-antioxidant treatment does not appear to modify the prognosis of NH, at least in severe cases. Liver transplantation, with 50% long-term survival, remains the treatment of choice and should be promptly offered to those infants who do not improve with supportive medical treatment.
