One or two phenylacetylide (PA) ligand(s) were successfully removed from the IrAu12 superatomic core of [IrAu12(dppe)5(PA)2]+ (dppe=1,2‐bis(diphenylphosphino)ethane) by reaction with controlled amounts of tetrafluoroboric acid. Optical and nuclear magnetic resonance spectroscopies and density functional theory calculations revealed the formation of open Au site(s) on the IrAu12 core of [IrAu12(dppe)5(PA)1]2+ and [IrAu12(dppe)5]3+ with the remaining structure intact. Isocyanide was efficiently trapped at the open electrophilic site on [IrAu12(dppe)5(PA)1]2+, whereas a dimer or trimer of the IrAu12 superatoms was formed using diisocyanide as a linker. Background. The practice of leaving the abdomen open after an emergency laparotomy has gained increasing popularity recently. Negative pressure wound therapy (NPWT) is known as an effective technique in the management of an open abdomen (OA). A new device, the ABThera™ Open Abdomen Negative Pressure Therapy System (KCI USA, San Antonio, TX, USA), was specifically designed to achieve a temporary abdominal closure (TAC) in the management of an OA. This study was aimed at presenting a successful experience of treating a case of abdominal wall necrosis caused by a perforated ascending colon using the ABThera System. Case Presentation. A 66-year-old man was admitted to our hospital with complaints of severe pain in the abdomen. On admission, abdominal contrast-enhanced computed tomography (CT) showed fluid collection, an air pocket in the subcutaneous fat layer of the abdominal wall, and edematous changes in the adipose tissue in the peritoneum and abdominal wall. Based on a diagnosis of peritonitis resulting from a perforated ascending colon, emergency surgery was performed. A right hemicolectomy, ileostomy construction, and debridement of the necrotic tissues were performed. However, necrotizing fasciitis rapidly spread; therefore, more necrotic tissue was debrided in a second operation. The abdominal wall defect was left open, and the ABThera System was used in the management of the OA; this device promoted wound healing. A reduction was observed in the size of the open wound with visible granulation tissue. The defect was finally covered with a mesh split-thickness skin graft and anterolateral thigh flap. Conclusions. In the management of a case of a massive wound with infection, it can be of great benefit to treat the wound with NPWT initially to decrease its size. The ABThera System could facilitate early and safe management of an OA by surgeons.

Some taxa of mammals live in water, all of which evolved from land-dwelling ancestors. In the family Mustelidae (Mammalia: Carnivora), most species live on land, while otters, comprising the subfamily Lutrinae, inhabit aquatic environments, which include the almost exclusively aquatic sea otters (Enhydra lutris). Thus, the transition from a terrestrial to an aquatic lifestyle has occurred within this family. Despite potentially different selection pressures on body size in aquatic and terrestrial habitats, no divergence in the evolutionary pattern of body size between otters and other mustelids has previously been shown using models of trait evolution on a phylogeny. We applied models that explicitly incorporated lineage-specific directional selection to the evolution of body mass in living mustelids. Using a simulation-based likelihood and approximate Bayesian computation approach, we demonstrated lineage-specific directional selection for larger body mass in otters, which is distinct from other mustelids. There was no evidence of a difference between sea otters and other otters in the strength of directional selection for larger body mass. Additionally, our analyses supported no difference in the rate at which body mass evolves in both directions between otters and other mustelids. These findings suggest that the evolution of body mass in otters is associated with selective advantages of larger size rather than the relaxation of constraints on body size in aquatic habitats, like other aquatic mammals such as sirenians, cetaceans, and pinnipeds. This study compared the effectiveness and safety of 120-degree (nasal) and 240-degree (bilateral) incisions in Tanito Microhook Trabeculotomy (TMH) combined with cataract surgery in patients with open-angle glaucoma. From a pool of 185 eyes, 67 eyes from 67 subjects were selected for each incision group using propensity score matching to align age, sex, glaucoma type, and preoperative intraocular pressure (IOP). The study found that preoperative IOP, initially 18.6 mmHg in both groups, decreased to 13.2 mmHg in the nasal group and 12.8 mmHg in the bilateral group 12 months postoperatively, representing reductions of 29% and 31%, respectively. Similarly, medication scores decreased from 3.4 to 2.7 in the nasal group and from 3.1 to 2.5 in the bilateral group. Notably, the bilateral incision group exhibited a significantly higher hyphema red blood cell score compared to the nasal group (p < 0.0001). Across the study period, other parameters such as IOP, medication score, visual acuity, anterior chamber flare, corneal endothelial cell density, visual field mean deviation, and the frequency of surgical complications other than hyphema were similar between the groups. The study concluded that TMH combined with cataract surgery is equally effective and safe regardless of incision width, although narrower incisions resulted in reduced early postoperative hyphema.
