PURPOSE: The purpose of this study was to evaluate the scar pattern of ACL due to reconstruction and to compare anterior laxity with each scar type. METHODS: A total of 77 patients who had undergone ACL reconstruction were collected. They consisted of 47 males and 30 females, and the average age was 31.4 years. We investigated the course from injury to operation, the experience of giving way, and the present of meniscus tearing. Anterior laxity was also measured for anterior-to-posterior drawer distance compared to the normal opposite knee using KT-2000 preoperatively. Arthroscopic evaluation of the scar pattern was evaluated using a modified Crain's classification: partial rupture (I), scarring to the roof of the notch (II), scarring to the lateral wall more anterior and distal (III), scarring to the posterior cruciate ligament (IV), and resorption (IV). RESULTS: There were 15 patients in group I, 9 group II, 8 group III, 12 in group IV, and 25 in group V. The mean value of KT-2000 was 2.82 mm in group I, 3.05 mm in group II, 4.56 mm in group III, 4.13 mm in group IV, and 5.59 mm in group V. The value of KT-2000 in-group V was significantly greater than in groups I and II (P<0.01). The course from injury to operation, experience of giving way and presence of meniscus tearing were not correlated to the scar pattern. [ABSTRACT FROM AUTHOR]

Developments of the large negative ion source have been progressed in the high-energy, high-power, and long-pulse neutral beam injector for JT-60 Super Advanced. Countermeasures have been studied and tested for critical issues of grid heat load and voltage holding capability. As for the heat load of the acceleration grids, direct interception of D- ions was reduced by adjusting the beamlet steering. As a result, the heat load was reduced below an allowable level for long-pulse injections. As for the voltage holding capability, local electric field was mitigated by tuning gap lengths between large-area acceleration grids in the accelerator. As a result, the voltage holding capability was improved up to the rated value of 500 kV. Background: Miriplatin (formerly SM-11355), a novel lipophilic platinum complex developed to treat hepatocellular carcinoma, is administered into the hepatic artery using an oily lymphographic agent (Lipiodol Ultra-Fluide) as a carrier. We clarified the usefulness of miriplatin as an agent for transarterial chemoembolization.Methods: Platinum compounds released from miriplatin into serum, medium and Earle's balanced salt solution were examined. Then, miriplatin and cisplatin were administered to rats bearing hepatoma AH109A tumors in livers. Platinum concentrations in tissues and DNA were assessed.Results: Miriplatin showed a more sustained release than cisplatin. Dichloro[(1R, 2R)-1, 2-cyclohexane diamine-N, N']platinum, the most abundant platinum compound released from miriplatin, was as effective as cisplatin in inhibiting the growth of cells. Miriplatin was selectively disposed of in tumors, maintained in tumors longer than cisplatin and caused apparent tumor regression inducing platinum-DNA adducts to form and massive apoptosis.Conclusion: Miriplatin appears to be a suitable chemotherapeutic agent for transarterial chemoembolization. The first long pulse production of high power D- ion beams has been demonstrated in the JT-60 U negative ion sources, each of which was designed to produce 22 A, 500 keV D- ion beams. Voltage holding capability and the grid power loading were examined for long pulse production of high power D- ion beams. From the correlation between voltage holding and the light intensity of cathodoluminescence from the Fiber Reinforced Plastic insulators, the acceleration voltage for stable voltage holding capability was found to be less than 320–340 kV where the light was sufficiently suppressed. By tuning the extraction voltage, the grid power loadings in the ion sources were decreased to the allowable levels for long pulse injection without a significant reduction of the beam power. After tuning the acceleration and extraction voltages, D- ion beams of 12.5 and 9.8 A were produced at 340 keV with cesium seeding at a rate of ∼14 μg/s into the ion sources. The pulse duration of these D- ion beams was extended step by step, and then was successfully extended up to 18 s without degradation of the negative ion production. The D- ion beams were neutralized to yield 3.6 MW D0 beams by a gas cell, and then injected into the JT-60 U plasma. Further, a slight reduction of D- ion beam power allowed the longer injection duration of 21 s at a D0 beam power of 3.2 MW. The success in the long pulse production of a high power D- ion beam shows that negative ion beams can be produced during a few tens of seconds without degradations of negative ion production and the voltage holding in a large Cs-seeded negative ion source. Based on previous studies on the spatial uniformity of the negative ion beam, the external magnetic filter was replaced to a novel tent-shaped magnetic filter in the JAEA 10 A negative ion source. The line-cusp field configuration on the source chamber was also changed to form a symmetric magnetic field like many of positive ion sources aiming at high proton yield. This magnetic field configuration allows fast electrons emitted from filament cathodes to rotate azimuthally inside the source chamber. The source configuration thus prevents localization of fast electrons due to their B×grad B drift in the filter field. As a result, the H- ion beam profile extracted from a wide region of 340×170 mm2 showed reduction of standard deviation from 16% in the original to 7.9% with the tent filter. The negative ion source with the tent filter satisfied the requirement of the beam uniformity for a large negative ion source in the ITER neutral beam injection. [ABSTRACT FROM AUTHOR]

For the ITER neutral beam system, reasearch and development of a vacuum-insulated MeV accelerator has been carried out at Japan Atomic Energy Research Institute. After a successful 1 MV insulation in vacuum for more than 2 h, H--ion-beam acceleration test is in progress to fulfill the ITER requirement of the current density of 200 A/m2 at 1 MeV, with good beam optics quality of <7 mrad divergence. For such a high-current-density H--ion-beam acceleration, the Kamaboko negative-ion source was operated at high input power, with increased filament number and magnetic filter strength. The optimum perveance was investigated by scanning the extraction and acceleration voltages, together with the beam divergence measurement. As a consequence, H--ion beams of 146 A/m2 (total ion-beam current: 206 mA) were obtained at 836 keV, of which power density is in the “ITER relevant” level. Such high-power-density beams with a divergence as low as 5 mrad were obtained at the optimum perveance corresponding to that of the ITER (200 A/m2 at 1 MeV). [ABSTRACT FROM AUTHOR]

Abstract: Requirement and technical issues of the neutral beam (NB) system are discussed for a fusion DEMO plant. The NB system for DEMO should be characterized by high efficiency, high energy, high reliability in year-long steady operation, and low maintenance frequency. From the viewpoint of high efficiency, use of conventional electrostatic accelerator is realistic. Due to operation under radiation environment, vacuum insulation is essential in the accelerator. According to the insulation design guideline, it was clarified that a beam energy of 1.5–2MeV is possible in the accelerator. Development of filamentless, and cesium free ion source is required to achieve low maintenance frequency, based on the existing high current/high current density negative ion production technology. The gas neutralization is not applicable due to its low efficiency (60%). R&D on an advanced neutralization scheme such as plasma neutralization (efficiency: >80%) is required. Recently, development of cw high power semiconductor laser is in progress. The paper shows a conceptual design of a high efficiency laser neutralizer utilizing the new semiconductor laser array. Abstract: The origin of the beam non-uniformity, that is one of the key issues in JT-60 negative ion sources, was experimentally examined by measuring correlations between the intensity of the H− ion beam and plasma parameters, such as an electron temperature and plasma density in the JAERI 10 A negative ion source. The ion source was operated without cesium. Although a uniform filter field was applied, the electron temperature near plasma grid (PG) was higher than 1eV in the upper half region of the longitudinal direction, where the spatial beam intensity decreased rapidly. This indicates that the beam non-uniformity is caused by a higher electron temperature than 1eV, namely, by a leakage of the high-energy electrons toward the PG. To investigate the path of high-energy electrons leaking into the PG, a metal plate of 50mm×50mm was placed in the plasma. By placing the plate in a weak magnetic field of <30G leading to a driver region near the upper wall, the electron temperature near the PG was sufficiently reduced to 1eV in the upper half region, where the beam intensity was simultaneously increased. As a result, a 20% gain of the beam current was obtained. Further, the source of the high-energy electrons leaking toward the PG was examined by changing the relative filament position to the upper wall. It is found that the filaments closest to the upper wall, i.e., at 80mm from the upper wall is one of the sources of the high-energy electrons leaking into the PG. [Copyright &y& Elsevier]

High energy accelerator and high current ion source development have been carried out at JAERI for negative-ion based neutral beam (N-NB) systems of JT-60U and ITER. One of R&D issues on the ITER prototype 5-staged electrostatic accelerator was voltage holding capability of the accelerator insulator column. By installing a stress ring, which reduces electric field concentration at the triple junction (interface of metal flange, insulator, and vacuum), 300 kV is held at each stage instead of rated voltage of 200 kV. At present the accelerator insulator column sustains 900 kV stably with the rings in all five stages. In the JT-60U N-NB system, a beamlet deflection by distorted electric field was found at the bottom of the extractor. Correcting the distorted field, reduction in beam divergence was confirmed for the overall beam. As the result, the heat load on the NB port limiter of JT-60U, located about 20 m away from the accelerator, was reduced to less than a half of the previous value before the correction. Consequently, we have succeeded in 10 s continuous injection of H0 beam with the NB power of 2.6 MW at 355 keV. [Copyright &y& Elsevier]

A 57-year-old man was referred to our hospital because of acute cardiac failure and acute renal insufficiency. Laboratory data showed elevation of serum immune complex levels and antineutrophil cytoplasmic antibody (ANCA) titers, with cytoplasmic pattern (C-ANCA) on indirect immunofluorescence (IIF), and proteinase 3 specificity (PR3-ANCA) on solid-phase enzyme-linked immunosorbent assay (ELISA). Hemodialysis therapy was initiated, and this relieved the symptoms of cardiac failure. Echocardiography revealed three-grade aortic insufficiency and two large floating vegetations on the aortic valve. Considering the risk of embolism, we immediately performed aortic valve replacement and surgically removed the vegetations, subsequently giving antibiotic therapy. Six weeks after the operation, the patient's renal function showed marked improvement and the serological abnormalities, except for ANCA titers, had normalized, resulting in no need for dialysis. A renal biopsy specimen revealed diffuse proliferative glomerulonephritis (GN) with crescents including more than 50% of glomeruli, and granular deposits of IgM, C3, and C1q on immunofluorescence. ANCA titers remained high, but the patient's renal function has been stable, indicating a discrepancy between ANCA titers and his clinical course. In this patient, treatment by immediate surgical intervention, performed during the acute phase with active GN and highly reduced renal function, led to dramatic renal recovery. This case suggests that surgical removal of vegetations in the early stage of crescentic GN may result in a good renal outcome in patients with rapidly progressive GN associated with endocarditis. Although it has been suggested that ANCA may have some relationship to GN in endocarditis, in this patient, its pathogenetic significance is questionable. [ABSTRACT FROM AUTHOR]

Symptoms of cholestasis, including epigastralgia, fever, and jaundice, with marked increases in peripheral eosinophils and serum IgE in a 20-year-old man are reported here. Endoscopic retrograde cholangio-pancreatography (ERCP) detected constrictions of the bile ducts, compatible with primary sclerosing cholangitis (PSC). The symptoms and blood parameters of liver dysfunction were associated with the degree of eosinophilia and high serum IgE levels. During corticosteroid therapy, all of these parameters improved, and morphologic improvements of the bile ducts were also observed. The pathogenesis of PSC may be explained, in part, by the concept of hypereosinophilic syndrome or allergic reaction. [ABSTRACT FROM AUTHOR]

High power and long-pulse negative ion extractor, which is composed of the plasma grid (PG) and the extraction grid (EXG), is newly developed toward the neutral beam injector for heating and current drive of future fusion machines such as ITER, JT-60 Super Advanced and DEMO reactor. The PG is designed to enhance surface production of negative ions efficiently by applying the chamfered aperture. The efficiency of the negative ion production for the discharge power increased by a factor of 1.3 against that of the conventional PG. The EXG is also designed with the thermal analysis to upgrade the cooling capability for the long pulse operation of >1000 s required in ITER. Though the magnetic field for electron suppression is reduced to 0.75 of that in the conventional EXG due to this upgrade, it was experimentally confirmed that the extracted electron current can be suppressed to the allowable level for the long pulse operation. In order to realize neutral beam systems in International Thermonuclear Experimental Reactor whose target is to produce a 1 MeV, 200 A/m² during 3600 s D- ion beam, the electrostatic five-stages negative ion accelerator so-called "MeV accelerator" has been developed at Japan Atomic Energy Agency. To extend pulse length, heat load of the acceleration grids was reduced by controlling the ion beam trajectory. Namely, the beam deflection due to the residual magnetic field of filter magnet was suppressed with the newly developed extractor with a 0.5 mm off-set aperture displacement. The new extractor improved the deflection angle from 6 mrad to 1 mrad, resulting in the reduction of direct interception of negative ions from 23% to 15% of the total acceleration power, respectively. As a result, the pulse length of 130 A/m², 881 keV H- ion beam has been successfully extended from a previous value of 0.4 s to 8.7 s. The first neutral beam (NB) injection system of the Korea Superconducting Tokamak Advanced Research (KSTAR) tokamak was partially completed in 2010 with only 1/3 of its full design capability, and NB heating experiments were carried out during the 2010 KSTAR operation campaign. The ion source is composed of a JAEA bucket plasma generator and a KAERI large multi-aperture accelerator assembly, which is designed to deliver a 1.5 MW, NB power of deuterium at 95 keV. Before the beam injection experiments, discharge, and beam extraction characteristics of the ion source were investigated. The ion source has good beam optics in a broad range of beam perveance. The optimum perveance is 1.1-1.3 μP, and the minimum beam divergence angle measured by the Doppler shift spectroscopy is 0.8°. The ion species ratio is D+:D2+:D3+ = 75:20:5 at beam current density of 85 mA/cm2. The arc efficiency is more than 1.0 A/kW. In the 2010 KSTAR campaign, a deuterium NB power of 0.7-1.5 MW was successfully injected into the KSTAR plasma with a beam energy of 70-90 keV. L-H transitions were observed within a wide range of beam powers relative to a threshold value. The edge pedestal formation in the Ti and Te profiles was verified through CES and electron cyclotron emission diagnostics. In every deuterium NB injection, a burst of D-D neutrons was recorded, and increases in the ion temperature and plasma stored energy were found. [ABSTRACT FROM AUTHOR]

Voltage holding test on MeV accelerator indicated that sustainable voltage was a half of that of ideal quasi-Rogowski electrode. It was suggested that the emission of the clumps is enhanced by a local electric field concentration, which leads to discharge initiation at lower voltage. To reduce the electric field concentration in the MeV accelerator, gaps between the grid supports were expanded and curvature radii at the support corners were increased. After the modifications, the accelerator succeeded in sustaining -1 MV in vacuum without beam acceleration. However, the beam energy was still limited at a level of 900 keV with a beam current density of 150 A/m2 (346 mA) where the 3 × 5 apertures were used. Measurement of the beam profile revealed that deflection of the H- ions was large and a part of the H- ions was intercepted at the acceleration grid. This causes high heat load on the grids and the breakdowns during beam acceleration. To suppress the direct interception, new grid system was designed with proper aperture displacement based on a 3D beam trajectory analysis. As the result, the beam deflection was compensated and the voltage holding during the beam acceleration was improved. Beam parameter of the MeV accelerator was increased to 980 keV, 185 A/m2 (427 mA), which is close to the requirement of ITER accelerator (1 MeV, 200 A/m2). [ABSTRACT FROM AUTHOR]
