Abstract

The Global Alliance for Genomics and Health (GA4GH) aims to accelerate biomedical advances by enabling the responsible sharing of clinical and genomic data through both harmonized data aggregation and federated approaches. The decreasing cost of genomic sequencing (along with other genome-wide molecular assays) and increasing evidence of its clinical utility will soon drive the generation of sequence data from tens of millions of humans, with increasing levels of diversity. In this perspective, we present the GA4GH strategies for addressing the major challenges of this data revolution. We describe the GA4GH organization, which is fueled by the development efforts of eight Work Streams and informed by the needs of 24 Driver Projects and other key stakeholders. We present the GA4GH suite of secure, interoperable technical standards and policy frameworks and review the current status of standards, their relevance to key domains of research and clinical care, and future plans of GA4GH. Broad international participation in building, adopting, and deploying GA4GH standards and frameworks will catalyze an unprecedented effort in data sharing that will be critical to advancing genomic medicine and ensuring that all populations can access its benefits.

Abstract

OBJECTIVES: AO Spine REsearch objectives and Common Data Elements for Degenerative Cervical Myelopathy [RECODE-DCM] is a multi-stakeholder consensus process aiming to promote research efficiency in DCM. It aims to establish the top 10 research uncertainties, through a James Lind Alliance Priority Setting Partnership [PSP]. Through a consensus process, research questions are generated and ranked. The inclusion of people with cervical myelopathy [PwCM] is central to the process. We hypothesized that presenting PwCM experience through word cloud generation would stimulate other key stakeholders to generate research questions better aligned with PwCM needs. This protocol outlines our plans to evaluate this as a nested methodological study within our PSP. METHODS: An online poll asked PwCM to submit and vote on words associated with aspects of DCM. After review, a refined word list was re-polled for voting and word submission. Word clouds were generated and an implementation plan for AO Spine RECODE-DCM PSP surveys was subsequently developed. RESULTS: Seventy-nine terms were submitted after the first poll. Eighty-seven refined words were then re-polled (which added a further 39 words). Four word clouds were generated under the categories of diagnosis, management, long-term effects, and other. A 1:1 block randomization protocol to assess word cloud impact on the number and relevance of PSP research questions was generated. CONCLUSIONS: We have shown it is feasible to work with PwCM to generate a tool for the AO Spine RECODE-DCM nested methodological study. Once the survey stage is completed, we will be able to evaluate the impact of the word clouds. Further research will be needed to assess the value of any impact in terms of stimulating a more creative research agenda.

Abstract

Prognostic factors for clinical outcome after spinal cord (SC) injury (SCI) are limited but important in patient management and education. There is a lack of evidence regarding magnetic resonance imaging (MRI) and clinical outcomes in SCI patients. Therefore, we aimed to investigate whether baseline MRI features predicted the clinical course of the disease. This study is an ancillary to the prospective North American Clinical Trials Network (NACTN) registry. Patients were enrolled from 2005-2017. MRI within 72 h of injury and a minimum follow-up of one year were available for 459 patients. Patients with American Spinal Injury Association impairment scale (AIS) E were excluded. Patients were grouped into those with (n = 354) versus without (n = 105) SC signal change on MRI T2-weighted images. Logistic regression analysis adjusted for commonly known a priori confounders (age and baseline AIS). Main outcomes and measures: The primary outcome was any adverse event. Secondary outcomes were AIS at the baseline and final follow-up, length of hospital stay (LOS), and mortality. A regression model adjusted for age and baseline AIS. Patients with intrinsic SC signal change were younger (46.0 (interquartile range (IQR) 29.0 vs. 50.0 (IQR 20.5) years, p = 0.039). There were no significant differences in the other baseline variables, gender, body mass index, comorbidities, and injury location. There were more adverse events in patients with SC signal change (230 (65.0%) vs. 47 (44.8%), p < 0.001; odds ratio (OR) = 2.09 (95% confidence interval (CI) 1.31-3.35), p = 0.002). The most common adverse event was cardiopulmonary (186 (40.5%)). Patients were less likely to be in the AIS D category with SC signal change at baseline (OR = 0.45 (95% CI 0.28-0.72), p = 0.001) and in the AIS D or E category at the final follow-up (OR = 0.36 (95% CI 0.16-0.82), p = 0.015). UDT was designed to effectively utilize the rapidly emerging high-speed wide area optical networks. It is built on top of UDP with reliability control and congestion control, which makes it quite easy to install. The congestion control algorithm is the major internal functionality to enable UDT to effectively utilize high bandwidth. Meanwhile, we also implemented a set of APIs to support easy application implementation, including both reliable data streaming and partial reliable messaging. The original UDT library has also been extended to Composable UDT, which can support various congestion control algorithms. We will describe in detail the design and implementation of UDT, the UDT congestion control algorithm, Composable UDT, and the performance evaluation. Abstract

Surface-station, radiosonde, and Doppler minisodar data from the Cooperative Atmosphere-Surface Exchange Study-1997 (CASES-97) field project, collected in a 60-km-wide array in the lower Walnut River watershed (terrain variation ~150 m) southeast of Wichita, Kansas, are used to study the relationship of the change of the 2-m potential temperature [[THETA].sub.2m] with station elevation [z.sub.e], [differential][[THETA].sub.2m]/[differential][z.sub.e] [equivalent to] [[THETA].sub.,ze] to the ambient wind and thermal stratification [differential][THETA]/[differential]z [equivalent to] [[THETA].sub.,z] during fair-weather nights. As in many previous studies, predawn [[THETA].sub.2m] varies linearly with [z.sub.e], and [[THETA].sub.,ze] ~ [[THETA].sub.,z] over a depth h that represents the maximum elevation range of the stations. Departures from the linear [[THETA].sub.2m]--elevation relationship ([[THETA].sub.,ze] line) are related to vegetation (cool for vegetation, warm for bare ground), local terrain (drainage flows from nearby hills, although a causal relationship is not established), and the formation of a cold pool at lower elevations on some days. The near-surface flow and its evolution are functions of the Froude number Fr = S/(Nh), where S is the mean wind speed from the surface to h, and N is the corresponding Brunt-Vaisala frequency. The near-surface wind is coupled to the ambient flow for Fr = 3.3, based on where the straight line relating [[THETA].sub.,ze] to ln Fr intersects the ln Fr axis. Under these conditions, [[THETA].sub.2m] is constant horizontally even though [[THETA].sub.,z] > 0, suggesting that near-surface air moves up- and downslope dry adiabatically. However, [[THETA].sub.2m] cools (or warms) everywhere at the same rate. The lowest Froude numbers are associated with drainage flows, while intermediate values characterize regimes with intermediate behavior. The evolution of [[THETA].sub.2m] horizontal variability [[sigma].sub.[THETA]] through the night is also a function of the predawn Froude number. For the nights with the lowest Fr, the [[sigma].sub.[THETA]] maximum occurs in the last 1-3 h before sunrise. For nights with Fr ~3.3 [[THETA].sub.,ze] [approximately equal to] 0) and for intermediate values, [[sigma].sub.[THETA]] peaks 2-3 h after sunset. The standard deviations relative to the [[THETA].sub.,ze] line reach their lowest values in the last hours of darkness. Thus, it is not surprising that the relationships of [[THETA].sub.,ze] to Fr and [[THETA].sub.,z] based on data through the night show more scatter, and [[THETA].sub.,ze] ~ 0.5[[THETA].sub.,z] in contrast to the predawn relationship. However, [[THETA].sub.,ze] [approximately equal to] 0 for ln Fr = 3.7, a value similar to that just before sunrise. A heuristic Lagrangian parcel model is used to explain the horizontal uniformity of time-evolving [[THETA].sub.2m] when the surface flow is coupled with the ambient wind, as well as both the linear variation of [[THETA].sub.2m] with elevation and the time required to reach maximum values of [[sigma].sub.[THETA] under drainage-flow conditions.
