Abstract

Examination of the brachial plexus with ultrasound is efficient because it allows many parts of the brachial plexus as well as the surrounding soft tissues to be assessed with high spatial resolution. The key to performing good ultrasound of the brachial plexus is being familiar with the anatomy and the common variants. That makes it possible to concentrate solely on the ultrasound appearances free of simultaneously wondering about the anatomy. Ultrasound of the brachial plexus is particularly good for assessing nerve sheath tumor, perineural fibrosis, metastases, some inflammatory neuropathies, neuralgic amyotrophy, and posttraumatic sequalae. It is limited in the assessment of thoracic outlet syndrome and in the acute/subacute trauma setting. This review addresses the anatomy, ultrasound technique, as well as pathology of the brachial plexus from the cervical foramina to the axilla. [ABSTRACT FROM AUTHOR]

Abstract

Objective This study aims to explore the clinical applicability and relevance of glycosaminoglycan Chemical Exchange Saturation Transfer (gagCEST) for intervertebral disc. Methods 25 subjects ranging in age from 24 yrs to 74 yrs were enrolled. gagCEST was acquired using a single-slice TSE sequence on a 3T. Saturation used a continuous rectangular RF pulse with B 1 =0.8 μT and a fixed duration time =1100 ms. Sagittal image was obtained firstly without saturation pulse, and then saturated images were acquired at 52 offsets ranging from ±0.125 to ±7 parts per million (ppm). MR T2 relaxivity map was acquired at the identical location. Six subjects were scanned twice to assess scan-rescan reproducibility. Results GagCEST intraclass correlation coefficient (ICC) of six subjects was 0.759 for nucleus pulposus (NP) and 0.508 for annulus fibrosus (AF). Bland-Altman plots showed NP had a mean difference of 0.10% (95% limits of agreement: −3.02% to 3.22%); while that of AF was 0.34% (95% limits of agreement: −2.28% to 2.95%). For the 25 subjects, gag CEST in NP decreased as disc degeneration increased, with a similar trend to T2 relaxivity. Gag CEST of AF showed a better correlation with disc degeneration than T2 relaxivity. Conclusion GagCEST in NP and AF decreased as disc degeneration increased, while gagCEST in AF showed a better correlation than T2 relaxivity. [ABSTRACT FROM AUTHOR]

231. High accuracy of ultrasound in diagnosing the presence and type of groin hernia.

Abstract

Background: To evaluate the accuracy of ultrasound (US) in diagnosing the the presence and type of groin hernia.Methods: We retrospectively studied the results of 172 US examinations of the groin in 151 patients (101 men and 50 women; mean age, 59 years) who had undergone US examination for suspected groin hernia. In total, 119 of the groin hernias had been diagnosed on US, and 108 (91%) had required subsequent surgery. All patients who had had positive results for hernia on US and did not undergo surgery (n = 11) and most of the patients whose US results had been negative for hernia (n = 48) underwent limited MRI or CT scanning. We determined the sensitivity, specificity, and accuracy of US in diagnosing the presence and type of groin hernia. To identify any change in the accuracy of US over time at our institution, we compared the sensitivity, specificity, and accuracy of its use from January 2002 through December 2010 (n = 54 groins) with those from January 2011 through December 2012 (n = 118 groins).Results: The overall rates of sensitivity and specificity of US for diagnosing the presence of groin hernia were 96% and 96%. These rates reflect improvements from 92% and 88% prior to 2011 to 98% and 100% beginning in 2011. In addition, the overall accuracy of US for diagnosing the type of groin hernia was 96%. This also improved over time at our center from 91% prior to 2011 to 98% beginning in 2011.Conclusions: US is highly accurate at diagnosing the presence and type of groin hernia.
