210. Pandia : Comprehensive contention-sensitive thread placement

Abstract

Pandia is a system for modeling the performance of in-memory parallel workloads. It generates a description of a workload from a series of profiling runs, and combines this with a description of the machine's hardware to model the workload's performance over different thread counts and different placements of those threads. The approach is "comprehensive" in that it accounts for contention at multiple resources such as processor functional units and memory channels. The points of contention for a workload can shift between resources as the degree of parallelism and thread placement changes. Pandia accounts for these changes and provides a close correspondence between predicted performance and actual performance. Testing a set of 22 benchmarks on 2 socket Intel machines fitted with chips ranging from Sandy Bridge to Haswell we see median differences of 1.05% to 0% between the fastest predicted placement and the fastest measured placement, and median errors of 8% to 4% across all placements. Pandia can be used to optimize the performance of a given workload - for instance, identifying whether or not multiple processor sockets should be used, and whether or not the workload benefits from using multiple threads per core. In addition, Pandia can be used to identify opportunities for reducing resource consumption where additional resources are not matched by additional performance - for instance, limiting a workload to a small number of cores when its scaling is poor., QC 20171201

211. Intractable ascites associated with mycophenolate in a simultaneous kidney-pancreas transplant patient: a case report.

Abstract

BACKGROUND: Mycophenolic acid (MPA), either given as an ester pro-drug or as an enteric-coated sodium salt, is the most commonly prescribed anti-proliferative immunosuppressive agent used following organ transplantation and widely applied in immune-mediated diseases. Clinicians are well aware of common adverse reactions related to MPA treatment, in particular diarrhea, leukopenia and infections. Here we report a case of severe, persistent ascites associated with MPA treatment. The otherwise unexplained and intractable ascites, requiring repeated paracenteses for more than 8 months, rapidly ceased with stopping the MPA treatment. To our knowledge this is the first case of severe ascites associated with MPA treatment reported in the scientific literature. CASE PRESENTATION: A 45-year old female with type 1 diabetes mellitus received a simultaneous kidney-pancreas transplant. The surgery was uneventful. However, post-operatively she developed severe transudative ascites requiring in total more than 40 paracenteses treatments draining in the average 2.8 l of ascites fluid. The ascites formation persisted despite exclusion of a surgical complication, fully functioning kidney and pancreas allografts, lack of any significant proteinuria, normalization of circulating albumin levels, intensive use of diuretics and deliberate attempts to increase the intervals between the paracentesis treatments. Various differential diagnoses, including infectious, hepatic, vascular and cardiac causes were ruled out. Nine months after surgery enteric-coated mycophenolate sodium was switched to azathioprine after which ascites completely resolved. When mycophenolate was recommenced abdominal fullness and weight gain reoccurred. The patient had to be switched to long-term azathioprine treatment. More than 1 year post-conversion the patient remains free of ascites. CONCLUSION: MPA is the most widely used antimetabolite immunosuppressive agent. 212. Combination of Leflunomide and Everolimus for treatment of BK virus nephropathy

Abstract

BK nephropathy (BKN) is a common cause of graft dysfunction following kidney transplantation. Minimization of immunosuppressive therapy remains the first line of therapy, but this may lead to rejection and graft loss. In some cases, despite lowering immunosuppression, BK infection can persist, leading to chronic damage and kidney failure. Currently, there is no specific anti-BK viral therapy. Recent in vitro experiments have demonstrated a reduction in BK viral replication when infected cells are treated with the combination of Leflunomide and Everolimus. This study aims to explore the effect of this drugs combination on viral clearance and graft function in patients with persistent disease despite reduction in immunosuppression. We treated three patients with combination Leflunomide and Everolimus. Data on medical history, biochemical parameters and viral loads were collected. Significant improvement in viral loads was observed in two cases with resolution of viremia in another (Table 1). Two recipients had preserved allograft function. The remaining graft was lost because of combination of obstruction and BKN. No adverse reactions such as bone marrow toxicity were observed. Combination of Leflunomide and Everolimus is safe and should be considered as a rescue therapy in treatment of BKN, especially in those who fail to clear this infection despite reduction of immunosuppressive therapy.

Abstract

Copyright © 2016 by the Society of Critical Care Medicine and the World Federation of Pediatric Intensive and Critical Care Societies.Objectives: Pediatric severe sepsis remains a significant global health problem without new therapies despite many multicenter clinical trials. We compared children managed with severe sepsis in European and U.S. PICUs to identify geographic variation, which may improve the design of future international studies. Design: We conducted a secondary analysis of the Sepsis PRevalence, OUtcomes, and Therapies study. Data about PICU characteristics, patient demographics, therapies, and outcomes were compared. Multivariable regression models were used to determine adjusted differences in morbidity and mortality. Setting: European and U.S. PICUs. Patients: Children with severe sepsis managed in European and U.S. PICUs enrolled in the Sepsis PRevalence, OUtcomes, and Therapies study. Interventions: None. Measurements and Main Results: European PICUs had fewer beds (median, 11 vs 24; p < 0.001). European patients were younger (median, 1 vs 6 yr; p < 0.001), had higher severity of illness (median Pediatric Index of Mortality-3, 5.0 vs 3.8; p = 0.02), and were more often admitted from the ward (37% vs 24%). Invasive mechanical ventilation, central venous access, and vasoactive infusions were used more frequently in European patients (85% vs 68%, p = 0.002; 91% vs 82%, p = 0.05; and 71% vs 50%; p < 0.001, respectively). Raw morbidity and mortality outcomes were worse for European compared with U.S. patients, but after adjusting for patient characteristics, there were no significant differences in mortality, multiple organ dysfunction, disability at discharge, length of stay, or ventilator/vasoactive-free days. Conclusions: Children with severe sepsis admitted to European PICUs have higher severity of illness, are more likely to be admitted from hospital wards, and receive more intensive care therapies than in the United States. The lack of significant differences in morbidity and mortality after adjusting for patient characteristics suggests that the approach to care between regions, perhaps related to PICU bed availability, needs to be considered in the design of future international clinical trials in pediatric severe sepsis.

Abstract

The formation of F centers (oxygen vacancies bearing two electrons) in near-surface MgO films, not the presence of (Li+O-) centers, is directly responsible for the catalytic oxidative coupling of methane into ethane. A high-temperature kinetics study reveals that the (Li+O-) centers instead promote the formation of the F centers, which are sites of low coordination, that directly lead to methane activation. Thus, even at constant lithium concentration but higher temperatures (1000-1300 K) at which F centers form abundantly, ethane production increases tenfold.

Abstract

The studies have shown that gas-phase CO pressure higher than 0.1 Torr is needed to segregate a sufficient amount of Pd to the surface of a well-annealed AuPd(100) sample to form contiguous Pd sites. The reaction kinetics has shown that the higher reaction rate is achieved on a surface with higher coverages of contiguous Pd sites.

Abstract

Solution chemistry methods are used for preparing bimetallic CuPt alloy nanoparticle catalysts with controllable sized and shapes. The mixed-metal nanoparticles have displayed excellent catalytic properties for CO oxidation, with light-off temperatures that are nearly 200 K below those of conventional supported Pt catalysts.
