Abstract

ObjectiveTo determine the safety and pharmacokinetics of erythropoietin (Epo) given in conjunction with hypothermia for hypoxic-ischemic encephalopathy (HIE). We hypothesized that high dose Epo would produce plasma concentrations that are neuroprotective in animal studies (ie, maximum concentration = 6000-10000 U/L; area under the curve = 117000-140000 U*h/L).MethodsIn this multicenter, open-label, dose-escalation, phase I study, we enrolled 24 newborns undergoing hypothermia for HIE. All patients had decreased consciousness and acidosis (pH < 7.00 or base deficit ≥ 12), 10-minute Apgar score ≤ 5, or ongoing resuscitation at 10 minutes. 204. Drug Involvement among Alcoholic Men: Relationships to Psychopathology and Adaptation.

Abstract

Though significant evidence suggests that drug-using alcoholics (DUA) are more damaged psychologically, socially, and medically than non-drug-using alcoholics (NDUA), current research on adults offers no clear explanation for the differences. This study examined the psychological and demographic differences between (other) drug-using and non-drug-using alcoholics in a systematically drawn, population-based nonclinical sample of males (N=193) who were convicted of driving while impaired or driving while under the influence. Respondents varied in extent of their drug use and were categorized into one of five groups ranging from drug abusing/dependent alcoholics to controls who used neither alcohol nor other drugs at clinical levels. Higher levels of drug involvement were associated with higher rates of antisocial behavior and alcohol-related problems, and were inversely related to level of mental health, adaptive functioning, socioeconomic status, and education. These results indicate that alcohol and drug use are related to patterns of adaptation among a variety of non-drug specific domains, including extent of psychopathology, and demographic indices of adaptation. Overall, there was a downward trends in the level of education and socioeconomic status as the level of substance abuse increased. These findings are consistent with a theoretical view which posits that increasing drug involvement is part of a larger syndrome of social failure and psychological distress that does not appear to be drug specific in nature. 211. Fundamental Results on a Class of Affine Polar Grassmann Codes and Their Duals

Abstract

In this manuscript, we study affine polar Grassmann codes. These are linear codes associated to an affine part of the projective embedding of a polar Grassmannian. In particular, the length, dimension, and minimum distance of the affine polar Grassmann codes derived from the symplectic Grassmannian and the Hermitian Grassmannian of totally isotropic spaces are determined. It is also shown that affine Hermitian Grassmann codes have a better minimum distance than affine Grassmann codes while having the same length, dimension, and alphabet size. We also study their dual codes and their related incidence structure.

Abstract

Background and Purpose: Quantitative CT perfusion (CTP) thresholds for assessing the extent of ischemia in patients with acute ischemic stroke (AIS) have been established; relative cerebral blood flow (rCBF) <30% is typically used for estimating estimated ischemic core volume and Tmax (time to maximum) >6 seconds for critical hypoperfused volume in AIS patients with large vessel occlusion (LVO). In this study, we aimed to identify the optimal threshold values for patients presenting with AIS secondary to distal medium vessel occlusions (DMVOs). Methods: In this retrospective study, consecutive AIS patients with anterior circulation DMVO who underwent pretreatment CTP and follow‐up MRI/CT were included. The CTP data were processed by RAPID (iSchemaView, Menlo Park, CA) to generate estimated ischemic core volumes using rCBF <20%, <30%, <34%, and <38% and critical hypoperfused volumes using Tmax (seconds) >4, >6, >8, and >10. Final infarct volumes (FIVs) were obtained from follow‐up MRI/CT within 5 days of symptom onset. Diagnostic performance between CTP thresholds and FIV was assessed in the successfully and unsuccessfully recanalized groups. Results: Fifty‐five patients met our inclusion criteria (32 female [58.2%], 68.0 ± 12.1 years old [mean ± SD]). Recanalization was attempted with intravenous tissue‐type plasminogen activator and mechanical thrombectomy in 27.7% and 38.1% of patients, respectively. Twenty‐five patients (45.4%) were successfully recanalized. In the successfully recanalized patients, no CTP threshold significantly outperformed what is used in LVO setting (rCBF < 30%). All rCBF CTP thresholds demonstrated fair diagnostic performances for predicting FIV. In unsuccessfully recanalized patients, all Tmax CTP thresholds strongly predicted FIV with relative superiority of Tmax >10 seconds (area under the receiver operating characteristic curve =.875, p =.001). Conclusion: In AIS patients with DMVOs, longer Tmax delays than Tmax > 6 seconds, most notably, Tmax > 10 seconds, best predict FIV in unsuccessfully recanalized patients. No CTP threshold reliably predicts FIV in the successfully recanalized group nor significantly outperformed rCBF < 30%. [ABSTRACT FROM AUTHOR]

Abstract

Inspired by the extraordinary ability of metallic resonant nano-cavities to transmit light toward a substrate beneath them, the authors introduce quite a simple design of nano-cavities displaying enhanced antireflective properties in the UV–vis band. With the aid of an evolutionary algorithm, which is based on social rules, the authors drive the dimensions of a hexagonal periodic array of nano-cavities to induce an enhanced reflectance decrease in a silicon substrate as well as a strong increase in the transmittance through it. By using finite-element simulations in the real space, the physical mechanism behind the light propagation is subsequently unveiled. [ABSTRACT FROM AUTHOR]
