Abstract

Objectives: There is sparsity of studies evaluating blood pressure in children with sickle cell disease (SCD), which have shown inconsistent results. Few of the studies have documented lower office blood pressure (BP) in SCD patients, whereas, others have shown presence of masked hypertension and abnormal ambulatory blood BP monitoring (ABPM). Thus, the present study was conducted to examine 24 h ABPM parameters and renal dysfunction in children with SCD and compare them with healthy controls.Methods: A cross-sectional study was conducted on 56 children (30 children having SCD and 26 controls). ABPM and evaluation of renal functions including serum creatinine, serum urea, urinary creatinine, urinary protein and specific gravity was performed.Results: Spot urinary protein to creatinine ratio was found to be higher in patients with SCD (63.3%) as compared to controls (p < 0.001). Proteinuria was observed in 1/4th of the SCD patients less than ten years of age. Masked hypertension was present in 2 (6.6%) patients, ambulatory hypertension in 4 (13.3%), ambulatory pre-hypertension in 1 (3.3%) and abnormal dipping in 60%. A statistically significant correlation of BMI for age Z-score and standard deviation score (SDS/Z) of 24 h systolic BP (r = 0.56, p = 0.002); estimated glomerular filtration rate (eGFR) with 24 h diastolic BP SDS (r = -0.52; p = 0.038) and age with e GFR (r = 0.54; p = 0.025) was found in the present study.Conclusions: The present study corroborates that ABPM abnormalities (ambulatory hypertension, non-dipping pattern, ambulatory prehypertension) and early onset proteinuria are significant findings in patients with SCD. This underscores the importance of regular screening for proteinuria and ABPM in routine care, for early detection and prevention of progressive renal damage in SCD. [ABSTRACT FROM AUTHOR]

Abstract

Family firm innovation has become a topic of vibrant scholarly debate. In this chapter, the authors first briefly outline recent developments in this research stream. They then propose a circular process model of family firm innovation, linking it to extant knowledge on family firm innovation and potential future research streams. This model includes new elements, such as feedback loops, initial and final decisions, stakeholder feedback, a learning and adaptation process, a family-centered non-economic goals paddlewheel, and influencing factors, with details on the input and output phases. Drawing on this model, the chapter uncovers some important research gaps and trends that future studies in this field should seek to fill. In particular, the authors point to important and current trends in innovation, including digitalization, agile management, cyber-security, and their implications for family firms and family firm research.

Abstract

Concurrent macular telangiectasia type 2 (MacTel) and branch retinal vein occlusion (BRVO) have not been described before and can pose a diagnostic dilemma. The current report highlights the role of multicolour (MC) imaging and blue reflectance (BR) imaging in diagnosing this multi-pathology. A 60-year-old male presented with diminution of vision in the right eye. BR imaging showed parafoveal hyperreflectance, characteristic of MacTel, and hyperreflective parafoveal retinal vessels indicating vascular changes in RVO. MC imaging, particularly the BR component is a promising tool to aide in the diagnosis of concurrent MacTel and BRVO, guiding treatment decisions and prognosticating treatment outcomes.

Abstract

Cooperative positioning (CP) utilises information sharing among multiple nodes to enable positioning in Global Navigation Satellite System (GNSS)-denied environments. This paper reports the performance of a CP system for pedestrians using Ultra-Wide Band (UWB) technology in GNSS-denied environments. This data set was collected as part of a benchmarking measurement campaign carried out at the Ohio State University in October 2017. Pedestrians were equipped with a variety of sensors, including two different UWB systems, on a specially designed helmet serving as a mobile multi-sensor platform for CP. Different users were walking in stop-and-go mode along trajectories with predefined checkpoints and under various challenging environments. In the developed CP network, both Peer-to-Infrastructure (P2I) and Peer-to-Peer (P2P) measurements are used for positioning of the pedestrians. It is realised that the proposed system can achieve decimetre-level accuracies (on average, around 20 cm) in the complete absence of GNSS signals, provided that the measurements from infrastructure nodes are available and the network geometry is good. In the absence of these good conditions, the results show that the average accuracy degrades to meter level. Further, it is experimentally demonstrated that inclusion of P2P cooperative range observations further enhances the positioning accuracy and, in extreme cases when only one infrastructure measurement is available, P2P CP may reduce positioning errors by up to 95% . The complete test setup, the methodology for development, and data collection are discussed in this paper. In the next version of this system, additional observations such as the Wi-Fi, camera, and other signals of opportunity will be included.

Abstract

BACKGROUND: ERCC1, a component of nucleotide excision repair pathway, is known to repair DNA breaks induced by platinum drugs. We sought to ascertain if ERCC1 expression dynamics and a single nucleotide polymorphism (SNP) rs11615 are biomarkers of sensitivity to oxaliplatin therapy in patients with colorectal cancer (CRC). METHODS: Western blot and qPCR for ERCC1 expression was performed from PBMCs isolated from patients receiving oxaliplatin-based therapy at specified timepoints. DNA was also isolated from 59 biorepository specimens for SNP analysis. Clinical benefit was determined using progression free survival (PFS) for metastatic CRC. RESULTS: ERCC1 was induced in PBMC in response to oxaliplatin in 13/25 patients with mCRC (52%). Median PFS with ERCC1 induction was 190d compared to 237d in non-induced patients (HR 2.35, CI 1.005-5.479; p=0.0182). ERCC1 rs11615 SNP analysis revealed that 43.3% harbored C/C, 41.2%-T/C and 15.5%-T/T genotype. Median PFS was significantly lower with C/C or T/C (211 and 196d) compared to T/T (590d; p=0.0310). CONCLUSIONS: ERCC1 was induced in a sub-population of patients undergoing oxaliplatin treatment, which was associated with poorer outcome, suggesting this could serve as a marker of oxaliplatin response. C/C or C/T genotype in ERCC1 rs11615 locus decreased benefit from oxaliplatin.

250. Reliably Learning the ReLU

Abstract

We give the first dimension-efficient algorithms for learning Rectified Linear Units (ReLUs), which are functions of the form x 7→ max(0, w · x) with w ∈ Sn−1 . Our algorithm works in the challenging Reliable Agnostic learning model of Kalai et al. (2012) where the learner is given access to a distribution D on labeled examples but the labeling may be arbitrary. We construct a hypothesis that simultaneously minimizes the false-positive rate and the loss on inputs given positive labels by D, for any convex, bounded, and Lipschitz loss function. The algorithm runs in polynomial-time (in n) with respect to any distribution on Sn−1 (the unit sphere in n dimensions) and for any error parameter ∈ = Ω(1/ log n) (this yields a PTAS for a question raised by F. Bach on the complexity of maximizing ReLUs). These results are in contrast to known efficient algorithms for reliably learning linear threshold functions, where must be Ω(1) and strong assumptions are required on the marginal distribution. We can compose our results to obtain the first set of efficient algorithms for learning constantdepth networks of ReLU with fixed polynomial-dependence in the dimension. For depth-2 networks of sigmoids, we obtain the first algorithms that have a polynomial dependency in all parameters. Our techniques combine kernel methods and polynomial approximations with a “dual-loss” approach to convex programming. As a byproduct we obtain a number of applications including the first set of efficient algorithms for “convex piecewise-linear fitting” and the first efficient algorithms for noisy polynomial reconstruction of low-weight polynomials on the unit sphere.
