Purpose We sought to compare objectively computed tomography ( CT) scanner performance for three clinically relevant protocols using a task-based image quality assessment method in order to assess the potential for radiation dose reduction. Methods Four CT scanners released between 2003 and 2007 by different manufacturers were compared with four CT scanners released between 2012 and 2014 by the same manufacturers using ideal linear model observers ( MO): prewhitening ( PW) MO and channelized Hotelling ( CHO) MO with Laguerre-Gauss channels for high-contrast spatial resolution and low-contrast detectability ( LCD) performance, respectively. High-contrast spatial resolution was assessed using a custom-made phantom that enabled the computation of the target transfer function ( TTF) and noise power spectrum ( NPS). Low-contrast detectability was assessed using a commercially available anthropomorphic abdominal phantom providing equivalent diameters of 24, 29.6, and 34.6 cm. Three protocols were reviewed: a head (trauma) and an abdominal (urinary stones) protocol were applied to assess high-contrast spatial resolution performance; and another abdominal (focal liver lesions) protocol was applied for LCD. The liver protocol was tested using fixed and modulated tube currents. The PW MO was proposed for assessing high-contrast detectability performance of the various CT scanners. Results Compared with older generation CT scanners, three newer systems displayed significant improvements in high-contrast detectability over that of their predecessors. A fourth, newer system had lower performance. The CHO MO was appropriate for assessing LCD performance and revealed that an excellent level of image quality could be obtained with newer scanners at significantly lower dose levels. Conclusions This study shows that MO can objectively benchmark CT scanners using a task-based image quality method, thus helping to estimate the potential for further dose reductions offered by the latest systems. Patient dose optimisation in computed tomography (CT) should be done using clinically relevant tasks when dealing with image quality assessments. In the present work, low-contrast detectability for an average patient morphology was assessed on 56 CT units, using a model observer applied on images acquired with two specific protocols of an anthropomorphic phantom containing spheres. Images were assessed using the channelised Hotelling observer (CHO) with dense difference of Gaussian channels. The results were computed by performing receiver operating characteristics analysis (ROC) and using the area under the ROC curve (AUC) as a figure of merit. The results showed a small disparity at a volume computed tomography dose index (CTDIvol) of 15 mGy depending on the CT units for the chosen image quality criterion. For 8-mm targets, AUCs were 0.999 ± 0.018 at 20 Hounsfield units (HU) and 0.927 ± 0.054 at 10 HU. For 5-mm targets, AUCs were 0.947 ± 0.059 and 0.702 ± 0.068 at 20 and 10 HU, respectively. Evaluating image quality by using receiver operating characteristic studies is time consuming and difficult to implement. This work assesses a new iterative algorithm using a channelised Hotelling observer (CHO). For this purpose, an anthropomorphic abdomen phantom with spheres of various sizes and contrasts was scanned at 3 volume computed tomography dose index (CTDIvol) levels on a GE Revolution CT. Images were reconstructed using the iterative reconstruction method adaptive statistical iterative reconstruction-V (ASIR-V) at ASIR-V 0, 50 and 70 % and assessed by applying a CHO with dense difference of Gaussian and internal noise. Both CHO and human observers (HO) were compared based on a four-alternative forced-choice experiment, using the percentage correct as a figure of merit. The results showed accordance between CHO and HO. Moreover, an improvement in the low-contrast detection was observed when switching from ASIR-V 0 to 50 %. Anthropomorphic model observers are mathe- matical algorithms which are applied to images with the ultimate goal of predicting human signal detection and classification accuracy across varieties of backgrounds, image acquisitions and display conditions. A limitation of current channelized model observers is their inability to handle irregularly-shaped signals, which are common in clinical images, without a high number of directional channels. Here, we derive a new linear model observer based on convolution channels which we refer to as the “Filtered Channel observer” (FCO), as an extension of the channelized Hotelling observer (CHO) and the nonprewhitening with an eye filter (NPWE) observer. In analogy to the CHO, this linear model observer can take the form of a single template with an external noise term. To compare with human observers, we tested signals with irregular and asymmetrical shapes spanning the size of lesions down to those of microcalfications in 4-AFC breast tomosynthesis detection tasks, with three different contrasts for each case. Whereas humans uniformly outperformed conventional CHOs, the FCO observer outperformed humans for every signal with only one exception. Additive internal noise in the models allowed us to degrade model performance and match human performance. We could not match all the human performances with a model with a single internal noise component for all signal shape, size and contrast conditions. This suggests that either the internal noise might vary across signals or that the model cannot entirely capture the human detection strategy. However, the FCO model offers an efficient way to apprehend human observer performance for a non-symmetric signal. [ABSTRACT FROM AUTHOR]

Purpose: Signal detection on 3D medical images depends on many factors, such as foveal and peripheral vision, the type of signal, and background complexity, and the speed at which the frames are displayed. In this paper, the authors focus on the speed with which radiologists and naïve observers search through medical images. Prior to the study, the authors asked the radiologists to estimate the speed at which they scrolled through CT sets. They gave a subjective estimate of 5 frames per second (fps). The aim of this paper is to measure and analyze the speed with which humans scroll through image stacks, showing a method to visually display the behavior of observers as the search is made as well as measuring the accuracy of the decisions. This information will be useful in the development of model observers, mathematical algorithms that can be used to evaluate diagnostic imaging systems. Methods: The authors performed a series of 3D 4-alternative forced-choice lung nodule detection tasks on volumetric stacks of chest CT images iteratively reconstructed in lung algorithm. The strategy used by three radiologists and three naïve observers was assessed using an eye-tracker in order to establish where their gaze was fixed during the experiment and to verify that when a decision was made, a correct answer was not due only to chance. In a first set of experiments, the observers were restricted to read the images at three fixed speeds of image scrolling and were allowed to see each alternative once. In the second set of experiments, the subjects were allowed to scroll through the image stacks at will with no time or gaze limits. In both static-speed and free-scrolling conditions, the four image stacks were displayed simultaneously. All trials were shown at two different image contrasts. Results: The authors were able to determine a histogram of scrolling speeds in frames per second. The scrolling speed of the naïve observers and the radiologists at the moment the signal was detected was measured at 25-30 fps. For the task chosen, the performance of the observers was not affected by the contrast or experience of the observer. However, the naïve observers exhibited a different pattern of scrolling than the radiologists, which included a tendency toward higher number of direction changes and number of slices viewed. Conclusions: The authors have determined a distribution of speeds for volumetric detection tasks. The speed at detection was higher than that subjectively estimated by the radiologists before the experiment. Objective: To compare image quality of a standard-dose (SD) and a low-dose (LD) cervical spine CT protocol using filtered back-projection (FBP) and iterative reconstruction (IR). Materials and methods: Forty patients investigated by cervical spine CT were prospectively randomised into two groups: SD (120 kVp, 275 mAs) and LD (120 kVp, 150 mAs), both applying automatic tube current modulation. Data were reconstructed using both FBP and sinogram-affirmed IR. Image noise, signal-to-noise (SNR) and contrast-to-noise (CNR) ratios were measured. Two radiologists independently and blindly assessed the following anatomical structures at C3-C4 and C6-C7 levels, using a four-point scale: intervertebral disc, content of neural foramina and dural sac, ligaments, soft tissues and vertebrae. They subsequently rated overall image quality using a ten-point scale. Results: For both protocols and at each disc level, IR significantly decreased image noise and increased SNR and CNR, compared with FBP. SNR and CNR were statistically equivalent in LD-IR and SD-FBP protocols. Regardless of the dose and disc level, the qualitative scores with IR compared with FBP, and with LD-IR compared with SD-FBP, were significantly higher or not statistically different for intervertebral discs, neural foramina and ligaments, while significantly lower or not statistically different for soft tissues and vertebrae. The overall image quality scores were significantly higher with IR compared with FBP, and with LD-IR compared with SD-FBP. Conclusion: LD-IR cervical spine CT provides better image quality for intervertebral discs, neural foramina and ligaments, and worse image quality for soft tissues and vertebrae, compared with SD-FBP, while reducing radiation dose by approximately 40 %. Background: The potential effects of ionizing radiation are of particular concern in children. The model-based iterative reconstruction VEO is a technique commercialized to improve image quality and reduce noise compared with the filtered back-projection (FBP) method. Objective: To evaluate the potential of VEO on diagnostic image quality and dose reduction in pediatric chest CT examinations. Materials and methods: Twenty children (mean 11.4 years) with cystic fibrosis underwent either a standard CT or a moderately reduced-dose CT plus a minimum-dose CT performed at 100 kVp. Reduced-dose CT examinations consisted of two consecutive acquisitions: one moderately reduced-dose CT with increased noise index (NI = 70) and one minimum-dose CT at CTDI 0.14 mGy. Standard CTs were reconstructed using the FBP method while low-dose CTs were reconstructed using FBP and VEO. Two senior radiologists evaluated diagnostic image quality independently by scoring anatomical structures using a four-point scale (1 = excellent, 2 = clear, 3 = diminished, 4 = non-diagnostic). Standard deviation (SD) and signal-to-noise ratio (SNR) were also computed. Results: At moderately reduced doses, VEO images had significantly lower SD ( P < 0.001) and higher SNR ( P < 0.05) in comparison to filtered back-projection images. Further improvements were obtained at minimum-dose CT. The best diagnostic image quality was obtained with VEO at minimum-dose CT for the small structures (subpleural vessels and lung fissures) ( P < 0.001). The potential for dose reduction was dependent on the diagnostic task because of the modification of the image texture produced by this reconstruction. Conclusions: At minimum-dose CT, VEO enables important dose reduction depending on the clinical indication and makes visible certain small structures that were not perceptible with filtered back-projection. Purpose: Quantification of myocardial blood flow (MBF) with generator-produced Rb is an attractive alternative for centres without an on-site cyclotron. Our aim was to validate Rb-measured MBF in relation to that measured using O-water, as a tracer 100% of which can be extracted from the circulation even at high flow rates, in healthy control subject and patients with mild coronary artery disease (CAD). Methods: MBF was measured at rest and during adenosine-induced hyperaemia with Rb and O-water PET in 33 participants (22 control subjects, aged 30 ± 13 years; 11 CAD patients without transmural infarction, aged 60 ± 13 years). A one-tissue compartment Rb model with ventricular spillover correction was used. The Rb flow-dependent extraction rate was derived from O-water measurements in a subset of 11 control subjects. Myocardial flow reserve (MFR) was defined as the hyperaemic/rest MBF. Pearson's correlation r, Bland-Altman 95% limits of agreement (LoA), and Lin's concordance correlation ρ (measuring both precision and accuracy) were used. Results: Over the entire MBF range (0.66-4.7 ml/min/g), concordance was excellent for MBF ( r = 0.90, [Rb-O-water] mean difference ± SD = 0.04 ± 0.66 ml/min/g, LoA = −1.26 to 1.33 ml/min/g, ρ = 0.88) and MFR (range 1.79-5.81, r = 0.83, mean difference = 0.14 ± 0.58, LoA = −0.99 to 1.28, ρ = 0.82). Hyperaemic MBF was reduced in CAD patients compared with the subset of 11 control subjects (2.53 ± 0.74 vs. 3.62 ± 0.68 ml/min/g, p = 0.002, for O-water; 2.53 ± 1.01 vs. 3.82 ± 1.21 ml/min/g, p = 0.013, for Rb) and this was paralleled by a lower MFR (2.65 ± 0.62 vs. 3.79 ± 0.98, p = 0.004, for O-water; 2.85 ± 0.91 vs. 3.88 ± 0.91, p = 0.012, for Rb). Myocardial perfusion was homogeneous in 1,114 of 1,122 segments (99.3%) and there were no differences in MBF among the coronary artery territories ( p > 0.31). Conclusion: Quantification of MBF with Rb with a newly derived correction for the nonlinear extraction function was validated against MBF measured using O-water in control subjects and patients with mild CAD, where it was found to be accurate at high flow rates. Background: Radiation dose exposure is of particular concern in children due to the possible harmful effects of ionizing radiation. The adaptive statistical iterative reconstruction (ASIR) method is a promising new technique that reduces image noise and produces better overall image quality compared with routine-dose contrast-enhanced methods.Objective: To assess the benefits of ASIR on the diagnostic image quality in paediatric cardiac CT examinations.Materials and Methods: Four paediatric radiologists based at two major hospitals evaluated ten low-dose paediatric cardiac examinations (80 kVp, CTDI(vol) 4.8-7.9 mGy, DLP 37.1-178.9 mGy·cm). The average age of the cohort studied was 2.6 years (range 1 day to 7 years). Acquisitions were performed on a 64-MDCT scanner. All images were reconstructed at various ASIR percentages (0-100%). For each examination, radiologists scored 19 anatomical structures using the relative visual grading analysis method. To estimate the potential for dose reduction, acquisitions were also performed on a Catphan phantom and a paediatric phantom.Results: The best image quality for all clinical images was obtained with 20% and 40% ASIR (p < 0.001) whereas with ASIR above 50%, image quality significantly decreased (p < 0.001). With 100% ASIR, a strong noise-free appearance of the structures reduced image conspicuity. A potential for dose reduction of about 36% is predicted for a 2- to 3-year-old child when using 40% ASIR rather than the standard filtered back-projection method.Conclusion: Reconstruction including 20% to 40% ASIR slightly improved the conspicuity of various paediatric cardiac structures in newborns and children with respect to conventional reconstruction (filtered back-projection) alone. The effect of copper (Cu) filtration on image quality and dose in different digital X-ray systems was investigated. Two computed radiography systems and one digital radiography detector were used. Three different polymethylmethacrylate blocks simulated the pediatric body. The effect of Cu filters of 0.1, 0.2, and 0.3 mm thickness on the entrance surface dose (ESD) and the corresponding effective doses (EDs) were measured at tube voltages of 60, 66, and 73 kV. Image quality was evaluated in a contrast-detail phantom with an automated analyzer software. Cu filters of 0.1, 0.2, and 0.3 mm thickness decreased the ESD by 25-32%, 32-39%, and 40-44%, respectively, the ranges depending on the respective tube voltages. There was no consistent decline in image quality due to increasing Cu filtration. The estimated ED of anterior-posterior (AP) chest projections was reduced by up to 23%. No relevant reduction in the ED was noted in AP radiographs of the abdomen and pelvis or in posterior-anterior radiographs of the chest. Cu filtration reduces the ESD, but generally does not reduce the effective dose. Cu filters can help protect radiosensitive superficial organs, such as the mammary glands in AP chest projections. The aim of this study was to evaluate a low-dose CT with oral contrast medium (LDCT) for the diagnosis of acute appendicitis and compare its performance with standard-dose i.v. contrast-enhanced CT (standard CT) according to patients' BMIs. Eighty-six consecutive patients admitted with suspicion of acute appendicitis underwent LDCT (30 mAs), followed by standard CT (180 mAs). Both examinations were reviewed by two experienced radiologists for direct and indirect signs of appendicitis. Clinical and surgical follow-up was considered as the reference standard. Appendicitis was confirmed by surgery in 37 (43%) of the 86 patients. Twenty-nine (34%) patients eventually had an alternative discharge diagnosis to explain their abdominal pain. Clinical and biological follow-up was uneventful in 20 (23%) patients. LDCT and standard CT had the same sensitivity (100%, 33/33) and specificity (98%, 45/46) to diagnose appendicitis in patients with a body mass index (BMI) >or= 18.5. In slim patients (BMI<18.5), sensitivity to diagnose appendicitis was 50% (2/4) for LDCT and 100% (4/4) for standard CT, while specificity was identical for both techniques (67%, 2/3). This work aimed at assessing the doses delivered in Switzerland to paediatric patients during computed tomography (CT) examinations of the brain, chest and abdomen, and at establishing diagnostic reference levels (DRLs) for various age groups. Forms were sent to the ten centres performing CT on children, addressing the demographics, the indication and the scanning parameters: number of series, kilovoltage, tube current, rotation time, reconstruction slice thickness and pitch, volume CT dose index (CTDI(vol)) and dose length product (DLP). Per age group, the proposed DRLs for brain, chest and abdomen are, respectively, in terms of CTDI(vol): 20, 30, 40, 60 mGy; 5, 8, 10, 12 mGy; 7, 9, 13, 16 mGy; and in terms of DLP: 270, 420, 560, 1,000 mGy cm; 110, 200, 220, 460 mGy cm; 130, 300, 380, 500 mGy cm. Combined positron emission tomography and computed tomography (PET/CT) scanners play a major role in medicine for in vivo imaging in an increasing number of diseases in oncology, cardiology, neurology, and psychiatry. With the advent of short-lived radioisotopes other than 18F and newer scanners, there is a need to optimize radioisotope activity and acquisition protocols, as well as to compare scanner performances on an objective basis. The Discovery-LS (D-LS) was among the first clinical PET/CT scanners to be developed and has been extensively characterized with older National Electrical Manufacturer Association (NEMA) NU 2-1994 standards. At the time of publication of the latest version of the standards (NU 2-2001) that have been adapted for whole-body imaging under clinical conditions, more recent models from the same manufacturer, i.e., Discovery-ST (D-ST) and Discovery-STE (D-STE), were commercially available. We report on the full characterization both in the two- and three-dimensional acquisition mode of the D-LS according to latest NEMA NU 2-2001 standards (spatial resolution, sensitivity, count rate performance, accuracy of count losses, and random coincidence correction and image quality), as well as a detailed comparison with the newer D-ST widely used and whose characteristics are already published. The purpose was to compare the image quality and patient dose between 4- and 16-row MDCT units and to evaluate the dispersion of the dose delivered for common clinical examinations. Four 4- and 16-row MDCT units were used in the study. Image noise levels from images of a CatPhan phantom were compared for all units using a given CTDI(vol) of 15.0+/-1.0 mGy. Standard acquisition protocols from ten centres, shifted from 4- to 16-row MDCT (plus one additional centre for 16-row MDCT), were compared for cerebral angiography and standard chest and abdomen examinations. In addition, the protocols used with 16-row MDCT units for diagnosis of the unstable shoulder and for cardiac examinations were also compared. The introduction of 16-MSCT units did not reduce the performance of the detectors. Concerning the acquisition protocols, a wide range in practice was observed for standard examinations; DLP varied from 800 to 5,120 mGy x cm, 130 to 860 mGy x cm, 410 to 1,790 mGy x cm and 850 to 2,500 mGy x cm for cerebral angiography, standard chest, standard abdomen and heart examinations, respectively. The introduction of 16-row MDCT did not, on average, increase the patient dose for standard chest and abdominal examinations. Current practice in particle visualization renders particle position data directly onto the screen as points or glyphs. Using a camera placed at a fixed position, particle motions can be visualized by rendering trajectories or by animations. Applying such direct techniques to large, time dependent particle data sets often results in cluttered images in which the dynamic properties of the underlying system are difficult to interpret. In this case study we take an alternative approach to the visualization of ion motions. Instead of rendering ion position data directly, we first extract meaningful motion information from the ion position data and then map this information onto geometric primitives. Our goal is to produce high-level visualizations that reflect the physicists' way of thinking about ion dynamics. Parameterized geometric icons are defined to encode motion information of clusters of related ions. In addition, a parameterized camera control mechanism is used to analyze relative instead of only absolute ion motions. We apply the techniques to simulations of Fourier transform mass spectrometry (FTMS) experiments. The data produced by such simulations can amount to 5 · 104 ions and 105 timesteps. This study compared different acquisition protocols performance to detect small liver metastases (<2 cm). Thirty consecutive patients with histologically proven hepatic metastases were explored by MDCT at the liver equilibrium phase by four successive acquisitions. We compared the following protocols (1-4): 5/30/1.5 (section thickness/table speed/pitch); 5/15/0.75; 5/11.25/0.75; and 2.5/15/1.5 with the same X-ray dose. The gold standard was based on patient radiological follow-up. Evolutive lesions were considered as true positive (TP). The described lesions, not found on the follow-up exams despite tumoral progression, were considered as false positive (FP). Stable lesions could not be considered as metastasis and were eliminated. One hundred and seventy-six lesions were detected: 61 TP and 91 FP. Twenty-four lesions were eliminated. The mean kappa values for protocols 1, 2, 3 and 4 were, respectively, 0.43, 0.68, 0.73 and 0.51 (0.61-0.80: substantial agreement) and the mean areas under the ROC curve were, respectively, 0.76, 0.87, 0.86 and 0.80. The results of protocols 2 and 3 were significantly superior to those of protocols 1 and 4. Purpose: It is generally assumed that the biodistribution and pharmacokinetics of radiolabelled antibodies remain similar between dosimetric and therapeutic injections in radioimmunotherapy. However, circulation half-lives of unlabelled rituximab have been reported to increase progressively after the weekly injections of standard therapy doses. The aim of this study was to evaluate the evolution of the pharmacokinetics of repeated 131 1-rituximab injections during treatment with unlabelled rituximab in patients with non-Hodgkin's lymphoma (NHL). Methods: Patients received standard weekly therapy with rituximab (375 mg/m2) for 4 weeks and a fifth injection at 7 or 8 weeks. Each patient had three additional injections of 185 MBq 131I-rituximab in either treatment weeks 1, 3 and 7 (two patients) or weeks 2, 4 and 8 (two patients). The 12 radiolabelled antibody injections were followed by three whole-body (WB) scintigraphic studies during 1 week and blood sampling on the same occasions. Additional WB scans were performed after 2 and 4 weeks post 131I-rituximab injection prior to the second and third injections, respectively. Results: A single exponential radioactivity decrease for WB, liver, spleen, kidneys and heart was observed. Biodistribution and half-lives were patient specific, and without significant change after the second or third injection compared with the first one. Blood T½β, calculated from the sequential blood samples and fitted to a bi-exponential curve, was similar to the T½ of heart and liver but shorter than that of WB and kidneys. Effective radiation dose calculated from attenuation-corrected WB scans and blood using Mirdose 3. 1 was 0.53+0.05 mSv/MBq (range 0.48-0.59 mSv/MBq). Radiation dose was highest for spleen and kidneys, followed by heart and liver. Conclusion: These results show that the biodistribution and tissue kinetics of 131I-rituximab, while specific to each patient, remained constant during unlabelled antibody therapy. The purpose of this study was to compare the diagnostic performance of axial and coronal views in multidetector CT enteroclysis (MDCTE). We retrospectively evaluated 48 patients with pathological correlation investigated by MDCTE for small bowel disorders. After nasojejunal administration of 2 l of 5% methylcellulose axial arterial and venous acquisition of MDCTE was followed by coronal reconstructions using equal slice thicknesses of 2.5 mm with 2 mm increments. Spatial resolution of both planes was evaluated by phantom. Three radiologists independently read axial and coronal images concerning 12 pathological features. The interobserver agreement and time of reading was calculated. Sensitivity and specificity resulted from comparison with histopathology (n=39) or follow-up (n=9). Phantom study revealed higher spatial resolution for axial than coronal views, whatever reconstruction interval was used. However, spatial frequency always remained high. Most pathological signs, such as bowel wall thickening (BWT), bowel wall enhancement (BWE) and intraperitoneal fluid (IPF), showed better interobserver agreement on axial than coronal views (BWT: 0.61 vs. 0.44; BWE: 0.56 vs. 0.5; IPF:0.53 vs. 0.43). The Wilcoxon signed-rank test revealed significantly higher sensitivity for axial than coronal views (P=0.0453); the time of reading was significantly shorter for the latter (P=0.0146). The diagnostic value of axial slices is superior to coronal reconstructions despite the reduced data volume and display of the physiological course of bowel loops on the coronal plane. Radiation protection is assured by respecting three general principles which are: the justification of the practice; the optimisation of the protection; and the individual dose limits. The justification of the practice is due to the fact that exposure to ionising radiation has deleterious effects on health. The principle of optimisation is introduced since some of these effects are considered as non-threshold ones, and thus one has to reduce exposure to levels as low as reasonably achievable (i.e. ALARA principle). The principle of individual limits is introduced as a safeguard to prevent situations where the respect of the two first principles would not be sufficient to protect individuals. The first objective of this contribution is to state the dose limits for exposure of diagnostic imaging personnel and explain the reasons for allowing a larger dose equivalent for radiation workers than for the general population. The second objective is to present various methods and devices that may be used to reduce personnel exposure during diagnostic X-ray examinations (especially interventional fluoroscopic procedures). A judicious application of radiation protection techniques, such as the limitation of patient exposure, the increase of the distance from the source of scatter, and shielding, will significantly reduce personnel exposure. The aim was to propose a strategy for finding reasonable compromises between image noise and dose as a function of patient weight. Weighted CT dose index (CTDI(w)) was measured on a multidetector-row CT unit using CTDI test objects of 16, 24 and 32 cm in diameter at 80, 100, 120 and 140 kV. These test objects were then scanned in helical mode using a wide range of tube currents and voltages with a reconstructed slice thickness of 5 mm. For each set of acquisition parameter image noise was measured and the Rose model observer was used to test two strategies for proposing a reasonable compromise between dose and low-contrast detection performance: (1) the use of a unique noise level for all test object diameters, and (2) the use of a unique dose efficacy level defined as the noise reduction per unit dose. Published data were used to define four weight classes and an acquisition protocol was proposed for each class. The protocols have been applied in clinical routine for more than one year. CTDI(vol) values of 6.7, 9.4, 15.9 and 24.5 mGy were proposed for the following weight classes: 2.5-5, 5-15, 15-30 and 30-50 kg with image noise levels in the range of 10-15 HU. The proposed method allows patient dose and image noise to be controlled in such a way that dose reduction does not impair the detection of low-contrast lesions. To investigate the impact of a partial model-based iterative reconstruction (ASiR-V) on image quality in thoracic oncologic multidetector computed tomography (MDCT), using human and mathematical model observers. Twenty cancer patients examined with regular-dose thoracic-abdominal-pelvic MDCT were retrospectively included. Thoracic images reconstructed using a sharp kernel and filtered back-projection (reference) or ASiR-V (0-100%, 20% increments; follow-up) were analysed by three thoracic radiologists. Advanced quantitative physical metrics, including detectability indexes of simulated 4-mm-diameter solid non-calcified nodules and ground-glass opacities, were computed at regular and reduced doses using a custom-designed phantom. All three radiologists preferred higher ASiR-V levels (best = 80%). Increasing ASiR-V substantially decreased noise magnitude, with slight changes in noise texture. For high-contrast objects, changing the ASiR-V level had no major effect on spatial resolution; whereas for lower-contrast objects, increasing ASiR-V substantially decreased spatial resolution, more markedly at reduced dose. For both high- and lower-contrast pulmonary lesions, detectability remained excellent, regardless of ASiR-V and dose levels, and increased significantly with increasing ASiR-V levels (all p < 0.001). While high ASiR-V levels (80%) are recommended to detect solid non-calcified nodules and ground-glass opacities in regular-dose thoracic oncologic MDCT, care must be taken because, for lower-contrast pulmonary lesions, high ASiR-V levels slightly change noise texture and substantially decrease spatial resolution, more markedly at reduced dose. Objective. The objective of this study was to assess the age-related variations of brain perfusion through quantitative cerebral perfusion computed tomography (CT) results in children without brain abnormality. Methods. Brain perfusion CT examinations were performed in 77 children, aged 7 days to 18 years. These patients were admitted at our institution for both noncontrast and contrast-enhanced cerebral CT. Only children whose conventional cerebral CT and clinical/radiologic follow-up, including additional investigations, were normal were taken into account for this study (53 of 77). Results. The average regional rCBF amounts to 40 (mL/100 g per minute) for the first 6 months of life, peaks at ~130 (mL/100 g per minute) at ~2 to 4 years of age, and finally stabilizes at ~50 (mL/100 g per minute) at ~7 to 8 years of age, with a small increase of rCBF values at ~12 years of age. The rCBF in the gray matter averages 3 times that in the white matter, except for the first 6 months of life. The global CBF represents 10% to 20% of the global cardiac output for the first 6 months of life, peaks at ~55% by 2 to 4 years of age, and finally stabilizes at ~15% by 7 to 8 years of age. Specific age-related evolution patterns were identified in the different anatomic areas of the cerebral parenchyma, which could be related to the development of neuroanatomic structures and to the emergence of corresponding cognitive functions. Conclusions. Quantitative perfusion CT characterization of brain perfusion shows specific age variations. Brain perfusion of each cortical area evolves according to a specific time course, in close correlation with the psychomotor development.
