The article reports on risk in information technology (IT) projects which lead to out-of-control projects that can disrupt companies and careers. Research on the so-called black swan of unpredictable events which cause financial loss and mismanagement of IT change initiatives is discussed. The study focused on public agencies and other U.S.-based projects including one at Levi Strauss dry goods company that had a budget of $5 million and a cost overrun of more than $100 million. Software and configuration problems that Airbus had which prevented the aircraft company from delivering its A380 jet transport on time are noted.
