Abstract

Sjögren's syndrome (SjS) accompanied by other systemic autoimmune rheumatic connective tissue diseases has historically been termed 'secondary' in contrast to 'primary' SjS as a standalone entity. However, it is a matter of a long-standing debate whether the prefixes 'primary' and 'secondary', including a temporal component, are obsolete in the terminology of SjS. We review the history and the pathophysiological, chronological, genetic, histological and clinical data underlying the concept of 'secondary' SjS. There are important unintended consequences of the nomenclature; notably 'secondary' SjS has been much less researched and is often excluded from clinical trials. We argue for further research, a change in terminology and more stringent classification. Further we highlight possible opportunities for trials in SjS and other systemic autoimmune diseases that might contribute to an advance in care for all patients with SjS.

216. Tracking Salal (Gaultheria shallon) and Western Red Cedar (Thuja plicata) Dieoff in Pacific Spirit Regional Park

Abstract

In the spring of 2019, multiple citizens raised concerns of western red cedar (Thuja plicata) and salad (Gaultheria shallon) disease and death in Pacific Spirit Regional Park (PSRP), Vancouver, B.C. Upon investigation by Metro Vancouver Parks staff, a few cases of such die off was confirmed. The purpose of this observational study was to identify and record the current extent of die-off of both western red Cedar and salal. The study also aimed to determine spatial trends of disease and die-off that might indicate the types of further research required, such as research into hydrology, topography, and soil structure. The geographic location and physical characteristics of each dying plant within view of existing trail infrastructure was collected, with the trails acting as transects. The data has been synthesized into the form of a Google MyMap™, which was made available to staff at Metro Vancouver Regional Parks and can be disseminated to organizations affiliated with the park, so that they may assess and compare population changes in the future. Although analysis of salal data was ultimately not conducted in this report due to low sample size, general trends for western red cedar indicate a higher concentration of disease and mortality near the eastern edge of the park possibly associated with disturbance in the form of residential construction, as well as near other localized sites of human disturbance of topography and soils. For example, concentrations of disease seem to exist near the Metro Vancouver Camosun drinking water reservoir, and near mounds of clay, gravel, concrete, or asphalt possibly deposited during trail construction or construction of UBC campus facilities such as the Sedgewick library. These findings should be taken as indications for further research, as no hypothesis testing was performed during this study. Initial findings suggest a review of construction and development practices in and around Pacific Spirit Regional Park, including appropriate removal and past and present construction related wastes and continued development of vegetation and soil restoration capacity.

Abstract

The therapeutic management of Sjögren syndrome (SjS) has not changed substantially in recent decades: treatment decisions remain challenging in clinical practice, without a specific therapeutic target beyond the relief of symptoms as the most important goal. In view of this scenario, the European League Against Rheumatism (EULAR) promoted and supported an international collaborative study (EULAR SS Task Force) aimed at developing the first EULAR evidence and consensus-based recommendations for the management of patients with SjS with topical and systemic medications. The aim was to develop a rational therapeutic approach to SjS patients useful for healthcare professionals, physicians undergoing specialist training, medical students, the pharmaceutical industry and drug regulatory organisations following the 2014 EULAR standardised operating procedures. The Task Force (TF) included specialists in rheumatology, internal medicine, oral health, ophthalmology, gynaecology, dermatology and epidemiology, statisticians, general practitioners, nurses and patient representatives from 30 countries of the 5 continents. Evidence was collected from studies including primary SjS patients fulfilling the 2002/2016 criteria; when no evidence was available, evidence from studies including associated SjS or patients fulfilling previous sets of criteria was considered and extrapolated. The TF endorsed the presentation of general principles for the management of patients with SjS as three overarching, general consensus-based recommendations and 12 specific recommendations that form a logical sequence, starting with the management of the central triplet of symptoms (dryness, fatigue and pain) followed by the management of systemic disease. Abstract

Law enforcement officers (often called school resource officers or SROs) are an increasingly common feature in schools across the United States. Although SROs' roles vary across school contexts, there has been little examination of why. One possible explanation is that SROs perceive threats differently in different school contexts and that the racial composition of schools may motivate these differences. To investigate this possibility, this study analyzes interviews with 73 SROs from two different school districts that encompass schools with a variety of racial compositions. Across both districts, SROs perceived three major categories of threats: student-based, intruder-based, and environment-based threats. However, the focus and perceived severity of the threats varied across districts such that SROs in the district with a larger proportion of White students were primarily concerned about external threats (i.e. intruder-based and environment-based) that might harm the students, whereas SROs in the district with a larger proportion of Black students were primarily concerned with students themselves as threats. We consider how these results relate to understandings of school security, inequality among students, racially disparate experiences with school policing, and school and policing policy. Abstract

School resource officers (SROs) have become increasingly common in schools in the United States and this growth of police in schools has taken place as part of a trend of school criminalization. In a school criminalization framework, scholars have focused on physical security measures and punitive outcomes for students, however, the subtler dimensions have been less explored. We draw on 26 interviews where SROs were asked to detail their typical day to investigate whether, and how, SROs criminalize school spaces through routine tasks. In spite of the multi-faceted roles of the SRO, the SROs in this study framed a typical day through a crime control logic where school spaces are criminogenic hotspots and the student body is ripe with potential offenders. School resource officer (SRO) behavior varies across schools, but little is known about what shapes their behavior. Social ecological theories state that features of communities shapes individual behavior, including police officers. This may similarly apply to SROs. This study uses the 2015 to 2016 School Survey on Crime and Safety to test the extent to which three aspects of a school's context related to behavior management (i.e., security measures, disciplinary environment, and restorative practices) shape SROs' involvement in three roles: law enforcement, teacher, and mentor. Using a generalized structural equation model to examine the relationships between school context and SRO roles, consistent with ecological theories, we find that school context shapes SRO roles. Abstract

An emerging clinical issue associated with immune-oncology agents is the collateral effects on the tolerability of concomitant medications. One report of this phenomenon was the increased incidence of hypersensitivity reactions observed in patients receiving concurrent immune checkpoint inhibitors (ICIs) and sulfasalazine (SLZ). Thus, the aim of this study was to characterize the T cells involved in the pathogenesis of such reactions, and recapitulate the effects of inhibitory checkpoint blockade on de-novo priming responses to compounds within in vitro platforms. A regulatory competent human dendritic cell/T-cell coculture assay was used to model the effects of ICIs on de novo nitroso sulfamethoxazole- and sulfapyridine (SP) (the sulfonamide component of SLZ) hydroxylamine-specific priming responses. The role of T cells in the pathogenesis of the observed reactions was explored in 3 patients through phenotypic characterization of SP/sulfapyridine hydroxylamine (SPHA)-responsive T-cell clones (TCC), and assessment of cross-reactivity and pathways of T-cell activation. Augmentation of the frequency of responding drug-specific T cells and intensity of the T-cell response was observed with PD-1/PD-L1 blockade. Monoclonal populations of SP- and SPHA-responsive T cells were isolated from all 3 patients. A core secretory effector molecule profile (IFN-γ, IL-13, granzyme B, and perforin) was identified for SP and SPHA-responsive TCC, which proceeded through Pi and hapten mechanisms, respectively. Data presented herein provides evidence that drug-responsive T cells are effectors of hypersensitivity reactions observed in oncology patients administered ICIs and SLZ. Perturbation of drug-specific T-cell priming is a plausible explanation for clinical observations of how an increased incidence of these adverse events is occurring. [ABSTRACT FROM AUTHOR]
