- Abstract
In recent years, evidence has emerged for a bidirectional relationship between sleep and neurological and psychiatric disorders. First, sleep-wake disorders (SWDs) are very common and may be the first/main manifestation of underlying neurological and psychiatric disorders. Secondly, SWDs may represent an independent risk factor for neuropsychiatric morbidities. Thirdly, sleep-wake function (SWF) may influence the course and outcome of neurological and psychiatric disorders. This review summarizes the most important research and clinical findings in the fields of neuropsychiatric sleep and circadian research and medicine, and discusses the promise they bear for the next decade. The findings herein summarize discussions conducted in a workshop with 26 European experts in these fields, and formulate specific future priorities for clinical practice and translational research. More generally, the conclusion emerging from this workshop is the recognition of a tremendous opportunity offered by our knowledge of SWF and SWDs that has unfortunately not yet entered as an important key factor in clinical practice, particularly in Europe. Strengthening pre-graduate and postgraduate teaching, creating academic multidisciplinary sleep-wake centres and simplifying diagnostic approaches of SWDs coupled with targeted treatment strategies yield enormous clinical benefits for these diseases.

203. Long-Term Cognitive and Behavioral Outcomes following Resolution of Sleep Disordered Breathing in Preschool Children

- Abstract
This study aimed to determine the long term effects of resolution of SDB in preschool children, either following treatment or spontaneous recovery, on cognition and behavior. Children diagnosed with SDB at 3-5y (N = 35) and non-snoring controls (N = 25), underwent repeat polysomnography (PSG) and cognitive and behavioral assessment 3 years following a baseline study. At follow-up, children with SDB were grouped into Resolved and Unresolved. Resolution was defined as: obstructive apnea hypopnea index (OAHI) ≤1 event/h; no snoring detected on PSG; and no parental report of habitual snoring. 57% (20/35) of children with SDB received treatment, with SDB resolving in 60% (12/20). 43% (15/35) were untreated, of whom 40% (6/15) had spontaneous resolution of SDB. Cognitive reduced between baseline and follow-up, however this was not related to persistent disease, with no difference in cognitive outcomes between Resolved, Unresolved or Control groups. Behavioral functioning remained significantly worse in children originally diagnosed with SDB compared to control children, regardless of resolution. Change in OAHI did not predict cognitive or behavioral outcomes, however a reduction in nocturnal arousals, irrespective of full resolution, was associated with improvement in attention and aggressive behavior. These results suggest that resolution of SDB in preschool children has little effect on cognitive or behavioral outcomes over the long term. The association between sleep fragmentation and behavior appears independent of SDB, however may be moderated by concomitant SDB. This challenges the assumption that treatment of SDB will ameliorate associated cognitive and behavioural deficits and supports the possibility of a SDB phenotype.

- Abstract
Occipital sources of resting-state electroencephalographic (EEG) alpha rhythms are abnormal, at the group level, in patients with amnesic mild cognitive impairment (MCI) and Alzheimer's disease (AD). Here, we evaluated the hypothesis that amplitude of these occipital sources is related to neurodegeneration in occipital lobe as measured by magnetic resonance imaging. Resting-state eyes-closed EEG rhythms were recorded in 45 healthy elderly (Nold), 100 MCI, and 90 AD subjects. Neurodegeneration of occipital lobe was indexed by weighted averages of gray matter density, estimated from structural MRIs. EEG rhythms of interest were alpha 1 (8-10.5 Hz) and alpha 2 (10.5-13 Hz). EEG cortical sources were estimated by low-resolution brain electromagnetic tomography. Results showed a positive correlation between occipital gray matter density and amplitude of occipital alpha 1 sources in Nold, MCI, and AD subjects as a whole group (r = 0.3, p = 0.000004, N = 235). Furthermore, there was a positive correlation between the amplitude of occipital alpha 1 sources and cognitive status as revealed by Mini Mental State Examination score across all subjects (r = 0.38, p = 0.000001, N = 235). Finally, amplitude of occipital alpha 1 sources allowed a moderate classification of individual Nold and AD subjects (sensitivity: 87.8%; specificity: 66.7%; area under the receiver operating characteristic curve: 0.81). These results suggest that the amplitude of occipital sources of resting-state alpha rhythms is related to AD neurodegeneration in occipital lobe along pathologic aging.

- Abstract
Background: For the past century, ferroalloy industries in Brescia province, Italy produced particulate emissions enriched in manganese (Mn), lead (Pb), zinc (Zn), copper (Cu), cadmium (Cd), chromium (Cr), iron (Fe), and alu- minum (Al). This study assessed metal concentrations in soil and vegetables of regions with varying ferroalloy industrial activity levels. Methods: Home gardens (n = 63) were selected in three regions of varying ferroalloy plant activity durations in Brescia province. Total soil metal concentration and extractability were measured by X-Ray Fluorescence (XRF), aqua regia extraction, and modified Community Bureau of Reference (BCR) sequential extraction. Unwashed and washed spinach and turnips cultivated in the same gardens were analyzed for metal concentrations by flame atomic absorption spectrometry. Results: Median soil Al, Cd, Fe, Mn, Pb, and Zn concentrations were significantly higher in home gardens near ferroalloy plants compared to reference home gardens. The BCR method yielded the most mobile soil fraction (the sum of extractable metals in Fractions 1 and 2) and all metal concentrations were higher in ferroalloy plant areas. Unwashed spinach showed higher metal concentrations compared to washed spinach. However, some metals in washed spinach were higher in the reference area likely due to history of agricultural product use. Over 60% of spinach samples exceeded the 2- to 4-fold Commission of European Communities and Codex Alimentarius Commission maximum Pb concentrations, and 10% of the same spinach samples exceeded 2- to 3-fold maximum Cd concentrations set by both organizations. Turnip metal concentrations were below maxi- mum standard reference values. Conclusions: Prolonged industrial emissions increase median metal concentrations and most soluble fractions (BCR F1 + F2) in home garden soils near ferroalloy plants. Only few studies are available on sleep disorders in Angelman syndrome (AS), a neurodevelopmental disorder with several behavior disturbances. The aim of this study was to determine the prevalence of sleep disorders in a relatively large group of AS subjects, compared to that of age-matched controls. Fourty-nine consecutive parents of patients with AS (26 males and 23 females aged 2.3–26.2 years) were interviewed and filled out a comprehensive sleep questionnaire. Based on their genetic etiology, four groups were defined: deletion of chromosome 15q11–13 (25 subjects); methylation imprinting mutation (six subjects), UBE3A mutations (seven subjects) and paternal uniparental disomy (five subjects). In the remaining cases genetic testings were negative. A significantly high frequency of disorders of initiating and maintaining sleep, prolonged sleep latency, prolonged wakefulness after sleep onset, high number of night awakenings and reduced total sleep time were found in our AS patients, as compared to age-matched controls. We also found other types of sleep disorders, never reported before, such as enuresis, bruxism, sleep terrors, somnambulism, nocturnal hyperkinesia, and snoring. No differences were found between the four genetic aetiology groups. Moreover, we did not find important improvement of sleep disturbances from pre-pubertal to post-pubertal ages. Our data confirm the significant presence of sleep/wake rhythms fragmentation, peculiar of AS, and also demonstrate the presence of several other types of sleep disturbances in this syndrome.

In the framework of a research program on innovative safety systems, ENEA has in progress a R&D activity aimed at developing a device able to increase the reliability of in-pool Heat Exchanger passive Residual Heat Removal systems.This device, so-called PERSEO (in-Pool Energy Removal System for Emergency Operation), enables the control of the removed power in the pool avoiding the installation of valve on pressurized loops, thus reducing thermal-mechanical constraints and thermal-hydraulic instability.Demonstration tests were performed in the PERSEO facility installed at SIET thermal-hydraulic research centre in Piacenza (Italy).This paper deals with the description of PERSEO experimental facility and its operating conditions, and presents the most significant experimental results, compared with the analytical pre-test ones obtained with Relap5 code, providing material to discuss the validity of this device.

The design of Advanced Light Water Reactors is characterized by the implementation of innovative passive safety systems in order to improve and simplify safety. These systems rely on natural phenomena (gravity injection, natural circulation, etc.) to improve their reliability and performance. Decay Heat Removal (DHR) is one of the main safety functions covered in a passive way by implementing in-pool heat exchangers, which require the actuation of isolation valves on the primary or secondary loop of LWRs to initiate their function. The important thermal-mechanical constrains that affect these valves can impair the reliability of such systems.ENEA (Ente per le Nuove Tecnologie, l'Energia e l'Ambiente) in collaboration with SIET (Società Informazioni Esperienze Termoidrauliche) developed an innovative device, so-called PERSEO (in-Pool Energy Removal System for Emergency Operation), that enables the control of the removed power by modifying the thermal exchange conditions in the pool, thus avoiding the installation of mechanical valves on the primary circuit and improving the system reliability. This device represents an evolution of the Thermal Valve device, already studied by ENEA and CEA within the framework of a EU FISA Project.PERSEO consists of two pools connected at the bottom and top, one containing the heat exchanger and the other full of cold water. During normal operation the former one is empty as the valve on the bottom connecting line is closed; thus inhibiting the heat removal from primary side. The valve opening starts the Decay Heat Removal function that relies on the natural circulation establishing between the two pools. The steam produced in the first pool, through a steam duct, is driven and condensed in the second one, which guarantees the water reserve for long term energy removal. An injector is installed at the exit of the steam duct inside the second pool for improving the water mixing and avoiding thermal stratification phenomena. Full-scale component and integral tests were performed at SIET thermal-hydraulic research center in Piacenza (Italy) in order to study specific phenomena and to verify the validity and performance of PERSEO.A complete assessment of the reliability and efficiency of a DHR system implementing the PERSEO device requires a numerical analysis of the overall plant response to a selection of accidental scenarios. Industrial system code like CATHARE (best estimate French code) can be used to this purpose. The experimental data acquired in the test campaign have allowed the development and qualification of a CATHARE one-dimensional model of PERSEO. This paper after describing the full-scale tests and the CATHARE model presents the satisfactory results of the post-test analysis and provides a better understanding of the limitations of the one-dimensional model in predicting all the physical phenomena involved.

An updated search for heavy narrow resonances decaying to muon or electron pairs using the CMS detector is presented. Data samples from pp collisions at √s = 7 TeV and 8 TeV at the LHC, with integrated luminosities of up to 5.3 and 4.1 fb-1, respectively, are combined. No evidence for a heavy narrow resonance is observed. The analysis of the combined data sets excludes, at 95% confidence level, a Sequential Standard Model Z'SSM resonance lighter than 2590 GeV, a superstring-inspired Z'y lighter than 2260 GeV, and Kaluza–Klein gravitons lighter than 2390 (2030) GeV, assuming that the coupling parameter k/MPl is 0.10 (0.05). These are the most stringent limits to date. Data samples from pp collisions at root s = 7 TeV and 8 TeV at the LHC, with integrated luminosities of up to 5.3 and 4.1 fb(-1), respectively, are combined. No evidence for a heavy narrow resonance is observed. The analysis of the combined data sets excludes, at 95% confidence level, a Sequential Standard Model Z '(SSM) resonance lighter than 2590 GeV, a superstring-inspired Z '(psi) lighter than 2260 GeV, and Kaluza-Klein gravitons lighter than 2390 (2030) GeV, assuming that the coupling parameter k/(M) over bar (Pl) is 0.10 (0.05). Since 1994 the International Fusion Materials Irradiation Facility is under development. Up till now only design activities have been performed aimed at providing a reference design, evaluating remaining design uncertainty, reducing the costs and the key technology risk factors to reach the specified requirements with sufficient availability and reliability. From the beginning ENEA is engaged in the design of all the systems. In particular for the Lithium Target System, its activities are mainly focused on risk analysis, transient analysis, thermal-hydraulics and stability of lithium jet. This paper deals with the analysis of the behaviour of the Lithium Target System under normal and incident conditions, performed with a version of the RELAP5/Mod3.2 code modified to allow for specific features of the system itself (Lithium and organic oil as cooling fluids).

Within the framework of its program on innovative safety systems, ENEA has in progress an R&D activity aimed at increasing the reliability of Decay Heat Removal (DHR) passive systems that implement in-pool heat exchangers. At present this activity concerns the PERSEO project aimed at the development of a system that enables the control of the removed power in the pool instead of the installation of mechanical valve on pressurized loops, thus reducing thermalmechanical constrains and thermal-hydraulic instability. PERSEO (in-Pool Energy Removal System for Emergency Operation), which is an evolution of the Thermal Valve concept studied by ENEA and CEA (Fr), will be tested at SET laboratories in Piacenza (Italy) by modifying the existing PANTHERS IC-PCC utilised in the past for testing a full scale module of the SBWR |in-pool heat exchanger. This paper is mainly conceived to compare some pre-tests calculations performed with RELAP and CATHARE of this system in order to confirm the validity of the proposed configuration and to better understand the limitations of the two different analytical models.

The PERSEO project (in-Pool Energy Removal System for Emergency Operation) is part of a research activity on cooling systems, suitable to remove the decay heat from the primary side of a LWR by means of pool immersed heat exchangers. Two examples of such systems are the GE-SBWR IC and the AP-600 PRHR. The PERSEO project is aimed at investigating the possibility of moving the high pressure primary side triggering valve, present in the above said systems, to the atmospheric pressure pool side, in order to increase the system reliability by reducing the presence of active devices on the reactor primary circuit.

The SIP-1 apparatus (Sistema di Iniezione Passiva) was conceived, designed, numerically simulated and tested by the SIET company as an innovative depressurisation and make-up device for the New Generation LWRs. In particular it is suitable to cope with those accidents where pressure in the circuit must be dumped to allow low pressure injection systems to intervene. ENEA sponsored all the research activity, starting from the SIP-1 design, its numerical simulation with the Relap5 code, the realisation of an experimental facility up to the test execution and post-test calculations. An experimental campaign on the GEST-SIP1 facility was performed in July 2000. The facility is mainly constituted by an U-tube Steam Generator simulator (1:1 elevation and 1:50 volume scaling factors), utilised as pressure vessel, which a proper model of SIP-1 apparatus has been connected to. A series of LOCAs was simulated by varying the break size and location and different steady conditions were investigated to verify the stability of SIP-1, the lack of unexpected interventions and the actuation modalitie. This paper deals with the description of the GEST-SIP1 experimental facility, the SIP-1 operating principles, the most meaningful results of the tests and the capability of the Relap5 code in reproducing phenomena and events in the post-test analysis of a test.

The main peculiarity of SIP-1 is the capability of depressurising a system by cold water injection, rather than by discharging mass to the outlet, as in the common depressurisation systems. The water injection is passively actuated on low level occurrence in the Reactor Pressure Vessel or in the Steam Generator secondary side. Moreover the simple geometrical layout of the system, constituted by a limited number of components and pipes, makes it interesting for future applications. ENEA sponsored all the research activity, starting from the SIP-1 design, its numerical simulation with the Relap5 code, the realisation of an experimental facility up to the test execution and post-test calculations. This paper deals with the description of the GEST-SIP1 experimental facility, the SIP-1 operating principles, the most meaningful results of the tests performed in July 2000 the capability of the Relap5 code in reproducing phenomena and events. All results confirm the effectiveness of SIP-1 capabilities to depressurise and refill the system other than its stability during the normal operating conditions of the plant.

Recently, several genome-wide association studies (GWASs) have led to the discovery of nine new loci of genetic susceptibility in Alzheimer's disease (AD). However, the landscape of the AD genetic susceptibility is far away to be complete and in addition to single-SNP (single-nucleotide polymorphism) analyses as performed in conventional GWAS, complementary strategies need to be applied to overcome limitations inherent to this type of approaches. We performed a genome-wide haplotype association (GWHA) study in the EADI1 study (n=2025 AD cases and 5328 controls) by applying a sliding-windows approach. After exclusion of loci already known to be involved in AD (APOE, BIN1 and CR1), 91 regions with suggestive haplotype effects were identified. In a second step, we attempted to replicate the best suggestive haplotype associations in the GERAD1 consortium (2820 AD cases and 6356 controls) and observed that 9 of them showed nominal association. In a third step, we tested relevant haplotype associations in a combined analysis of five additional case–control studies (5093 AD cases and 4061 controls). We consistently replicated the association of a haplotype within FRMD4A on Chr.10p13 in all the data set analyzed (OR: 1.68; 95% CI: (1.43–1.96); P=1.1 × 10−10). We finally searched for association between SNPs within the FRMD4A locus and Aβ plasma concentrations in three independent non-demented populations (n=2579). We reported that polymorphisms were associated with plasma Aβ42/Aβ40 ratio (best signal, P=5.4 × 10−7). In conclusion, combining both GWHA study and a conservative three-stage replication approach, we characterised FRMD4A as a new genetic risk factor of AD.

Moreover, it underlines the SPES facility capability to simulate IB-LOCAs in addition to SB-LOCAs and other kind of transients which it was designed for.

Background: The optimal management of patients with cryptogenic ischemic stroke found to have a patent foramen ovale (PFO) at diagnostic workup remains unclear. The aims of this observational multicenter study were to evaluate: (1) the risk of recurrent cerebrovascular events in patients with cryptogenic minor ischemic stroke or transient ischemic attack (TIA) and PFO who either underwent percutaneous PFO closure or received only medical treatment, and (2) the risk factors associated with recurrent events. Methods: Consecutive patients (aged 55 years or less) with first-ever cryptogenic minor ischemic stroke or TIA and PFO were recruited in 13 Italian hospitals between January 2006 and September 2007 and followed up for 2 years. Results: 238 patients were included in the study (mean age 42.2 ± 10.0 years; 118 males); 117 patients (49.2%) received only antithrombotic therapy while 121 patients underwent percutaneous PFO closure (50.8%). Stroke as the qualifying event was more common in the medical treatment group (p = 0.01). The presence of atrial septal aneurysm and evidence of 20 bubbles or more on transcranial Doppler were more common in the PFO closure group (p = 0.002 and 0.02). Eight patients (6.6%) experienced a nonfatal complication during PFO closure. At the 2-year follow-up, 17 recurrent events (TIA or stroke; 3.6% per year) were observed; 7 of these events (2.9% per year) occurred in the percutaneous PFO closure group and 10 events (4.2% per year) in the medical treatment group. The rate of recurrent stroke was 0.4% per year in patients who underwent percutaneous closure (1 event) and 3.4% per year in patients who received medical treatment (8 events). On multivariate analysis, percutaneous closure was not protective in preventing recurrent TIA or stroke (OR = 0.1, 95% CI = 0.02–1.5, p = 0.1), while it was barely protective in preventing recurrent stroke (OR = 0.1, 95% CI = 0.0–1.0, p = 0.053). Conclusions: The results of this observational, nonrandomized study suggest that PFO closure might be superior to medical therapy for the prevention of recurrent stroke. Periprocedural complications were the trade-off for this clinical benefit. Controlled randomized clinical trials comparing percutaneous closure with medical management are required.

The only established genetic determinant of non-Mendelian forms of Alzheimer's disease (AD) is the epsilon4 allele of the apolipoprotein E gene (APOE). Recently, it has been reported that the P86L polymorphism of the calcium homeostasis modulator 1 gene (CALHM1) is associated with the risk of developing AD. In order to independently assess this association, we performed a meta-analysis of 7,873 AD cases and 13,274 controls of Caucasian origin (from a total of 24 centers in Belgium, Finland, France, Italy, Spain, Sweden, the UK, and the USA). Our results indicate that the CALHM1 P86L polymorphism is likely not a genetic determinant of AD but may modulate age of onset by interacting with the effect of the epsilon4 allele of the APOE gene.

- Abstract
Clinical assessment and management of sleep disturbances in patients with mild cognitive impairment and dementia has important clinical and social implications. Poor sleep results in an increased risk of morbidities and mortality in demented patients and is a source of stress for caregivers. Sleep disturbances show high prevalence in mild cognitive impairment and dementia patients and they are often associated one to another in the same patient. A careful clinical evaluation of sleep disorders should be performed routinely in the clinical setting of individuals with cognitive decline. The Sleep Study Group of the Italian Dementia Research Association (SINDem) reviewed evidence from original research articles, meta-analyses and systematic reviews published up to December 2013. The evidence was classified in quality levels (I, II, III) and strength of recommendations (A, B, C, D, E). Where there was a lack of evidence, but clear consensus, good practice points were provided. These recommendations may not be appropriate for all circumstances and should therefore be adopted only after a patient's individual characteristics have been carefully evaluated.

In 2007, the American Academy of Sleep Medicine (AASM) completed a new manual for the scoring of sleep and associated events. The AASM manual is divided into separate sections relative to the parameters reported for polysomnography. The present commentary, accomplished by a Task Force of the Italian Association of Sleep Medicine, focuses on sleep scoring data, arousal rules, movement and respiratory events. Comparisons with the previous Rechtschaffen and Kales system are detailed and a number of methodological weaknesses are pointed out. Major comments address the 30-s scoring epochs, the restrictive approach to arousals and EEG activating patterns, the incomplete quantification of motor events and the thresholds for the definition of hypopnea. Since the new AASM manual is an iterative process, proposals for discussion and re-examination of the agreed criteria with other national and international organizations are encouraged.
