Jejuno-ileal diverticula are usually multiple and mainly localized to the proximal jejunum. Usually these diverticula are asymptomatic but can also present with complications including small bowel obstruction, diverticular bleeding, and diverticulitis. The management of small bowel diverticula is based on the clinical presentation. Here, we report a case where jejunal diverticulum caused liver indentation which required liver resection. 232. Effectiveness of a topical local anaesthetic spray as analgesia for dressing changes: A double-blinded randomised pilot trial comparing an emulsion with an aqueous lidocaine formulation

Abstract

Background: Partial thickness skin graft wounds are painful. Topically applied lidocaine has been used for analgesia in several clinical trials. This study compared the effectiveness of two different formulations of topical local anaesthetic for dressing changes of partial thickness skin graft donor sites.Methods: A double-blind randomised controlled, pilot trial was conducted in 29 patients undergoing split thickness skin graft surgery. Subjects were randomised to either a 3% lidocaine emulsion formulation ‘‘Treatment E’’ (NOPAYNETM) or a 4% aqueous solution ‘‘Treatment A’’ (XylocaineTM). Subjects received one spray per 3 cm2 of donor site area followed by up to two further spays as required. Endpoints included pain intensity measured by the numerical rating scale (NRS) up to 1 h after dressing change commencement, sting sensation, overall satisfaction and lidocaine plasma concentration.Results: The 60 min pain scores for E and A were 1.3 ±0.3 (mean ±SEM) and 1.8 ±0.4 (ƿ = 0.98) respectively. Nearly 90% of patients were very satisfied with their treatment. The mean plasma concentrations of lidocaine for A and E were 0.132 mg/l and 0.040 mg/l respectively (ƿ = 0.069).Conclusion: The topical local anaesthetic formulations achieved low pain scores during dressing changes. The safety profile was potentially improved with the emulsion formulation of lidocaine.

Abstract

Currently majority of epidemiological studies and research on menstrual disorders across the countries indicate that menstrual hygiene is prime concern for every country for their women empowerment. As government and many nongovernmental organizations are serving for it continuously but still it needs support form community pharmacist for better education and awareness amongst adult women. The current study was done with an intention to study and made awareness about menstrual hygiene in working women of silvassa region. There were 3 basic parameters like basic information about menstruation, awareness on menstrual hygiene, source of information were assessed during the study. The study was randomized open label questionnaire based feedback analysis study by using percentage analysis tool for data analysis. The questionnaire was prepared under the supervision of clinical pharmacist. From the analysis of result 90% participants had knowledge about menstruation and it's caused by physiological and hormones induced condition. 80% of participants have regular periods and bleeding. But only 72% have symptoms related knowledge in menstruation. 88% were not aware regarding world menstrual hygiene day. Only 41 % are carrying precautionary sanitary napkins with them. These results indicate participants know the basics regarding menstruation but lack of information regarding hygiene and precautionary measures available this days. Ventricular pseudoaneurysm is an uncommonly encountered complication of myocardial infarction (MI) in the era of percutaneous coronary intervention. Its presentation can be very non-specific, and diagnosis requires a high index of suspicion. Urgent surgical repair is generally warranted to prevent potentially catastrophic complications. We present a case of patient who presented several days after his index MI. He was ultimately diagnosed with a ruptured pseudoaneurysm, and despite best efforts had a complicated hospital course. 247. Visceral transplantation in patients with intestinal‐failure associated liver disease: Evolving indications, graft selection, and outcomes

Abstract

Intestinal failure (IF)‐associated liver disease (IFALD) is widely recognized as a lethal complication of long‐term parenteral nutrition. The pathophysiology of IFALD is poorly understood but appears to be multifactorial and related to the inflammatory state in the patient with IF. Visceral transplant for IFALD includes variants of intestine, liver, or combined liver–intestine allografts. Graft selection for an individual patient depends on the etiology of IF, abdominal and vascular anatomy, severity of IFALD, and potential for intestinal rehabilitation. The past decade has witnessed dramatic improvement in the management of IFALD, principally due to improved lipid emulsion formulations and the multidisciplinary care of the patient with IF. As the recognition and treatment of IFALD continue to improve, the requirement of liver‐inclusive visceral grafts appears to be decreasing, representing a paradigm shift in the care of the patient with IF. This review highlights the current indications, graft selection, and outcomes of visceral transplantation for IFALD. In this minireview, the authors describe how intestinal‐failure associated liver disease (IFALD) influences intestine transplant indications, graft selection, and outcomes, and why the recent dramatic improvement in the medical treatment of IFALD is changing the field of intestine transplantation.

We describe our technique for isolation and ligation of the small lymphatics at the root of the mesentery during the procurement of the small intestine and report our experience with the management of lymphatic leaks posttransplant.

Abstract

Paclitaxel is widely used in the treatment of patients with metastatic breast cancer (MBC). Formulations of paclitaxel contain surfactants and solvents or albumin derived from human blood. The use of co-solvents such as polyoxyethylated castor oil is thought to contribute to toxicity profile and hypersensitivity reactions as well as leaching of plasticizers from polyvinyl chloride bags and infusion sets. Currently, nab-paclitaxel, an albumin-bound paclitaxel in nanometer range continues to be the preferred taxane formulation used in clinic. This study (CTRI/2010/091/001116) investigated the efficacy and tolerability of a polyoxyethylated castor oil- and albumin-free formulation of paclitaxel [paclitaxel injection concentrate for nanodispersion (PICN)] compared with nab-paclitaxel in women with refractory MBC. The current study was a multicenter, open-label, parallel-group, randomized, comparative phase II/III trial evaluating the efficacy and safety of PICN (260 mg/m [ n = 64] and 295 mg/m [ n = 58] every 3 weeks) compared with nab-paclitaxel (260 mg/m every 3 weeks [ n = 58]) in women 18 and 70 years old with confirmed MBC. Overall response rate (ORR) was assessed with imaging every 2 cycles. An independent analysis of radiologic data was performed for evaluable patients. Progression-free survival (PFS) was a secondary efficacy measure. Independent radiologist-assessed ORRs in the evaluable population of women aged ≥70 years were 35, 49, and 43 % in the PICN 260 mg/m, PICN 295 mg/m, and nab-paclitaxel 260 mg/m arms, respectively. Median PFS in the evaluable population was 23, 35, and 34 weeks in the PICN 260 mg/m, PICN 295 mg/m, and nab-paclitaxel 260 mg/m arms, respectively. Adverse events occurred in similar proportions of patients across treatment arms. Hypersensitivity reactions were not frequently observed with the clinical use of PICN across the treatment cohorts. In women with metastatic breast cancer, PICN at 260 and 295 mg/m every 3 weeks was effective and well tolerated and showed similar tolerability compared with nab-paclitaxel 260 mg/m every 3 weeks. Statistically, significant differences were not observed in the PICN and nab-paclitaxel treatment arms for radiologist-assessed ORR or median PFS. The novel paclitaxel formulation, PICN, offers apart from efficacy, potential safety advantage of decreased use of corticosteroid pretreatment and the absence of the risk of transmission of blood product-borne disease.
