- Abstract
Overall, 70–85% of individuals will present with chronic low back pain (CLBP). Concepts for its management are multimodal and multidisciplinary. If conservative management fails, surgery becomes an option. Spinal stabilization with fusion is considered a valid option for treating CLBP refractory to conservative management. Efforts have been made to make spinal fusion more efficient as well as more accurate. In a dedicated operating room equipped with intraoperative 3D fluoroscopy and an advanced navigation system, we have demonstrated that these new technologies can help to improve pedicle screw placement during spinal fusion procedures while decreasing the drawbacks associated with imaging utilization. For the first time in humans, we have also safely and efficiently applied osteodifferentiated adipose mesenchymal stem cells to spinal fusion as a graft material. Background No definitive evidence exists on how intracranial hypertension should be treated in patients with traumatic brain injury (TBI). It is therefore likely that centers and practitioners individually balance potential benefits and risks of different intracranial pressure (ICP) management strategies, resulting in practice variation. The aim of this study was to examine variation in monitoring and treatment policies for intracranial hypertension in patients with TBI. Methods A 29-item survey on ICP monitoring and treatment was developed on the basis of literature and expert opinion, and it was pilot-tested in 16 centers. The questionnaire was sent to 68 neurotrauma centers participating in the Collaborative European Neurotrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study. Results The survey was completed by 66 centers (97% response rate). Centers were mainly academic hospitals (n = 60, 91%) and designated level I trauma centers (n = 44, 67%). The Brain Trauma Foundation guidelines were used in 49 (74%) centers. Approximately 90% of the participants (n = 58) indicated placing an ICP monitor in patients with severe TBI and computed tomographic abnormalities. There was no consensus on other indications or on peri-insertion precautions. We found wide variation in the use of first- and second-tier treatments for elevated ICP. Approximately half of the centers were classified as using a relatively aggressive approach to ICP monitoring and treatment (n = 32, 48%), whereas the others were considered more conservative (n = 34, 52%). Conclusions Substantial variation was found regarding monitoring and treatment policies in patients with TBI and intracranial hypertension. The results of this survey indicate a lack of consensus between European neurotrauma centers and provide an opportunity and necessity for comparative effectiveness research. - Abstract
Acute subdural hematoma (ASDH) is a type of intracranial haemorrhage following head impact, with high mortality rates. Bridging vein (BV) rupture is a major cause of ASDH, which is why a biofidelic representation of BVs in finite element (FE) head models is essential for the successful prediction of ASDH. We investigated the mechanical behavior of BVs in the KTH FE head model. First, a sensitivity study quantified the effect of loading conditions and mechanical properties on BV strain. It was found that the peak rotational velocity and acceleration and pulse duration have a pronounced effect on the BV strains. Both Young's modulus and diameter are also negatively correlated with the BV strains. A normalized multiple linear regression model using Young's modulus, outer diameter and peak rotational velocity to predict the BV strain yields an adjusted -value of 0.81. Secondly, cadaver head impact experiments were simulated with varying sets of mechanical properties, upon which the amount of successful BV rupture predictions was evaluated. The success rate fluctuated between 67 and 75%. To further increase the predictive capability of FE head models w.r.t. BV rupture, future work should be directed towards improvement of the BV representation, both geometrically and mechanically., QC 20170522

33 p, ill., Le Centre fédéral d’Expertise des Soins de Santé (KCE) publie un guide de pratique clinique sur la prise en charge de l’un de nos principaux « maux de civilisation », le mal de dos, qu’il soit lumbago aigu ou lombalgie chronique. Pour cerner le problème sous toutes ses facettes, les chercheurs du KCE se sont entourés de 31 experts représentant les principales professions de santé concernées. L’accent est mis sur la « démédicalisation » : guérison spontanée dans la majorité des cas, importance de rester physiquement actif, inutilité des examens d’imagerie et nécessité d’identifier les personnes pour lesquelles existe un risque élevé de passage à la chronicité afin de leur proposer d’emblée une approche spécifique.

239. Postoperative bracing after lumbar surgery: a survey amongst spinal surgeons in Belgium.

Purpose: Bracing is frequently prescribed following lumbar surgery for degenerative conditions. However, previous studies failed to demonstrate the advantage of postoperative lumbar bracing in both short- and long-term outcome in terms of pain, quality of life and fusion rate. The purpose of this study was to assess the prescription patterns and rationale for postoperative bracing amongst spinal surgeons in Belgium.Methods: A 16-item online survey was distributed by email to spinal surgeons affiliated to the Spine Society of Belgium (N = 252).Results: A total of 105 surgeons (42%) completed the survey. The overall bracing frequency following lumbar surgery was 38%. A brace was more often prescribed following the fusion procedures (52%) than after the non-fusion procedures (21%) (p < 0.0001). The majority of surgeons (59%) considered bracing after at least one type of lumbar surgery. Orthopaedic surgeons (73%) reported a significantly higher rate of prescribing postoperative bracing compared to neurosurgeons (44%) (p = 0.003). Pain alleviation (67%) was the main goal for prescribing a postoperative brace. A total of 42% of the surgeons aimed to improve fusion rate by bracing after lumbar fusion procedures. A quasi-equal level of the scientific literature (29%), personal experience (35%) and teaching from peers (36%) was reported to contribute on the attitudes towards prescribing bracing.Conclusions: Postoperative bracing was prescribed by Belgian spinal surgeons following more than one-third of lumbar procedures. This was underpinned by beliefs regarding pain alleviation and higher fusion rate. Interestingly, based on the scientific literature these beliefs have been demonstrated to be false. Cerebrovascular autoregulation (CA) capacity can be impaired in the aftermath of acute brain injuries. Altered physiological states, such as hypo- and hypercapnia, affect CA. Although these effects have been demonstrated in several animal experiments, the exact effect of PaCO₂ on the plateau of cerebral blood flow (CBF) across the spectrum of arterial blood pressures has not been fully disclosed.
