Böbrekleri aorta abdominalis’ten L1-2 seviyesinden çıkan a. renalisler besler. Bu damarlar genellikle arteria mesenterica superior’un başlangıcının hemen altında aorta’dan çıkar. İnsanlarda her böbreğin a. renalis’i genellikle bir tane olmasına rağmen bazen iki (%10-%12), üç (%1) çok nadir olarak dört ya da beş adet olduğu bildirilmiştir. Bu çalışmada çeşitli sağlık problemleri nedeniyle Erciyes Üniversitesi Tıp Fakültesi hastanesinde anjiografi ve bilgisayarlı tomografi tetkikleri yapılan 105 (77 erkek, 28 kadın) hastanın arşiv taramasında a. renalislerin değerlendirdik. İncelediğimiz 105 olgunun 85(%81)’inde her iki böbreği besleyen birer a. renalis’in olduğu, 20 (%19.0) tanesinde iki tane a. renalis olduğu saptandı. Üç veya daha fazla a. renalis’i olan böbreğe rastlanmadı. A. renalis orijinlerinin çoğunlukla sağda 56(%53.3), solda 52(%49.5) L1-L2 vertebra düzeyleri arasından çıktığı bulgularımızın yapılmış çalışmalarla paralellik gösterdiği anlaşılmıştır. Tek a. renalis’i olan böbreğin transplantasyonunda, komplikasyon ve böbrek kaybı birden fazla a. renalis’i olan böbreğe göre daha az olduğu bilinmektedir. Bu konunun öneminden dolayı a. renalis’in sayı varyasyonlarını araştırmayı amaçladık. Çalışmada elde ettiğimiz sonuçların kanaatindeyiz, Renal arteries, originates from the abdominal aorta at the level of L1-2, feed the kidneys. These vessels usually originate from aorta, just below the inception of the superior mesenteric artery. Each kidney usually has only one renal artery in human body, however, it has been reported that double (%10-%12), triple (%1) and very uncommonly 4 or 5 pieces arteries might also be seen. In this study, we retrospectively examined the anatomy of the renal arteries of 105 patients (77 male 28 female), registered to Erciyes University Medical Faculty Hospital due to various health problems and were performed angiography and computerized tomography. It was determined that each kidney had one renal artery in 85 (81%) of 105 patients and had double renal arteries in 20 cases. It has not been determined any triple or more renal arteries. Our study findings that show that origins of renal arteries commonly occur between the vertebral columns of L1-L2 on the right side 56 (53.3%) and on the left side 52 (49.5%) are concordant with the findings of previous studies. The transplantation of the kidney with single renal artery has lower risk of complication and kidney lost compared to transplantation of the kidney with more than a single artery. Thus, we aimed to study the numeral variations of the renal artery due to the importance of this subject. We believe that the result of this study might be useful for the kidney surgery

Small cell prostate cancer constitutes less than 1% of all prostate cancers and has a poor prognosis. A 60-year-old male patient presented with dysuria, pollakiuria, and nocturia of about 1-year duration.The total PSA level at admission was 47.50 ng/mL. The prostate needle biopsy result was reported as adenocarcinoma Gleason 5 + 3. The patient underwent transurethral prostate resection (TUR-P) and bilateral orchiectomy. The TUR-P pathology result was consistent with small cell neuroendocrine carcinoma. He was offered systemic chemotherapy but refused it. Examinations and tests at the third postoperative month showed diffuse liver metastasis and vertebral bone metastasis. He died at the 6 months after surgery.

A 19-year-old male patient was admitted with flank pain, which had lasted intermittently for four years. In X-ray, there was a radiopacity with a dimension of 6 × 4 cm on the left pelvic bone. Intravenous pyelography revealed a huge left megaureter with a stone in the lower end and grade V hydronephrosis. A left ureterolithotomy, left nipple ureteroneocystostomy, and psoas hitch operation was performed. A voiding cystourethrogram taken three months after the operation showed no reflux, and in IVP there was reduced dilatation of the collecting system when compared to the ureter before the operation.

Urinary bladder carcinoma is the second most common cancer of the urinary system. The recurrence rate in the upper urinary system (UUS) for urothelial cancers is around 3% following radical cystectomy. The followup generally consists of imaging studies and urinary cytology, although there are no prospective data on the frequency, the mode, and the duration of followup. In patients carefully selected according to risk factors, kidney-sparing minimally invasive methods (ureteroscopic procedures, percutaneous approach, and local drug instillation) appear as contemporary alternatives for low-grade and low-stage primary UUS. In this paper, we present the patient who underwent radical cystectomy with urinary diversion ureterocutaneostomy, was diagnosed with widespread bilateral UUS tumors and recurrent tumor at the urostomy site at active followup, for which he was given local Bacillus Calmette-Guérin (BCG) and cryotherapy, and was followed by disease-free for 2 years thereafter.

Solitary fibrous tumor is a spindle cell neoplasm mostly originating from pleura; however, it has also recently been reported to be extrapleural. A 57-year-old man presented with left lumbal pain. Ultrasonography and computed tomography showed a cystic lesion of 14 × 11 cm with solid areas and septations in middle and lower poles of the left kidney. Radical nephrectomy was performed. Immunohistochemical studies showed strong reactions with CD34 and CD99. A nuclear positivity with Ki-67 was observed in less than 1% of cells. Despite repeated stainings with vimentin, no clear tumor evaluation could be made due to artifacts. The tumor was negative with Bcl-2, desmin, HMB-45, S100, FVIII, and CD31. Histopathological and molecular studies made the diagnosis of a solitary fibrous tumor. The patient is now currently free of disease at the 26th month of followup.

Introduction: The aim of this study is to evaluate the results of patients with the isolated greater tubercul fractures (GTF) of humerus who conservatively or surgically treated. Materials and methods: 23 patients were admitted (11 women, 12 men) with GTF (54% dominant side of 24 shoulder) between 2008 and 2011. All of patient were evaluated with MRI as preoperatively soft tissue injury. There was supraspinatus muscle tear together with GTF in 3 case and their shoulder were arthroscopically assisted rotator cuff repair. Osteosynthesis was performed in 3 case with screw and fixation in 2 case with suture. GTF of 16 shoulder were conservatively treated. Shoulder function after treatment were evaluated according to Constant score (CS), Quick-DASH score (QDS) and VAS score. Results: Three patients with screw fixation had CS 68,6 points, QDS 12,8 points, VAS score 1,66. Two patients with suture fixation had 67,5 CS points, 5,1 QDS pointsp, and 2 VAS score. Three patients with rotator cuff tear repair had 82,6 CS points, 16,6 QDS points, and 2,33 VAS score. In patients with conservatively treated mean CS was 63,9 points, QDS 26,1 points, VAS 2,96 points. Conclusions: In this study, conservative and surgical treatment results were satisfactory. In terms of pain the results were excellent. In conclusion, GTFs are rare but trouble-free with a good observation and sufficient rehabilitation., Amaç: Bu çalışmanın amacı, izole humerus tüberkulum majus kırığı (TMK) bulunan hastaların konservatif ve cerrahi tedavi sonuçlarını değerlendirmektir. Gereç ve yöntem: 2008-2011 yılları arasında kliniğimize başvuran ve TMK bulunan 23 olgu (11 kadın, 12 erkek)\'nun 24 omuzu (% 54 dominant) çalışmaya dahil edildi. Tüm olguların travma sonrası MR görüntüleri ile yumuşak doku patolojileri değerlendirildi. Üç olguda TMK\'ya eşlik eden supraspinatus yırtığı tespit edildi. Kırıkla birlikte supraspinatus yırtığı tespit edilen üç olguya osteosentez yapılmaksızın rotator manşet onarımı yapıldı. Üç olguda vida ile osteosentez, iki olguda sutur (ethibond) ile kırık tespit edildi. Onbeş olgunun on altı omuzu konservatif yöntemle tedavi edildi. Tedavi sonrası omuz fonksiyonlarının değerlendirilmesinde Constant skorlaması (CS), Quick- Dash skorlaması (QDS) ve Görsel Analog Skala (VAS) kullanıldı. Bulgular: Vida ile osteosentez yapılan üç olgunun ortalama CS puanı 68,6, QDS puanı 12,8 ve VAS değerleri 1,66 olarak bulundu. TMK\' nın sutur ile yerine dikildiği iki olgunun CS puanı 67,5, Quick-DASH skoru 5,7 ve VAS değerleri 2 olarak bulundu. Rotator manşet yırtığı onarımı yapılan üç olgunun Constant skoru 82,6, Quick-DASH skoru 16,6 ve VAS değerleri 2,33 olarak tespit edildi. Konservatif olarak tedavi edilen 16 olgunun CS puanı 63,9, QDS puanı 26,1 ve VAS değerleri 2,96 puan olarak bulundu. Sonuç: Çalışmamızda konservatif ve cerrahi tedavi uygulanan olgularımızın fonksiyonel sonuçları tatminkar bulundu. Ağrı açısından sonuçlarımız ise mükemmel bulunmuştur. Sonuç olarak TMK\'lar nadir görülen, yakın takip ve yeterli rehabilitasyonla sorunsuz iyileşen yaralanmalardır.

Ortadoğu siyasal coğrafyasının en belirgin özelliklerinden biri, uzun yıllardır bölgede hüküm süren dirençli otoriter rejimlerdir. Otoriter rejimler yalnızca güncel siyaset üzerinde değil, yürüttükleri politikalarla, bölgenin ekonomik, toplumsal ve kültürel yapısı üzerinde de uzun erimli ve belirleyici etkilere sahiptir. Bu nedenle, Ortadoğu siyasetini ve toplumlarını anlayabilmek için, otoriter rejimlerin kaynaklarını ve işleyiş mekanizmalarını kavramak büyük öneme sahiptir. Bu tez, Ortadoğu'da otoriter rejimler üzerine akademik literatürün bir bilançosunu sunmayı hedeflemektedir. Otoriter rejimleri incelerken hangi kuramsal yaklaşımların benimsendiği, bu yaklaşımların literatüre olan katkıları ve sorunlu yönleri eleştirel bir değerlendirmeye tabi tutulacaktır. Böylece, Ortadoğu'da otoriter rejimleri uzun yıllar ayakta tutan kültürel, ekonomik, siyasal ve toplumsal dinamikleri ana hatlarıyla ortaya koyabileceğime inanıyorum. Bu amaç doğrultusunda, ilk olarak, mevcut literatür otoriter rejimlerin kaynakları ve rejimlerin kendilerini sürdürme stratejileri bakımından iki ana başlık halinde sınıflandırılacaktır. Literatürün sınıflandırılması ve alt başlıklar halinde incelenmesinin ardından, son bölüm, otoriter rejimler nasıl incelenmeli sorusuna cevap arayan bir tartışmaya ayrılmıştır. Bu bölümde ilk olarak, son yılların yeni otoriteryanizm tartışmaları ele alınacaktır. Yeni otoriteryanizm çalışmalarının vaatleri ve sınırlılıkları ortaya konulduktan sonra, otoriter rejimleri siyasal iktidar perspektifinden okumanın, ana akım rejim çalışmalarının sıkıştığı dar formalist bakış açısını zenginleştireceği, siyasal rejimleri ortaya çıkaran toplumsal faktörleri ve rejimlerin maddi ve sembolik yaşam stratejilerini bütün veçheleriyle kavramamıza yardımcı olabileceği ileri sürülecektir. Burada, siyasal iktidar perspektifi olarak önerilen yaklaşıma, Pierre Bourdieu'nun alan teorisi ve Michael Mann'ın siyasal iktidar üzerine geliştirdiği kavramsallaştırmalardan hareketle varılmıştır. One of the most prominent characteristics of the political geography of Middle East is durable authoritarian regimes that have been prevalent in the region for a long time. Authoritarian regimes have decisive and long run influences on economic, social and cultural structures of the region as well as contemporary politics. Therefore, it is crucial to grasp the orgins and the dynamics of the authoritarian regimes in order to understand Middle East societies and politics. The purpose of this thesis is to offer a review of the academic literature on the Middle East authoritarian regimes. A critical appraisal will be held with respect to theoretical approaches which have been employed to analyse authoritarian regimes in the Middle East in terms of their contributions to the literature and their shortcomings. Hence, I hope, I can figure out the economic, political and social dynamics that lie behind the Middle East authoritarian regimes. To this end, the extant literature will be classified into two main headings with regard to origins and survival strategies of the authoritarian regimes. Having classified the literature, the last part of this thesis is devoted to a discussion on how can we scrutinize the political regimes. First, recent debates on the new authoritarianism in comparative politics will be evaluated. After evaulating the promises and limits of the new authoritarianism literature, it will be argued that analysing authoritarian regimes in the light of political power approach might widen and deepen the narrow perspective of mainstream regime studies. Furthermore, a power oriented approach might contribute to apprehending the origins and, material and symbolic survival strategies of political regimes. The political power approach that will be offered in the last chapter derives from the field theory of Pierre Bourdieu and power concepts of Michael Mann. 155
