Abstract

Objectives: This study sought to investigate gender-related differences in patients with atrial fibrillation (AF) in Europe. Background: Gender-related differences may play a significant role in AF. Methods: We analyzed the data of 5,333 patients (42% female) enrolled in the Euro Heart Survey on Atrial Fibrillation. Results: Compared with men, the women were older, had a lower quality of life (QoL), had more comorbidities, more often had heart failure (HF) with preserved left ventricular systolic function (18% vs. 7%, p < 0.001), and less often had HF with systolic dysfunction (17% vs. 26%, p < 0.001). Among patients with typical AF symptoms (56% of women, 49% of men), there was no gender-related difference in the choice of rate or rhythm control. Among patients with atypical or no symptoms (44% of women, 51% of men), women less frequently underwent rhythm control (39% vs. 51%, p < 0.001) than did men. Women underwent less electrical cardioversion (22% vs. 28%, p < 0.001). Prescription of oral anticoagulants was identical (65%) in both genders. One-year outcome was similar except that women had a higher chance for stroke (odds ratio 1.83 in multivariable regression analysis, p = 0.019). Conclusions: Women with AF had more comorbidities, more HF with preserved systolic function, and a lower QoL than men. In the large group with atypical or no symptoms, women were treated appropriately more conservatively with less rhythm control than men. Women had a higher chance for stroke. Long-term QoL changes and other morbidities and mortality were similar. © 2007 American College of Cardiology Foundation.

Abstract

Background The aim of the study was to assess the effect on cardiac events of adding perindopril 8 mg once daily to standard preventive therapy in the subgroup of EUROPA patients with previous revascularization and without previous myocardial infarction (MI). Methods We conducted a subgroup analysis of the EUROPA study patients according to their revascularization and previous MI history. Among the 12218 patients of EUROPA, we identified 6709 (54.9%) patients who had a previous revascularization. Approximately equal proportions had undergone percutaneous coronary intervention (3122) or coronary artery bypass grafting (3136). Of the revascularized patients, 3047 (24.9%) patients had not experienced a previous MI. Results Out of the 6709 revascularized patients, 3340 were treated with perindopril and 3369 with placebo. Baseline characteristics were similar to the whole EUROPA population in terms of demographics, medical history, physical examination (heart rate, blood pressure), and medications at screening. The mean patient age was 60 years, and 85% were men. The relative risk reduction with perindopril 8 mg was 17.3% (95% CI 1.3%-30.8%, P = .035) for the composite primary end point of cardiovascular death, nonfatal MI, and resuscitated cardiac arrest and was 23% (95% CI 4.9%-37.6%, P = .015) for fatal and nonfatal MI. In the 3047 revascularized patients without a history of MI, perindopril was associated with a relative risk reduction of 31.7% for fatal and nonfatal MI (95% CI 4.4%-51.2%, P = .026). Conclusion Perindopril 8 mg daily is beneficial for primary and secondary prevention of cardiac events in patients with coronary artery disease without clinical evidence of heart failure including those with previous revascularization.

Abstract

BACKGROUND: The EUropean trial on Reduction Of cardiac events with Perindopril in stable coronary Artery disease (EUROPA) trial has recently reported. OBJECTIVE: To assess the cost effectiveness of perindopril in stable coronary heart disease in the UK. METHODS: Clinical and resource use data were taken from the EUROPA trial. Costs included drugs and hospitalisations. Health-related quality of life values were taken from published sources. A cost-effectiveness analysis is presented as a function of the risk of a primary event (non-fatal myocardial infarction, cardiac arrest or cardiovascular death) in order to identify people for whom treatment offers greatest value for money. RESULTS: The median incremental cost of perindopril for each quality-adjusted life year (QALY) gained across the heterogeneous population of EUROPA was estimated as 9700 pounds(interquartile range 6400-14,200 pounds). Overall, 88% of the EUROPA population had an estimated cost per QALY below 20,000 pounds and 97% below 30,000 pounds. For a threshold value of cost effectiveness of 30,000 pounds per QALY gained, treatment of people representing the 25th, 50th (median) and 75th centiles of the cost effectiveness distribution for perindopril has a probability of 0.999, 0.99 and 0.93 of being cost effective, respectively. Cost effectiveness was strongly related to higher risk of a primary event under standard care. CONCLUSIONS: Whether the use of perindopril can be considered cost effective depends on the threshold value of cost effectiveness of healthcare systems. For the large majority of patients included in EUROPA, the incremental cost per QALY gained was lower than the apparent threshold used by the National Institute for Health and Clinical Excellence in the UK.

Abstract

When the first official version of the WRB was released at the 17th World Congress of Soil Science at Montpellier in 1998, the fortunate decision was taken to freeze the system for at least eight years in order to allow thorough reflection and field data collection. Since then, the WRB has been tested worldwide and has proven to be very valuable. However, a lot of time was spent discussing the necessity either to add/delete qualifiers to Reference Soil Groups, their ranking, and the ultimate rationale behind the WRB system. This paper summarizes the discussions on the Reference Groups and the qualifiers and provides a rationale for their ranking. It appears that a unified system of ranking rigorously applied to all WRB Reference Groups makes little sense. Therefore, a logical ranking system is implemented that adjusts to the very character of the different Reference Groups while respecting, as much as possible, some general principles for overall ranking.

Abstract

The objective of this paper is to elaborate on soil surveys in view of land evaluation, with emphasis on how soil surveys can meet the information demand for land evaluation. The paper starts by explaining what soil survey and land evaluation is about. Elements of soil survey and land evaluation relating to scale and purpose are discussed. The main focus will be on the linkage between soil survey, soil survey interpretation and physical land evaluation. Soil classification is a key component of soil surveys. The International Union of Soil Sciences (IUSS)-endorsed soil classification system "World Reference Base for Soil Resources" is briefly elaborated upon as a means to summarize the wealth of information from soil profiles for the purpose of land evaluation. The different steps from soil parameters, to land characteristics and land qualities for use in land evaluation are explained. The theory is illustrated by a worked example: land evaluation for irrigated rice in the West-African Sahel, wherein the Fertility Capability Classification (FCC) system is used to make the bridge between soil and land information

Abstract

In collaboration with farmers, rice varieties were evaluated under small-scale irrigation in two villages of south east Tanzania for two consecutive cropping seasons (1999/2000 –2000/2001). The objectives were to give farmers access to new improved rice varieties; to identify the selection criteria farmers consider important in irrigated rice production; and to come to grips with their arguments. Farmers were provided with eleven improved varieties, which they compared with their own ones. Farmers' preferred varieties with short to medium maturity period, which produce many tillers and mature uniformly; and with long translucent aromatic grains for their own use and marketing. This study identified qualitative and quantitative evaluation criteria which farmers are using for selecting rice varieties. The implication for further research on rice in south east Tanzania is that the breeding programme should incorporate these attributes to address farmers' preferences, rather than to go for absolute maximum yield levels.

Abstract

Despite demonstrated environmental and economic benefits, the adoption of conservation agriculture (CA) often remains lower than expected. Adoption of a complex and composite technology such as CA requires a thorough understanding of site-specific determinants of and constraints in the adoption process. In this paper, we investigate the adoption of CA among medium-scale farmers in the Mexican Bajío using farm survey data. We differentiate between full and partial adoption of composite technology, and disentangle the different steps in the adoption process, including awareness, using Heckman selection models. The results indicate that institutional factors and farmers’ education affect awareness of CA but not adoption itself. Farm size is not an important determinant in the adoption process but farmers’ tenancy position does affect both awareness and adoption. A main constraint for full but not for partial adoption is livestock ownership, which complicates residue retention on the field, as crop residues are baled for fodder.

Abstract

Thoracic aortic aneurysms and dissections (TAAD) is a serious condition with high morbidity and mortality. It is estimated that 20% of non-syndromic TAAD cases are inherited in an autosomal-dominant pattern with variable expression and reduced penetrance. Mutations in myosin heavy chain 11 (MYH11), one of several identified TAAD genes, were shown to simultaneously cause TAAD and patent ductus arteriosus (PDA). We identified two large Dutch families with TAAD/PDA and detected two different novel heterozygote MYH11 variants in the probands. These variants, a heterozygote missense variant and a heterozygote in-frame deletion, were predicted to have damaging effects on protein structure and function. However, these novel alterations did not segregate with the TAAD/PDA in 3 out of 11 cases in family TAAD01 and in 2 out of 6 cases of family TAAD02. No mutation was detected in other known TAAD genes. Thus, it is expected that within these families other genetic factors contribute to the disease either by themselves or by interacting with the MYH11 variants. Such an oligogenic model for TAAD would explain the variable onset and progression of the disorder and its reduced penetrance in general. We conclude that in familial TAAD/PDA with an MYH11 variant in the index case caution should be exercised upon counseling family members. Specialized surveillance should still be offered to the non-carriers to prevent catastrophic aortic dissections or ruptures. Furthermore, our study underscores that segregation analysis remains very important in clinical genetics. Abstract

AIMS: To determine the prevalence of heart failure and symptomatic as well as asymptomatic left ventricular systolic dysfunction in the general population.METHODS AND RESULTS: In 5540 participants of the Rotterdam Study (age 68.9+/-8.7 years, 2251 men) aged 55-95 years, the presence of heart failure was determined by assessment of symptoms and signs (shortness of breath. ankle oedema and pulmonary crepitations) and use of heart failure medication. In 2267 subjects (age 65.7+/-7.4 years, 1028 men) fractional shortening was measured. The overall prevalence of heart failure was 3.9% (95% CI 3.0+/-4.7) and did not differ between men and women. The prevalence increased with age, with the exception of the highest age group in men. Fractional shortening was higher in women and did not decrease appreciably with age. The prevalence of left ventricular systolic dysfunction (fractional shortening CONCLUSIONS: The prevalence of heart failure is appreciable and does not differ between men and women. The majority of persons with left ventricular systolic dysfunction can be regarded as having asymptomatic left ventricular systolic dysfunction.

Abstract

1. Many man-made reservoirs in the semi-arid highlands of Northern Ethiopia (Tigray) are characterised by the occurrence of intensive blooms of cyanobacteria and a dominance of small riverine fishes belonging to the genus Garra. 2. We carried out enclosure experiments to test for the effect of these small fish on abiotic characteristics, phytoplankton biomass and zooplankton community structure in the pelagic of two reservoirs (Gereb Awso and Tsinkanet). Two experiments were carried out in each of the reservoirs, one at the end of the rainy season (highest water level) and one at the end of the dry season (lowest water level). 3. The presence of Garra in general increased the amount of suspended matter, nutrient concentrations (total nitrogen and total phosphorus), phytoplankton and Microcystis biomass (including the proportion of Microcystis in the phytoplankton community), and reduced water transparency. The positive effect of the presence of Garra on nutrient concentrations and phytoplankton productivity indicate that Garra has the potential to affect food web functioning indirectly through bottom-up effects, by enhancing nutrient concentrations through sediment resuspension and excretion of nutrients. Indeed, population densities of the cladoceran zooplankton taxa Ceriodaphnia and Diaphanosoma also showed an overall increase in enclosures with Garra. 4. However, our data also provide some evidence for a potential of Garra to exert top-down control on large bodied daphnids (Daphnia carinata, D. barbata), although such effect varied among experiments. The limited capability of Garra to control zooplankton communities mainly reflects the low efficiency of these small, riverine and benthos-oriented fish in foraging on zooplankton and suggests the existence of an unoccupied niche for zooplanktivorous fish in the majority of the reservoirs. 5. Abstract

The semi-arid highlands of Northern Ethiopia (Tigray) have numerous small reservoirs that have been created by microdams in an attempt to provide water supply for irrigation and livestock drinking. Although the reservoirs have substantial added value to residents, their use as water resource is jeopardized by eutrophication and a high occurrence of blooms of toxic cyanobacteria. So far, there is no systematic information available on the limnological and aquatic ecological characteristics of these dams. We carried out a standardized survey of 32 reservoirs and assessed a wide set of morphometric, abiotic and biotic variables. The sampling was performed during two seasons, September-October 2004 (end of the wet season/start of the dry season) and April-May 2005 (towards the end of the dry season). Using multivariate analysis, we revealed dominating patterns of variable associations and compared the variability in these patterns among seasons. According to standardized PCA and RDA analyses, the most important axis of variation was mainly represented by a gradient in nutrients and altitude that was also positively associated with phytoplankton biomass, suspended matter and oxygen concentration, and negatively with water transparency. For most variables, correlations between the wet and dry season were weak, which suggests that individual reservoirs behaved rather differently in their response to seasonal changes. Nevertheless, a Mantel correlation (r = 0.32; P = 0.035) showed a weak but significant overall concordance in the variable association patterns among seasons. A number of reservoirs became very shallow or fell dry in the dry season, a process that was associated with an increase in suspended matter and conductivity and a decrease in transparency. These reservoirs contained lower amounts of fish and tended to be less eutrophic than the deeper, permanent systems, as they had lower levels of phosphorus and chlorophyll-a. Abstract

Land reclamation by rock removal has wide potential in areas with shallow or rocky soils. In northwest Syria, this practice is hindered in its implementation by a lack of physical soil suitability data, principally soil rockiness and soil depth to hard rock. These soil properties were surveyed in a limited study area, resulting in a hard-boundary thematic soil map (64-94% accuracy per property). Bayesian inference is proposed as a low-cost upscaling method that yields a set of pixel-based probability maps, providing improved input for spatial decision support models. Whereas the achieved spatial upscaling (from 2510 to 19 100 ha) outweighed the decrease in overall accuracy (down to 26-57%), probability maps require dedicated validation and manipulation procedures. This research contributes to methods for the creation, validation and interpretation of probabilistic soil property maps for quantitative land evaluation. First, we evaluated three postprocessing and validation methods for probabilistic soil property maps, identifying the use of 'prediction rates' as the best approach in a spatial planning context. Next, we demonstrated how the maps can support decision-making for land management activities by simulating the expected losses and gains from interventions, in a decision-theoretic approach. Based on the simulations, the investments in large-scale derocking projects in northwest Syria will pay off in terms of increased agricultural productivity in less than 10 years.
