Abstract

Background: Coronavirus Disease 2019 (COVID-19) continues to have a devastating impact across the world. A number of pre-existing common clinical conditions were reported to represent risk factors for more severe COVID-19 outcomes. Hereditary Hemorrhagic Telangiectasia (HHT) is a rare vascular heritable disorders, characterized by complications secondary to visceral Arterio-Venous Malformations. The impact of HHT, as well as for many Rare Diseases (RDs) on infection susceptibility profile and clinical adverse outcome risk is an unresolved issue.Objectives: The main objectives were: to assess the clinical features and outcomes of HHT patients infected with COVID-19; to compare the relative infection risk in these patients with the Italian general population throughout the first pandemic wave; to investigate the factors potentially associated with severe COVID-19 outcome in HHT patients, and the possible impact of COVID-19 infection on HHT-related symptoms/complications. Finally, we aimed to estimate how the lockdown-associated wearing of personal protective equipment/individual protection devices could affect HHT-related telangiectasia bleeding frequency.Methods: The study is a nation-wide questionnaire-based survey, with a multi-Center retrospective cross-sectional design, addressed to the whole Italian HHT population. COVID-19 cases, occurring throughout the first pandemic wave, were collected by a questionnaire-based semi-structured interview. Only the cases ascertained by laboratory confirmation (molecular/serological) were included for epidemiological estimates. Information concerning eventual SarS-Cov-2 infection, as well as regarding HHT-related manifestations and HHT-unrelated co-morbidities were collected by the questionnaire. Prevalence data were compared to Italian general population in the same period.Results: The survey disclosed 9/296 (3.04%) COVID-19 cases, 8/9 of them being resident in Lombardy, the main epidemic epicenter. Pneumonia was reported by 4/9 patients, which prompted hospital admission and intensive care management in 2 cases. No fatal outcome was recorded. After careful refinement of epidemiological analysis, the survey evidenced overlapping infection risk in HHT compared to general population.Conclusions: COVID-19 infection profile parallels geographical distribution of epidemic foci. COVID-19 in HHT patients can lead to highly variable clinical profile, likely overlapping with that of general population. 227. Daptomycin Pharmacokinetics and Pharmacodynamics in Patients on Methadone Substitution Therapy.

Abstract

Background and Objective: When administered for severe infections in intravenous drug users (IDUs) at a daily dose of 6 mg/kg, daptomycin displayed abnormal pharmacokinetic parameters compared with those seen in healthy volunteers; specifically, decreased trough and maximum concentrations (Ctrough; Cmax) and increased clearance (CL). The objective of this study was to evaluate the pharmacokinetics and pharmacodynamics of daptomycin administered at a daily dosage of 12 mg/kg for Staphylococcus aureus infective endocarditis (IE) in patients concomitantly treated with methadone, and to compare the results with those published in the literature for healthy controls treated with the same daily dose. Methods: Antibiotic treatment included daptomycin (12 mg/kg daily) in combination with an antistaphylococcal β-lactam (cefazolin 2 g three times a day). The minimum inhibitory concentration (MIC) of Staphylococcus aureus isolated through blood cultures was used to calculate pharmacokinetic and pharmacodynamic parameters such as the ratio of the area under the concentration–time curve over 24 h to the MIC (AUC0–24/MIC) and Cmax/MIC. Results: Five IDUs hospitalized for IE were enrolled. The mean measured daptomycin Cmax and Ctrough were 54.1 μg/mL (CV: 0.32) and 8.7 μg/mL (CV: 0.59), respectively; the mean calculated AUC0–24 was 742.7 μg × h/mL (CV: 0.31). The estimated average volume of distribution at the steady state (Vd,ss) and the half-life (t1/2) were 316.5 mL/kg (CV: 0.53) and 14.4 h (CV: 0.30), respectively. The mean daptomycin clearance from plasma normalized for body weight (CLwp) was 17.3 mL/(h × kg) (CV: 0.33). The calculated average Cmax and AUC0–24 (183.7 µg/mL and 1277.4 µg × h/mL, respectively) were lower than and statistically significantly different from (p < 0.001 and p = 0.001, respectively) those expected for healthy volunteers. Conclusions: Treatment of Staphylococcus aureus IE in IDUs on methadone treatment requires the use of high daptomycin daily doses in order to achieve satisfactory pharmacodynamic parameters. Abstract

Toxoplasma gondii (T. gondii) and cytomegalovirus (CMV) infections are typically asymptomatic infections, but they can have serious consequences mainly in newborns and immunocompromised patients. In many parts of the world, these infections are routinely screened during pregnancy (toxoplasmosis) and, in others, high-risk individuals are tested using fully automated screening assays. In this study, we investigated the performance of the three fully automated immunoassays, LIAISON® XL DiaSorin, Abbott Architect and Roche Cobas®, for the determination of specific IgG antibodies to Cytomegalovirus and Toxoplasma gondii in human serum or plasma samples in terms of prevalence of CMV and Toxo IgG detected, and both sensitivity and specificity. Performance of the LIAISON® assays was investigated compared to two other assays, ARCHITECT (CMV IgG and Toxo IgG assays) and Cobas® (CMV IgG and Toxo IgG assays). Discrepant anti CMV IgG and anti Toxoplasma IgG samples were tested for IgM to CMV and Toxoplasma to exclude early acute infection where IgG could be detected differently by the methods. Overall, for both CMV IgG and Toxo IgG, the LIAISON® assay was better than both the Cobas® and ARCHITECT assays in terms of CMV and Toxo IgG detected, and both diagnostic sensitivity and specificity performance although the difference is statistically significant only compared to Cobas®.

234. Can we innovate how we measure scientific impact?

Abstract

Each year, all the Scientific Institutes of Medical Research in Italy communicate the complete list of scientific publications in their research areas to a central database at the Ministry of Health (MoH), which is necessary for funding allocation. With FileMaker 11 software, we have created a dedicated database that collect citations and Altmetrics of all research articles produced by our researchers at the Foundation sent to the Ministry. The DOI (Digital Object Identifier) and Web of Science Accession Number and Scopus EID Identifier have been inserted for each article. Launching the update, the system is able to connect to both Web of Science, through the Accession Number, to Scopus , by EID identifier, and to Altmetric.com through API codes based on DOI. Data can then be broken down by year, department, or unit. The system included data from year 2011 to 2016 (for a total of n.3.756 articles). We plan to assess the correlation between Altmetrics, citation counts and traditional indexes, by Spearman’s rank correlation coefficient, by department and by author. Also, we plan to analyse trend over time of both Altmetrics and traditional indexes, to assess whether a temporal lag exist between paper publication and actual impact. Furthermore, the possibility to have the citations of all articles from the two traditional databases (Scopus and Wos) will allow us to study the differences in the h-indices between them. Abstract

Background: Statins and sildenafil have been shown to exert beneficial effects in cardiac injury. We hypothesized that antenatal maternal administration of simvastatin and/or sildenafil might also promote benefits in cardiac remodeling of congenital diaphragmatic hernia (CDH). Therefore, we performed micro-CT image analysis and histology of the heart after antennal treatment in experimental nitrofen-induced CDH.Methods: At 9.5 days post conception (dpc), pregnant rats were exposed to nitrofen. At 16 and 20 dpc fetuses were treated with simvastatin and/or sildenafil. At 21 dpc postmortem micro-CT and autopsy were performed.Results: All nitrofen-treated fetuses had a lower birth weight compared to controls; in the simvastatin-treated group, a significant improvement in CDH was noted. Impairment of the lung and liver was also noted in CDH. Compared to controls, CDH rats showed lower ventricular mass, with greater left ventricular thickness; simvastatin decreased the ventricular mass and improved wall thickness. CDH rats exhibited myocardial hypotrophy, severe vascular depression in the left ventricle, and intense interstitial edema compared to controls and nitrofen-exposed animals without CDH. In CDH, the cardiac morphology appeared deformed with left ventricular wall verticalization. Simvastatin improved cardiac myocyte appearance and heart morphology.Conclusion: The potential to treat CDH with antenatal simvastatin may improve the management of this malformation.
