204. Implementation of a Collaborative HIV and Hepatitis C Screening Program in Appalachian Urgent Care Settings

Abstract

Introduction: With the current hepatitis C (HCV) epidemic in the Appalachian region and the risk of human immunodeficiency virus (HIV) co-infection, there is a need for increased secondary prevention efforts. The purpose of this study was to implement routine HIV and HCV screenings in the urgent care setting through the use of an electronic medical record (EMR) to increase a provider’s likelihood of testing eligible patients.  Methods: From June 2017 through May 2018, EMR-based HIV and HCV screenings were implemented in three emergency department-affiliated urgent care settings: a local urgent care walk-in clinic; a university-based student health services center; and an urgent care setting located within a multi-specialty clinic. EMR best practice alerts (BPA) were developed based on Centers for Disease Control and Prevention (CDC) guidelines and populated on registered patients who qualified to receive HIV and/or HCV testing. Patients were excluded from the study if they chose to opt out from testing or the provider deemed it clinically inappropriate. Upon notification of a positive HIV and/or HCV test result through the EMR, patient navigators (PNs) were responsible for linking patients to their first medical appointment.  Results: From June 2017 through May 2018, 48,531 patients presented to the three urgent care clinics. Out of 27,230 eligible patients, 1,972 patients (7.2%) agreed to be screened for HIV; for HCV, out of 6,509 eligible patients, 1,895 (29.1%)  agreed to be screened. Thirty-one patients (1.6%) screened antibody-positive for HCV, with three being ribonucleic acid confirmed positives. No patients in either setting were confirmed positive for HIV; however, two initially screened HIV-positive. PNs were able to link 17 HCV antibody-positive patients (55%) to their first appointment, with the remainder having a scheduled future appointment.  Conclusion: Introducing an EMR-based screening program is an effective method to identify and screen eligible patients for HIV and HCV in Appalachian urgent care settings where universal screenings are not routinely implemented.

Abstract

In this paper, a novel method for automatic planogram compliance checking in retail chains is proposed without requiring product template images for training. Product layout is extracted from an input image by means of unsupervised recurring pattern detection and matched via graph matching with the expected product layout specified by a planogram to measure the level of compliance. A divide and conquer strategy is employed to improve the speed. Specifically, the input image is divided into several regions based on the planogram. Recurring patterns are detected in each region respectively and then merged together to estimate the product layout. Experimental results on real data have verified the efficacy of the proposed method. Abstract

Recent research on finding important intermediate nodes in a network suspected to contain criminal activity is highly dependent on network centrality values. Betweenness centrality, for example, is widely used to rank the nodes that act as brokers in the shortest paths connecting all source and all the end nodes in a network. However both the shortest path node betweenness and the linearly scaled betweenness can only show rankings for all the nodes in a network. In this paper we explore the mathematical concept of pair-dependency on intermediate nodes, adapting the concept to criminal relationships and introducing a new source-intermediate reliance measure. To illustrate our measure, we apply it to rank the nodes in the Enron email dataset and the Noordin Top Terrorist networks. We compare the reliance ranking with Google PageRank, Markov centrality as well as betweenness centrality and show that a criminal investigation using the reliance measure, will lead to a different prioritisation in terms of possible people to investigate. While the ranking for the Noordin Top terrorist network nodes yields more extreme differences than for the Enron email transaction network, in the latter the reliance values for the set of finance managers immediately identified another employee convicted of money laundering., Comment: Paper version 3.0

223. Using shortest path to discover criminal community

Abstract

Extracting communities using existing community detection algorithms yields dense sub-networks that are difficult to analyse. Extracting a smaller sample that embodies the relationships of a list of suspects is an important part of the beginning of an investigation. In this paper, we present the efficacy of our shortest paths network search algorithm (SPNSA) that begins with an "algorithm feed", a small subset of nodes of particular interest, and builds an investigative sub-network. The algorithm feed may consist of known criminals or suspects, or persons of influence. This sets our approach apart from existing community detection algorithms. We apply the SPNSA on the Enron Dataset of e-mail communications starting with those convicted of money laundering in relation to the collapse of Enron as the algorithm feed. The algorithm produces sparse and small sub-networks that could feasibly identify a list of persons and relationships to be further investigated. In contrast, we show that identifying sub-networks of interest using either community detection algorithms or a k-Neighbourhood approach produces sub-networks of much larger size and complexity. When the 18 top managers of Enron were used as the algorithm feed, the resulting sub-network identified 4 convicted criminals that were not managers and so not part of the algorithm feed. We also directly tested the SPNSA by removing one of the convicted criminals from the algorithm feed and re-running the algorithm; in 5 out of 9 cases the left out criminal occurred in the resulting sub-network.

Abstract

Tracing criminal ties and mining evidence from a large network to begin a crime case analysis has been difficult for criminal investigators due to large numbers of nodes and their complex relationships. In this paper, trust networks using blind carbon copy (BCC) emails were formed. We show that our new shortest paths network search algorithm combining shortest paths and network centrality measures can isolate and identify criminals' connections within a trust network. A group of BCC emails out of 1,887,305 Enron email transactions were isolated for this purpose. Abstract

Background Surgical treatment of intracerebral hemorrhage (ICH) is unproven, although meta-analyses suggest that both early conventional surgery with craniotomy and minimally invasive surgery (MIS) may be beneficial. We aimed to demonstrate the safety, feasibility, and promise of efficacy of early MIS for ICH using the Aurora Surgiscope and Evacuator. Methods We performed a prospective, single arm, phase IIa Simon's two stage design study at two stroke centers (10 patients with supratentorial ICH volumes ≥20 mL and National Institutes of Health Stroke Scale (NIHSS) score of ≥6, and surgery commencing <12 hours after onset). Positive outcome was defined as ≥50% 24 hour ICH volume reduction, with the safety outcome lack of significant ICH reaccumulation. Results From December 2019 to July 2020, we enrolled 10 patients at two Australian Comprehensive Stroke Centers, median age 70 years (IQR 65--74), NIHSS score 19 (IQR 19--29), ICH volume 59 mL (IQR 25--77), at a median of 227 min (IQR 175--377) post-onset. MIS was commenced at a median time of 531 min (IQR 437--628) post-onset, had a median duration of 98 min (IQR 77-- 110), with a median immediate postoperative hematoma evacuation of 70% (IQR 67--80%). A positive outcome was achieved in 5/5 first stage patients and in 4/5 second stage patients. One patient developed significant 24 hour ICH reaccumulation; otherwise, 24 hour stability was observed (median reduction 71% (IQR 61--80), 5/9 patients <15 mL residual). Three patients died, unrelated to surgery. There were no surgical safety concerns. At 6 months, the median modified Rankin Scale score was 4 (IQR 3--6) with 30% achieving a score of 0--3. Conclusion In this study, early ICH MIS using the Aurora Surgiscope and Evacuator appeared to be feasible and safe, warranting further exploration. Trial registration number ACTRN12619001748101. Abstract

Background: The relationship between baseline perihematomal edema (PHE) and inflammation, and their impact on survival after intracerebral hemorrhage (ICH) are not well understood. Objective: Assess the association between baseline PHE, baseline C-reactive protein (CRP), and early death after ICH. Methods: Analysis of pooled data from multicenter ICH registries. We included patients presenting within 24 h of symptom onset, using multifactorial linear regression model to assess the association between CRP and edema extension distance (EED), and a multifactorial Cox regression model to assess the association between CRP, PHE volume and 30-day mortality. Results: We included 1,034 patients. Median age was 69 (interquartile range [IQR] 59--79), median baseline ICH volume 11.5 (IQR 4.3--28.9) mL, and median baseline CRP 2.5 (IQR 1.5--7.0) mg/L. In the multifactorial analysis [adjusting for cohort, age, sex, log-ICH volume, ICH location, intraventricular hemorrhage (IVH), statin use, glucose, and systolic blood pressure], baseline log-CRP was not associated with baseline EED: for a 50% increase in CRP the difference in expected mean EED was 0.004 cm (95%CI 0.000--0.008, p = 0.055). In a further multifactorial analysis, after adjusting for key predictors of mortality, neither a 50% increase in PHE volume nor CRP were associated with higher 30-day mortality (HR 0.97; 95%CI 0.90--1.05, p = 0.51 and HR 0.98; 95%CI 0.93--1.03, p = 0.41, respectively). Conclusion: Higher baseline CRP is not associated with higher baseline edema, which is also not associated with mortality. Abstract

Southern California experienced unprecedented megadrought between 2012 and 2018. During this time, Malosma laurina, a chaparral species normally resilient to single‐year intense drought, developed extensive mortality exceeding 60% throughout low‐elevation coastal populations of the Santa Monica Mountains. We assessed the physiological mechanisms by which the advent of megadrought predisposed M. laurina to extensive shoot dieback and whole‐plant death. We found that hydraulic conductance of stem xylem (Ks, native) was reduced seven to 11‐fold in dieback adult and resprout branches, respectively. Staining of stem xylem vessels revealed that dieback plants experienced 68% solid‐blockage, explaining the reduction in water transport. Following Koch's postulates, persistent isolation of a microorganism in stem xylem of dieback plants but not healthy controls indicated that the causative agent of xylem blockage was an opportunistic endophytic fungus, Botryosphaeria dothidea. We inoculated healthy M. laurina saplings with fungal isolates and compared hyphal elongation rates under well‐watered, water‐deficit, and carbon‐deficit treatments. Relative to controls, we found that both water deficit and carbon‐deficit increased hyphal extension rates and the incidence of shoot dieback. Summary statement: We conclude that the ultimate cause of mortality of Malosma laurina in field populations was megadrought and that the proximal cause of mortality was the transition of endemic Botryosphaeria dothidea to an opportunistic pathogen that amplified protracted water stress through physical blockage of xylem water transport. Abstract

Background and purposeRecent positive randomized trials of endovascular therapy for ischemic stroke used predominantly stent retrievers. We pooled data to investigate the efficacy and safety of stent thrombectomy using the Solitaire device in anterior circulation ischemic stroke.MethodsPatient-level data were pooled from trials in which the Solitaire was the only or the predominant device used in a prespecified meta-analysis (SEER Collaboration): Solitaire FR With the Intention for Thrombectomy as Primary Endovascular Treatment (SWIFT PRIME), Endovascular Treatment for Small Core and Anterior Circulation Proximal Occlusion With Emphasis on Minimizing CT to Recanalization Times (ESCAPE), Extending the Time for Thrombolysis in Emergency Neurological Deficits-Intra-Arterial (EXTEND-IA), and Randomized Trial of Revascularization With Solitaire FR Device Versus Best Medical Therapy in the Treatment of Acute Stroke Due to Anterior Circulation Large Vessel Occlusion Presenting Within Eight Hours of Symptom Onset (REVASCAT). The primary outcome was ordinal analysis of modified Rankin Score at 90 days. The primary analysis included all patients in the 4 trials with 2 sensitivity analyses: (1) excluding patients in whom Solitaire was not the first device used and (2) including the 3 Solitaire-only trials (excluding ESCAPE). Secondary outcomes included functional independence (modified Rankin Score 0-2), symptomatic intracerebral hemorrhage, and mortality.ResultsThe primary analysis included 787 patients: 401 randomized to endovascular thrombectomy and 386 to standard care, and 82.6% received intravenous thrombolysis. The common odds ratio for modified Rankin Score improvement was 2.7 (2.0-3.5) with no heterogeneity in effect by age, sex, baseline stroke severity, extent of computed tomography changes, site of occlusion, or pretreatment with alteplase. The number needed to treat to reduce disability was 2.5 and for an extra patient to achieve independent outcome was 4.25 (3.29-5.99). Successful revascularization occurred in 77% treated with Solitaire device. The rate of symptomatic intracerebral hemorrhage and overall mortality did not differ between treatment groups.ConclusionsSolitaire thrombectomy for large vessel ischemic stroke was safe and highly effective with substantially reduced disability. Benefits were consistent in all prespecified subgroups.

Abstract

This paper aims to articulate observations of a technical organization during corporate restructuring. It conducts a systematic literature review to identify factors of drivers, actions, and outcomes within a corporate spin-off, the authors then conducted three case studies of spin-offs using those findings using surveys and interviews of real-world executives who managed a corporate restructuring spin-off. Additionally, this paper gives further insight into the relationships of actions and outcomes of spin-offs, finding that there are correlations between change management, systematic change and organizational restructuring to performance, effects on employees, and market fit strategy. Engineering managers can use this paper to understand the dynamics within a corporate restructuring and act accordingly. 246. How Principals Learn to Lead: The Comparative Influence of On-the-Job Experiences, Administrator Credential Programs, and the ISLLC Standards in the Development of Leadership Expertise among Urban Public School Principals

Abstract

The purpose of this study was to examine the comparative influence of college administrator credential programs, on-the-job experiences, and the ISLLC Standards in the development of leadership expertise among urban public school principals. An exploratory, ex-post-facto research design used both quantitative and qualitative approaches. A survey of 101 randomly selected urban school principals from 25 of America's largest metropolitan school districts was given, followed by telephone interviews with a subset of 20 randomly selected survey participants. Data were analyzed through the use of inferential and descriptive statistics and descriptive narratives. On-the-job experiences were significantly more important in developing leadership expertise than college credential programs on each of 41 ISLLC-based learning tasks. However, college credential programs are also important sources of leadership development. Several significant differences were found between comparative ratings of learning tasks by subgroups (experience, gender, school type). Respondents ranked 78% of the ISLLC Standards as very important to the field of school leadership. Interview subjects expressed concern about lack of preparation in budgeting, data analysis, teacher evaluation, and change management. (Contains 8 tables and 3 footnotes)
