201. Learning Over Dirty Data Without Cleaning

Abstract

Real-world datasets are dirty and contain many errors. Examples of these issues are violations of integrity constraints, duplicates, and inconsistencies in representing data values and entities. Learning over dirty databases may result in inaccurate models. Users have to spend a great deal of time and effort to repair data errors and create a clean database for learning. Moreover, as the information required to repair these errors is not often available, there may be numerous possible clean versions for a dirty database. We propose DLearn, a novel relational learning system that learns directly over dirty databases effectively and efficiently without any preprocessing. DLearn leverages database constraints to learn accurate relational models over inconsistent and heterogeneous data. Its learned models represent patterns over all possible clean instances of the data in a usable form. 207. Comparative Genomics in Cultivated and Wild Brassicaceae Species

Abstract

This study explores the genetic complexities and evolutionary dynamics of plantadaptation across species within the Brassicaceae family, focusing on Brassica napus,Streptanthus diversifolius, and exploring gene expression related to seed germination andclimate adaptation in the Streptanthoid complex. For Brassica napus, an important oilseed cropand an allotetraploid hybrid of Brassica rapa and Brassica oleracea, we utilized third generationsequencing and assembly technologies to generate a new high quality genome assembly of thesynthetic cultivar Da-Ae. This work identifies homoeologous exchange hotspots, illuminatingthe genetic rearrangements essential for hybrid viability. In parallel, we present a chromosome-level genome assembly for Streptanthus diversifolius, also utilizing third generation sequencingtechnologies. This assembly sheds light on the species' ability to adapt to diverse Californianenvironments, potentially facilitated by a tribe-specific whole genome duplication event,enhancing its capacity to thrive in inhospitable conditions like serpentine soils. Further, throughgermination assays and RNA sequencing across multiple California Jewelflower species, weconstruct gene co-expression networks to correlate with germination and climate adaptationtraits. Our findings reveal distinct gene expression patterns driven by climate variations andposit that positive selection has shaped these networks, optimizing germination timing foradaptive success. Collectively, these studies enhance our understanding of the geneticunderpinnings of plant adaptability and evolution, showcasing the intricate relationshipbetween genomic architecture, environmental adaptation, and evolutionary innovation in theBrassicaceae family.

Abstract

The development of core entrustable professional activities (EPA) for entering residency and Accreditation Council for Graduate Medical Education's milestones have spurred thinking about the fourth year of medical school as a transition to residency. In this monograph, we lay out our specialty focused post-clerkship curriculum and report learner and residency director perceptions over the first three years of implementation. Ongoing curricular monitoring has reinforced core principles but has also informed actionable quality improvement efforts. EPA-focused learning experiences, integration of specialty-specific milestones, addition of the feedforward process, and accessible mentorships have been key curricular elements to guide the transition to residency.

Abstract

IntroductionThe AAMC prioritizes promoting a diverse and culturally competent workforce which is thought to have a positive impact on the health of people living in the US. There is a lack of diversity in the current landscape of academic medicine and strategies are needed to effect change. This module introduced undergraduate and graduate medical trainees to leadership skills and opportunities in curriculum innovation and reform by learning about and interacting with the office of medical education (OME) at their institutions.MethodsWe implemented a workshop using small-group case discussions and didactics to help medical students and residents learn how to: (1) describe the structure and functions of an OME, (2) describe leadership competencies associated with various roles within the OME, and (3) identify opportunities for trainees to engage with the OME on curricular innovation and reform, especially advancing diversity and inclusion.ResultsAcross three sites, 45 learners completed partial or full workshop evaluations. Of learners, 22 (49%) were not knowledgeable and 13 (29%) were somewhat knowledgeable in identifying leadership opportunities for trainees to become engaged through the OME. There was a statistically significant increase in confidence after the workshop in "discussing an interdisciplinary approach to the creation of a medical education innovation," and, "assessing the need for curricula change." Over 90% of attendees agreed learning objectives were met.DiscussionThis workshop succeeded in promoting awareness of the structure and function of OMEs and confidence in seeking opportunities to become engaged in medical education, especially in advancing diversity and inclusion.

Abstract

PurposeTo assess psychological effects of the initial peak phase of the COVID-19 pandemic on United States (US) medical students in clinical training to anticipate sequelae and prepare for future outbreaks.MethodsAuthors emailed a cross-sectional survey in April-May, 2020 to students in clinical training years at six US medical schools which included validated General Anxiety Disorder (GAD-7) and Primary Care-PTSD (PC-PTSD-5) screening tools, and asked students about pandemic-related stress and specific concerns. Authors used quantitative and thematic analysis to present results.ResultsOf 2511 eligible students, 741 responded (29.5%). Most students (84.1%) reported at least "somewhat" increased levels of stress and anxiety related to the pandemic. On the GAD-7, 34.3% showed mild, 16.1% moderate, and 9.5% severe anxiety symptoms, with 39.6% demonstrating no/minimal symptoms. One quarter (25.4%) screened positive for PTSD risk symptoms. Top concerns of students chosen from a pre-populated list included inadequate COVID-19 testing, undiagnosed or asymptomatic spread and racial or other disparities in the pandemic. In thematic analysis, students' reactions to removal from clinical learning included: understanding the need to conserve PPE (32.2%), a desire to help (27.7%), worry over infectious risk to others (25.4%) and self (21.2%), and lost learning opportunities (22.5%). Female students were significantly more likely to report anxiety and PTSD risk symptoms. Asian students had a greater risk of moderate anxiety and those underrepresented in medicine (UIM) had greater risk of moderate and severe anxiety symptoms compared to white students.ConclusionsDuring the initial peak phase of COVID-19, over 60% of US medical students screened positive for pandemic-related anxiety and one quarter were at risk for PTSD. Female and UIM students were significantly more affected. Medical schools should consider broad support of students, and targeted outreach to female and UIM students.

222. 'Why Am I in All of These Pictures?' from Learning Stories to Lived Stories: The Politics of Children's Participation Rights in Documentation Practices

Abstract

In this paper, we report on Phase One of a small action research project that examined how Learning Stories were put into practice at one Scottish nursery. Specifically, the paper looks at young children's participation rights and how they were enacted within the authorship of the stories. The project used an action research approach in which qualitative data about participants' current experiences with the stories was used to spark reflection, experimentation and change in documentation practices. Drawing on Phase One data from young children, parents and practitioners at the nursery, our findings illustrate the complex enactment of children's participation rights, including children's right to information, freedom of expression and their right to express their views and have those views taken into account. The paper concludes that more work needs to be done in the field of Learning stories to (a) acknowledge the complex political and material considerations at play in the creation of pedagogical documentation and (b) to accommodate children's own authorship, through flexible, non (or less) written methods.

226. Bioleaching of chalcopyrite using native Acidithiobacillus ferrooxidans isolated in a mining area in Kitwe Zambia

Abstract

This study aimed to assess the influence of the physical-chemical factors (particle size, shake flask speed, starting pH, and pulp density) in bioleaching of Zambian chalcopyrite using a novel native isolated Acidithiobacillus ferrooxidans strain DJN1 2021 from samples collected within mining area at Copper Nkana slag dump also known as the Black Mountain in Kitwe, Copperbelt Province in Zambia. The bacterium isolation was done using the modified 9 K selective media and was identified as A. ferrooxidans by using conventional procedures and 16S rRNA gene sequencing. The resulting sequences were analyzed by trimming and generating consensus sequences using Bio-edit software. Blasting results produced a significant alignment of 99.6% similarity with the corresponding sequences of A. ferrooxidans in GenBank. The phylogenetic analysis entails that the isolated strain was identified as A. ferrooxidans and named A. ferrooxidans strain DJN1 2021 with accession number MZ713091. Bioleaching of chalcopyrite (CuFeS2) by A. ferrooxidans (DJN1 2021) was conducted in shake flasks (100 ml) with a 9 K medium. The experiments were tested to optimize the initial pH, particle size, pulp density, and shake flask speed in the recovery of copper from its ore. The results revealed that all of the factors significantly influence the bioleaching process. When the initial pH was 1.8, copper was extracted at the rate of 30.6% within 30 bioleaching days. This was the maximum dissolution observed. The effect of revolution speed in the three different shake flask speeds (i.e. 50 rpm, 90 rpm, and 150 rpm) showed a significant difference in the rate of copper dissolution. A marked difference was observed between shake flask speeds of 50 rpm and 90 rpm with dissolutions of 29.4% and 30.0% respectively. Abstract

The coronavirus disease 2019 (COVID-19) pandemic caused by SARS coronavirus 2 (SARS-CoV-2) has caused significant morbidity and mortality for patients and stressed healthcare systems worldwide. The clinical features, disease course, and serologic response of COVID-19 among immunosuppressed patients such as solid organ transplant (SOT) recipients, who are at presumed risk for more severe disease, are not well characterized. We describe our institutional experience with COVID-19 among 10 SOT patients, including the clinical presentation, treatment modalities, and outcomes of 7 renal transplant recipients, 1 liver transplant recipient, 1 heart transplant recipient, and 1 lung transplant recipient. In addition, we report the serologic response in SOT recipients, documenting a positive IgG response in all 7 hospitalized patients. We also review the existing literature on COVID-19 in SOT recipients to consolidate the current knowledge on COVID-19 in the SOT population for the transplant community.

Abstract

PurposeAccording to the Williams Institute, 1.4 million U.S. adults identify as transgender. Many experience health care disparities. Professional organizations call for medical education to improve transgender care, but what curricula are being delivered is unknown. The goal of this study was to conduct the first comprehensive, national survey of transgender health care curricula in physician assistant (PA) education.MethodThe authors sent a questionnaire to program directors (PDs) at all 236 U.S. PA programs in June 2018. They categorized programs as those that currently deliver at least 1 hour of transgender health content and those who do not (Teaching/Not Teaching). They examined differences between Teaching and Not Teaching programs using chi-square tests, and they evaluated comments for themes.ResultsThe response rate was 100%. Of the 236 programs, 202 (85.6%) teach transgender content and 34 (14.4%) do not. According to PDs, most transgender content was delivered in medical interviewing (44.1%) or women's health (31.4%) and the most common transgender health topics included differentiating between sex and gender and between behavior and identity (78.8%), followed by health disparities (77.5%) and barriers to care (75.0%). PDs most commonly cited a lack of time (51.3%) and faculty knowledge (35.6%) as barriers for teaching transgender health topics. Half of the PDs (50.4%) ranked transgender health as very or extremely important. The authors detected statistically significant differences between Teaching and Not Teaching programs based on geographic region (P = .01), perceived importance (P ≤ .001), and presence of knowledgeable faculty (P = .01). Presence of knowledgeable faculty was significantly associated with perceived importance (P = .01).ConclusionsThis is the first comprehensive, nationwide survey of transgender health education in U.S. PA programs. A key finding is that the presence of expert faculty is significantly associated with delivery and perceived importance of transgender health curricula.

Abstract

Terpenoid metabolism plays vital roles in stress defense and the environmental adaptation of monocot crops. Here, we describe the identification of the terpene synthase (TPS) gene family of the panicoid food and bioenergy model crop foxtail millet (Setaria italica). The diploid S. italica genome contains 32 TPS genes, 17 of which were biochemically characterized in this study. Unlike other thus far investigated grasses, S. italica contains TPSs producing all three ent-, (+)- and syn-copalyl pyrophosphate stereoisomers that naturally occur as central building blocks in the biosynthesis of distinct monocot diterpenoids. Conversion of these intermediates by the promiscuous TPS SiTPS8 yielded different diterpenoid scaffolds. Additionally, a cytochrome P450 monooxygenase (CYP99A17), which genomically clustered with SiTPS8, catalyzes the C19 hydroxylation of SiTPS8 products to generate the corresponding diterpene alcohols. The presence of syntenic orthologs to about 19% of the S. italica TPSs in related grasses supports a common ancestry of selected pathway branches. Among the identified enzyme products, abietadien-19-ol, syn-pimara-7,15-dien-19-ol and germacrene-d-4-ol were detectable in planta, and gene expression analysis of the biosynthetic TPSs showed distinct and, albeit moderately, inducible expression patterns in response to biotic and abiotic stress. In vitro growth-inhibiting activity of abietadien-19-ol and syn-pimara-7,15-dien-19-ol against Fusarium verticillioides and Fusarium subglutinans may indicate pathogen defensive functions, whereas the low antifungal efficacy of tested sesquiterpenoids supports other bioactivities. Together, these findings expand the known chemical space of monocot terpenoid metabolism to enable further investigations of terpenoid-mediated stress resilience in these agriculturally important species.

Abstract

The concept of providing focused, competency-based LGBTQ+ health education outside the setting of health professional programs, specifically for undergraduates, is quite uncharted. However, the issue at the core of our rationale is one shared by those with and without clinical exposure: how to best support the development of cultural competence in providers who are or will be caring for LGBTQ+ patients. Traditional health professional education programs have enacted a number of curricular initiatives in this regard, designed for advanced learners. By focusing specifically on the undifferentiated learner, we offer a new perspective on the timing of LGBTQ+ health-related education. Our course is not intended to supplant the critical learning and application that must occur in the clinic or hospital room. Rather, we present a framework for cultivating understanding of the healthcare issues faced by the LGBTQ+ community that may help a learner to acquire and apply skills subsequently with greater cultural competence.

BackgroundWe aimed to validate Decipher to predict adverse pathology (AP) at radical prostatectomy (RP) in men with National Comprehensive Cancer Network (NCCN) favorable-intermediate risk (F-IR) prostate cancer (PCa), and to better select F-IR candidates for active surveillance (AS).MethodsIn all, 647 patients diagnosed with NCCN very low/low risk (VL/LR) or F-IR prostate cancer were identified from a multi-institutional PCa biopsy database; all underwent RP with complete postoperative clinicopathological information and Decipher genomic risk scores. The performance of all risk assessment tools was evaluated using logistic regression model for the endpoint of AP, defined as grade group 3-5, pT3b or higher, or lymph node invasion.ResultsThe median age was 61 years (interquartile range 56-66) for 220 patients with NCCN F-IR disease, 53% classified as low-risk by Cancer of the Prostate Risk Assessment (CAPRA 0-2) and 47% as intermediate-risk (CAPRA 3-5). Decipher classified 79%, 13% and 8% of men as low-, intermediate- and high-risk with 13%, 10%, and 41% rate of AP, respectively. Decipher was an independent predictor of AP with an odds ratio of 1.34 per 0.1 unit increased (p value = 0.002) and remained significant when adjusting by CAPRA. Notably, F-IR with Decipher low or intermediate score did not associate with significantly higher odds of AP compared to VL/LR.ConclusionsNCCN risk groups, including F-IR, are highly heterogeneous and should be replaced with multivariable risk-stratification. In particular, incorporating Decipher may be useful for safely expanding the use of AS in this patient population.
