BACKGROUND: We reviewed our experience with the Sorin Pericarbon (Sorin, Saluggia, Italy) valve implanted in the aortic position. METHODS: From January 1990 to January 1996, 143 consecutive patients had a Pericarbon valve implanted in the aortic position. The mean age was 75+/-5 years. Seventy-eight patients (55%) were in New York Heart Association (NYHA) class III or IV. Sixty patients (42%) had one or more concomitant procedures (51 coronary artery bypass grafting [CABG], 7 carotid endarterectomies, 9 others). RESULTS: The hospital mortality rate was 12% (17 of 143 patients). The follow-up was 100% complete and the median time was 42 months (range, 2 to 79 months). There were 36 late deaths, 20 being cardiac-related: 5 non-valve-related, 11 valve-related, and 4 sudden unexpected deaths. The 5-year actuarial survival was 57%+/-5%. There were 6 early valve failures related to a calcific stenosis at a median time of 36 months (range, 5 to 66 months). Three patients had to undergo another operation and one of these patients died. One patient died the day before the planned reoperation and 2 patients are followed with a symptomatic aortic stenosis but refuse reoperation. Freedom from structural deterioration was 93%+/-3% at 4 years. Echocardiographic examination was obtained in 73 patients at a median time of 42 months (range, 4 to 79 months). Four additional asymptomatic patients were found to have calcifications of their prosthesis. The 5-year freedom from thromboembolic events and from endocarditis were, respectively, 87%+/-5% and 92%+/-3%. CONCLUSION: The surprisingly high rate of early failure due to calcific stenosis and of thromboembolic events of the Pericarbon valve implanted in the aortic position in the elderly made us discontinue its use in our institution.

The gastroepiploic artery has been widely used for complete arterial myocardial revascularization of young patients. Gastric complications related to the harvesting of this artery are exceptional. We describe here a case of ischemic gastric ulcer due to the use of a gastroepiploic artery in a patient with severe celiac trunk disease. The patient was cured by angioplasty completed by a stenting procedure.

From July 1989 to July 1994, a total of 44 popliteal-to-distal artery bypasses were performed in 36 patients (29 men and seven women, mean age 62 +/- 10 years). These procedures accounted for 8.8% of all infrainguinal revascularizations performed during that period. Risk factors included diabetes in 33 patients (92%), smoking in 18 (50%), and coronary artery disease in 15 (42%). Prior to revascularization all patients were at risk of limb loss. Tissue necrosis was present in 31 cases (71%), ulceration in eight cases (18%), and rest pain in five cases (11%). Patency of the femoral and popliteal arteries was confirmed prior to surgery in all cases. Intraoperative percutaneous angioplasty of the superficial femoral artery was performed in three cases. Proximal anastomosis was made to the distal popliteal artery in all cases. A total of 52 distal anastomoses (eight sequential bypasses) were made on the following arteries: posterior tibial artery in 13 cases, anterior tibial artery in eight cases, peroneal artery in six cases, plantar artery in two cases, and dorsalis pedis artery in 21 cases. The greater saphenous vein was used as graft material in 42 cases (95%) and the lesser saphenous vein in two cases (5%). No patient died during hospitalization. Early bypass occlusion occurred in three cases (6.8%) and led to amputation in all cases. Secondary patency and limb salvage rates at 3 years calculated using the actuarial method were 74% and 82% respectively. Bypass thrombosis due to superficial femoral artery deterioration was not observed in any case. The present study indicates that popliteal-to-distal artery bypass is a simple, durable, and low-risk method of lower limb revascularization. Medium-term results are promising and support routine use of popliteal-to-distal artery bypass for limb salvage in patients without significant stenosis of the superficial femoral artery.

This study examines how the recording of the lumbar and subcortical components of the posterior tibial nerve (PTN) SEPs may usefully replace that of cortical components in situations in which these components cannot be reliably obtained (infants, high concentrations of halogenated gasses). Lumbar, brain-stem, and cortical PTN SEPs were intraoperatively monitored in 7 patients undergoing repair of aortic coarctation under variable isoflurane concentration (up to 1.2%). Four patients were less than 1 year old. Two distinct activities were evidenced at the lumbar level in all of the patients: the dorsal root component (DRC) and the dorsal horn negativity (DHN). The equivalent of the adult P30 (lemniscal positivity; LP) was also present in all of the patients, whatever their age or the concentration of isoflurane. By contrast, the parietal activities were absent intraoperatively in the youngest patients. Spinal-cord ischemia consecutive to aortic cross-clamping gave rise to early DHN changes and later alterations of the LP in the two patients in which it occurred, while the DRC and the peripheral nerve activities remained unchanged. This elective sensitivity of the DHN is likely due to it being dependent on the gray matter of the spinal cord, the basal metabolism of which is greater than that of the white matter and to the situation of the DHN generator in a watershed zone of the spinal cord. This study emphasizes the interest of PTN SEPs for spinal-cord monitoring in vascular surgery and the importance of combining the recording of parietal activities with that of the lumbar spinal components.

OBJECTIVE: Our objective was to investigate the influence of preoperative myocardial ultrastructure and metabolism on recovery of contractile function after coronary artery bypass grafting in patients with coronary artery disease and left ventricular dysfunction. METHODS: Dynamic positron emission tomography with 13N-ammonia and 18F-deoxyglucose was used to assess myocardial perfusion and glucose uptake in 53 patients scheduled for coronary revascularization because of coronary artery disease and left ventricular dysfunction. The degree of tissue fibrosis and the presence of potentially reversible alterations of cardiomyocytes (loss of myofilaments and accumulation of glycogen) were quantified from transmural biopsy specimens. These were harvested from the center of the dysfunctional area during the operation and analyzed with a light microscope. The recovery of contractile performance was assessed from the changes in left ventricular function at contrast ventriculography or echocardiography before and 6 months after the operation. RESULTS: According to postoperative changes in regional wall motion, left ventricular function was considered to have improved in 34 patients, whereas dysfunction persisted in 19 patients. In patients with improved wall motion, ejection fraction rose by 12% and end-systolic volume decreased by 28%. By contrast, in patients with persistent dysfunction, ejection fraction decreased by 6% and end-systolic volume increased by 25%. Before revascularization, myocardium with reversible dysfunction displayed higher levels of absolute myocardial blood flow, higher myocardial glucose uptake, less tissue fibrosis, and more altered cardiomyocytes than myocardium with persistent dysfunction. Significant correlations were found between regional blood flow and the surface of the biopsy specimen covered by fibrosis, as well as between glucose uptake and the density of altered cardiomyocytes. Between April 1990 and August 1996, 138 patients (median age 42 years, range 10 days to 66 years) underwent aortic valve or root replacement with allografts (n = 70) or autografts (n = 68). Aortic stenosis was the main indication (80 patients, 58%) followed by aortic incompetence (31 patients, 22%). Twenty-five patients had endocarditis (18%) and two patients had truncus repair (2%). Some 140 allograft valves were inserted (70 pulmonary, 70 aortic). Most of the valves were conditioned and cryopreserved by the European Homograft Bank, Brussels, Belgium. Subcoronary implantation was performed in 46 patients, intraluminal cylinder in 29 and root replacement in 63. Peroperative transoesophageal echocardiography was routinely used. Three patients required valves replacement by mechanical valves at the time of surgery due to technical failure. Seven patients (5%) died early, while 131 were followed up from 1 to 76 months (mean 32 months). There were two late deaths. All survivors are in NYHA class I and currently free of any medication. There have been no thromboembolic events. Three patients developed endocarditis (2%). Transthoracic echocardiography was performed routinely in all patients; an initial study showed grade 1 aortic incompetence in nine patients and grade 2 in two. Late studies up to 6 years after surgery showed progression of aortic incompetence in 18 of 43 survivors with subcoronary implantation and in four of 75 survivors who underwent other techniques (P < 0.002). In congenital patients with preoperative aortic incompetence, the left ventricular function has been prospectively analysed by echocardiography. Risk factors have been identified as dilated ventricles with spherical shapes, thin wall and reduced velocities by measuring ejection fraction and velocity of shortening of myocardial fibres. Ventricular assist devices are widely used as a bridge for cardiac transplantation patients. The purpose of the present study was to evaluate retrospectively, whether the use of ventricular assist devices could increase survival after early or late retransplantation. Over the past 10 years, 219 patients were transplanted in the authors' centre. Eight of 11 retransplanted patients required preoperative insertion of a ventricular assist device. Five assist systems were implanted in the early period after transplantation ( < 30 days) and three later after transplantation ( > 30 days). After early retransplantation, two patients survived and three died. All three late retransplantations were successful and all patients are still alive with a mean (s.d.) follow-up of 25(22) months. Ventricular assist devices are helpful for late retransplantation but do not improve poor survival after early retransplantation. Such devices should not be set up for early retransplantation.

OBJECTIVE: The conventional surgical treatment of isolated critical stenosis of the left main coronary artery (LMCA) leads to the definitive occlusion of LMCA, restores only a retrograde perfusion to a rather extensive myocardial area and consumes bypass material. Direct surgical angioplasty avoids these inconveniences. METHODS: Between June 1985 and August 1996, 49 surgical angioplasties have been performed in 47 patients. LMCA was approached posteriorly in the first 11 procedures, and an anterior approach was preferred in the last 38 because of better exposure. The onlay patch consisted of saphenous vein in 37 cases; pericardium was used in 12 cases, and only for ostial stenosis. RESULTS: No technical failure occurred in the last 28 cases. 44 procedures, (90%), succeeded, but 1 patient (2.3%) died later of a massive air embolism, and 2 patients needed conventional CABG after 3 and 5 months, respectively. The 35 survivors still benefiting from a successful LMCA angioplasty on the long term are free of ischemia after a mean follow-up of 75 months (2-136). Angiographic restudy was obtained in 30 patients (70%) at an average of 38 months and revealed an excellent result in 26 (87%). In 10 patients, a late angiographic restudy at an average of 71 months (32-119) still revealed a perfect result. CONCLUSION: Provided that well-defined contra-indications (involvement of the distal bifurcation, heavy calcification) are respected, LMCA surgical angioplasty deserves a place in the array of surgical strategies.

This paper presents the results of intraoperative median nerve SEP monitoring in 205 successive patients undergoing isolated carotid endarterectomy (CE) (N = 172) or CE followed by coronary bypass (CBP) and/or vascular replacement (VR) (N = 33). The left and right median nerves were alternately stimulated and recordings performed on 4 channels: cervical, ipsi-and contralateral parietal, and frontal. SEPs were qualitatively rated in terms of mild, moderate, or severe ipsilateral, contralateral, or bilateral abnormalities. The SEP abnormalities were subdivided into 5 categories as a function of their relationships with intraoperative events: no alterations (67.3%), early or late SEP alterations after carotid cross-clamping (15.6%), SEP alterations after a drop in blood pressure (occurring outside of or within the cross-clamping period) (15.1%), SEP alterations of a most likely embolic origin (2.4%), SEP changes after head positioning (1%), and SEP changes after a modification of the anesthetic regimen (1.5%). Only moderate to severe SEP alterations occurring soon after carotid crossclamping justified shunt installation in 16% of the cases. SEP alterations after a drop in blood pressure were reversed merely by restoring blood pressure. The neurological outcome was uneventful in 94.2% of cases. Of the 12 patients who developed neurological sequellae, only one case presented transient sequellae after isolated CE without SEP changes white most cases either had undergone combined CE and CBP and/or VR (6 cases) or had presented SEP alterations of embolic origin (3 cases). We conclude that our system of qualitative rating of SEPs proved very sensitive to intraoperative hemodynamic disturbances or macroembolisms. (C) 1997 Elsevier Science Ireland Ltd.

Assessment of regional myocardial perfusion and glucose uptake with positron emission tomography (PET) identifies dysfunctional myocardium that shows improvement in function after revascularization. Yet little is known about the ultrastructural patterns of ischemic injury in myocardium with and without postoperative functional improvement in relation to residual perfusion and metabolism. Therefore dynamic PET with [13N]ammonia and 18F-labeled deoxyglucose was performed in 24 patients with coronary artery disease and anterior wall dysfunction undergoing bypass surgery. Transmural biopsies were obtained from the dysfunctional area during surgery and analyzed by optical and electron microscopy to quantify the presence of fibrosis and cardiomyocytes. As judged from the postoperative changes in contraction, left ventricular function improved in 16 patients. In myocardium that showed improved function after revascularization, preoperative flow and glucose uptake were higher by PET than in persistently dysfunctional myocardium. In tissue samples from myocardium that improved postoperatively, there were more cardiomyocytes, including a larger proportion of cells with excess glycogen stores (35 vs. 21%), and there was less fibrosis (24 vs. 49%) than in tissue samples from myocardium that did not improve functionally. Preoperative perfusion and postoperative wall motion were inversely correlated with the amount of tissue fibrosis, whereas preoperative 18F-labeled deoxyglucose uptake correlated positively with the amount of cardiomyocytes showing excess glycogen stores.

BACKGROUND: Both thallium imaging and low-dose dobutamine echocardiography have been proposed to predict the reversibility of left ventricular (LV) dysfunction in patients with coronary disease. The present study was designed to evaluate whether the use of these techniques during the preoperative assessment of coronary patients with depressed LV function can improve our ability to identify those likely to have improved LV function after surgery. METHODS AND RESULTS: Forty consecutive patients (age, 60 +/- 10 years) with coronary disease and an ejection fraction < or = 35% underwent dobutamine echocardiography (10 micrograms/kg per minute) and exercise-redistribution-reinjection thallium single photon emission computed tomography (SPECT) before coronary revascularization by bypass surgery (n = 33) or angioplasty (n = 7). Recovery of LV function was evaluated by echocardiography 5.3 +/- 2.4 months after revascularization. According to the changes in end-systolic volume and ejection fraction after revascularization, the patients were categorized into groups with (n = 19) and without (n = 21) postoperative functional improvement, defined as a > 5% increase in ejection fraction and > 10 mL decrease in end-systolic volume. Before revascularization, patients with improved postoperative function had smaller end-diastolic volume and less wall motion abnormalities than those with persistent dysfunction. They also showed greater improvement of wall motion score with dobutamine (6.1 +/- 2.4 versus 1.8 +/- 4.2 grades, P < .001) and smaller thallium defect score after exercise (38 +/- 12 versus 47 +/- 14 grades, P = .04). Discriminant analysis selected the improvement in wall motion score with dobutamine and baseline end-diastolic volume as independent predictors of postoperative recovery. From April 1990 to May 1994, 89 patients (median age, 42 years; range, 10 days to 66 years) underwent aortic valve or root replacement with allografts or autografts. Thirteen patients were less than 18 years old at the time of operation. Indication for aortic valve replacement was aortic stenosis (50 patients, 56%), small stenotic prosthesis (2 patients, 2%), aortic valve endocarditis (19 patients, 21%), isolated aortic regurgitation (17 patients, 19%), and type II truncus arteriosus (1 patient, 1%). The subcoronary implantation was used in 45 patients (group A), and implantation of an intraluminal cylinder (16 patients) or complete root replacement (28 patients) was performed in the remaining 44 patients (group B). The Ross procedure was performed in 22 patients. Intraoperative transesophageal echocardiography was used routinely. Five patients died in the early postoperative period (6%), 2 in group A and 3 in group B. Three other patients required immediate replacement of a failing graft by a mechanical prosthesis (1 in group A and 2 in group B). There has been no late death. All survivors remained in New York Heart Association functional class I and were free of thromboembolic complications. Endocarditis occurred in 2 patients, 1 year after operation. Both were successfully treated medically. Echocardiographic studies were obtained serially in every patient. Four patients, 2 in group A and 2 in group B underwent reoperation because of mild-to-moderate aortic regurgitation (rate of reoperation, 5%). A restrictive definition of ischemic mitral regurgitation (IMR) has allowed us to select a more uniform cohort of 41 patients having undergone mitral valve surgery for IMR between January 1993 and March 1995. Thirty-six patients (88%) presented with chronic and five with acute IMR. All patients had at least one significant stenosis in the circumflex area. Left ventricular ejection fraction averaged 35%. Transesophageal echocardiography (TEE) revealed a > or = 3+ MR in 30 patients and an intermittent, fluctuating or grade 2+ MR in 11 (27%). Annulus dilatation was found in all cases, and it was the only mechanism in 10 (24.4%). Leaflet restrictive motion was clearly seen in 17 cases (41.5%) and leaflet prolapse in 14 (34%). In doubtful cases, an intra-operative dynamic testing using TEE, and associating a loading test and an afterload test, led to the indication of a valve procedure in 11/19 patients (58%). An average of three distal coronary anastomoses per patient were constructed. Mitral valve replacement (MVR) was unavoidable in four patients (9.8%); at least the posterior leaflet subvalvular apparatus was preserved in all. Repair of the mitral valve (Mvrep) was achieved in 37 cases (90%). Ring annuloplasty alone was performed in 27 cases (73%). In the remaining 10 cases, leaflet prolapse was corrected by various artifices such as flip-over technique, quadrangular resection, papillary muscle plication or commissuroplasty. At the 10th postoperative day, a residual MR was found in 4/34 cases (11.8%), only after isolated ring annuloplasty. The four patients who have died in the ICU after MVrep belonged to the same group of isolated ring annuloplasty; this mortality of 4/27 (14.8%) illustrates the mediocre prognosis of marked annulus dilatation and impaired LV function with restrictive leaflet motion. During the period 1970-1993, 116 patients (63 men, 53 women) with native aortic valvular infective endocarditis were treated surgically. The mean age was 37 years. The main causative organisms were streptococci and staphylococci. Indication for surgery was cardiac failure (70 cases), uncontrolled sepsis (30), peripheral emboli (11) and overwhelming destruction of the aortic valve (five). Hospital and late mortality rates were 8% and 11% respectively. Patients who died in hospital and those who presented a paravalvular leakage had a ring abscess associated with aortic wall destruction. Among 34 patients screened for cerebral septic emboli the condition was confirmed in 15, of whom six were symptom-free. Thus, it is believed that in the presence of root abscess, surgery should be undertaken promptly, regardless of the cardiac status. BACKGROUND: The gastroepiploic artery is increasingly used as an alternative arterial coronary bypass conduit. In vitro studies have reported differences in vasoreactivity among various types of coronary graft conduits, susceptible to influencing the adaptation of myocardial blood flow and long-term patency rate. METHODS AND RESULTS: To evaluate in vivo the vasoreactivity of gastroepiploic artery grafts implanted long-term, nine angiographically smooth grafts implanted to the distal right or to the left circumflex coronary artery were studied with quantitative angiography 6 to 36 months after surgery. Angiograms were obtained on 35mm cinefilms in basal conditions, after injection of methylergometrine (0.4 mg IV), and after intragraft injection of 1 mg isosorbide dinitrate. In basal conditions, there was no difference in luminal diameter between gastroepiploic and coronary arteries (1.64 +/- 0.32 versus 1.51 +/- 0.31 mm; P = NS). After methylergometrine, a constriction was observed in all gastroepiploic artery grafts (-14 +/- 6% of basal diameter) and in all but one grafted coronary artery (-6 +/- 5%). After isosorbide dinitrate, a dilation was consistently observed in all gastroepiploic artery grafts (+26 +/- 9%) and grafted coronary arteries (+14 +/- 7% of basal). Changes in lumen diameter in response to these constrictor and dilator stimuli, either expressed in absolute values or in percentage of control were significantly greater (P < .001) in gastroepiploic artery grafts than in grafted coronary arteries. CONCLUSIONS: Gastroepiploic artery grafts implanted long-term are more reactive than grafted coronary arteries to ergometrine and nitrates. This response differs from that previously reported of internal mammary artery grafts to the same pharmacological vasoactive stimuli. We sequentially recorded subcortical (P14) and cortical (N20) somatosensory evoked potentials (SEPs) in 32 patients undergoing deep hypothermic circulatory arrest (CA). Under normal hemodynamic conditions, hypothermia initially produced N20 disappearance at a mean nasopharyngeal temperature of 20.4 +/- 2.6 degrees C (range 14.5 to 26.1 degrees C) and P14 disappearance at a mean of 16.9 +/- 2.0 degrees C (range 12.4 to 20.2 degrees C). On rewarming, P14 reappeared at mean temperature of 19.3 +/- 4.0 degrees C (range 13.5 to 29.2 degrees C) and N20 at a mean of 21.1 +/- 4.1 degrees C (range 14.3 to 29.6 degrees C). The delay of SEP reappearance after restoration of blood flow correlated significantly with CA duration (r = 0.74 for P14, and r = 0.62 for N20; p < 0.01). Neurological recovery was uneventful in 23 patients; 5 patients presented with neurological sequelae (minor or transient in 4; no recovery from anesthesia and death after 48 hours in 1), and 4 patients died during operation. Twenty-three of 24 surviving patients in whom P14 disappearance was the criterion that hypothermia was deep enough to perform CA (duration: 17 to 94 min) had a normal neurological outcome. By contrast, all surviving patients in whom cortical SEPs disappeared at higher temperatures presented neurological sequelae. In conclusion, the neurophysiological monitoring of brain stem activity, as specifically provided by SEPs, enables determination of the optimal temperature for CA, and demonstrates superiority of SEP monitoring over the use of EEG.

Various thrombo-embolic complications have been related to the antiphospholipid syndrome. The authors report the case of a patient with a primary antiphospholipid syndrome associated with a right auricular mass highly suggesting a myxoma by both echocardiography and nuclear magnetic resonance imaging. Meanwhile, the histological examination of the operative specimen showed a well organized and calcified thrombus. Intracardiac thrombus has been rarely reported as a complication of the antiphospholipid syndrome. The preoperative investigations can't differenciate such a thrombus with a myxoma and the diagnosis is usually made postoperatively.

Sotalol is a beta-adrenergic blocking drug with the additional property of lengthening the cardiac action potential. These electrophysiologic properties render the drug attractive for use in the prevention of postoperative supraventricular arrhythmias (SVA), and previous studies have suggested that it was indeed effective. The hemodynamic response to sotalol and its safety early after coronary artery bypass graft (CABG) surgery were therefore studied. Forty-two patients undergoing CABG were randomly assigned either to receive sotalol to prevent postoperative SVA (25 patients) or to serve as controls (17 patients). Sotalol was started 6 hours after surgery if patients had a cardiac index > 2.8 L/min/m2 with a pulmonary capillary wedge pressure < 15 mmHg, and if they had no contraindications to the use of beta-blockers. The drug was given as a loading infusion of 1 mg/kg over 2 hours, followed by a maintenance infusion of 0.15 mg/kg/h for 24 hours. Three hours later, patients received the first oral dose of 80 mg to be repeated every 8 or 12 hours. Adverse effects necessitating discontinuation of the drug (bradycardia < 50 beats/min, systolic blood pressure < 90 mmHg, or cardiac index < 2.2 L/min/m2) occurred in six patients (24%) and were mainly related to the loading infusion. The hemodynamic data for patients who completed the study were characterized by a significant fall of the cardiac index caused by a lower heart rate without significant change of the stroke volume index. The incidence of paraparesis or paraplegia following surgical repair of aneurysms of the thoracic or thoraco-abdominal aorta has been reported to be in the range of 2% to 25%. These complications are likely to occur in two circumstances: first, too long a period of aortic cross-clamping associated with a low distal perfusion pressure (< 60 mmHg); second, the exclusion of critical intercostal or lumbar arteries. The first mechanism can be prevented by short cross-clamping time or an efficient atrio-femoral shunt. The second mechanism can theoretically be prevented by reimplantation on the graft of critical arteries. However, this procedure often has to be performed in a blind fashion and can sometimes uselessly increase the cross-clamping duration, thereby increasing the risk of the occurrence of the former mechanism. [Revision of Midline Sternotomies After Cardiac-surgery in 1988 - Retrospective Study] Twenty-three of 514 (4.6%) patients who underwent cardiac surgery in 1988 with cardiopulmonary bypass were later reoperated for sternal complications. Several risk factors, including pre-, intra and postoperative variables, have been investigated. A significant correlation was found with the presence of diabetes mellitus, the type of operation (bilateral internal mammary artery grafting), early reoperations, haemodynamic alterations, pulmonary infections and incidence of serious non infectious complications. The different sternal complications are analyzed from a clinical and therapeutic point of view.
