Lipoprotein (a) [Lp(a)] is a lipoprotein with multiple deleterious characteristics and is a recognized cardiovascular (CV) risk factor. The pro-atherogenic, pro-thrombotic, and pro-inflammatory features of Lp(a) are associated not only with atherosclerotic vascular disease but also with aortic valve calcification and all-cause mortality. One of the most interesting aspects of Lp(a) is that its level is determined by genetics in more than 90% of cases, with lifestyle habits having very little influence. Therefore, the recommendation is to test it, at least, once in a lifetime. Contrary to previous beliefs, evidence in recent decades has shown that women have the same or even greater CV risk than men of the same age, attributed to female sex hormones. Different stages of a woman's life can impact on Lp(a) levels, from newborn to menopause, including other critical moments such as menarche and pregnancy. The main objective of this review is to describe and analyze the effect of different specific periods of a woman's life on Lp(a) levels and the potential clinical relevance on their CV risk.

204. Can all children flourish in Nordic communities? A study of how elementary school children perceive their well-being

Few Nordic studies put children’s experiences of social inequality at their centre. This article explores the children’s perceptions of their education and well-being within two Nordic cities. The participants in this cross-national comparative qualitative study are children living in polarised urban metropolitan areas in Norway and Sweden. Our analysis is based on 27 group interviews with 91 boys and girls between the ages of 9-10 and 11-12. Theoretical inspiration is found in the term ‘to flourish’ as utilised by Erik Olin Wright and Erik Allardt’s theory for operationalising well-being from a childhood welfare perspective. The researchers interviewed the children regarding a) belongings and resources (having) b) interests and connections to their surroundings (what they love), c) how they experience their role in their environment and d) in their educational system (being). The findings reveal that even though the Nordic education systems attempt to ensure educational equity, the children in this study indicate a lack of communication between educational communities within and between schools, disparities in available resources, and great differences in what the school communities require from them. The findings show that pupils of lower SES families are excluded, and many show resilience; those children living in upper socio-economic status (SES) areas are given the opportunity to adapt more to the system’s requests without a system that adapts to their needs. Nordic cities still provide unequal childhoods for all children, which has both immediate and long-term impacts on their lives. The study shows how the Nordic educational systems produce competitive systems and school segregation that cannot compensate for inequality and offer well-being and the possibility for all of their students to live flourishing lives.

El área máxima de copa es una variable importante en la estimación del nivel de competencia que afecta al potencial de crecimiento y productividad de las especies. Por lo tanto, el objetivo del presente estudio fue ajustar modelos de regresión para predecir el diámetro máximo de copa para Pinus cooperi Blanco en la región forestal de El Salto, en el estado de Durango. Particularmente se utilizaron 95 árboles de diferentes categorías diamétricas y creciendo libres de competencia. De manera independiente, se analizaron tres modelos de regresión, lineal simple, polinómico de segundo grado y potencial, para predecir el diámetro máximo de copa en función del diámetro normal, altura total, altura a la base de la copa, edad y longitud total de la copa. Los resultados evidenciaron que el modelo polinómico de segundo grado con la inclusión del diámetro normal como variable independiente presentó el mejor ajuste en la predicción del diámetro máximo de copa. En general el diámetro normal fue la variable más significativa para la predicción del diámetro máximo de copa en Pinus cooperi. Con base a los resultados se pueden identificar dos escenarios de productividad del bosque relacionados con factor de competencia de copa (FCC), cuando este es del 100 % o superior, la productividad maderable es óptima y cuando es menor al 100% la productividad no es óptima pudiendo estar atribuido a la poca o nula competencia y deficiencia en el uso del suelo.

BackgroundHealth Promoting Schools (HPS) have emerged as a powerful framework to promote healthy behaviors in many countries. However, HPS still present several challenges, highlighting the excessive workload involved in the accreditation, design, implementation, and evaluation processes. In this sense, a resource to facilitate the implementation processes may have a positive impact on the support of HPS. ObjectiveThe aim of this study was to describe the co-design processes undertaken and resulting learnings to develop the Red Escuelas Promotoras de Salud (network of health promoting schools; REDEPS)-Gestion platform to facilitate the accreditation, design, implementation procedures, and evaluation processes of the Aragon's Health-Promoting School Network. MethodsThe Double Diamond Design Approach was used to co-design this web-platform. The different stakeholders that participated in this co-design, progressed through a 4-stage reflective phase, to discover, define, develop, and deliver the REDEPS-Gestion platform. ResultsParticipants agreed that the functions of the REDEPS-Gestion platform should permit the management of both the educational centers and the administration such as accreditation processes, definition and review intervention projects, and preparation and review of the different progress reports to evaluate the HPS. Despite co-design being a well-established approach to creative practice, especially within the public sector, some challenges emerged during the co-design process, such as engaging and facilitating stakeholders’ participation or the complexity of combining the interests of all stakeholders. This approach allowed us to identify the main barriers for future users and implement platform improvements. ConclusionsWe hope that the REDEPS-Gestion platform will therefore be able to contribute to facilitating the implementation of HPS. The Double Diamond Design Approach used to co-design this web platform was an efficient and feasible methodological design approach. The REDEPS-Gestion platform will facilitate HPS implementation in Aragon as well as all the processes involving HPS. Future work will determine its effectiveness in improving HPS implementation. 207. Development and validation of a National Prostalgia scale to measure acceptance and desire for cultural change

IntroductionDemographic changes occurring throughout the world, as well as different forms of climate and social movements, are examples of the constant cultural change people experience. Many of these changes reflect broad political and existential threats. We developed and tested the concept of national prostalgia, which we define as the longing of the nation’s future. According to Cultural Inertia theory, national prostalgia is a psychological propeller that facilitates acceptance of cultural change.MethodsWe conducted two correlational studies to develop and test the construct, and the predictive, divergent, and convergent validity of national prostalgia by comparing it to two prospection scales that measure future-thinking, and a third study to experimentally test if national prostalgia can be manipulated. We hypothesized that national prostalgia would predict higher acceptance to cultural change. Change was operationalized as eco-friendly intentions, engagement of new norms created during the COVID-19 pandemic and lowering prejudiced attitudes–beyond the effect of national nostalgia, prospection, optimism, and openness to new experiences.ResultsStudy 1 demonstrated that national prostalgia was a better predictor than prospection for higher eco-friendly intentions and acceptance of new normal norms, even when controlling for national nostalgia (i.e., a psychological anchor). In study 2, national prostalgia predicted higher eco- friendly intentions and acceptance of new normal norms, even when controlling for prospection, optimism, and openness to new experiences. National prostalgia did not predict outgroup derogation or white nationalism–variables that indicate prejudice. Study 3 replicated the basic effects, but manipulations designed to influence national prostalgia did not have their predicted effects.DiscussionThus, national prostalgia is a psychological propeller that goes above and beyond the effect of prospection and we now have a reliable and valid scale to measure national prostalgia.

This Letter presents the first measurement of the impact-parameter dependent angular anisotropy in the decay of coherently photoproduced ρ0 mesons. The ρ0 mesons are reconstructed through their decay into pion pairs. The measured anisotropy corresponds to the amplitude of the cos(2ϕ) modulation, where ϕ is the angle between the two vectors formed by the sum and the difference of the transverse momenta of the pions, respectively. The measurement was performed by the ALICE Collaboration at the LHC using data from ultraperipheral Pb–Pb collisions at a center-of-mass energy of sNN=5.02 TeV per nucleon pair. Different impact-parameter regions are selected by classifying the events in nuclear-breakup classes. The amplitude of the cos(2ϕ) modulation is found to increase by about one order of magnitude from large to small impact parameters. Theoretical calculations describe the measured cos(2ϕ) anisotropy and its impact-parameter dependence as the result of a quantum interference effect at the femtometer scale, arising from the ambiguity regarding which of the nuclei is the photon source in the interaction.

In this letter, measurements of (anti)alpha production in central (0–10%) Pb–Pb collisions at a center-of-mass energy per nucleon–nucleon pair of sNN = 5.02 TeV are presented, including the first measurement of an antialpha transverse-momentum spectrum. Owing to its large mass, the production of (anti)alpha is expected to be sensitive to different particle production models. The production yields and transverse-momentum spectra of nuclei are of particular interest because they provide a stringent test of these models. The averaged antialpha and alpha spectrum is compared to the spectra of lighter particles, by including it into a common blast-wave fit capturing the hydrodynamic-like flow of all particles. This fit is indicating that the (anti)alpha also participates in the collective expansion of the medium created in the collision. A blast-wave fit including only protons, (anti)alpha, and other light nuclei results in a similar flow velocity as the fit that includes all particles. A similar flow velocity, but a significantly larger kinetic freeze-out temperature is obtained when only protons and light nuclei are included in the fit. The coalescence parameter B4 is well described by calculations from a statistical hadronization model but significantly underestimated by calculations assuming nucleus formation via coalescence of nucleons. Similarly, the (anti)alpha-to-proton ratio is well described by the statistical hadronization model. On the other hand, coalescence calculations including approaches with different implementations of the (anti)alpha substructure tend to underestimate the data.

Introducción: Un nivel de colesterol-LDL > 190 mg/dL indica hipercolesterolemia severa de origen monogénico y/o poligénico. Los puntajes de riesgo genético (GRS) evalúan potenciales causas poligénicas. Objetivo: Aplicar el GRS de seis SNP (GRS-6) en individuos con hipercolesterolemia severa. Material y métodos: Se estudiaron 69 sujetos del registro de detección de hipercolesterolemia familiar (HF) en Argentina. Resultados: Con 44 individuos con fenotipo de HF sin variantes genéticas que indicaran origen monogénico (HF/M−) y 26 sujetos de control, se estableció el valor de corte de GRS-6 en > 0.76, con sensibilidad de 0.59 y especificidad de 0.69; 15 sujetos HF/M− (34 %) presentaron GRS-6 positivo. Los valores medios del GRS-6 en los pacientes con HF/M−, HF/M+ y los sujetos de control fueron 0.72 ± 0.17, 0.66 ± 0.17 y 0.70 ± 0.13 (p = 0.43). No existieron diferencias significativas en los valores de colesterol, ni en el puntaje clínico, entre los casos con GRS-6 positivo versus negativo. En 32 % de los casos se obtuvieron valores positivos para contribución poligénica con GRS-6 versus 20 % con GRS-10, puntaje que previamente había sido aplicado (p = 0.003). Conclusión: Describimos el primer valor de corte para el GRS-6 en una población latinoamericana, y concluimos que este GRS podría aportar a la evaluación de la contribución poligénica en los pacientes con hipercolesterolemia severa en la población argentina de manera similar a la de otras poblaciones europeas.

Illicit or non-normative sentimental relationships appear repeatedly in many of the short stories that comprise James Joyce’s Dubliners (1914). This type of emotional link did not have any room in end-of-century Catholic Ireland, and any unorthodox relationship was regarded or punished as sinful and socially unacceptable, following the strict morality of the times. In this article, I intend to analyse some of the most significant stories in Dubliners, in order to dissect the ways in which late nineteenth-century Dublin’s double standards punished any subject steering away from established social norms concerning marriage and acceptable relationships, either by forcing the reclusion of the subjects to the domestic/private sphere or by imposing a normative marriage on them, or even by pushing them to the brink of madness, alcoholism, or suicide.

La presente investigación que se desarrolló en la ciudad de Cuenca, Ecuador, tuvo como objetivo elaborar un modelo de gestión para el Destino Turístico Inteligente (DTI), acorde a la realidad actual de la ciudad que se presenta dentro de los procesos económicos, de investigación, desarrollo tecnológico e innovación. Como resultado de la investigación se propone un modelo de gestión que considera la posibilidad de mejorar la gestión de manera integral y horizontal del turismo en la ciudad de Cuenca, con un efecto multiplicador por medio del uso de las Tecnologías de la Información y Comunicación (TIC), que es una realidad en el mundo. La validación del modelo se realizó mediante los métodos Delphi y Anochi, a expertos y usuarios, que mostraron resultados de alta satisfacción.

Background: Charcot foot is a severe complication of diabetes mellitus. Amputation is associated with 5-year mortality rates as high as 70%, and the overall treatment cost for diabetic foot surpasses that of conditions such as cancer or depression. Objectives: To compare clinical, quality-of-life, and cost outcomes related to Charcot foot management through two distinct treatments: amputation and resection with stabilization using circular external fixation (CEF). Methods: This retrospective study included all adult patients treated at our unit between 2008 and 2022 for acute diabetic foot with infected ulcers. The allocation to treatment groups was based on the timing of patient enrollment. We gathered anthropometric, diagnostic, and surgical data, documenting individualized costs for preoperative, postoperative, and rehabilitation phases. Health status was assessed using the EQ-5D-3L questionnaire, and recorded data included mortality. Results: A total of 31 patients (18 amputations; 13 CEF) were included. Amputees exhibited significantly higher mortality compared to those with a CEF (44.8% vs 7.7%, p = 0.045). The estimated 3-year survival was 60.8% for amputees and 90% for the CEF group (log-rank test, p = 0.096). Detecting people carrying firearms in outdoor or indoor scenes usually identifies (or avoids) potentially dangerous situations. Nevertheless, the automatic detection of these weapons can be greatly affected by the scene conditions. Commonly, in real scenes these firearms can be seen from different perspectives. They also may have different real and apparent sizes. Moreover, the images containing these targets are usually cluttered, and firearms can appear as partially occluded. It is also common that the images can be affected by several types of distortions such as impulse noise, image darkening or blurring. All these perceived variabilities could significantly degrade the accuracy of firearm detection. Current deep detection networks offer good classification accuracy, with high efficiency and under constrained computational resources. However, the influence of practical conditions in which the objects are to be detected has not sufficiently been analyzed. Our article describes an experimental study on how a set of selected image distortions quantitatively degrade the detection performance on test images when the detection networks have only been trained with images that do not present the alterations. The analyzed test image distortions include impulse noise, blurring (or defocus), image darkening, image shrinking and occlusions. In order to quantify the impact of each individual distortion on the firearm detection problem, we have used a standard YOLOv5 network. Our experimental results have shown that the increased addition of impulse salt-and-pepper noise is by far the distortion that affects the most the performance of the detection network.

Organic solar cells (OSCs), especially those employing bulk heterojunction architecture, present a promising avenue in renewable energy technology. These devices utilize organic materials and can be doped by solvents such as dimethylformamide (DMF), tetrahydrofuran (THF), and dimethyl sulfoxide (DMSO). Solvent doping (DMF, THF, and DMSO) is observed to augment the efficiency of OSCs. However, a trade‐off exists between the volume of solvent used and the device's efficiency. The judicious selection of solvents is crucial as it directly impacts the environmental footprint of the fabrication process and the power conversion efficiency. Notably, the use of solvents in OSC fabrication contributes to reducing the environmental impact across various categories, in particular Abiotic Depletion, Global Warming, and Human Toxicity. Among the solvents studied, THF demonstrates the most significant reduction in environmental impact. Therefore, optimizing the choice and volume of solvents in OSC fabrication is paramount for achieving both enhanced device performance and minimal environmental footprint.

217. Using computational learning for non-melanoma skin cancer and actinic keratosis near-infrared hyperspectral signature classification

Background: The early detection of Non-Melanoma Skin Cancer (NMSC) is essential to ensure patients receive the most effective treatment. Diagnostic screening tools for NMSC are crucial due to high confusion rates with other types of skin lesions, such as Actinic Keratosis. Nevertheless, current means of diagnosing and screening patients rely on either visual criteria, that are often conditioned by subjectivity and experience, or highly invasive, slow, and costly methods, such as histological diagnoses. From this, the objectives of the present study are to test if classification accuracies improve in the Near-Infrared region of the electromagnetic spectrum, as opposed to previous research in shorter wavelengths. Methods: This study utilizes near-infrared hyperspectral imaging, within the range of 900.6 and 1454.8 nm. Images were captured for a total of 125 patients, including 66 patients with Basal Cell Carcinoma, 42 with cutaneous Squamous Cell Carcinoma, and 17 with Actinic Keratosis, to differentiate between healthy and unhealthy skin lesions. A combination of hybrid convolutional neural networks (for feature extraction) and support vector machine algorithms (as a final activation layer) was employed for analysis. In addition, we test whether transfer learning is feasible from networks trained on shorter wavelengths of the electromagnetic spectrum. Results: The implemented method achieved a general accuracy of over 80 %, with some tasks reaching over 90 %. F1 scores were also found to generally be over the optimal threshold of 0.8. The best results were obtained when detecting Actinic Keratosis, however differentiation between the two types of malignant lesions was often noted to be more difficult. These results demonstrate the potential of near-infrared hyperspectral imaging combined with advanced machine learning techniques in distinguishing NMSC from other skin lesions. Transfer learning was unsuccessful in improving the training of these algorithms. Conclusions: We have shown that the Near-Infrared region of the electromagnetic spectrum is highly useful for the identification and study of non-melanoma type skin lesions. While the results are promising, further research is required to develop more robust algorithms that can minimize the impact of noise in these datasets before clinical application is feasible.

Objective. The purpose of this study was to describe the experiences of individuals with post-coronavirus 19 (COVID-19) condition symptoms who underwent a supervised telerehabilitation and home-based respiratory muscle training (TSHB-RMT) program. Methods. A qualitative descriptive study was conducted. Participants were recruited using purposeful sampling. The inclusion criteria consisted of: patients aged over 18 years who presented persistent COVID-19 symptoms of fatigue and dyspnea for at least 3 months after the COVID-19 diagnosis. In total, 28 patients were included. In-depth interviews and researcher field notes were used to collect the data. A thematic analysis was performed. Results. Three themes reflect the patients' perspective on the TSHB-RMT before the program (reasons for participating), during the treatment program, and upon completion of the study. Among the reasons for participation, participants highlighted the absence of improvement and treatment, and feeling abandoned and forgotten by the health system. The treatment required discipline on behalf of the patients. Mondays and Tuesdays were the most difficult days for performing the therapy, and the physical therapist was perceived as a tool for adherence, change, and a source of validated information. The patients perceived positive effects quite soon; however, it was necessary to extend the follow-up after completing the program because they abandoned the program due to the lack of guidance for exercise supervision. Conclusion. This study described relevant aspects that physical therapist professionals should consider when providing TSHB-RMT treatment. Impact. TSHB-RMT requires discipline, perseverance, effort, and a commitment to the group. The physical therapist is perceived as the tool that facilitates adherence and participation. The effects are rapidly perceived, leading to improved self-confidence and autonomy; however, it is necessary to increase the follow-up time.
