226. Do Hospital Data Breaches Reduce Patient Care Quality?

Abstract

Objective: To estimate the relationship between a hospital data breach and hospital quality outcome Materials and Methods: Hospital data breaches reported to the U.S. Department of Health and Human Services breach portal and the Privacy Rights Clearinghouse database were merged with the Medicare Hospital Compare data to assemble a panel of non-federal acutecare inpatient hospitals for years 2011 to 2015. The study panel included 2,619 hospitals. Changes in 30-day AMI mortality rate following a hospital data breach were estimated using a multivariate regression model based on a difference-in-differences approach. Results: A data breach was associated with a 0.338[95% CI, 0.101-0.576] percentage point increase in the 30-day AMI mortality rate in the year following the breach and a 0.446[95% CI, 0.164-0.729] percentage point increase two years after the breach. For comparison, the median 30-day AMI mortality rate has been decreasing about 0.4 percentage points annually since 2011 due to progress in care. The magnitude of the breach impact on hospitals' AMI mortality rates was comparable to a year's worth historical progress in reducing AMI mortality rates. Conclusion: Hospital data breaches significantly increased the 30-day mortality rate for AMI. Data breaches may disrupt the processes of care that rely on health information technology. Financial costs to repair a breach may also divert resources away from patient care. Abstract

Pregnancy imposes a substantial metabolic burden on women through weight gain and insulin resistance. Lactation reduces the risk of maternal postpartum diabetes, but the mechanisms underlying this benefit are unknown. Here, we identified long-term beneficial effects of lactation on β cell function, which last for years after the cessation of lactation. We analyzed metabolic phenotypes including β cell characteristics in lactating and non-lactating humans and mice. Lactating and non-lactating women showed comparable glucose tolerance at 2 months after delivery, but after a mean of 3.6 years, glucose tolerance in lactated women had improved compared to non-lactated women. In humans, the disposition index, a measure of insulin secretory function of β cells considering the degree of insulin sensitivity, was higher in lactated women at 3.6 years after delivery. In mice, lactation improved glucose tolerance and increased β cell mass at 3 weeks after delivery. Amelioration of glucose tolerance and insulin secretion were maintained up to 4 months after delivery in lactated mice. During lactation, prolactin induced serotonin production in β cells. Secreted serotonin stimulated β cell proliferation through serotonin receptor 2B in an autocrine and paracrine manner. In addition, intracellular serotonin acted as an antioxidant to mitigate oxidative stress and improved β cell survival. Together, our results suggest that serotonin mediates the long-term beneficial effects of lactation on female metabolic health by increasing β cell proliferation and reducing oxidative stress in β cells.

Abstract

BACKGROUND: Gabapentinoid use for long-term cancer pain control may be problematic, given unclear mechanisms of action and increased concerns for physical dependence. The purpose of this report is to examine trends of gabapentinoid use among US adults with cancer from 2005 to 2015. METHODS: We conducted a serial, cross-sectional study using data from the Medical Expenditure Panel Survey (MEPS). We performed multiple logistic regression to examine the annual percentages of gabapentinoid users, which were adjusted for age, sex, and US region of residence. The amount of gabapentinoid prescriptions filled in 2015 was also estimated. RESULTS: The adjusted percentage of gabapentinoid users in 2015 was 5.60% (3.79%, 7.41%), 2.39 times greater than the percentage in 2005 (p < .001). By 2015, the number of gabapentinoid prescriptions had grown to approximately 3.52 million (2.40 million, 4.65 million). CONCLUSION: We observed greater than a twofold increase in the trend of gabapentinoid medication use among US adults with cancer. Investigations on the long-term efficacy of gabapentinoids for complex pain syndromes, and mitigation of risks, is essential to guide informed clinical management and keep patients safe.

Abstract

The practical use of nanoparticle superlattices (NPSLs) which are of great interest as materials with designed functionalities is often limited by their lack of structural stability under various utilization conditions. Here, we report a new method for directly synthesizing NPSL fully embedded in hierarchically porous silica which provides exceptional stability and efficient pathways for reactant molecules, making the NPSL highly efficient catalyst. The superlattices made of 12 nm gold nanoparticles exhibit exceptionally high catalytic activity for CO oxidation at low temperature, showing higher activity than that of small gold nanoparticles (ca. 3 nm) supported on metal oxides. The gold NPSL also shows unprecedented stability, maintaining its structural stability and catalytic activity without any signature of degradation over a month of continuous catalytic reaction, which present one significant step forward to realizing the great potentials of gold catalysts in automotive emission control and green chemistry industry., Comment: 1. Abstract

ObjectiveThere is a lack of standardisation in the terminology used to describe gout. The aim of this project was to develop a consensus statement describing the recommended nomenclature for disease states of gout.MethodsA content analysis of gout-related articles from rheumatology and general internal medicine journals published over a 5-year period identified potential disease states and the labels commonly assigned to them. Based on these findings, experts in gout were invited to participate in a Delphi exercise and face-to-face consensus meeting to reach agreement on disease state labels and definitions.ResultsThe content analysis identified 13 unique disease states and a total of 63 unique labels. The Delphi exercise (n=76 respondents) and face-to-face meeting (n=35 attendees) established consensus agreement for eight disease state labels and definitions. The agreed labels were as follows: 'asymptomatic hyperuricaemia', 'asymptomatic monosodium urate crystal deposition', 'asymptomatic hyperuricaemia with monosodium urate crystal deposition', 'gout', 'tophaceous gout', 'erosive gout', 'first gout flare' and 'recurrent gout flares'. There was consensus agreement that the label 'gout' should be restricted to current or prior clinically evident disease caused by monosodium urate crystal deposition (gout flare, chronic gouty arthritis or subcutaneous tophus).ConclusionConsensus agreement has been established for the labels and definitions of eight gout disease states, including 'gout' itself. The Gout, Hyperuricaemia and Crystal-Associated Disease Network recommends the use of these labels when describing disease states of gout in research and clinical practice.
