201. No increased risk of tuberculosis‐related immune reconstitution inflammatory syndrome with integrase inhibitor‐based antiretroviral therapy in people with HIV with profound immunosuppression.

Abstract

Introduction: The issue of whether integrase inhibitors (INSTIs) may confer a higher risk of paradoxical tuberculosis‐related immune reconstitution inflammatory syndrome (TB‐IRIS) compared with other classes of antiretroviral in people with HIV with a profound level of immunosuppression remains insufficiently explored. We aimed to assess whether such a higher risk exists by examining a cohort of patients with TB‐HIV initiating antiretroviral therapy (ART) in Hong Kong. Methods: This was a retrospective review of 133 patients registered in the TB‐HIV Registry of the Department of Health during the period 2014–2021. Results: Sixteen of 70 patients (22.9%; 95% confidence interval [CI] 13.0–32.7) and 14 of 63 patients (22.2%; 95% CI 12.0–32.5) from the INSTI and non‐INSTI groups experienced TB‐IRIS (p = 0.920). The median intervals between ART initiation and IRIS among patients from the two groups were similar (3 weeks [interquartile range IQR 2.0–7.8] vs. 4 weeks [IQR 2.0–5.1], p = 0.620). The proportion of patients requiring steroid therapy were similar, as were the hospitalization rates. There was no IRIS‐related death in either group. The risk of TB‐IRIS with INSTI versus non‐INSTI was also similar in a stratified analysis in a subgroup of patients with a baseline CD4 count of <50 μL (10/33 [30.3%; 95% CI 14.6–46.0] vs. 10/22 [45.5%; 95% CI 24.7–66.3], p = 0.252) and another subgroup of patients with ART initiated within 4 weeks of anti‐TB treatment (10/26 [38.5%; 95% CI 19.8–57.2] vs. 10/23 [43.5%; 95% CI 23.2–63.7], p = 0.721). Conclusion: Our cohort study did not offer support for an increased risk of TB‐IRIS with INSTIs compared with non‐INSTIs, even in severely immunocompromised people with HIV. [ABSTRACT FROM AUTHOR]

202. A Hybrid Lens to Realize Electrical Real‐Time Super‐Resolution Imaging.

Abstract

Real‐time dynamic super‐resolution focusing technology is crucial for various imaging applications. However, the diffraction limit significantly impedes the achievement of real‐time dynamic super‐resolution imaging. Prior studies within this domain, such as super‐resolution fluorescence imaging and structured illumination microscopy, heavily rely on fluorescent labels and intricate algorithms. This article proposes a novel approach to achieving real‐time dynamic super‐resolution imaging at microwave frequency by integrating the Mikaelian lens derived from conformal transformation optics with the space‐time‐coding metasurface antenna. Real‐time dynamic super‐resolution focusing with a resolution ranging from 0.3λ to 0.4λ is demonstrated at the periphery of the Mikaelian lens with a numerical aperture (NA) of 0.54. The proposed hybrid lens exhibits the capacity to discern features separated by about one‐third of a wavelength with high precision. The work offers a universal solution for achieving dynamic real‐time super‐resolution imaging electrically, which can be extended to terahertz waves, visible light, and other wave fields, such as acoustic and flexural waves. AbstractIntroductionMethodsResultsDiscussionConclusionsThe toxicokinetics of methyl salicylate after unintentional or intentional ingestion of medicinal oil containing methyl salicylate has not been well studied. We aimed to characterize the trajectory of serum salicylate concentrations and to evaluate factors associated with the peak serum salicylate concentration and the time from ingestion to peak concentration.This was a retrospective cohort study of consecutive patients reported to the Hong Kong Poison Control Centre for laboratory-confirmed methyl salicylate poisoning by all local public emergency departments between 1 July 2008 and 30 June 2023. We analyzed cases with at least three serum salicylate concentrations. Multivariable generalized linear regression was used to identify factors significantly associated with the peak serum concentration and the time from ingestion to peak concentration.We included 41 patients (median age 81.0 years; 32 women and nine men). The median time from ingestion to the first peak serum salicylate concentration was 5.6 h (IQR: 3.2–10.8 h). Multiple regression showed that gastric aspiration (adjusted regression coefficient [β] − 2.50; 95% CI: −3.93 to −1.08; P = 0.001) and single-dose activated charcoal (adjusted β − 1.22; 95% CI: −2.02 to −0.42; P = 0.003) were significantly associated with a lower peak concentration, after adjusting for patient age, sex, exposure due to intentional self-harm, reported ingested dose, time from ingestion to emergency department presentation, vomiting, concurrent use of aspirin (acetylsalicylic acid) and other medications that affect gastric emptying or gastric acid secretion, blood pH, serum albumin concentration, and creatinine clearance.The serum salicylate concentration did not peak as quickly as generally believed, highlighting the importance of continued monitoring. Gastric aspiration and single-dose activated charcoal may help reduce gastrointestinal absorption, but their impact on clinical outcomes remains unclear.Given the median time of 5.6 h (IQR: 3.2–10.8 h) from ingestion to the peak salicylate concentration, gastric aspiration and single-dose activated charcoal can be considered in patients up to a few hours after medicinal oil ingestion when the airway is protected. Abstract

As the proliferation of video content continues, and many video archives lack suitable metadata, therefore, video retrieval, particularly through example‐based search, has become increasingly crucial. Existing metadata often fails to meet the needs of specific types of searches, especially when videos contain elements from different modalities, such as visual and audio. Consequently, developing video retrieval methods that can handle multi‐modal content is essential. An innovative Multi‐modal Video Search by Examples (MVSE) framework is introduced, employing state‐of‐the‐art techniques in its various components. In designing MVSE, the authors focused on accuracy, efficiency, interactivity, and extensibility, with key components including advanced data processing and a user‐friendly interface aimed at enhancing search effectiveness and user experience. Furthermore, the framework was comprehensively evaluated, assessing individual components, data quality issues, and overall retrieval performance using high‐quality and low‐quality BBC archive videos. The evaluation reveals that: (1) multi‐modal search yields better results than single‐modal search; (2) the quality of video, both visual and audio, has an impact on the query precision. Compared with image query results, audio quality has a greater impact on the query precision (3) a two‐stage search process (i.e. searching by Hamming distance based on hashing, followed by searching by Cosine similarity based on embedding); is effective but increases time overhead; (4) large‐scale video retrieval is not only feasible but also expected to emerge shortly. Abstract

The relationship between training load and injury risk in basketball is an important area in sports injury prevention and performance enhancement; however, there is limited conclusive evidence of their associations. The aim of this systematic review was to examine the evidence of the relationship between training load and injury risk in basketball, which is one of the most common sports worldwide. This systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A comprehensive literature search was conducted on the PubMed, SCOPUS, and Web of Science databases up until March 2024. The search aimed to identify studies that prospectively and/or retrospectively examined the relationship between training load and injury risk in basketball. Inclusion criteria were limited to studies published before February 2024. The quality of each study was assessed using the Newcastle–Ottawa Quality Assessment Scale and Oxford Centre for Evidence-Based Medicine levels of evidence. A narrative synthesis of the findings was performed. A total of 14 articles met the inclusion criteria and were included in the review. Of these, 11 studies reported at least partially statistically significant results, providing evidence of a relationship between training load and injury risk. 211. Moral Education in Hong Kong Kindergartens: An Analysis of the Preschool Curriculum Guides

Abstract

Adolescent problems in Hong Kong have brought questions about their moral development and mental health to light. Moral education for preschool children will be strategically important to address these issues because applying early educational interventions to preschool children will effectively enhance their personal development. This article highlights the inadequacy of the Hong Kong preschool curriculum guides in the promotion of moral development in children. The lack of a consensus about the values and character that preschool children should develop is a fundamental problem of implementing preschool moral education in Hong Kong. If moral education is regarded as a school curriculum, either formal or informal, the objectives, content and assessment should be closely adhered to one another. In addition, the learning and teaching activities should also be in line with student's developmental characteristics and experiences. The O-C-A model is then proposed for moral education in the preschool settings of Hong Kong. Advice has also been made for the promotion of moral education in Hong Kong's early childhood settings.

212. The Earnings Advantage of University Education: A Case Study of the Financial Sector in Hong Kong

Abstract

There has been a massification of higher education in Hong Kong since the late 1980s. In addition to programmes offered by local higher education institutions, there are plenty of off-shore degree programmes provided by overseas institutions. In the meantime, Hong Kong has been restructured to an international financial centre, and its financial sector has become one of the largest employers. This cross-sectional study based on Hong Kong's 2011 census data examined the economic effects of university education in terms of earnings in the financial sector of Hong Kong. The major findings about the earnings effect in the sector are: firstly, a match between education and occupation does not necessarily bring about an earnings advantage; secondly, it refutes previous studies reporting that women are disadvantaged in terms of pay as compared with men; thirdly, workers holding an off-shore degree suffer an earnings disadvantage compared with those holding a local or overseas degree. The results also revealed that the provision of off-shore degree programmes in Hong Kong might be of concern to the government and community.

Abstract

The lack of representative nasopharyngeal carcinoma (NPC) models has seriously hampered research on EBV carcinogenesis and preclinical studies in NPC. Here we report the successful growth of five NPC patient-derived xenografts (PDXs) from fifty-eight attempts of transplantation of NPC specimens into NOD/SCID mice. The take rates for primary and recurrent NPC are 4.9% and 17.6%, respectively. Successful establishment of a new EBV-positive NPC cell line, NPC43, is achieved directly from patient NPC tissues by including Rho-associated coiled-coil containing kinases inhibitor (Y-27632) in culture medium. Spontaneous lytic reactivation of EBV can be observed in NPC43 upon withdrawal of Y-27632. Whole-exome sequencing (WES) reveals a close similarity in mutational profiles of these NPC PDXs with their corresponding patient NPC. Whole-genome sequencing (WGS) further delineates the genomic landscape and sequences of EBV genomes in these newly established NPC models, which supports their potential use in future studies of NPC.

Abstract

We identified a homozygous missense alteration (c.75C>A, p.D25E) in CLCC1, encoding a presumptive intracellular chloride channel highly expressed in the retina, associated with autosomal recessive retinitis pigmentosa (arRP) in eight consanguineous families of Pakistani descent. The p.D25E alteration decreased CLCC1 channel function accompanied by accumulation of mutant protein in granules within the ER lumen, while siRNA knockdown of CLCC1 mRNA induced apoptosis in cultured ARPE-19 cells. TALEN KO in zebrafish was lethal 11 days post fertilization. The depressed electroretinogram (ERG) cone response and cone spectral sensitivity of 5 dpf KO zebrafish and reduced eye size, retinal thickness, and expression of rod and cone opsins could be rescued by injection of wild type CLCC1 mRNA. Clcc1+/- KO mice showed decreased ERGs and photoreceptor number. Together these results strongly suggest that intracellular chloride transport by CLCC1 is a critical process in maintaining retinal integrity, and CLCC1 is crucial for survival and function of retinal cells.

Abstract

Endotracheal tube obstruction by a mucus plug causing a ball-valve effect is a rare but significant complication. The inability to pass a suction catheter through the endotracheal tube with high peak and plateau pressure differences are classical features of an endotracheal tube obstruction. A case is described of endotracheal tube obstruction from a mucus plug that compounded severe respiratory acidosis and hypotension in a patient who simultaneously had abdominal compartment syndrome. The mucus plug was not identified until a bronchoscopic assessment of the airway was performed. Due to the absence of classical signs, the delayed identification of the obstructing mucus plug exacerbated diagnostic confusion. It resulted in various treatments being trialed whilst the patient continued to deteriorate from the evasive offending culprit. We suggest that earlier and more routine use of bronchoscopy should be employed in an intensive care unit, especially as a definitive way to rule out endotracheal obstruction.

248. Variability in GRMHD simulations of Sgr A$^*$: Implications for EHT closure phase observations

Abstract

The observable quantities that carry the most information regarding the structures of the images of black holes in the interferometric observations with the Event Horizon Telescope are the closure phases along different baseline triangles. We use long time span, high cadence, GRMHD+radiative transfer models of Sgr A$^*$ to investigate the expected variability of closure phases in such observations. We find that, in general, closure phases along small baseline triangles show little variability, except in the cases when one of the triangle vertices crosses one of a small regions of low visibility amplitude. The closure phase variability increases with the size of the baseline triangle, as larger baselines probe the small-scale structures of the images, which are highly variable. On average, the jet-dominated MAD models show less closure phase variability than the disk-dominated SANE models, even in the large baseline triangles, because the images from the latter are more sensitive to the turbulence in the accretion flow. Our results suggest that image reconstruction techniques need to explicitly take into account the closure phase variability, especially if the quality and quantity of data allow for a detailed characterization of the nature of variability. This also implies that, if image reconstruction techniques that rely on the assumption of a static image are utilized, regions of the $u-v$ space that show a high level of variability will need to be identified and excised., Comment: submitted to apj. 12 pages, 12 figures

We consider the generalization of the Navier-Stokes equations from $\mathbb R^n$ to the Riemannian manifolds. There are inequivalent formulations of the Navier-Stokes equations on manifolds due to the different possibilities for the Laplacian operator acting on vector fields on a Riemannian manifold. 250. Particle Acceleration and the Origin of X-ray Flares in GRMHD simulations of Sgr A*

Abstract

Significant X-ray variability and flaring has been observed from Sgr A* but is poorly understood from a theoretical standpoint. We perform GRMHD simulations that take into account a population of non-thermal electrons with energy distributions and injection rates that are motivated by PIC simulations of magnetic reconnection. We explore the effects of including these non-thermal electrons on the predicted broadband variability of Sgr A* and find that X-ray variability is a generic result of localizing non-thermal electrons to highly magnetized regions, where particles are likely to be accelerated via magnetic reconnection. The proximity of these high-field regions to the event horizon forms a natural connection between IR and X-ray variability and accounts for the rapid timescales associated with the X-ray flares. The qualitative nature of this variability is consistent with observations, producing X-ray flares that are always coincident with IR flares, but not vice versa, i.e., there are a number of IR flares without X-ray counterparts.
