The aim of this study was to estimate whether male welding has an impact on couple fecundability (the probability of conceiving in a menstrual cycle). A sample of Danish couples without previous reproductive experience was recruited nationwide by postal letters to members of the union of metal workers and three other trade unions. Among 430 included couples, 201 males were metal workers and 130 were welders. The couples were followed for a maximum of six menstrual cycles from termination of birth control until a clinical pregnancy was detected. Compared with nonwelding metal workers the fecundability odds ratio (OR) of male exposure to welding was 0.86 (95% confidence interval [CI] 0.58-1.28). An interaction between male smoking and welding was found; within smokers the OR for welding was 0.40 (95% CI 0.17-0.95) and within nonsmokers it was 1.22 (95% CI 0.74-1.99). Previous welding exposure was negatively related to fecundability among smokers (OR 0.84 per year with mild steel welding, OR 0.76 per year with stainless steel welding). No significant results were found when comparing with an external group of nonmetal workers. Decreased fecundability among smoking welders attributable to both current and previous welding exposure is possible, but these findings were the results of subanalyses that were not part of the a priori hypothesis.

Welding may involve hazards to the male reproductive system, but previous studies of semen quality have produced inconsistent results. We studied the effects of welding on markers of semen quality in a Danish nationwide sample of 430 first-time pregnancy planners without earlier reproductive experience. Couples were recruited among members of the union of metal workers and three other trade unions and were followed from termination of birth control until pregnancy for a maximum of six menstrual cycles. The males provided semen samples in each cycle. Median sperm density for welders was 56 x 10(6)/mL (52.5 x 10(6)/mL and 50.0 x 10(6)/mL in two reference groups). No statistically significant differences attributable to welding were found in proportions of morphologically normal sperm, sperm motility assessed by computer-aided sperm analysis, or sex hormones (testosterone, follicle-stimulating hormone, and luteinizing hormone). These negative findings may not apply to populations with high-level exposure to welding fume or to welders exposed to other putative hazards, e.g., heat.

Recent studies have focused on reproductive health of men in the general population. However, semen samples are difficult to obtain within sampling frames that allow comparisons. Blood samples are easier to obtain than ejaculates. Therefore, serum biomarkers of spermatogenesis are of major interest for population studies. FSH has previously been used as a marker of spermatogenesis, although it is also influenced by the hypothalamus. Serum inhibin B was recently suggested as a possible, more direct serum marker of spermatogenesis in men with testicular disorders. In a Danish nationwide collaborative study, we found an unexpected difference in semen concentration between two groups of men recruited from two different centres. We, therefore, analyzed reproductive hormones in blood, including inhibin B, to test whether the observed difference in semen concentration was reflected in the reproductive hormones. From 1992 to 1995, a total of 430 men, 20-35 yr old, who lived with a partner and who had not previously attempted to achieve a pregnancy, were recruited. The couples were enrolled into the study in one of two centres (centre A, n = 231; and centre B, n = 199) when they discontinued birth control. At enrollment, they provided a semen sample (n = 419), and a blood sample was drawn (n = 349). The semen analysis was performed in accordance with the WHO 1992 guidelines, and interlaboratory differences were tested. Inhibin B was measured in an enzyme immunometric assay, which has previously been described. All blood samples were analyzed in the same laboratory. Median sperm concentration and the percentage of morphologically normal spermatozoa were significantly higher among men from centre A (56.0 mill/mL and 42.5%), compared with men from centre B (44.8 mill/mL and 39%). Men from centre B had a significantly higher median FSH (3.42 IU/L) and a lower inhibin B (186 pg/mL) than men from centre A (3.21 IU/L and 209 pg/mL). 212. Task based exposure assessment in ergonomic epidemiology : a study of upper arm elevation in the jobs of machinists, car mechanics, and house painters.

AIMS: To explore the precision of task based estimates of upper arm elevation in three occupational groups, compared to direct measurements of job exposure. METHODS: Male machinists (n = 26), car mechanics (n = 23), and house painters (n = 23) were studied. Whole day recordings of upper arm elevation were obtained for four consecutive working days, and associated task information was collected in diaries. For each individual, task based estimates of job exposure were calculated by weighting task exposures from a collective database by task proportions according to the diaries. These estimates were validated against directly measured job exposures using linear regression. The performance of the task based approach was expressed through the gain in precision of occupational group mean exposures that could be obtained by adding subjects with task based estimates to a group of subjects with measured job exposures in a "validation" design. RESULTS: In all three occupations, tasks differed in mean exposure, and task proportions varied between individuals. Task based estimation proved inefficient, with squared correlation coefficients only occasionally exceeding 0.2 for the relation between task based and measured job exposures. Consequently, it was not possible to substantially improve the precision of an estimated group mean by including subjects whose job exposures were based on task information. CONCLUSIONS: Task based estimates of mechanical job exposure can be very imprecise, and only marginally better than estimates based on occupation. It is recommended that investigators in ergonomic epidemiology consider the prospects of task based exposure assessment carefully before placing resources at obtaining task information. Strategies disregarding tasks may be preferable in many cases

OBJECTIVES: It was hypothesized that the welding of stainless steel involves a risk of male-mediated developmental toxicity because of exposure to mutagenic substances, including hexavalent chromium. The purpose of the present study was to corroborate or refute earlier findings that spouses of stainless steel welders have an increased risk of spontaneous abortion.METHODS: The occurrence of spontaneous abortion among 2520 pregnancies of spouses of 1715 married metal workers from 1977 through 1987 was examined. Occupational histories were collected with a postal questionnaire in a previous study. Information on children born live, spontaneous abortion, and induced abortion was obtained from national medical registers.RESULTS: The proportion of spontaneous abortions was not increased for pregnancies at risk from stainless steel welding when compared with pregnancies not at risk (odds ratio 0.78, 95% confidence interval 0.55-1.1). The risk estimate was robust to adjustment for potential confounding effects of maternal age and parity and male smoking and alcohol consumption.CONCLUSIONS: This study does not corroborate earlier findings that spouses of stainless steel welders have increased risk of spontaneous abortion. A reanalysis indicated that earlier findings were probably biased because the job exposure of male metal workers is apparently modified by the outcome of their partners' first pregnancy.

- Abstract
The human cytokine growth-regulated oncogene (GRO)-α is a small glycoprotein secreted by monocytes, endothelial cells, fibroblasts, synovial cells, and some tumor cells such as melanoma cells. It is structurally related to IL-8 and can activate neutrophils, whereas it induces chemotaxis, exocytosis, and a respiratory burst in neutrophils. To date, its functions on T lymphocytes have not been well established. We report here that recombinant human (rh)GRO-α is a potent chemoattractant for freshly isolated T lymphocytes, but not for anti-CD3 mAb-activated T lymphocytes. It attracts CD4 and CD8 T lymphocyte subsets to an equal extent. The migrating T lymphocytes toward rhGRO-α are predominantly CD45RO memory CD4 and CD8 subsets. The chemotactic migration of T lymphocytes toward rhGRO-α is stimulated via the IL-8 receptors on the cells. This process can be augmented by IFN-γ and TNF-α, and inhibited by IL-4, IL-10, and IL-13. In addition, we also document that on T lymphocytes there exist IL-8 receptors that can be up-regulated by IFN-γ, TNF-α, and IL-2. Our results demonstrate that rhGRO- α gene encodes for an inflammatory mediator that stimulates the directional migration of T lymphocytes. It may thus be another important mediator in the diseases in which T lymphocytes form the major constituent of the cellular infiltration.

Several studies suggest that welding is detrimental to the male reproductive system. Welding fume and radiant heat are of interest as possible causal factors. This study investigates semen quality and sex hormone concentrations among 17 manual metal arc alloyed steel welders with a moderate exposure to radiant heat (globe temperature ranging from 31.1 degrees to 44.8 degrees C), but without substantial exposure to welding fume toxicants. During exposure to heat the skin temperature in the groin increased on average by 1.4 degrees C (SE +/- 0.72 degrees C). Sperm count and motile sperm count were non-significantly reduced among welders in comparison with two different reference groups. Within the group of welders the proportion of sperm with normal shape declined significantly after six weeks of exposure to heat and increased after a break in exposure. Sperm count and sperm concentration had the same pattern of intraindividual change in relation to exposure to radiant heat, but the changes were not statistically significant. No consistent changes in concentrations of sex hormones were found. The welders investigated were more exposed to radiant heat than welders in general. The results suggest that the study group of welders experienced a reversible decrease in semen quality, most likely caused by a moderate exposure to radiant heat (about five hours a day through several weeks). It remains to be established if even lower levels of exposure to radiant heat in the general population of welders has any impact on semen quality and fertility.

- Abstract
Inhaled nitric oxide (iNO) has been extensively used in the treatment of severe hypoxaemic respiratory failure and/or pulmonary hypertension. Although the majority of studies have almost consistently demonstrated a beneficial effect on oxygenation and a reduction in pulmonary artery pressures, none of the randomised trials have reported a reduced mortality using iNO on patients with severe respiratory failure. In neonates with severe respiratory failure and pulmonary hypertension, however, iNO can reduce the need of extracorporal membrane oxygenation. Prior to using iNO on adult patients we suggest other measures to be taken (i.e. optimising ventilator settings, ventilation in prone position).
