205. Assessment of antimicrobial and anti-biofilm activities of lemongrass and bay leaf extracts on microorganisms from fermented cereal-based porridges in northern Benin

Abstract

Lemongrass (Cymbopogon citratus) and bay leaf (Laurus nobilis) are condiments in food processing. This study aimed to assess the antimicrobial and anti-biofilm activities of lemongrass and bay leaf extract on the microorganisms isolated from cereal-based fermented porridges produced in northern Benin. The phytochemical components of lemongrass and bay leaf extracts were analyzed qualitatively using two solvents: water and alcohol. The antimicrobial activity of the extracts was tested using the solid media diffusion method, and the minimum concentration was determined using the microdilution method. The extracts were also evaluated for their ability to inhibit biofilm formation using qualitative assays. The two plants have different phytochemical profiles. Tannins, flavonoids, mucilage, and alkaloids were detected in both plants. Cyanogenic derivatives, O-heterosides, and genin-reduced O-heterosides were absent. L. nobilis contained anthocyanins, quinone derivatives, saponosides, reducing compounds, and free anthracenes, absent in C. citratus. The inhibition diameters ranged from 4.4 to 26.3, depending on the strains and extracts, and varied significantly. The MICs for Escherichia coli ranged from 4.5 mg/ml to 7 mg/ml, staphylococci from 3.75 mg/ml to 10 mg/ml, and lactic bacteria from 2.5 mg/ml to 4.44 mg/ml. The CMB varied between 8.125 mg/ml and 14 mg/ml for E. coli, 7.5 mg/ml to 20 mg/ml for staphylococci, and 6 mg/ml to 10 mg/ml for lactic bacteria. It is worth noting that staphylococci are the most biofilm-producing strains (80%), followed by E. coli (60%), and lactic bacteria produce less biofilm (30%). The findings of this research confirm the presence of a wide range of biological activities, including antibacterial and anti-biofilm, in the aqueous and ethanolic extracts of these two aromatic plants. Further research is required to identify and isolate the active components responsible for their medicinal properties and ascertain their operation mode.

Abstract

Rationale Androgen deprivation therapy (ADT) is pivotal in treating recurrent prostate cancer and is often combined with external beam radiation therapy (EBRT) for localized disease. However, for metastatic castration-resistant prostate cancer, EBRT is typically only used in the palliative setting, because of the inability to radiate all sites of disease. Systemic radiation treatments that preferentially irradiate cancer cells, known as radiopharmaceutical therapy or targeted radionuclide therapy (TRT), have demonstrable benefits for treating metastatic prostate cancer. Here, we explored the use of a novel TRT, 90Y-NM600, specifically in combination with ADT, in murine prostate tumor models.Methods 6-week-old male FVB mice were implanted subcutaneously with Myc-CaP tumor cells and given a single intravenous injection of 90Y-NM600, in combination with ADT (degarelix). The combination and sequence of administration were evaluated for effect on tumor growth and infiltrating immune populations were analyzed by flow cytometry. Sera were assessed to determine treatment effects on cytokine profiles.Results ADT delivered prior to TRT (ADT→TRT) resulted in significantly greater antitumor response and overall survival than if delivered after TRT (TRT→ADT). Studies conducted in immunodeficient NRG mice failed to show a difference in treatment sequence, suggesting an immunological mechanism. Myeloid-derived suppressor cells (MDSCs) significantly accumulated in tumors following TRT→ADT treatment and retained immune suppressive function. However, CD4+ and CD8+ T cells with an activated and memory phenotype were more prevalent in the ADT→TRT group. Depletion of Gr1+MDSCs led to greater antitumor response following either treatment sequence. Chemotaxis assays suggested that tumor cells secreted chemokines that recruited MDSCs, notably CXCL1 and CXCL2. The use of a selective CXCR2 antagonist, reparixin, further improved antitumor responses and overall survival when used in tumor-bearing mice treated with TRT→ADT.Conclusion The combination of ADT and TRT improved antitumor responses in murine models of prostate cancer, however, this was dependent on the order of administration. This was found to be associated with one treatment sequence leading to an increase in infiltrating MDSCs. Combining treatment with a CXCR2 antagonist improved the antitumor effect of this combination, suggesting a possible approach for treating advanced human prostate cancer.

Abstract

Radiation therapy (RT) is a pillar of cancer therapy used by more than half of all cancer patients. Clinically, RT is mostly delivered as external beam radiation therapy (EBRT). However, the scope of EBRT is limited in the metastatic setting, where all sites of disease need to be irradiated. Such a limitation is attributed to radiation-induced toxicities, for example on bone marrow and hematologic toxicities, resulting from a large EBRT field. Radiopharmaceutical therapy (RPT) has emerged as an alternative to EBRT for the irradiation of all sites of metastatic disease. While RPT can reduce tumor burden, it can also impact the immune system and anti-tumor immunity. Understanding these effects is crucial for predicting and managing treatment-related hematological toxicities and optimizing their integration with other therapeutic modalities, such as immunotherapies. Here, we review the immunomodulatory effects of α- and β-particle emitter-based RPT on various immune cell lines, such as CD8+ and CD4+ T cells, natural killer (NK) cells, and regulatory T (Treg) cells. We briefly discuss Auger electron-emitter (AEE)-based RPT, and finally, we highlight the combination of RPT with immune checkpoint inhibitors, which may offer potential therapeutic synergies for patients with metastatic cancers.

El voleibol Camagüeyano ha sido portador de una rica historia y de ella se ha escogido como necesidad de estudio las ideas y concepciones del medallista de bronce olímpico Victoriano Sarmiento Bío contenidas en su quehacer deportivo- educacional y que constituyen una contribución al pujante desarrollo de la escuela cubana de voleibol. La elección de ello se debe, junto a la significación de su imagen para la comunidad de jugadores, entrenadores y profesores cubanos, la necesidad de sistematizar todo su legado como fuente de información para las actuales generaciones de jugadores y entrenadores. A partir de una minuciosa búsqueda bibliográfica se ha podido comprobar que no existen referencias de su trayectoria como jugador, profesor y entrenador. Tales circunstancias actúan como condicionantes y prueban la existencia de elementos contributivos, no sistematizados hasta ahora; en consecuencia, el trabajo se dirige a valorar las ideas acerca de la preparación de los jóvenes voleibolistas y su enseñanza contenidas en su obra deportivo laboral que constituyen una contribución al desarrollo del voleibol camagüeyano, a la escuela cubana de voleibol y con ello a la educación cubana en que se integra su actividad científica y educacional. En esta investigación se utilizan las concepciones teóricas que explican la realización de los estudios histórico-educacionales.

Abstract

BackgroundStarting in November 2020, the US Food and Drug Administration (FDA) has issued Emergency Use Authorizations (EUAs) for multiple novel virus-neutralizing monoclonal antibody therapies, including bamlanivimab monotherapy (now revoked), bamlanivimab and etesivimab, casirivimab and imdevimab (REGEN-COV), and sotrovimab, for treatment or postexposure prophylaxis of Coronavirus disease 2019 (COVID-19) in adolescents (≥12 years of age) and adults with certain high-risk conditions. Previous guidance is now updated based on new evidence and clinical experience.MethodsA panel of experts in pediatric infectious diseases, pediatric infectious diseases pharmacotherapy, and pediatric critical care medicine from 18 geographically diverse US institutions was convened. Through a series of teleconferences and web-based surveys, a guidance statement was developed and refined based on a review of the best available evidence and expert opinion.ResultsThe course of COVID-19 in children and adolescents is typically mild, though more severe disease is occasionally observed. Evidence supporting risk stratification is incomplete. Randomized controlled trials have demonstrated the benefit of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2)-specific monoclonal antibody therapies in adults, but data on safety and efficacy in children or adolescents are limited. Potential harms associated with infusion reactions or anaphylaxis are reportedly low in adults.ConclusionsBased on evidence available as of August 31, 2021, the panel suggests a risk-based approach to administration of SARS-CoV-2 monoclonal antibody therapy. Therapy is suggested for the treatment of mild to moderate COVID-19 in adolescents (≥12 years of age) at the highest risk of progression to hospitalization or severe disease. Therapeutic decision-making about those at moderate risk of severe disease should be individualized. Use as postexposure prophylaxis could be considered for those at the highest risk who have a high-risk exposure but are not yet diagnosed with COVID-19. Clinicians and health systems should ensure safe and timely implementation of these therapeutics that does not exacerbate existing healthcare disparities.

Abstract

The COVID-19 pandemic in Indonesia is still ongoing despite a decrease in the number of cases compared to last year. Based on BPS data, it is assumed that some people believe the COVID-19 pandemic is still ongoing and some believe this pandemic is over. The results of previous research show that the level of prevention behavior for COVID-19 is influenced by the level of media consumption, which is mediated by personal responsibility and the moderating role of one's health orientation. This experimental research aims to analyze the relationship between consumption of information about COVID-19 which influences prevention behavior. The method used is Quasi Experimental: The Time Series Experiment, by comparing prevention behavior after giving treatment in the form of COVID-19 information for 2 weeks in each group. The results of this study indicate that health orientation does not affect a person's personal responsibility, then differences in prevention behavior and also the duration of change in a person's preventive behavior against COVID-19 are influenced by their health orientation.

Abstract

This study explores novel techniques to improve the detection accuracy of skarn iron deposits using advanced image-processing methodologies. Leveraging the capabilities of ASTER image, band ratio (BR) images, and principal component analysis (PCA) alongside the power of 3D convolutional neural networks (3D-CNNs), the research aims to enhance the precision and efficiency of ore detection in complex geological environments. The proposed method employs a specific 3D-CNN architecture accepting input as a 7 × 7 × C image patch, where C represents the combined number of selected ASTER image bands, principal component (PC) bands, and computed BR images. To evaluate the accuracy of the proposed method, five distinct image band combinations, including the proposed band combination, were tested and evaluated based on the overall accuracy (OA), average accuracy (AA), and kappa coefficient. The results demonstrated that while the incorporation of BR images alongside ASTER bands initially seemed promising, it introduced significant confusion in certain classifications, leading to unexpected misclassification rates. Surprisingly, utilizing solely ASTER bands as input parameters yielded higher accuracy rates (OA = 93.13%, AA = 91.96%, kappa = 90.91%) compared with scenarios involving the integration with band ratios (OA = 87.02%, AA = 79.15, kappa = 82.60%) or the integration of BR images to PC bands (OA = 87.78%, AA = 82.39%, kappa = 83.81%). However, the amalgamation of ASTER bands with selected PC bands showed slight improvements in accuracy (OA = 94.65%, AA = 92.93%, kappa = 93.45%), although challenges in accurately classifying certain features persisted. Ultimately, the proposed combination of ASTER bands, PC bands, and BR images (proposed band combination) presented the most visually appealing and statistically accurate results (OA = 96.95%, AA = 94.87%, kappa = 95.93%), effectively addressing misclassifications observed in the other combinations. These findings underscore the synergistic contributions of each of the ASTER bands, PC bands, and BR images, with the ASTER bands proving pivotal for optimal skarn classification, the PC bands enhancing intrusions classification accuracy, and the BR images strengthening wall rock classification accuracy. In conclusion, the proposed combination of input image bands emerges as a robust and comprehensive methodology, demonstrating unparalleled accuracy in the remote sensing detection of skarn iron minerals.

Abstract

Purpose Distinguishing intradural extramedullary (IDEM) spinal ependymoma from myxopapillary ependymoma is challenging due to the location of IDEM spinal ependymoma. This study aimed to investigate the utility of clinical and MR imaging features for differentiating between IDEM spinal and myxopapillary ependymomas. Materials and Methods We compared tumor size, longitudinal/axial location, enhancement degree/ pattern, tumor margin, signal intensity (SI) of the tumor on T2-weighted images and T1-weighted image (T1WI), increased cerebrospinal fluid (CSF) SI caudal to the tumor on T1WI, and CSF dissemination of pathologically confirmed 12 IDEM spinal and 10 myxopapillary ependymomas. Furthermore, classification and regression tree (CART) was performed to identify the clinical and MR features for differentiating between IDEM spinal and myxopapillary ependymomas. Results Patients with IDEM spinal ependymomas were older than those with myxopapillary ependymomas (48 years vs. 29.5 years, p < 0.05). A high SI of the tumor on T1W1 was more frequently observed in IDEM spinal ependymomas than in myxopapillary ependymomas (p = 0.02). Conversely, myxopapillary ependymomas show CSF dissemination. Increased CSF SI caudal to the tumor on T1WI was observed more frequently in myxopapillary ependymomas than in IDEM spinal ependymomas (p < 0.05). Dissemination to the CSF space and increased CSF SI caudal to the tumor on T1WI were the most important variables in CART analysis. Conclusion Clinical and radiological variables may help differentiate between IDEM spinal and myxopapillary ependymomas.

Abstract

Abstract Background While Ghana has a good track record in the Expanded Programme on Immunization, there are substantial challenges with regards to subsequent vaccinations, particularly after the first year of life of the child. Given that the last dose of the RTS, S/AS01E vaccine against malaria is administered at 24 months, there is a high likelihood of default. Hence, it is imperative to understand the dynamics and reasons for the defaults to enable the development of effective implementation strategies. This study explored why caregivers default on the RTS, S/AS01E vaccine from the perspective of health service providers and caregivers. Methods This study employed an exploratory, descriptive approach. Using a purposive sampling technique, caregivers who defaulted and health service providers directly involved in the planning and delivery of the RTS, S/AS01E vaccine at the district level were recruited. A total of five health service providers and 30 mothers (six per FGD) participated in this study. Data analysis was done using NVivo-12 following Collaizi’s thematic framework for qualitative analysis. The study relies on the Standards for Reporting Qualitative Research. Results Reasons for defaulting included the overlap of timing of the last dose and the child starting school, disrespectful attitudes of some health service providers, concerns about adverse side effects and discomforts, travel out of the implementing district, the perception that the vaccines are too many, and lack of support from partners. Conclusion To reduce the occurrence of defaulting on the RTS, S/AS01E vaccine programme, stakeholders must reconsider the timing of the last dose of the vaccine. The schedule of the RTS, S/AS01E vaccine should be aligned with the established EPI schedule of Ghana. This will significantly limit the potential of defaults, particularly for the last dose. Also, the findings from this study underscore a need to encourage male partner involvement in the RTS, S/AS01E vaccine programme. Health promotion programmes could be implemented to raise caregivers’ awareness of potential adverse reactions and discomforts—this is necessary to prepare the caregiver for the vaccine process psychologically.

Abstract

The coronavirus disease (COVID-19) pandemic leveraged telemedicine worldwide mainly due to the need for social distancing, patient safety, and infection prevention. The Hospital das Cl?nicas da Faculdade de Medicina da Universidade de S?o Paulo (HCFMUSP) was a key reference site in the treatment of COVID-19 severe cases in the country. To continue patient's health care, it became necessary to increase the number of teleconsultations and standardize it institutionally. Herein, we briefly described how the HCFMUSP improved the teleconsultation health care service during the COVID-19 pandemic, highlighting the implementation of important innovations and the throughout standardization process, including patients and professional workflow. We also detailed the methodology used to implement or improve teleconsultation in a medical/multidisciplinary specialty at HCFMUSP. All these efforts made the HCFMUSP reach the goal of converting 15% of all face-to-face consultations into teleconsultations only in 2021. In addition, there were more than 370,000 teleconsultations until the end of 2022. Our experience has shown that having a supporting team, a digital certification process, and the data integration were key factors toward the successful implementation of the teleconsultation services. We believe that progressing toward teleconsultation will improve the population covered by health care services in Brazil, as well as contribute to a reduction of waiting time, and solving costs to health care institutions and patients. We expect this report of our experience in teleconsultation implementation could inspire and guide other health care institutions in the development of telemedicine.

Abstract

Since the great droughts of the 1970s and even today, climate risks have been the major challenge facing by the main agricultural and pastoral activities in the Sahel and particularly in Niger. This article maps the impacts of climate threats on food security in three regions of Niger and suggests resilience solutions. The vulnerability analysis approach describing the cause-and-effect relationships between climate change and its impacts on populations, economic sectors, and socio-ecological systems, associated with the notion of impact chains inspired from the ClimProspect model, is used to characterize exposure, sensitivity, and potential impacts. The analysis shows that drought constitutes the main risk which affects agro-silvopastoral production through reductions in food production, the number of animals and their productivity, agricultural income, the disappearance of certain valuable tree species with the consequence of food insecurity and even food and nutrition crises and entrenched poverty. The factors that explain the vulnerability of rural households to food crises are land degradation and insufficiency, poverty, dysfunction of mechanisms and devices for crisis prevention and management and especially the rainfed and rudimentary nature of agro-pastoral activities. They are grouped into four groups - V1, V2, V3 and V4 - serving as benchmarks or decision support tools for efficient and effective responses established in the form of classes of climate risk resilience needs.

Purpose/objective(s)Previous deep learning (DL) algorithms for brain metastasis (BM) detection and segmentation have not been commonly used in clinics because they produce false-positive findings, require multiple sequences, and do not reflect physiological properties such as necrosis. The aim of this study was to develop a more clinically favorable DL algorithm (RLK-Unet) using a single sequence reflecting necrosis and apply it to automated treatment response assessment.Methods and materialsA total of 128 patients with 1339 BMs, who underwent BM magnetic resonance imaging using the contrast-enhanced 3D T1 weighted (T1WI) turbo spin-echo black blood sequence, were included in the development of the DL algorithm. Fifty-eight patients with 629 BMs were assessed for treatment response. The detection sensitivity, precision, Dice similarity coefficient (DSC), and agreement of treatment response assessments between neuroradiologists and RLK-Unet were assessed.ResultsRLK-Unet demonstrated a sensitivity of 86.9% and a precision of 79.6% for BMs and had a DSC of 0.663. Segmentation performance was better in the subgroup with larger BMs (DSC, 0.843). The agreement in the response assessment for BMs between the radiologists and RLK-Unet was excellent (intraclass correlation, 0.84).ConclusionRLK-Unet yielded accurate detection and segmentation of BM and could assist clinicians in treatment response assessment.
