202. Ecological momentary assessment of smoking behavior in persistent pain patients

Smoking is associated with chronic pain and pain-related functional impairment. Some studies suggest that pain activates smoking urges and others suggest that smoking is analgesic. We evaluated these associations using ecological momentary assessment, a method for real-time measurement of health-related phenomena.For 1 week, 36 chronic pain patients who smoked a mean of 17.5 (SD=9.4) cigarettes per day completed multiple daily assessments on a handheld computer.The sample included 67% women and 39% whites; 67% had back pain, with an average (SD) worst pain severity during the past week of 8.6 (1.5) on a 0 to 10 numeric rating scale. Patients completed an average (SD) of 44 (24) random assessments. At each assessment, the patient recorded pain "right now" on a 0 to 10 scale, whether he/she was "about to smoke," and if he/she had "just smoked in the past 30 minutes," pain before smoking. After controlling for other significant correlates of pain, patients who were about to smoke had more pain than at other times (M [SD]=6.5 [2.3] vs. 5.2 [2.4]; P0.01), but pain before and after smoking was not different (M [SD]=6.1 [2.2] vs. 5.9 [2.3]; P=0.18).These findings support the hypothesis that smoking behavior is triggered by pain, but smoking is not analgesic. Future studies should clarify potential explanatory mechanisms for this pain-related trigger and evaluate tailored cessation strategies for pain patients.

Melinjo (Gnetum gnemon L.) is a beneficial agroforestry plant that contains antioxidant compounds broadly studied as a natural source in pharmaceutical and nutraceutical uses. The antioxidant compounds benefit human health and plant to develop defense mechanisms under environmental stresses, such as drought stress. This study aims to evaluate the defense mechanism of melinjo under drought stress. Polyethylene glycol (PEG) was used to simulate drought stress to melinjo seedlings for 5, 10, 20, and 40 days compared to zero-day control without PEG. The inhibition and detoxification activities of 2,2′-azino-bis (3-ethylbenzothiazoline-6-sulfonic acid) (ABTS), 2,2-diphenyl-1-picrylhydrazyl (DPPH), hydroxyl, superoxide radical, and superoxide dismutase (SOD) were determined to evaluate antioxidant capabilities during drought stress. Lipid peroxidation and hydrogen peroxide (H2O2) contents were also analyzed to evaluate the level of cellular damage during drought stress. The seedling's growth was significantly reduced as a response to the longer duration of drought stress treatment. On the contrary, the level of lipid peroxidation and H2O2 were increased, followed by the escalation of antioxidant activities. This result indicates that many antioxidant compounds act together to develop a defense mechanism in melinjo seedlings, and the different duration of drought stresses influences their capabilities.

206. Implementation research training for learners in low- and middle-income countries: evaluating behaviour change after participating in a massive open online course

Abstract Background Implementation research (IR) can play a critical role in the delivery of disease control interventions, particularly in low- and middle-income countries (LMICs). The growing demand for IR training has led to the development of a range of training programmes and university courses, the majority of which can not be accessed by learners in LMICs. This article reports on the evaluation of the massive open online course (MOOC) developed by the Special Programme for Research and Training in Tropical Diseases hosted by WHO on the topic of IR with a focus on infectious diseases of poverty. This study followed the Kirkpatrick Model to evaluate training programmes with a specific focus on post-training changes in behaviour. Methods MOOC participants were invited to take part in an anonymous online survey examining their knowledge of IR and how they applied it in their professional practice approximately 1–1.5 years after completing their course. The survey contained 43 open-ended, multiple choice and Likert-type questions. Descriptive statistics were calculated for the quantitative data and responses to the open-ended questions were thematically coded. Results A total of 748 MOOC participants responded to the survey. The demographic profile of the survey respondents aligned with that of the MOOC participants, with nearly 70% of respondents originating from Africa. Responses to the quantitative and open-ended survey questions revealed that respondents’ knowledge of IR had improved to a large extent as a result of the MOOC, and that they used the knowledge and skills gained in their professional lives frequently and had consequently changed their professional behaviour. Respondents most often cited the problem-solving aspect of IR as a substantial area of behavioral change influenced by participating in the MOOC. Conclusions These findings indicate that the MOOC was successful in targeting learners from LMICs, in strengthening their IR knowledge and contributing to their ability to apply it in their professional practice. The utility of MOOCs for providing IR training to learners in LMICs, where implementation challenges are encountered often, makes this platform an ideal standalone learning tool or one that could be combined with other training formats.

Background Idiopathic recurrent pericarditis (IRP) is an orphan disease that carries significant morbidity, partly driven by corticosteroid dependence. Innate immune modulators, colchicine and anti‐interleukin‐1 agents, pioneered in monogenic autoinflammatory diseases, have demonstrated remarkable efficacy in trials, suggesting that autoinflammation may contribute to IRP. This study characterizes the phenotype of patients with IRP and monogenic autoinflammatory diseases, and establishes whether autoinflammatory disease genes are associated with IRP. Methods and Results We retrospectively analyzed the medical records of patients with IRP (n=136) and monogenic autoinflammatory diseases (n=1910) attending a national center (London, UK) between 2000 and 2021. We examined 4 genes (MEFV, MVK, NLRP3, TNFRSF1A) by next‐generation sequencing in 128 patients with IRP and compared the frequency of rare deleterious variants to controls obtained from the Genome Aggregation Database. In this cohort of patients with IRP, corticosteroid dependence was common (39/136, 28.7%) and was associated with chronic pain (adjusted odds ratio 2.8 [95% CI, 1.3–6.5], P=0.012). IRP frequently manifested with systemic inflammation (raised C‐reactive protein [121/136, 89.0%] and extrapericardial effusions [68/136, 50.0%]). Pericarditis was observed in all examined monogenic autoinflammatory diseases (0.4%–3.7% of cases). Rare deleterious MEFV variants were more frequent in IRP than in ancestry‐matched controls (allele frequency 9/200 versus 2932/129 200, P=0.040). Conclusions Pericarditis is a feature of interleukin‐1 driven monogenic autoinflammatory diseases and IRP is associated with variants in MEFV, a gene involved in interleukin‐1β processing. We also found that corticosteroid dependence in IRP is associated with chronic noninflammatory pain. Together these data implicate autoinflammation in IRP and support reducing reliance on corticosteroids in its management.

Ascariasis is an endemic condition in poor countries with a tropical and subtropical climate, with a high prevalence in children. The gastrointestinal form is the most typical presentation. When there is massive infestation, complications can occur that cause intestinal obstruction and perforation, requiring emergency surgical intervention. We present the case of a 13-year-old male patient of rural origin, treated at the National Hospital of San Miguel de Uspantan del Quiche, Guatemala, due to abdominal cramp and fever. He reported the occurrence of two vomiting episodes the day before. Physical examination revealed malnutrition by default, pale skin, increased hydro-aerial sounds in the abdomen, with marked distension, pain, involuntary muscle contracture, and the presence of tympanism. Body temperature was regulated. The patient was transfused with red blood cells and treated with intravenous solutions. Anesthetic induction and endotracheal intubation were performed. A nasogastric tube was placed and intravenous metronidazole was administered as the only available antimicrobial prophylaxis. An exploratory laparotomy was performed through a median, supra and infraumbilical incision. A perforated small bowel volvulus was observed, with marked necrosis. Devolvulation, digital stimulation and packing of the devitalized intestinal loops were performed with warm compresses of saline solution, without recovering the blood supply. Intestinal resection and colostomy were performed due to generalized peritonitis. In the postoperative period, the expulsion of Ascaris lumbricoides through the colostomy stoma was visualized. Treatment with albendazole was established. The patient was discharged after ten days and with a favorable progress.

This study uses a case study approach to examine the development of a massive open online course (MOOC) on intervention and implementation research in infectious diseases of poverty for learners in low- and middle-income countries (LMICs). Implementation research (IR) seeks to understand and address barriers to effective implementation of health interventions, strategies, and policies. In recent years, IR has attracted increased interest, and corresponding demand for training, however, current training opportunities are not easily accessible to learners in LMICs. In 2017, the MOOC was introduced to a diverse range of learners to enhance access to training materials and has been offered yearly since. Findings are based on the experiences of the MOOC working group which included developers and facilitators, and on interpretations of data such as forum discussion activity and Facebook posts. The use of material from local contexts and in local languages, and professional facilitation of discussion forums was identified by the working group to be key considerations in developing the MOOC. Other findings include the importance of using clear instructions and preparing discussion questions to stimulate learner engagement. These findings add to the limited knowledge of MOOCs developed for LMICs and are of value to others developing professional development MOOCs in LMIC health contexts.

Introducción: el carcinoma hepatocelular (CHC) es una neoplasia con una elevada incidencia y una alta mortalidad. Objetivo: nuestro objetivo es la descripción de la historia natural de una cohorte de pacientes con CHC e identificar variables asociadas a supervivencia. Material y métodos: estudio retrospectivo y descriptivo de pacientes diagnosticados de CHC entre 1995-2002. Las variables cualitativas se expresaron mediante frecuencias y porcentajes y las cuantitativas con mediana y desviación típica. La supervivencia se estimó mediante el método de Kaplan Meyer y la prueba de Log Rank. Resultados: se analizaron un total de 154 pacientes con CHC. La ratio hombre-mujer fue de 2,9/1. La edad media de 68 ± 9 años. El 82% de los pacientes fueron exitus en un tiempo medio de seguimiento de 28 meses. La mediana de supervivencia fue de 21,5 meses (IC 95%: 16,98-26,04). Se realizó tratamiento con intención curativa en un 40,3% y el resto tratamiento paliativo. Las variables asociadas a la supervivencia fueron: presencia o no de ascitis, el número de lesiones en el momento del diagnóstico y si el tratamiento realizado fue con intención curativa o no. No hubieron diferencias estadísticamente significativas respecto a: sexo, edad, etiología de la cirrosis y estadio de Child en el momento del diagnóstico. Conclusiones: los factores asociados a una menor supervivencia en los pacientes con carcinoma hepatocelular fueron la presencia de ascitis y el número de lesiones en el momento del diagnóstico. La realización de tratamiento con intención curativa se asocia a una mayor supervivencia. Background: hepatocellular carcinoma (HCC) is a cancer with high incidence and mortality. Objective: our aim was to describe the natural history of a patient cohort with HCC, and to identify the factors associated with survival. Patients and methods: a retrospective and descriptive study of patients diagnosed with HCC between 1995 and 2002. Qualitative variables were expressed as frequencies and percentages. Quantitative variables were expressed as medians and standard deviations. Survival was calculated using the Kaplan-Meier method and log rank. Results: a total of 154 patients were analyzed. The men-to-women ratio was 2.9/1. Mean age was 68 ± 9 years. 82% of patients died during a median follow-up of 28 months. Median survival was 21.5 months (95% CI: 16.98-26.04). Curative treatment was done in 40.3% of diagnosed patients, and 59.7% of patients received palliative treatment. Factors associated with survival were: ascites, number of lesions at diagnosis, and curative treatment. No statistical differences were found for the next factors: age, sex, etiology of cirrhosis, and Child-Pugh stage at diagnosis. Conclusions: factors associated with low survival in patients with HCC were ascites and number of lesions. Curative treatment is associated with a higher survival when compared to palliative treatment.
