Abstract

Summary: Background: Primary cutaneous lymphomas (PCLs) are a heterogeneous group of T‐cell (CTCL) and B‐cell (CBCL) malignancies. Little is known about their epidemiology at initial presentation in Europe and about potential changes over time. Objectives: The aim of this retrospective study was to analyse the frequency of PCLs in the French Cutaneous Lymphoma Registry (GFELC) and to describe the demography of patients. Methods: Patients with a centrally validated diagnosis of primary PCL, diagnosed between 2005 and 2019, were included. Results: The calculated incidence was unprecedently high at 1·06 per 100 000 person‐years. The number of included patients increased yearly. Most PCL subtypes were more frequent in male patients, diagnosed at a median age of 60 years. The relative frequency of rare CTCL remained stable, the proportion of classical mycosis fungoides (MF) decreased, and the frequency of its variants (e.g. folliculotropic MF) increased. Similar patterns were observed for CBCL; for example, the proportion of marginal‐zone CBCL increased over time. Conclusions: Changes in PCL frequencies may be explained by the emergence of new diagnostic criteria and better description of the entities in the most recent PCL classification. Moreover, we propose that an algorithm should be developed to confirm the diagnosis of PCL by central validation of the cases. Abstract

BACKGROUND: Survival in mycosis fungoides (MF) is varied and may be poor. The PROCLIPI (PROspective Cutaneous Lymphoma International Prognostic Index) study is a web-based data collection system for early-stage MF with legal data-sharing agreements permitting international collaboration in a rare cancer with complex pathology. Clinicopathological data must be 100% complete and in-built intelligence in the database system ensures accurate staging. OBJECTIVES: To develop a prognostic index for MF. METHODS: Predefined datasets for clinical, haematological, radiological, immunohistochemical, genotypic, treatment and quality of life are collected at first diagnosis of MF and annually to test against survival. Biobanked tissue samples are recorded within a Federated Biobank for translational studies. RESULTS: In total, 430 patients were enrolled from 29 centres in 15 countries spanning five continents. Altogether, 348 were confirmed as having early-stage MF at central review. The majority had classical MF (81·6%) with a CD4 phenotype (88·2%). Folliculotropic MF was diagnosed in 17·8%. Most presented with stage I (IA: 49·4%; IB: 42·8%), but 7·8% presented with enlarged lymph nodes (stage IIA). A diagnostic delay between first symptom development and initial diagnosis was frequent [85·6%; median delay 36 months (interquartile range 12-90)]. This highlights the difficulties in accurate diagnosis, which includes lack of a singular diagnostic test for MF. CONCLUSIONS: This confirmed early-stage MF cohort is being followed-up to identify prognostic factors, which may allow better management and improve survival by identifying patients at risk of disease progression. This study design is a useful model for collaboration in other rare diseases, especially where pathological diagnosis can be complex.

Abstract

Kaposi's sarcoma (KS)is a multifocal neoplasm of lymphatic endothelium-derived cells infected with human herpesvirus 8. Four clinical subtypes are distinguished: the classic, the endemic, the epidemic subtype in HIV positive patients and the iatrogenic subtype. The diagnosis is primarily based on clinical features and confirmation by histology with immunohistochemistry. Cutaneous distribution and severity, mucosal, nodal and visceral involvement depend on the type of KS with in general indolent behaviour and chronic evolution in the classic subtype and the more severe forms in iatrogenic or epidemic subtypes. Management should aim at achieving disease control. For localised lesions, several local therapies have been developed without randomised trial comparisons. Radiotherapy, intralesional chemotherapies and electrochemotherapy have high response rates. Topical treatments—imiquimod or topical 9-cis-retinoid acid—can also be used. Systemic treatments are reserved for locally aggressive extensive and disseminated KS: the recommended first-line agents are pegylated liposomal doxorubicin (PLD)and paclitaxel. In CKS, PLD or low-dose interferon-alfa are the recommended first-line agents in younger patients. In AIDS-related KS, combination antiretroviral therapy is the first treatment option; specific systemic treatment is needed only in case of extensive disease and in the prevention and treatment of immune reconstitution inflammatory syndrome. In post-transplant KS, tapering down immunosuppressive therapy and switching to mammalian target of rapamycin (m-TOR)inhibitors are used. Follow-up schedules for patients with KS disease depend on aggressiveness of the disease.

Abstract

Background: Cutaneous peripheral T‐cell lymphoma, not otherwise specified (PTL NOS) is an aggressive, but poorly characterized neoplasm. Objectives: The European Organization for Research and Treatment of Cancer cutaneous lymphoma taskforce (EORTC CLTF) investigated 33 biopsies of 30 patients with primary cutaneous PTL NOS to analyse their clinical, histological, immunophenotypic features and outcome. Methods: Retrospective analysis of clinical data and histopathological features by an expert panel. Results: Cutaneous PTL NOS manifested clinically either with solitary or disseminated rapidly grown ulcerated tumours or disseminated papulo‐nodular lesions. Histologically, a mostly diffuse or nodular infiltrate in the dermis and often extending into the subcutis was found. Epidermotropism was rarely present and only mild and focal. Unusual phenotypes were frequent, e.g. CD3+/CD4−/CD8− and CD3+/CD4+/CD8+. Moreover, 18% of the cases exhibited an aberrant expression of the B‐cell marker CD20 by the tumour cells. All solitary tumours were located on the limbs and presented a high expression of GATA‐3 but this did not correlate with outcome and therefore could not serve as a prognostic factor. The prognosis was shown to be generally poor with 10 of 30 patients (33%) dying of lymphoma within the follow‐up of 36 months (mean value; range 3–144). The survival rates were 61% after 3 years (CI, 43–85%) and 54% after 5 years (CI, 36–81%). Small to medium‐sized morphology of tumour cells was associated with a better outcome than medium to large or large tumour cells. Age, gender, clinical stage, CD4/CD8 phenotype and GATA‐3 expression were not associated with prognosis. Chemotherapy was the most common treatment modality, but surgical excision and/or radiotherapy may represent an appropriate first‐line treatment for solitary lesions. Conclusions: Cutaneous PTL NOS shows an aggressive course in most patients independent of initial presentation, age and phenotype. Cytomorphology was identified as a prognostic factor. Abstract

This report describes the main activities carried out at the High Voltage Padova Test Facility HVPTF in 2018 and the ones related to the installation of a new high voltage test stand at the Consorzio RFX . The activities of the subtask 5.7 have been carried out according with the Work Programme (WP) 2018 between F4E and the Consorzio RFX, agreement on the Neutral Beam Test Facility. The activities mainly consisted in: ? studying of the X-ray spectra to investigate the nature of the physical phenomena which occurs during the high voltage conditioning ? testing instrumentation for precise current measurements and for determining type and position of discharges (visible imaging, IR imaging, RX spectra and Residual Gas Analyzer) ? maintenance of both positive and negative HV power supplies and the refurbishment of high voltage ceramic feedthroughs ? evaluation of other methods for improving voltage holding capabilities: tests of surface treatments on electrodes (e.g. alumina spray) ? testing the behaviour of "transparent anodes", i.e. anodes in which the surface is screened by a thin metallic mesh, aimed at zeroing the electric field at surface behind ? experimental study of two-electrode configurations subjected to a combination of Common-Mode Voltage (CMV) and differential-mode voltage to understand the different contribution of anode and cathode on the discharge initiation ? ordinary maintenance of the vacuum plant and controlling system ? construction of a small experimental set-up to measure with sufficient accuracy the FN emission insulating the electrodes by short vacuum gaps, the experimental set up has been called High Voltage Short Gap Test Facility (HVSGTF) The activity mentioned at the last point is partially reported in sec. 10. The specific technical note T5.7 - P1.4_RFX-IE-TN-59 has been prepared to indicate the data for the radiological safety assessment of the HVSGTF.

Abstract

The MITICA Central Safety System (MITICA CSS) implements a set of important protection functions related to personnel health and safety, both during and outside the operation of the MITICA test bed, providing continuous monitoring of potentially dangerous events, automated protection actions, and logging of events and data relevant for safety of people. The Technical Specification for the call for tender of the supply of the MITICA CSS was drafted in 2017 (ref. RFX_MITICA_TS_041), but some important details were still to be defined, also depending on the status of the procurement of plant systems interfacing with MITICA CSS. This document is intended to be the report of the main activities related to MITICA safety carried out in 2018, in order to better define the aspects needed to pass from the MITICA CSS "Draft" to the "Detailed" Techinical Specification.

Abstract

The ITER Heating Neutral Beam (HNB) injectors, one of the tools necessary both to achieve burning conditions and to control plasma instabilities, are characterized by such demanding parameters as to require the construction of a Neutral Beam Test Facility (NBTF) dedicated to their development and optimization. The NBTF was realized in Padua (Italy), with the direct contribution of the Italian government, through the Consorzio RFX as the host entity, IO, the in kind contributions of three Domestic Agencies (F4E, JADA, INDA) and the technical and scientific support of various European laboratories and universities. The NBTF hosts two experiments: SPIDER and MITICA. The former is devoted to the optimization of the HNB and DNB ion sources and to the achievement of the required source performances. MITICA is the full size prototype of the ITER HNB, with an ion source identical to the one used in SPIDER. This paper gives an overview of the progress of the NBTF realization with particular emphasis on issues discovered during this phase of activities and to the adopted solutions in order to minimize the impact on the schedule while maintaining the goals of the facilities. The realization of MITICA is well advanced; it is expected to enter into operation in 2023 due to the long procurement times of the in-vessel mechanical components. The power supply designed to operate at 1MV are in an advanced phase of realization; all the high voltage components have been installed and the complex insulation test phase has begun in 2018. The construction and installation of SPIDER plant systems was successfully completed with their integration into the facility. The mechanical components of the ion source were installed inside the vessel and connected to the plants. The integrated commissioning with control (CODAS), protection and safety systems ended positively and the first experimental phase began. Finally, the first results of the SPIDER experimentation and of the 1MV insulation tests on the MITICA high voltage components are presented.
