BackgroundDiarrhea and acute respiratory infection (ARI) are major causes of child mortality. We aimed to identify risk factors associated with diarrhea and ARI among children under 2 years of age in rural northern Bangladesh.MethodWe collected information on diarrhea and ARI in the previous 14 days and the previous 6 months at 6, 12, 18 and 24 months of age as part of a longitudinal, cluster randomized effectiveness trial, the Rang-Din Nutrition Study which enrolled 4011 pregnant women at ≤20 gestational weeks. Women and their children were followed up until 2 years postpartum. Information on household socioeconomic status, type of toilet, garbage disposal system, food insecurity, number of under-five children in the household, type of family, maternal characteristics and child characteristics was collected at baseline and/or at 6, 12, 18 and 24 months postpartum. Data on newborn health and feeding behaviors were collected within 72 h of delivery. Associations between potential risk factors and morbidity prevalence outcomes were assessed using logistic regression controlling for potential confounders.ResultsOut of 3664 live born children, we collected information from ~ 3350 children at 6, 12, 18 and 24 months of age. Diarrhea in the previous 14 days, and in the previous 6 months, was associated with maternal depression score and food insecurity; diarrhea in the previous 6 months was also associated with family type (nuclear vs. joint). ARI in the previous 14 days was associated with maternal depression score, type of toilet and garbage disposal, household food insecurity and sex. Cough or nasal discharge in the past 6 months was associated with maternal depression score, type of toilet and garbage disposal, household food insecurity, sex and perceived overall physical condition of the infant after birth.ConclusionMaternal depression and food insecurity appear to be important risk factors for diarrhea and respiratory infection among children under 2 years of age in this setting. These findings suggest that policies and programs that include strategies to address maternal mental health and household food insecurity may contribute to improved child health.Trial registrationThe trial was registered with the US National Institutes of Health at ClinicalTrials.gov, # NCT01715038 , with registration completed October 26, 2012.

Prenatal nutritional supplements may improve birth outcomes. This study aims to examine the effect of prenatal lipid-based nutrient supplements (LNS), compared with iron and folic acid (IFA), on general newborn physical condition and feeding behaviours. We conducted a cluster-randomized effectiveness trial that enrolled 4,011 pregnant women at ≤20 gestational weeks. LNS and IFA were provided to women in 48 and 16 clusters, respectively, for daily consumption until delivery. We collected data on household socio-economic, food insecurity, and maternal characteristics during early pregnancy and on newborn condition and feeding within 72 hr of delivery. We analysed intervention effects on these secondary outcomes using mixed models with analysis of covariance for continuous outcomes and logistic regression for dichotomous outcomes. Among 3,664 live births, intervention groups did not differ in newborn response, mother's rating of the general condition of her newborn, early initiation of breastfeeding (EIBF), suckling ability, or frequency and exclusivity of breastfeeding in the first 24 hr. If the mother perceived her infant to be healthy, EIBF was more likely (OR [95% CI]: 2.08 [1.46, 2.97]) and frequency of breastfeeding in the first 24 hr was greater (mean difference [95% CI]: 3.0 [1.91, 4.01]), but there was no difference in exclusive breastfeeding in the first 24 hr. Newborn condition and early breastfeeding practices were not affected by giving mothers prenatal LNS versus IFA. However, early breastfeeding practices were related to maternal perception of her newborn's condition. Thus, interventions to improve breastfeeding practices for newborns with poorer perceived health status may be useful.

This study was performed with the main objective of formulating and evaluating the potential of ethosomesl gel (Etho gel) to deliver nimodipine (NiM) for cardiovascular disease, a potent water insoluble anti-hypertensive drug via skin to reach the deeper layers of skin. The Box-Behnken design (BBD) was used to optimize the NiM-Eth to determine the impact of the independent and depended variables. The effectiveness of drug entrapment, vesicle size, and cumulative drug release were assessed for the NiM loaded ethosomes and NiM-Eth gel using carbopol 934 as a gelling agent. Fourier transform infrared spectroscopy (FTIR), Differential scanning calorimetry (DSC), Power X-ray diffraction (PXRD), and scanning electron microscopy (SEM) analysis were performed and analysed their physicochemical characters. Rat abdomen skin was used to investigate drug permeability and deposition. As compared to marketed products, NiM-Eth gel produced an improved drug permeability in ex vivo experiments. The mean AUC0 to AUC0-∞ of NiM-Eth gel when compared to oral formulation (Nymalize oral preparation) was found to be increased from 4.1 to 5.9 folds which was found to be resulted from first pass effect. Histophatlogical findings revealed that the maximum amount of NiM penetrated the stratum corneum of the skin and create drug depots in the deep layer. In summary, it can be said that NiM might be successfully prepared in NiM-Eth gel for transdermal drug delivery.

Background: Allelopathy is the process of a plant's chemical release into the environment having direct or indirect, beneficial or negative effects on another plant. In the present work, we investigated the allelopathic interaction of Acacia modesta in the different extracts to show significant effects regarding the growth and germination of test species. Objective: The main objective of this research is to study how the allelopathic activity of Acacia modesta affects the growth of three Brassica species in various extracts (Aqueous extracts, litter & mulching). Methodology: Fresh leaves of Acacia modesta were collected from the Botanical Garden, Islamia College Peshawar, dried in the shade, and ground, whereas the glass ware was rinsed and sterilized for at least 4 hours at 170°C. Seeds of the Brassica crop were used and checked for viability in H2O. The Petri dishes were incubated at 25°C for 72 hours, then the effects of various extracts were examined, and lastly, readings from the Petri dishes were obtained. Results: According to our findings, the leaf extracts of Acacia modesta had a substantial effect on the germination and growth of Brassica species. Brassica campestris (10g/48h dry & 5g/24h fresh), Brassica napus (10g/24h), and Brassica juncea (10g/48h dry & 5g/48h fresh) leaf extracts have a greater effect than the control. Conclusion: Our outcomes concluded that in comparison of the test species in all the experiments, Brassica campestris and Brassica juncea were highly affected by the allelopathy of A. modesta which necessitates further evaluation.

Abstract Accurate and quantitative investigation of the physical structure and fractal geometry of coal has important theoretical and practical significance for coal bed methane (CBM) development and the prevention of dynamic disasters such as coal and gas outbursts. This study investigates the pore structure and fractal characteristics of soft and hard coals using nitrogen and carbon dioxide (N2/CO2) adsorption. Coal samples from Pingdingshan Mine in Henan province of China were collected and pulverized to the required size (0.20–0.25 mm). N2/CO2 adsorption tests were performed to evaluate the specific surface area (SSA), pore size distribution (PSD), and pore volume (PV) using Braunuer-Emmett-Teller (BET), Barrett-Joyner-Halenda (BJH), and Density Functional Theory (DFT). The pore structure was characterized based on the theory of fractal dimensions. The results unveiled that the strength of coal has a significant influence on pore structure and fractal dimensions. There are significant differences in SSA and PV between both coals. The BJH-PV and BET-SSA obtained by N2-adsorption for soft coal are 0.029–0.032 cm3/g and 3.523–4.783 m2/g. While the values of PV and SSA obtained by CO2-adsorption are 0.037–0.039 cm3/g and 106.016–111.870 m2/g. Soft coal shows greater SSA and PV than hard coal, which is consistent with the adsorption capacity ( $${V}_{\mathrm{L}}$$ V L ). The fractal dimensions of soft and hard coal are respectively different. The Ding coal exhibits larger D 1 and smaller D 2, and the reverse for the Wu coal seam is observed. The greater the value of D 1 (complexity of pore surface) of soft coal is, the larger the pore surface roughness and gas adsorption capacity is. The results enable us to conclude that the characterization of pores and fractal dimensions of soft and hard coals is different, tending to different adsorption/desorption characteristics. In this regard, the results provide a reference for formulating corresponding coal and gas outburst prevention and control measures.

Abstract Background Vascular endothelial growth factor D (VEGFD), a member of the VEGF family, is implicated in angiogenesis and lymphangiogenesis, and is deemed to be expressed at a low level in cancers. S-nitrosylation, a NO (nitric oxide)-mediated post-translational modification has a critical role in angiogenesis. Here, we attempt to dissect the role and underlying mechanism of S-nitrosylation-mediated VEGFD suppression in lung adenocarcinoma (LUAD). Methods Messenger RNA and protein expression of VEGFD in LUAD were analyzed by TCGA and CPTAC database, respectively, and Assistant for Clinical Bioinformatics was performed for complex analysis. Mouse models with urethane (Ure)–induced LUAD or LUAD xenograft were established to investigate the role of S-nitrosylation in VEGFD expression and of VEGFD mutants in the oncogenesis of LUAD. Molecular, cellular, and biochemical approaches were applied to explore the underlying mechanism of S-nitrosylation-mediated VEGFD suppression. Tube formation and wound healing assays were used to examine the role of VEGFD on the angiogenesis and migration of LUAD cells, and the molecular modeling was applied to predict the protein stability of VEGFD mutant. Results VEGFD mRNA and protein levels were decreased to a different extent in multiple primary malignancies, especially in LUAD. Low VEGFD protein expression was closely related to the oncogenesis of LUAD and resultant from excessive NO-induced VEGFD S-nitrosylation at Cys277. Moreover, inhibition of S-nitrosoglutathione reductase consistently decreased the VEGFD denitrosylation at Cys277 and consequently promoted angiogenesis of LUAD. Finally, the VEGFDC277S mutant decreased the secretion of mature VEGFD by attenuating the PC7-dependent proteolysis and VEGFDC277S mutant thus reversed the effect of VEGFD on angiogenesis of LUAD. Conclusion Low-expression of VEGFD positively correlates with LUAD development. Aberrant S-nitrosylation of VEGFD negates itself to induce the tumorigenesis of LUAD, whereas normal S-nitrosylation of VEGFD is indispensable for its secretion and repression of angiogenesis of LUAD.

Background: Rubella is an acute viral disease generally mild in its clinical manifestations. However, it is most serious for pregnant women due to the high frequency of congenital abnormalities that it causes, known as congenital rubella syndrome. This study aimed to determine rubella seroprevalence among a group of Moroccan parturients and survey physicians’ opinions regarding the situation of congenital rubella syndrome in different regions of Morocco.Methods: We prospectively enrolled 1500 women hospitalized in the Maternity Souissi of Rabat for delivery and 14 pediatricians/neonatologists who were practicing in public and private centers representing different regions of Morocco. Data on rubella serological status were extracted from the medical records of mothers. The physicians involved in this study were visited to answer the survey in person, or they completed the survey by e-mail or phone discussion.Results: Among enrolled women, and after excluding those who did not bring back evidence of a serological assessment, 222 (17.5%) women performed a rubella serology of which 84.7% were immunized against rubella. The opinion poll of physicians showed an almost absence of reports on congenital rubella syndrome cases. All practitioners surveyed were interested to join the establishment of congenital rubella syndrome reporting system.Conclusion: To achieve the goals set for congenital rubella syndrome elimination, it is of primary importance to protect women from the childbearing age by vaccination and measures that help avoid any further contact with the virus. There is also a need to expand serological screening to detect and monitor seronegative women. In addition, it is necessary to diagnose and notify new cases of congenital rubella syndrome.

Abstract Background Tetratricopeptide repeat domain 7A (TTC7A, chromosome 2p21) is a highly conserved structural motif essential for multiprotein scaffolding and cell survival. Fewer than 60 cases of TTC7A deficiency have been reported globally. It produces multisystemic disease phenotypes which are lethal in two-thirds of patients, with a median survival age of 12 months. It is predominantly detected postnatally, often rendering medical and surgical interventions futile. Case presentation We report the antenatal sonographic and magnetic resonance imaging characteristics of a novel phenotype of TTC7A-deficiency presenting with gastrointestinal atresia. This has never been previously documented. The diagnosis was confirmed via whole-exome next-generation sequencing, thus facilitating prompt initiation of management and prolonging viability. Conclusions Novel insight into the prenatal morphological characteristics of TTC7A-deficiency phenotypes expands knowledge of this rare condition. Furthermore, antenatal recognition facilitates targeted investigation, genetic counselling, and earlier multidisciplinary intervention to prolong viability of this predominantly lethal condition.

Background: Deficiencies in islet ß-cell activity, which can arise from genetic, environmental, or immunological factors, result in a flat-out or relative decrease in insulin emission, which is the hallmark of T2DM. Gene polymorphisms and the incidence of type 2 diabetes have been linked in numerous studies. There is a correlation between T2DM risk and single-nucleotide polymorphisms (SNPs) found in the intergenic zones of multiple potential genes. lncRNAs are in excess of 200 nucleotides long. lncRNAs have been linked to type 2 diabetes in many ethnic groups. HOX antisense RNA transcription (HOTAIR) is a well-characterized long noncoding RNA that is found on human chromosome 12q13. Finding out if the rs4759314 HOTAIR gene polymorphism is linked to T2DM is the goal of the current study. Methods: This study was carried out on 192 subjects (96 healthy controls, and 96 non-insulin-dependent diabetic patients. HOTAIR genotype was determined by T-ARMS-PCR. Results: The predominance of the polymorphic genotype of HOTAIR polymorphism (GG) was essentially expanded in type two diabetic patients contrasted with the healthy group (15.6% vs. 5.2%). (P<0.01). Compared to carriers of the AA genotype, those carrying the GG genotype had the probability of getting T2DM four times higher (OR=3.92). Millions of people worldwide suffer from cancer, facing challenges such as treatments affecting healthy cells, suboptimal responses, adverse effects, recurrence risk, drug resistance, and nonspecific targeting. Chemoresistance leads to fatalities, but phytoactives show promise in cancer management despite limitations such as high metabolism, poor absorption, and high dosage requirements. Challenges in the large-scale isolation of phytoactive compounds, solubility, bioavailability, and targeting limit their development. Recent developments, including carbohydrate, lipid, and protein-based nanoparticles, have enhanced cancer treatment by improving the bioavailability and targeted delivery of phytoactives such as polyphenols, alkaloids, sulfur-containing compounds, flavonoids, and terpenes. Despite advancements, clinical application faces hurdles such as poor bioavailability and inconsistent immune responses. This article discusses the promise of phytoactive-loaded nanoformulations in cancer management, highlighting targeted drug delivery, unmet needs, and challenges. The main objective of the current research work was to synthesize mesoporous silica nanoparticles for controlled delivery of mometasone furoate for potential nasal delivery. The optimized sol–gel method was used for the synthesis of mesoporous silica nanoparticles. Synthesized nanoparticles were processed through Zeta sizer, SEM, TEM, FTIR, TGA, DSC, XRD, and BET analysis for structural characterization. The in vitro dissolution test was performed for the inclusion compound, while the Franz diffusion experiment was performed for permeability of formulation. For the determination of expression levels of anti-inflammatory cytokines IL-4 and IL-5, RNA extraction, reverse transcription, and polymerase chain reaction (RT-PCR) were performed. The MTT assay was also performed to determine cell viability. Synthesized and functionalized mesoporous silica nanoparticles showed controlled release of drugs. FT-IR spectroscopy confirmed the presence of the corresponding functional groups of drugs within mesoporous silica nanoparticles. Zeta sizer and thermal analysis confirmed the delivery system was in nano size and thermally stable. Moreover, a highly porous system was observed during SEM and TEM evaluation, and further it was confirmed by BET analysis. Greater cellular uptake with improved permeability characteristics was also observed. As compared to the crystalline drug, a significant improvement in the dissolution rate was observed. It was concluded that stable mesoporous silica nanoparticles with significant porosity were synthesized, efficiently delivering the loaded drug without any toxic effect. [ABSTRACT FROM AUTHOR]

In practical three‐phase PWM rectifiers, external disturbances, such as changes in load and grid voltage, and internal disturbances, such as DC capacitor uncertainty, have caused major problems, including DC‐link voltage fluctuations, harmonic distortion, and power factor degradation. They can also cause problems with power balance, stability, system dynamics, reduced efficiency, and increased stress. This paper proposes an enhanced linear state observer (ELSO) with a PLL‐less voltage‐oriented control (PLL‐less VOC) method for a three‐phase PWM rectifier in the synchronous coordinate frame (dq coordinate), aiming to simultaneously improve the DC‐link voltage dynamics, disturbance rejection ability, and the grid current control loop's transient response. The ELSO is suggested instead of the load current sensor not only to estimate the disturbance but also to estimate the overall switching losses of the PWM rectifier, resulting in good DC‐link voltage disturbance rejection and transient fluctuation suppression, high robustness against DC capacitance change, low size and cost, and high reliability. Furthermore, contrary to the traditional VOC method, where the PLL and Park transforms are important parts for controlling the rectifier in the dq coordinate, the PLL‐less VOC method can control the grid currents directly in the dq coordinate without needing PLL and Park transforms, which can offer a fast transient response and low computational burden. The frequency domain analysis of the ELSO is used to identify its gains and enhance its stability and disturbance rejection capability. Finally, hardware‐in‐the‐loop (HIL) simulation using the OPAL‐RT 4510 real‐time simulator is also given to confirm the superiority and efficacy of the suggested control method. Free radical polymerization technique was used to formulate Poloxamer-188 based hydrogels for controlled delivery. A total of seven formulations were formulated with varying concentrations of polymer, monomer ad cross linker. In order to assess the structural properties of the formulated hydrogels, Fourier Transform Infrared Spectroscopy (FTIR), Thermogravimetric analysis (TGA), Differential Scanning Calorimetry (DSC), Scanning electron microscopy (SEM), and X-ray diffraction (XRD) were carried out. To assess the effect of pH on the release of the drug from the polymeric system, drug release studies were carried in pH 1.2 and 7.4 and it was found that release of the drug was significant in pH 7.4 as compared to that of pH 1.2 which confirmed the pH responsiveness of the system. Different kinetic models were also applied to the drug release to evaluate the mechanism of the drug release from the system. To determine the safety and biocompatibility of the system, toxicity study was also carried out for which healthy rabbits were selected and formulated hydrogels were orally administered to the rabbits. The results obtained suggested that the formulated poloxamer-188 hydrogels are biocompatible with biological system and have the potential to serve as controlled drug delivery vehicles. Glycine max L. (soybean) is an important leguminous staple crop. In this study, nutritional value, bioactive compounds, and antioxidant properties of soybean milled fractions (MF) were determined. Protein contents in different milled fractions ranged from 4.6% to 47.2%, and fat contents ranged from 1.9% to 22.1%. MF amino acid contents ranged from 6.8% to 37.9 g/100 g. Glycitein and genistin were the most concentrated isoflavones in the cotyledon and germ fractions. Although MF phenolics of protocatechuic, chlorogenic, hydroxybenzoic, vanillic, caffeic, and sinapic acids were abundant in bound and free phenolic extracts. In addition, MF total phenolic indexes ranged from 65 to 270.1 GAE/100 g and total flavonoid 7.5 to 92.3 mg GAE/100 g, which were found to be the most active of FRAP (80.2–197.1) and ABTS (28.2–125.6 µmol TEAC/100 g), respectively. Aluminium metal matrix composites (AMMCs) using alumina (Al2O3) and silicon carbide (SiC) as reinforcement elements are gaining significant interest in various applications because of their excellent properties. In this study, Al2O3/SiC with compositions (0.5 wt.%, 1.5 wt.%, and 2.5 wt.% for each) were used as reinforcement elements in an aluminium alloy (AA2024-T6). The samples prepared were AA2024-T6 + (0.5Al2O3/0.5SiC), AA2024-T6 + (1.5Al2O3/1.5SiC), and AA2024-T6 + (2.5Al2O3/2.5SiC) using a stir-casting technique. The experimental study included density calculation mechanical properties, such as tensile strength, compressive strength and hardness. The study also included a microstructure examination of the fracture surface of the tensile specimens. The results showed that incorporating Al2O3/SiC as reinforcement materials into aluminium AA2024-T6 significantly improved its properties. Hence, increasing the reinforcement with compositions of 2.5Al2O3/2.5SiC into AA2024-T6 showed a drop in density and increased mechanical properties, such as ultimate tensile strength, compressive strength and hardness, compared to the base alloy (AA2024-T6). Furthermore, the scanning electron microscopy analysis revealed the uniform distribution of the reinforcement particles resulting in strong bonding with the matrix. The findings suggest that Al2O3/SiC reinforced with AA2024-T6 can be used in applications where a combination of lightweight and high strength is needed. [ABSTRACT FROM AUTHOR]

Objective: To determine the effect of Alpha Lipoic Acid (ALA) on blood glucose, lipid profile, and oxidative stress in streptozotocin-induced type II diabetic male Sprague Dawley rats. Study Design: Laboratory-based experimental study. Place and Duration of the Study: The study was carried out at the Physiology Research Lab, Army Medical College, Rawalpindi, Pakistan, from 20th January 2021 to 24th June 2021. Methods: Thirty Sprague Dawley (SD) rats were divided into three equal groups. Group I was given a normal standard rat diet. In Groups II and III after feeding a diet rich in fat and calories for 2 weeks, after feeding a diet rich in fat and calories for 2 weeks, a single low dose of streptozotocin was injected for Type 2 Diabetes Mellitus (T2DM) induction. Alpha Lipoic Acid 30 mg/kg body weight/day was administered intraperitoneally in Group III for 01 weeks after the development of T2DM. Results: The blood glucose, lipid profile, and serum Malondialdehyde levels were deranged in the diabetic group.
