Abstract Background Despite increasing female enrolment into medical schools, persistent gender gaps exist in the physician workforce. There are limited published data on female representation in the critical care medicine workforce. Methods To obtain a global perspective, societies (n = 84; 79,834 members (40,363 physicians, 39,471 non-physicians)) registered with the World Federation of Societies of Intensive and Critical Care Medicine were surveyed. Longitudinal data on female trainee and specialist positions between 2006-2017 were obtained from Australia and New Zealand. Data regarding leadership and academic faculty representation were also collected from national training bodies and other organisations of critical care medicine. Results Of the 84 societies, 23 had a registered membership of greater than 500 members. Responses were received from 27 societies (n = 55,996), mainly high-income countries, covering 70.1% of the membership. Amongst the physician workforce, the gender distribution was available from six (22%) participating societies—mean proportion of females 37 ± 11% (range 26–50%). Longitudinal data from Australia and New Zealand between 2006 and 2017 demonstrate rising proportions of female trainees and specialists. Female trainee and specialist numbers increased from 26 to 37% and from 13 to 22% respectively. Globally, female representation in leadership positions was presidencies of critical care organisations (0–41%), representation on critical care medicine boards and councils (8–50%) and faculty representation at symposia (7–34%). Significant gaps in knowledge exist: data from low and middle-income countries, the age distribution and the time taken to enter and complete training. Conclusions Despite limited information globally, available data suggest that females are under-represented in training programmes, specialist positions, academic faculty and leadership roles in intensive care. There are significant gaps in data on female participation in the critical care workforce. Further data from intensive care organisations worldwide are required to understand the demographics, challenges and barriers to their professional progress.

Chronic treatment with levodopa (LD) in Parkinson's disease (PD) can cause drug induced dyskinesias. Mucuna pruriens endocarp powder ( MPEP) contains several compounds including natural LD and has been reported to not cause drug-induced dyskinesias. We evaluated the effects of Mucuna pruriens to determine if its underlying mechanistic actions are exclusively due to LD. We first compared MPEP with and without carbidopa (CD), and LD+CD in hemiparkinsonian (HP)monkeys. Each treatment ameliorated parkinsonism.We then compared the neuronal firing properties of the substantia nigra reticulata (SNR) and subthalamic nucleus (STN) in HP monkeys with MPEP+CD and LD+CD to evaluate basal ganglia circuitry alterations. Both treatments decreased SNR firing rate compared to HP state. However, LD+CD treatments significantly increased SNR bursting firing patterns that were not seen with MPEP+CD treatments. No significant changes were seen in STN firing properties. We then evaluated the effects of a water extract of MPEP. Oral MPWE ameliorated parkinsonism without causing drug-induced dyskinesias. The distinctive neurophysiological findings in the basal ganglia and the ability to ameliorate parkinsonism without causing dyskinesias strongly suggest that Mucuna pruriens acts through a novel mechanism that is different from that of LD. Adsorbents prepared from pistachio nut shell, an agricultural waste biomass, were successfully used to remove Acid Violet 17 from an aqueous solution. The activated carbons PNS1, PNS2, and PNS3 were characterized by scanning electron microscope (SEM), Fourier Transform - Infra Red spectroscopy (FTIR) and (BET). The effect of pH, adsorbent dosage, and temperature on dye removal was studied. Maximum color removal was observed at pH 2. The adsorption increased with the increase in adsorbent dosage. As the adsorption capacity increased with the increase in temperature, the process was concluded to be endothermic. The experimental data were analyzed by the Langmuir and Freundlich isotherm models of adsorption. Equilibrium data fitted well with the Langmuir model. The rates of adsorption confirmed the pseudo-second order kinetics with good correlation values. The results indicated that the activated carbon prepared from pistachio nut shell can be effectively used for the removal of Acid Violet 17 from aqueous solution. [ABSTRACT FROM AUTHOR]

Parkinson's disease is a neurodegenerative disorder for which no neurorestorative therapeutic treatment is currently available. Oxidative stress plays an important role in the pathophysiology of Parkinson's disease. The ancient Indian medical system, Ayurveda, traditionally uses Mucuna pruriens to treat Parkinson's disease. In our earlier studies, Mucuna pruriens has been shown to possess antiparkinson and neuroprotective effects in animal models of Parkinson's disease. The antioxidant activity of Mucuna pruriens was demonstrated by its ability to scavenge DPPH radicals, ABTS radicals and reactive oxygen species. Mucuna pruriens significantly inhibited the oxidation of lipids and deoxyribose sugar. Mucuna pruriens exhibited divalent iron chelating activity and did not show any genotoxic/mutagenic effect on the plasmid DNA. These results suggest that the neuroprotective and neurorestorative effect of Mucuna pruriens may be related to its antioxidant activity independent of the symptomatic effect. Mucuna pruriens possesses significantly higher antiparkinson activity compared with levodopa in the 6-hydroxydopamine (6-OHDA) lesioned rat model of Parkinson's disease. The present study evaluated the neurorestorative effect of Mucuna pruriens cotyledon powder on the nigrostriatal tract of 6-OHDA lesioned rats. Mucuna pruriens cotyledon powder significantly increased the brain mitochondrial complex-I activity but did not affect the total monoamine oxidase activity ( in vitro). Unlike synthetic levodopa treatment, Mucuna pruriens cotyledon powder treatment significantly restored the endogenous levodopa, dopamine, norepinephrine and serotonin content in the substantia nigra. Nicotine adenine dinucleotide (NADH) and coenzyme Q-10, that are shown to have a therapeutic benefit in Parkinson's disease, were present in the Mucuna pruriens cotyledon powder. Earlier studies showed that Mucuna pruriens treatment controls the symptoms of Parkinson's disease. This additional finding of a neurorestorative benefit by Mucuna pruriens cotyledon powder on the degenerating dopaminergic neurons in the substantia nigra may be due to increased complex-I activity and the presence of NADH and coenzyme Q-10. Abstract: Improved understanding of the molecular mechanisms underlying dysregulated inflammatory responses in severe infection and septic shock is urgently needed to improve patient management and identify new therapeutic opportunities. The WNT signaling pathway has been implicated as a novel constituent of the immune response to infection, but its contribution to the host response in septic shock is unknown. Although individual WNT proteins have been ascribed pro- or anti-inflammatory functions, their concerted contributions to inflammation in vivo remain to be clearly defined. Here we report differential expression of multiple WNT ligands in whole blood of patients with septic shock and reveal significant correlations with inflammatory cytokines. Systemic challenge of mice with lipopolysaccharide (LPS) similarly elicited differential expression of multiple WNT ligands with correlations between WNT and cytokine expression that partially overlap with the findings in human blood. Molecular regulators of WNT expression during microbial encounter in vivo are largely unexplored. Analyses in gene-deficient mice revealed differential contributions of Toll-like receptor signaling adaptors, a positive role for tumor necrosis factor, but a negative regulatory role for interleukin (IL)-12/23p40 in the LPS-induced expression of Wnt5b, Wnt10a, Wnt10b, and Wnt11. Pharmacologic targeting of bottlenecks of the WNT network, WNT acylation and β-catenin activity, diminished IL-6, tumor necrosis factor, and IL-12/23p40 in serum of LPS-challenged mice and cultured splenocytes, whereas IL-10 production remained largely unaffected. Taken together, our data support the conclusion that the concerted action of WNT proteins during severe infection and septic shock promotes inflammation, and that this is, at least in part, mediated by WNT/β-catenin signaling.

In this paper, we propose an environment perception framework for autonomous driving using state representation learning (SRL). Unlike existing Q-learning based methods for efficient environment perception and object detection, our proposed method takes the learning loss into account under deterministic as well as stochastic policy gradient. Through a combination of variational autoencoder (VAE), deep deterministic policy gradient (DDPG), and soft actor-critic (SAC), we focus on uninterrupted and reasonably safe autonomous driving without steering off the track for a considerable driving distance. Our proposed technique exhibits learning in autonomous vehicles under complex interactions with the environment, without being explicitly trained on driving datasets. To ensure the effectiveness of the scheme over a sustained period of time, we employ a reward-penalty based system where a negative reward is associated with an unfavourable action and a positive reward is awarded for favourable actions. The results obtained through simulations on DonKey simulator show the effectiveness of our proposed method by examining the variations in policy loss, value loss, reward function, and cumulative reward for ‘VAE+DDPG’ and ‘VAE+SAC’ over the learning process.

In the distribution system, customers have increasingly use renewable energy sources and battery energy storage systems (BESS), transforming traditional loads into active prosumers. Therefore, methodologies are needed to provide prosumers with tools to optimize their investments and increase business opportunities. In this paper, a stochastic mixed integer linear programming (MILP) formulation is proposed to solve for optimal sizes of prosumer assets, considering the use of a BESS and photovoltaic (PV) units. The objective is to minimize the total cost of the system, which is defined as the combination of a solar PV system investment, BESS investment, maintenance costs of assets, and the cost of electricity supplied by the grid. The developed method defines the optimal size of PV units, the power/energy capacities of the BESS, and the optimal value for initial energy stored in the BESS. Both deterministic and stochastic approaches were explored. For each approach, the proposed model was tested for three cases, providing a varying combination of the use of grid power, PV units, and BESS. The optimal values from each case were compared, showing that there is potential to achieve more economic plans for prosumers when PV and BESS technologies are taken into account.

The Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) can be attributed to various causes such as CNS disorders like head trauma, stroke, pulmonary lesions like TB, bacterial pneumonia, as paraneoplastic syndromes in malignancies, drugs like antidepressants, antineoplastics like cyclophosphamide, vincristine, vinblastine, bortezomib, carboplatin and others. Cisplatin induced SIADH is very uncommon and hence, scarcely reported in literature. Here, we report two cases of dilutional hyponatremia secondary to SIADH that developed in two patients receiving radiotherapy with concomitant cisplatin.
