We constructed a database of cross-tropopause convection in the Asian monsoon region for the months of May through October of 2017 using overshooting tops (OTs), deep convective features that penetrate the local cirrus anvil layer and the local tropopause, with Meteosat-8 geostationary satellite detections. The database of 40 918 OTs represents a hemispheric record of convection covering the study domain from 10∘ S to 55∘ N and from 40 to 115∘ E. With this database, we analyzed the geographic, monthly, and altitude distribution of this convection and compared it to the convective distributions represented by satellite observations of outgoing longwave radiation (OLR) and precipitation. We find that cross-tropopause convection is most active during the months of May through August (with daily averages of these months above 300 OTs per day) and declines through September and October. Most of this convection occurs within Northern India and Southern India, the Bay of Bengal, and the Indian Ocean regions, which together account for 75.1 % of all OTs. We further identify distinct, differing seasonal trends within the study subregions. For the Northern India, Southern India, and Bay of Bengal regions, the distribution of OTs follows the development of the Asian monsoon, with its north–south movement across the study period. This work demonstrates that when evaluating the effects of convection on lower stratospheric composition over the Asian monsoon region, it is important to consider the impact of cross-tropopause convection specifically, as well as the contributions from both land-based and oceanic regions due to the significant geographic and monthly variation in convective activity.

Purpose Upper respiratory tract infections (URTI) and related symptoms are widespread and a common reason for visiting primary care with cough and sore throat being most prevalent. Despite their impact on daily activities, no studies have explored the impact on health-related quality of life (HRQOL) in representative general populations. We aimed to understand the short-term impact of the two most prevalent URTI symptoms on HRQOL. Methods Online 2020 surveys including acute (≤ 4 weeks) respiratory symptoms (sore throat and cough) and SF-36® health survey (all with 4-week recall) were analysed using analysis of covariance (ANCOVA) in comparison with adult US population norms. Linear T-score transformation of SF-6D utility (ranging from 0 to 1) enabled direct comparisons with SF-36. Results In total, 7563 US adults responded (average age: 52 years; range: 18–100 years). Sore throat and cough lasting at least several days were experienced by 14% and 22% participants, respectively. Chronic respiratory conditions were reported by 22% of the sample. Future studies on early self-care for symptom relief and its implications on HRQOL and health economics would be valuable to understand the benefits on healthcare burden and need for updating treatment guidelines.

ObjectiveEvaluate the effectiveness of alcohol sclerotherapy in postoperative breast and axillary seromas.MethodsThis was an IRB-approved retrospective review of consecutive patients from 2017 to 2021. The procedure involves aspiration of seroma fluid, injection of ethanol for 15 to 30 minutes, and then aspiration of the injected ethanol. Following review of the medical record, patient and procedure data were recorded. Success was defined as no recurrence of seroma. Statistical analysis was performed using a chi-square or t-test, as appropriate.ResultsTwenty seromas were treated in 19 patients (mean age, 63 years; range, 49–79) following mastectomy (9/20, 45%), lumpectomy (25%, 5/20), axillary nodal dissection (5/20, 25%), or chest wall surgery (1/20, 5%). The mean seroma volume was 146 ± 165 mL. There was a mean of 1.4 prior aspiration attempts (range, 0–3). Mean injected ethanol volume was 39 mL (range, 8–60 mL) for a mean of 26 ± 5 minutes. Seromas recurred in 85% (17/20) of cases, and repeat aspiration was performed in 35% (6/17). The mean recurrence time was 34 ± 29 days. The recurrent seromas were 31% smaller (mean, 80 ± 48 mL).
