IntroductionSkin-reducing mastectomy has been applied to several surgical techniques in which subcutaneous mastectomy is associated with various types of skin reduction, with preservation of a lower dermal flap to reinforce the inferior lateral seat of an implant. The aim of the study is to present a case series of patients with pendulous/ptotic and/or large-sized breasts treated for breast cancer at the Breast Surgery Unit of Istituto Nazionale Tumori IRCCS Fondazione “G. Pascale”, Naples, Italy, with the superomedial pedicle skin-reducing mastectomy technique, two-stage reconstruction, and transaxillary video-assisted technique, when a postoperative radiotherapy was indicated. We verified its effectiveness by discussing its results, especially in patients who are candidates for postmastectomy radiotherapy.Materials and methodsA single-center retrospective study was performed between January 2020 and March 2021 on a prospectively filled database of conservative mastectomies. Of the 64 patients who underwent nipple/skin-sparing mastectomies in the mentioned period, 17 (mean age 46 years, range 30–62 years) were treated with superomedial pedicle skin-reducing mastectomy, with two-stage breast reconstruction through transaxillary video-assisted replacement expander with definitive prosthesis and contralateral symmetrization, selected for postmastectomy radiotherapy.ResultsWe had only three minor complications. No flap necrosis, no infections, no breast seromas, and no reconstructive failures were observed. During follow-up of the patients treated with video-assisted reconstruction, there were no cases of infection, hematoma, implant rupture, or suture dehiscence in the reconstructed breast.DiscussionSkin-reducing mastectomy with superomedial pedicle is a safe and reliable procedure to treat breast cancer in selected patients, i.e., those with pendulous/ptotic and or large-sized breasts. Particularly, in patients who undergo postmastectomy radiotherapy, the two-stage reconstruction with video-assisted transaxillary endoscopic approach can find its main indication, using incisions positioned far from the mammary region, offering numerous advantages.

Whispering-gallery mode microresonators have gained wide popularity as experimental platforms for different applications, ranging from biosensing to nonlinear optics. Typically, the resonant modes of dielectric microresonators are stimulated via evanescent wave coupling, facilitated using tapered optical fibers or coupling prisms. However, this method poses serious shortcomings due to fabrication and access-related limitations, which could be elegantly overcome by implementing a free-space coupling approach; although additional alignment procedures are needed in this case. To address this issue, we have developed a new algorithm to excite the microresonator automatically. Here, we show the working mechanism and the preliminary results of our experimental method applied to a home-made silica microsphere, using a visible laser beam with a spatial light modulator and a software control.

In recent years, human populations’ exposure to microplastics via foods is becoming a topic of concern. Although microplastics have been defined as “emerging contaminants”, their occurrence in the environment and food is quite dated. This systematic review aims to investigate the discrepancies which are characterizing the research in the microplastics field in foods, with particular regard to sample preparations, microplastics’ concentrations and their effect on humans. For the selection of papers, the PRISMA methodology was followed. Discrepancies in the methodological approaches emerged and in the expression of the results as well, underlying the urgency in the harmonization of the methodological approaches. Uncertainties are still present regarding the adverse effects of microplastics on the human body. The scientific evidence obtained thus far is, in fact, not sufficient to demonstrate a concrete negative effect. This review has clearly underlined the need to standardise laboratory approaches to obtain useful results for better food safety management.

Mental health problems in early childhood are common, but there is a lack of psychiatric research on this age group. DC:0-5 is a multiaxial classification system for mental disorders in early childhood, providing a framework for standardizing clinical practice and research. However, research on the validity of DC:0-5 is scarce. The Developmental Psychiatry Diagnostic Challenges Study (DePsy) is a multi-site, prospective clinical study including six German early childhood mental health (ECMH) clinics. The main objective of the study is to contribute to the validation of Axis I and Axis II of DC:0-5. A second aim of the study is to describe the population of the participating clinics regarding diagnoses, family context, and treatment outcomes. Additionally, the impact of environmental risk factors, including parental Adverse Childhood Experiences (ACEs) and media use, on child psychopathology and caregiver–child relationships will be examined. Over two years, patients aged 0.0–5.9 years old will be enrolled in the study. Assessments include ICD-10 and DC:0-5 diagnoses, developmental tests, video-based observations of caregiver—child interactions, and questionnaires on child psychopathology, media use, parental stress, and treatment satisfaction. Study results will promote the standardization of assessment and treatment in ECMH clinics aiming to improve the development of patients and their families.

The β2-adrenergic receptor (β2AR) has provided a paradigm to elucidate how G protein-coupled receptors (GPCRs) control intracellular signaling, including the discovery that β-arrestins, which bind to ligand-activated GPCRs, are central for GPCR function. We used genome editing, conditional gene deletion, and small interfering RNAs (siRNAs) to determine the roles of β-arrestin 1 (β-arr1) and β-arr2 in β2AR internalization, trafficking, and signaling to ERK. We found that only β-arr2 was essential for β2AR internalization. Unexpectedly, β-arr1 and β-arr2 and receptor internalization were dispensable for ERK activation. Instead, β2AR signaled through Gαs and Gβγ subunits through a pathway that involved the tyrosine kinase SRC, the adaptor protein SHC, the guanine nucleotide exchange factor SOS, the small GTPase RAS, and the kinases RAF and MEK, which led to ERK activation. These findings provide a molecular framework for β2AR signaling through β-arrestin-independent pathways in key physiological functions and under pathological conditions.

A novel room‐temperature photodetector based on a silica‐microsphere optical resonator operating from the mid‐infrared to the terahertz spectral region is reported. The sensor relies on a free‐space visible laser whose frequency is locked to the center of a whispering‐gallery mode (WGM). The devised scheme automatically tracks any small change in the microresonator's size and refractive index that occurs upon illumination with radiation absorbed by silica. In this way, direct detection of quantum‐cascade laser beams around 4.3 and 111 μm wavelengths is shown, down to minimum noise‐equivalent power levels of ≈1 pW √Hz−1 with a dynamic range >100 dB. The microsphere resonator is then exploited, for the first time, as a photodetector in absorption spectroscopy of the 12CO2 fundamental ro‐vibrational band and of very‐weak 13CH3OH rotational lines.

Abstract Despite its known reconstructed volcanic history, the structural setting and present state of the Astroni Volcano of the Campi Flegrei caldera in Italy are still poorly defined. Through structural, geophysical, and geochemical investigations, we elucidate the structure and present volcanic activity of the Astroni Volcano, which hosts tuff cones, scoriae cones, lava domes, and lakes on the crater floor. A volcano‐tectonic analysis focused on the entire volcano edifice, coupled with electrical resistivity tomography of the shallower part of the Astroni crater, revealed the main rock formations, faults, and possible fluid patterns within the first 150 m depth. Two main NE–SW and NW–SE trending fault sets were imaged using electrical resistivity modeling and measurements along the wall of the volcanic edifice; they likely delimit a maar‐like structure resulting from the highest energetic subplinian Astroni 6 eruption event and acted as magma pathways during the late eruptive activity stage. A 3D view of the reconstructed resistivity model revealed both deep root‐conduit‐like structures and shallower dome‐like shapes for volcanic edifices on the crater floor. Gas and carbon compositions in the NNE sectors of the Astroni Lago Grande are similar to those of the Solfatara fumarole fluids, suggesting common hydrothermal origin and a possible link with a deep hydrothermal reservoir. This fluid‐emission area along the border of the younger volcanic structure exhibits a +40°C maximum soil‐temperature anomaly. The proposed volcano‐tectonic architecture should improve the unrest scenarios in case of reactivation in this Campi Flegrei caldera sector and the monitoring strategies for the Astroni Volcano.

An economy based on renewable energy sources is the hallmark of responsible companies. Climate policy and energy crisis commitments have led to a search for alternative ways to produce energy. Bioenergy is considered the most consistent renewable energy source due to its economic and environmental benefits. Biogas and biomethane are promising forms of renewable energy derived from widely available evergreen raw materials. Agricultural, animal, industrial and food wastes are excellent substrates used to produce clean and sustainable energy in a circular economy context. Their conversion into biogas and biomethane through the anaerobic digestion (AD) process is an efficient solution to the treatment of waste of different origins. The production and use of biomethane favor important environmental advantages, such as the reduction in greenhouse gas emissions compared with those deriving from the use of conventional fossil fuels. This review would like to highlight modern trends and approaches to evaluate processes and strategies to control biogas and biomethane production. In particular, the use of livestock waste for the digestion process and the reuse of the by-product as fertilizer, as well as the potential development of biogas and biomethane as prospects for the improvement and optimization of renewable energy sources, are discussed.

Postural instability (PI) in Parkinson’s disease (PD) exposes patients to an increased risk of falls (RF). While dopaminergic therapy and deep brain stimulation (DBS) improve motor performance in advanced PD patients, their effects on PI and RF remain elusive. PI and RF were assessed using a stabilometric platform in six advanced PD patients. Patients were evaluated in OFF and ON dopaminergic medication and under four DBS settings: with DBS off, DBS bilateral, and unilateral DBS of the more- or less-affected side. Our findings indicate that dopaminergic medication by itself exacerbated PI and RF, and DBS alone led to a decline in RF. No combination of medication and DBS yielded a superior improvement in postural control compared to the baseline combination of OFF medication and the DBS-off condition. Yet, for ON medication, DBS significantly improved both PI and RF. Among DBS conditions, DBS bilateral provided the most favorable outcomes, improving PI and RF in the ON medication state and presenting the smallest setbacks in the OFF state. Conversely, the more-affected side DBS was less beneficial. These preliminary results could inform therapeutic strategies for advanced PD patients experiencing postural disorders.

Preschool mental disorders are often associated with significant interpersonal problems, related to impaired affect recognition, theory of mind (ToM), and empathy. To date, these skills have not been studied together in preschoolers with externalizing behavior problems (EBPs). The aim of the present study was to investigate whether and to what extent preschool children with EBPs show impairments in affect recognition, ToM, and empathy. Preschoolers with EBPs, defined by current psychiatric treatment and T-scores ≥ 60 on the externalizing problem scale of the Child Behavior Checklist (CBCL/1½-5 or 6-18R) were compared to non-clinical controls (HCs), defined by no past and no current psychiatric treatment and T-scores < 60 on all CBCL broad-band scales. Groups were compared on affect recognition (NEuroPSYchological Assessment-II), affective ToM (Test of Emotion Comprehension), cognitive ToM (Extended Theory-of-Mind Scale), parent-reported emotional contagion, attention to others’ feelings, and prosocial action (Empathy Questionnaire), IQ and language (Wechsler Preschool and Primary Scale of Intelligence-III Matrices, Active and Passive Vocabulary test), controlling for age, sex, and language abilities. Compared to 28 HCs, 22 preschoolers with EBPs (total sample meanage = 5.5 years +/− 0.8 years, range= 4.2–6.9 years, males 66%) had significantly greater impairments in cognitive ToM (p = 0.0012, η2 = 0.266), attention to others’ feelings (p = 0.0049, η2 = 0.222), and prosocial action (p = 0.0070, η2 = 0.210), each representing strong effect sizes. EBPs were significantly related to cognitive domains, like prosocial action (r = −0.501), cognitive ToM (r = −0.425), and attention to others’ feelings (r = −0.332), but not to affective domains of social cognition. Social cognitive development may be impaired as early as preschool age and should be promoted before the child starts school.

Background and purpose: Although there is extensive evidence about the safety of monoclonal antibodies against calcitonin gene‐related peptide (anti‐CGRP mAbs) in combination with traditional drugs, scarce data are available on the safety of their combination with other mAbs. This study aimed to evaluate the 6‐month effectiveness and tolerability of anti‐CGRP mAbs in combination with other mAbs for different diseases. Methods: Patients included in the Italian Headache Registry and treated concomitantly with an anti‐CGRP mAb and another mAb were included. Effectiveness outcomes for migraine included reduction from baseline of monthly headache days (MHDs), Migraine Disability Assessment (MIDAS) score, Headache Impact Test‐6 (HIT‐6) scores, and Patients' Global Impression of Change (PGIC) scale. Adverse events (AEs) were recorded. Results: Thirty‐eight patients were included. In 27 patients (71.1%), the anti‐CGRP mAb was added to a previously ongoing mAb. Nine patients (23.7%) discontinued one of the two mAbs before the end of treatment (seven discontinued the anti‐CGRP mAb and two the other mAb). One patient discontinued for AEs. Anti‐CGRP mAbs were discontinued due to ineffectiveness (n = 5, 55.5%) and one each (11.1%) for clinical remission and lost to follow‐up. MHDs significantly decreased from baseline to 3 months (p < 0.0001) and 6 months (p < 0.001), as did the MIDAS and the HIT‐6 scores at 3 and 6 months (p < 0.001). For anti‐CGRP mAbs, 27.4% of patients reported PGIC ≥ 5 at 3 months and 48.3% at 6 months. Mild AEs associated with introduction of a second mAb were detected in six patients (15.8%). Conclusions: In this real‐world study, anti‐CGRP mAbs showed safety and effectiveness when administered concomitantly with other mAbs. [ABSTRACT FROM AUTHOR]

Plant leaves can be used to determine the atmospheric deposition of organic contaminants, including polycyclic aromatic hydrocarbons (PAHs), to assess the contamination status of an area. The purpose of this study was to develop an analytical method for the determination of PAHs deriving from atmospheric deposition using Malva sylvestris leaves. Analytes were recovered from the leaves of the plant using cyclohexane as an organic solvent and subsequent sonication. The percentage recoveries (R%) were good (from 65.8 ± 3.2 to 104.2 ± 16.9), together with the instrumental analytical parameters, including correlation coefficients (r) ≥ 0.995 for all PAHs. The instrumental analysis was carried out using GC-MS in total ion current and single ion monitoring at the same time. Real samples taken from urban environments have shown that they are not always the most contaminated. At the Palermo site, leaves were observed to have high amounts of PAHs due to the deposition of dust generated by combustion processes that occurred near the sampling site. Further studies are recommended to compare the use of plants and classical sampling systems for monitoring the atmospheric deposition of key contaminants toxic to human health. In volcanic regions, the analysis of Thermal InfraRed (TIR) satellite imagery for Land Surface Temperature (LST) retrieval is a valid technique to detect ground thermal anomalies. This allows us to achieve rapid characterization of the shallow thermal field, supporting ground surveillance networks in monitoring volcanic activity. However, surface temperature can be influenced by processes of different natures, which interact and mutually interfere, making it challenging to interpret the spatio-temporal variations in the LST parameter. In this paper, we use a workflow to detect the main thermal patterns in active volcanic areas by analyzing the Independent Component Analysis (ICA) results applied to satellite nighttime TIR imagery time series. We employed the proposed approach to study the surface temperature distribution at the Campi Flegrei caldera volcanic site (Southern Italy, Naples) during the 2013–2022 time interval. The results revealed the contribution of four main distinctive thermal patterns, which reflect the endogenous processes occurring at the Solfatara crater, the environmental processes affecting the Agnano plain, the unique microclimate of the Astroni crater, and the morphoclimatic aspects of the entire volcanic area. The prevalence of many chronic noncommunicable diseases has been steadily rising over the past six decades. During this time, over 350,000 new chemical substances have been introduced to the lives of humans. In recent years, the epithelial barrier theory came to light explaining the growing prevalence and exacerbations of these diseases worldwide. It attributes their onset to a functionally impaired epithelial barrier triggered by the toxicity of the exposed substances, associated with microbial dysbiosis, immune system activation, and inflammation. Diseases encompassed by the epithelial barrier theory share common features such as an increased prevalence after the 1960s or 2000s that cannot (solely) be accounted for by the emergence of improved diagnostic methods. Other common traits include epithelial barrier defects, microbial dysbiosis with loss of commensals and colonization of opportunistic pathogens, and circulating inflammatory cells and cytokines. In addition, practically unrelated diseases that fulfill these criteria have started to emerge as multimorbidities during the last decades. Here, we provide a comprehensive overview of diseases encompassed by the epithelial barrier theory and discuss evidence and similarities for their epidemiology, genetic susceptibility, epithelial barrier dysfunction, microbial dysbiosis, and tissue inflammation. There is not a welfare protocol for horses reared for meat production in semi-extensive systems. The aims of the study were to develop a specific welfare protocol suitable to be applied at pasture and on stable; and to evaluate whether the welfare items were influenced by the farming system (pasture vs stable). 52 non-animal-based measures (N-ABMs) and 14 animal-based measures (ABMs), classified into 6 thematic areas (training, feeding, facilities, ABMs, biosecurity, health management) were selected by a focus group. The protocol was applied on a total of 429 Catria horses located across 26 pastures during the warm seasons and on 7 stables during the cold seasons. Differences obtained within each horse-unit were calculated by using the diversity index (VARNC) and the distance from the ideal (dfi) index. Chi-square test was used for comparing the relative frequencies (%) of the answers (pasture vs stable). Most the welfare items were classified as adequate in both pasture and stable, yet differences were found within 'training' (p = 0.02) and 'feeding' (p < 0.01) areas in relation to the welfare items 'inspection of the animals' and 'feeding management'. Weaknesses of both pasture and stable were represented by some welfare items in the 'health management' and 'biosecurity' areas. After proper validation, the welfare protocol developed in the present study could help to fill the existing gap of knowledge on horse welfare assessment for semi-extensive system systems, providing support for official control of veterinarians and enabling the identification of key weakness to address preventive interventions. Iatrogenic aortic dissection (IAD) is a life‐threatening condition, primarily caused by arterial cannulation during cardiopulmonary bypass (CPB) in cardiac surgeries. Transesophageal echocardiography (TEE) is the first‐line diagnostic tool in the acute setting, but the presence of several artifacts can easily lead to misinterpretation. A 55‐year‐old man underwent coronary artery bypass grafting and implantation of central veno‐arterial extracorporeal membrane oxygenation (V‐A ECMO). TEE revealed what appeared to be an intimal dissection flap in the aortic arch and descending thoracic aorta, raising concerns for an IAD, which was not confirmed by computed tomographic angiography. This case highlights the pitfalls and limitations of echocardiography in the diagnosis of IAD, especially in settings with complex flow patterns such as during CPB or V‐A ECMO. The field of bioorthogonal chemistry has revolutionized our ability to interrogate and manipulate biological systems at the molecular level. However, the range of chemical reactions that can operate efficiently in biological environments without interfering with the native cellular machinery, remains limited. In this context, the rapidly growing area of photocatalysis offers a promising avenue for developing new type of bioorthogonal tools. The inherent mildness, tunability, chemoselectivity, and external controllability of photocatalytic transformations make them particularly well‐suited for applications in biological and living systems. This minireview summarizes recent advances in bioorthogonal photocatalytic technologies, with a particular focus on their potential to enable the selective generation of designed products within biologically relevant or living settings. Background: In recent years, in case of aortic valve replacement (AVR), a significant increase in the use of bioprostheses has been observed. The Perceval sutureless bioprosthesis has proven to be safe and reliable in the short and mid-term, with limited but promising long-term results. An updated systematic review with the long-term results of patients who underwent a sutureless bioprosthesis implantation with a Perceval biological valve is herewith presented. Methods: Studies published between 2015 and 2024, including the long-term outcomes—with clinical as well as echocardiographic information for up to five years—of patients who underwent a Perceval implantation for AVR were selected from the published literature. The Cochrane GRADE system was used to assess the study quality, and the risk of bias in non-randomized studies (ROBINS-I) tool was used to evaluate studies. Results: Ten studies were selected with an overall number of 5221 patients. The long-term survival ranged from 64.8 to 87.9%, freedom from structural valve degeneration (SVD) from 96.1 to 100%, freedom from significant paravalvular leak from 98.5 to 100%, freedom from prosthetic endocarditis from 90.7 to 99%, and freedom from reintervention from 94 to 100%. The long-term mortality ranged from 6.5 to 27.4%. SVD was observed in 0–4.8% patients. Significant paravalvular leak was observed in 0–3.4% patients, and infective endocarditis was observed in 0–3.4%. A bioprosthesis-related reintervention at long-term follow-up was required for 0–4.3% of patients, and 1.7–7.1% of patients required a late new pacemaker implantation. The transprosthetic mean pressure gradient ranged from 9 to 14.7 mmHg, peak pressure gradient ranged from 17.8 to 26.5 mmHg, and EOA ranged from 1.5 to 1.7 cm2. Conclusions: This systematic review shows that there is still a paucity of data about sutureless bioprostheses. Nevertheless, the clinical results from prospective studies or retrospective series are encouraging. Medium- and long-term results seem to support the increasing use of this type of prosthesis. Background: Paracetamol and ibuprofen are the most commonly used drugs for pain treatment in children and their combination has shown improved analgesic effect compared to treatment with either drug alone. Current literature lacks specific guidelines regarding the settings in which this combination should be adopted. Methods: The survey, conducted with Delphi methodology, involved 75 hospital and outpatient pediatricians with clinical experience in the management of pain in children. Pediatricians involved were asked to validate or not the results of the previous NominalGroup Tecnique (NGT) consensus and thus specify the optimal clinical settings in which the paracetamol/ibuprofen fixed-dose combination could be adopted. Results: The results confirm the importance of the fixed-dose paracetamol and ibuprofen combination for the control of mild-to-moderate acute pain in children. Particularly, this association seems to be appropriate in case of headache, earache, odontalgia and musculoskeletal pain, and in specific settings such as post-operative and post-procedural pain. The broadening of the panel brought to slight variations in clinical management practices between hospital and outpatient specialists. Nonetheless, overall consensus supports the notion that the fixed dose combination is more efficacious than monotherapies and it is well tolerated. Moreover, experts unanimously agree on the usefulness of the combination for caregivers, leading to improved adherence and effectiveness. Conclusions: Both the NGT consensus and the broader Delphi consensus confirm the usefulness of the paracetamol-ibuprofen fixed-dose combination in pediatric pain. This is attributed to its superior effectiveness compared to monotherapies, a good tolerability profile, and improved compliance and ease of use. Some pain settings related to chronic, inflammatory and rheumatological pathologies remain to be investigated to evaluate the use of this combination. [ABSTRACT FROM AUTHOR]
