203. Is there a biochemical basis for lower oesophageal sphincter (LOS) resting pressure?

Abstract

Myosin plays a fundamental role in the contraction of muscle cells. Some structural differences of this protein are related to the different physical properties of muscle--that is, speed of shortening. The typical myosin protein has a molecular weight of 460 000 daltons and is made up of six subunits: two heavy chains (200 000 daltons each) and two pairs of light chains (LC) weighting 15 000-25 000 daltons each (LC1 and LC2). Myosin was extracted from six specimens of human oesophagus in order to find out whether any biochemical differences exist between the oesophageal body and the lower oesophageal sphincter, to account for their different properties. Myosin was examined by two dimensional gel electrophoresis. Peptide mapping of myosin heavy chains was obtained by carrying out enzymatic digestion during the electrophoretic run. A quantitative difference between LC1 of the oesophageal body and the lower oesophageal sphincter was found in the circular fibres, thereby suggesting that two populations of muscle fibres exist in the inner coat of the oesophagus. 208. Vapour pressure of isotopic liquids

Abstract

The separation factor a in the liquid-vapour equilibrium of the isotopic diatomic systems12CO—13CO,14N2—14N15N, and16O2—16O18O was measured in the temperature range from boiling point to a vapour pressure of about 12 atm. Natural and enriched mixtures were used. A very good agreement with measurements carried out by other authors in a lower temperature range, using separated isotopes, was found for all systems, thus confirming the reliability of the static equilibration method. An approximate linear dependence of In α on 1/T2was found in the range of existence of the liquid explored so far. A brief discussion of the results is given. Con un metodo statico già descritto in articoli precedenti è stato misurato il fattore di separazione nell’equilibrio liquido-vapore dei sistemi isotopici biatomici12CO—13CO,14N2—114N15N, e16O2—16O18O tra la temperatura di ebollizione e la temperatura corrispondente a una pressione di vapore di circa 12 atm, adoperando miscele isotopiche naturali o arricchite. I risultati sperimentali cosi ottenuti sono anzitutto confrontati con quelli ottenuti da altri sperimentatori che hanno determinato il rapporto delle tensioni di vapore dei due isotopi separati in un intervallo di temperature inferiori al punto di ebollizione. L’accordo è molto buono, oomprovando cosi la completa equivalenza dei due metodi. Viene altresi mostrato che in tutto l’intervallo di temperatura finora esplorato il logaritmo del fattore di separazione è proporzionale all’inverso del quadrato della temperatura. I risultati vengono infine brevemente discussi alla luce delle teorie esistenti.

238. Esophageal strictures following nasogastric intubation

Abstract

Esophageal stricture is not reported to be a common complication of nasogastric intubation. Two patients who had a Levin tube inserted in the stomach for nutrition during a prolonged coma, and finally presented with a severe esophageal stricture were observed and treated. The clinical history of the patients suggests that the pathogenesis of this lesion is probably multifactorial, secondary to induced gastroesophageal reflux, impaired esophageal clearance, high gastric acid output, and use of steroids. Jejunostomy feeding, rather than nasogastric feeding, is probably a better means to provide nutritional support in comatose patients, thus averting the risk of such a serious complication. Esophageal replacement with left colon interposition appears the treatment of choice for these severe esophageal strictures.

Abstract

Distal esophageal function and LES competence were evaluated in 21 patients by means of esophageal manometry and standard pHmetry after Sujura mod. operation for bleeding esophageal varices. This operation involves complete devascularization of 10-12 cm. of distal esophagus, disruption of anatomic anti-reflux mechanism, excision of nearly one cm. of muscular tissue at lower esophageal sphincter level. Intraoperative manometry was performed in 8 patients and demonstrated a significant shortening of high pressure zone, while lower esophageal sphincter pressure did not show significant variations. Postoperatively gastroesophageal reflux at pHmetry was not observed (five patients had pathological reflux preoperatively). No significant variations of resting lower esophageal sphincter pressure and no peristaltic alterations were observed. In conclusion despite the marked anatomic alterations related to this procedure, sphincter dynamics and distal esophageal function remain nearly normal.
