Background/Aims: Altered anatomy is a challenge in endoscopic retrograde cholangiopancreatography (ERCP) for patients with Billroth II anastomosis. In this study, we investigated the overall success and role of endoscopist experience. Methods: Data of patients who underwent ERCP between 2014 and 2018 after a previous Billroth II operation were retrieved retrospectively from 2 tertiary FRCP centers. The procedures were performed by 2 endoscopists with different levels of experience. Clinical success was defined as extraction of the stone, placement of a stent through a malignant stricture, and clinical and laboratory improvements in patients. Results: Seventy-five patients were included. The technical success rate was 83% for the experienced endoscopist and 75% for the inexperienced endoscopist (p=0.46). OBJECTIVE: The present study aims to determine whether normal spontaneous vaginal delivery at term was associated with an inflammatory activation compared with scheduled cesarean section without previous onset of labor by detecting cord blood serum levels of interleukin-6 (IL-6) and interleukin-1β (IL-1β). STUDY DESIGN: Eighty-eight patients were studied (vaginal delivery n=44 and scheduled cesarean n=44) with cord venous blood sampling after delivery of the fetus. IL-6 and IL-1β levels were measured with commercial kits. All of the subjects had uncomplicated term singleton pregnancies. RESULTS: Both study groups showed no significant difference in maternal age, gestational week, birth weight, body mass index (p>0.05). Cord blood IL-6 and IL-1β levels were similar between spontaneous vaginal delivery and scheduled cesarean groups (3.1±2.0 pg/mL versus 3.1±2.9 pg/mL, p=0.62 and 2.0±1.7 pg/mL versus 2.3±1.8 pg/mL, p=0.46, respectively). Also maternal age, gestational week, birth weight and body mass index did not correlate with cord blood cytokine levels. CONCLUSION: In healthy and term pregnancies, the exposition of spontaneous vaginal delivery and labor is not associated with activation of inflammatory mediators compared to cesarean. The data of the present study did not support the increasing ratio of newborn complications (respiratory distress syndrome, bronchopulmonary dysplasia, the need for prolonged oxygen therapy and neonatal intensive care unit, allergic diseases of infancy period) after cesarean delivery due to inadequate increasing levels of cytokines in newborns delivered by cesarean.

Giriş: Günümüzde küreselleşmenin bir sonucu olarak toplumlar çok kültürlü hale gelmiştir. Sağlık profesyonellerinin kültürel açıdan yeterli hizmet sunabilmeleri ancak kültürlerarası iletişim yeterliliğinin sağlanması ile mümkündür. Dolayısıyla bu araştırmada ebelik öğrencilerinin kültürel duyarlılıkları ile kültürel beceri düzeyleri arasındaki ilişkinin incelenmesi amaçlanmıştır.Yöntem: Araştırma bir devlet üniversitesinde öğrenim gören 318 ebelik öğrencisi ile analitik ve kesitsel olarak gerçekleştirilmiştir. Veri toplamada Demografik Bilgi Formu, Kültürlerarası Duyarlılık Ölçeği ve Kültürlerarası Beceri Ölçeği kullanılmıştır. Araştırma verileri, ANOVA ve Pearson korelasyon testleri ile değerlendirilmiştir. Bulgular: Öğrencilerin Kültürel Duyarlılık Ölçeği (birinci sınıf: 77.25±8.95, ikinci sınıf: 77.98±7.45, üçüncü sınıf: 78.12±6.54, dördüncü sınıf: 78.87±5.78) ve Kültürel Beceri Ölçeği (birinci sınıf: 69.47±6.74, ikinci sınıf: 70.01±5.85, üçüncü sınıf: 70.86±6.74, dördüncü sınıf: 71.96±7.85) puan ortalamalarının sınıf seviyesine paralel olarak arttığı ancak bu artışın istatistiksel anlamlılık taşımadığı görülmüştür. Ayrıca öğrencilerin kültürel duyarlılık ve kültürel beceri düzeyleri arasında pozitif yönde orta şiddette ilişki saptanmıştır. Sonuç: Çalışmada öğrencilerin kültürel duyarlılık düzeyleri ile kültürel beceri düzeyleri arasında pozitif yönde orta şiddette anlamlı ilişki olduğu, sınıflara göre kültürel duyarlılık ve kültürel beceri düzeyleri arasında anlamlı ilişki bulunmadığı sonuçlarına ulaşılmıştır.

Purpose- The study aims to estimate the export income of Turkey under the effects Corona Virus Covid 19 Pandemics of 2020. Methodology- The study employs descriptive statistics analyses for macroeconomic indicators of leading export markets of Turkish economy.Findings- The analysis shows that Turkish Economy export income would decline 30 percent relative to 2019 due to sudden shock and lockdowns and losses, deaths and illnesses during the first quarter of 2020.Conclusion- The World Economy has been expected to shrink 3 percent at 2020. This is larger than 1.68 percent decline in growth rate at 2009. In 2009 financial crises, Turkish export volume decreased %30 in 2009. At 2020, the export of Turkey would decrease 30 percent due to decline of import demand of major export markets of EU, OECD and member countries of Islamic Cooperation Countries. However, the World Bank and IMF’s expectations suggest rapid growth in the World economy at 2021.

Özellikle özel durumlara sahip ebeveynler (engellilik, infertilite, anomalili bebeği olan, istenmeyen gebelik, çoğul gebelik) doğum sonrası döneme adaptasyon sağlamakta zorlanmaktadır. Engelli ebeveynlerin doğum sonu döneme uyumları uzayabilirken, infertil ebeveynlerde doğum sonu dönemde postpartum depresyon görülebilmekte ve anomalili bir bebeğe sahip olan ebeveynlerde post-travmatik stres yaşayabilmektedir. İstenmeyen gebelik sonucu bebek sahibi olan ebeveynlerde öfke, hayal kırıklığı, bebeği istememe gibi tepkiler görülebilmekte, çoğul gebeliklerde doğum sonrası psikolojik ve sosyal desteğe ihtiyaç duyulmaktadır. Bu derleme özel durumlara sahip ebeveynlerin doğum sonrası döneme adaptasyon süreçleri ile ilgili ebe ve hemşirelere farkındalık kazandırmak amacıyla hazırlanmıştır.

223. Nasal Anthropometric Analysis in Turkish Rhinoplasty Patients

This study aimed to determine the mean values of the nasal anthropometric measurements through 2-dimensional photogrammetry in for rhinoplasty patients. The study group included 72 healthy individuals, 36 men, and 36 women. The patients were aged 18 to 42 years. All patients had undergone primary rhinoplasty in our clinic between 2013 and 2020. Seven landmarks were identified from the preoperative photographs taken with the patient's head in neutral position in the frontal and lateral planes. Five distances (total nasal length, nasal bridge length, morphological nose width, alar length, and nasal tip protrusion) and 2 angles (nasofrontal and nasolabial) measurements were calculated. We noted a statistically significant difference between the male and female sexes in terms of total nasal length and morphological nose width (P0.05). The results were compared with the nasal anthropometric measurements reported in the literature for the Turkish population as well as other ethnic groups. We believe results of this study may facilitate preoperative planning in Turkish patients who request rhinoplasty and improve the success rate of the surgery.

Vehicle Routing Problem (VRP) is used to ensure that customer requirements are met on time and at minimum cost. With VRP, it is aimed to create routes that enable the movement of limited capacity vehicles or vehicles to customers in different regions, with the lowest cost from a depot. Therefore, the purpose of the Vehicle Routing Problem, which deals with product distribution between the depot and the customers, is to determine the vehicle routes in a way that will serve all customers by minimizing the total travel costs. In this study, it is aimed to minimize transportation costs by improving the capacity utilization of the vehicles, used for deliveries from the central depot of a flour mill, operating in the city center of Isparta to customers in the city and district centers of Isparta, Denizli, Burdur, Afyon and Antalya. Within the scope of the study, after the routes were determined with the Savings Algorithm, alternative solutions were obtained by fuzzying the demand data with the help of Fuzzy Logic in order to improve the capacity ratios of the vehicles., Araç Rotalama Problemi (ARP), müşteri gereksinimlerinin zamanında ve minimum maliyetle karşılanmasını sağlamak amacıyla kullanılmaktadır. ARP ile sabit bir depodan, sınırlı kapasiteli araç veya araçların farklı bölgelerde yer alan müşterilere en düşük maliyetle hareketlerini sağlayan rotalar oluşturulması hedeflenmektedir. O yüzden de, depo ile müşteriler arasında ürün dağıtımıyla ilgilenen Araç Rotalama Problemi’nin amacı, toplam seyahat maliyetlerini en aza indirerek tüm müşterilere hizmet verecek şekilde araç rotalarının belirlenmesidir. Bu çalışmada, Isparta il merkezinde faaliyet gösteren bir un fabrikasının merkez deposundan Isparta, Denizli, Burdur, Afyonkarahisar ve Antalya il ve ilçe merkezlerindeki müşterilere teslimat seferleri için kullanılan araçların kapasite kullanımları iyileştirilerek, ulaşım maliyetlerin en aza indirilmesi amaçlanmıştır. Çalışma kapsamında Kazançlar Algoritması ile dağıtım rotaları belirlendikten sonra, araçların doluluk oranlarının iyileştirilmesi amacıyla Bulanık Mantık yardımıyla talep verileri bulanıklaştırılarak alternatif çözümler elde edilmiştir.

Geriatric and arthritic dogs have special rehabilitation needs. In these dogs the functional status worsens because of the progressive loss of movement. Aging triggers changes in a wide variety of tissues. The incidence and severity of chronic diseases increase over time. Animals that reach 75% of the predicted lifespan are defined as geriatric. The aging process is affected by breed, genetics, nutrition, environment, and other factors. The life expectancy of dogs has increased due to medical and surgical advances in veterinary medicine. As a result owners are faced with a number of problems related to old age. While many pets are active during adolescence, most dogs begin to show signs of aging around the age of 6. Geriatric dogs with this condition are candidates for comprehensive evaluation and rehabilitation. Geriatric and arthritic dogs should be evaluated regularly to detect changes in their condition and to design the most accurate treatment plan. Pain and limitations are often seen in geriatric dogs, and their quality of life decreases. The use of pain management guidelines, multimodal medication, physical rehabilitation modalities and environmental changes ensures the dog has a quality life. Rehabilitation goals for aging dogs include promoting health, slowing the onset or progression of age-related health problems, and improving the dog's quality of life—if possible, life expectancy. This study emphasized the effects of aging, common musculoskeletal changes in the aging dog, quality of life, pain management and rehabilitation process of the arthritic dog., Geriatrik ve artritik köpeklerin durumlarına özel rehabilitasyona ihtiyaçları vardır. Yaşlanan köpeklerde ilerleyici hareket kaybıyla bağlantılı olarak fonksiyonel durum kötüleşir. Yaşlanma, çok çeşitli dokularda değişikliklere neden olur. Ayrıca yaşlanan köpeklerde kronik hastalıkların görülme sıklığı ve şiddeti de artmaktadır. Öngörülen yaşam süresinin %75’ine ulaşan hayvanlar geriatrik olarak tanımlanır. Yaşlanma süreci cins, genetik, beslenme, çevre ve diğer faktörlerden etkilenir. Veteriner hekimlikte tıbbi ve cerrahi ilerlemeler nedeniyle köpeklerin yaşam süresi uzamıştır. Bunun bir sonucu olarak, evcil hayvanların sahipleri yaşlılığa bağlı bir dizi sorunla karşı karşıya kalmaktadır. Pek çok evcil hayvan ergenlik çağında aktif olsa da, çoğu köpek 6 yaşından itibaren yaşlanma belirtileri göstermeye başlar. Bu durumdaki geriatrik köpekler kapsamlı bir değerlendirmeye ve rehabilitasyona ihtiyaç duyar. Geriatrik ve artritik köpekler, durumlarındaki değişiklikleri tespit edebilmek ve en doğru bakım planını tasarlayabilmek için mutlaka düzenli olarak değerlendirilmelidir. Geriatrik köpeklerde sıklıkla ağrı ve hareket kısıtlılığı görülür, yaşam kalitesi azalır. Ağrı yönetimi kılavuzlarının, multimodal ilaçların, fiziksel rehabilitasyon modalitelerinin ve çevresel düzenlemelerin kullanılması yaşlı köpeğin kaliteli bir yaşam sürmesini sağlar. Yaşlanan köpekler için rehabilitasyon hedefleri, sağlığı desteklemeyi, yaşa bağlı sağlık sorunlarının ilerlemesini yavaşlatmayı ve köpeğin yaşam kalitesini arttırmayı ve -mümkünse- yaşam süresini uzatmayı içerir. Bu çalışmada yaşlanmanın etkileri, yaşlanan köpekte yaygın görülen kas-iskelet sistemi değişiklikleri, yaşam kalitesi, ağrı yönetimi ve artritik köpeğin rehabilitasyon süreci üzerinde durulmaktadır.

Low-Level laser therapy (LLLT), known as photobiomodulation therapy, stimulates the mitochondrial respiratory chain and causes changes in cellular adenosine triphosphate or cyclic adenosine monophosphate levels. Lasers are classified according to their power, maximum exposure, and wavelength. The dose required to treat the tissue; it depends on wavelength, power density, tissue type, tissue condition, pigmentation, depth of target tissue and treatment technique. Lasers are a useful treatment in human and veterinary rehabilitation. Studies showing that cartilage properties are preserved with treatment, improvement in peripheral nerve injuries, and that they contribute to pain management in patients with osteoarthritis are promising for their use in veterinary rehabilitation., Fotobiyomodülasyon tedavisi olarak bilinen düşük yoğunluklu lazer tedavisi (Low Level Laser Therapy-LLLT), mitokondriyal solunum zincirini uyarır ve hücresel adenozin trifosfat veya siklik adenozin monofosfat seviyelerinde değişikliklere neden olur. Lazerler güçlerine, maksimum maruz kalma miktarına ve dalga boylarına göre sınıflandırılır. 227. Evaluation of Pathogens and Antibiotic Resistance Profiles Isolated from Lower Respiratory Tract Samples

Amaç: Bu çalışmada hastanemize başvuran hastalardan izole edilen bakteriyel alt solunum yolu enfeksiyonu etkenleri ve antibiyotik direnç profilleri ortaya konularak ampirik tedavinin planlanmasında klinisyenlere yol gösterici olmak ve bölgesel verilere katkı sağlamak amaçlanmaktadır.Yöntem: Hastanemiz farklı birimlerine 01 Ocak – 31 Aralık 2021 tarihleri arasında başvuran hastalardan gönderilen alt solunum yoluna ait tüm örnekler retrospektif olarak değerlendirildi. Tespit edilen etkenlerin tanımlama ve duyarlılık testleri VITEK 2 otomatize sistemi kullanılarak yapıldı. Antibiyotik duyarlılıkları ise European Committee on Antimicrobial Susceptibility Testing (EUCAST) önerileri doğrultusunda değerlendirildi.Bulgular: Toplam 923 alt solunum yolu örneğinden 461’inde anlamlı üreme saptandı. Aynı hastaya ait tekrarlayan örnekler ve az sayıda üreyen bakterilerin olduğu örnekler çıkarıldığında kalan 340 örneğe ait antibiyotik duyarlılık test sonuçları değerlendirildi. 340 örnekten 309’unda (%90,9) Gram negatif bakteriler izole edilirken 17’sinde (%5) Staphylococcus aureus, 14’ünde (%4,1) Candida albicans üredi. En sık izole edilen Gram negatif bakteriler sırasıyla Pseudomonas aeruginosa (121, %39,2), Acinetobacter baumannii (88, %28,5), Klebsiella pneumoniae (64, %20,7) ve Escherichia coli (36, %11,6) idi. A. baumannii izolatlarının karbapenem, piperasilin/tazobaktam, seftazidim ve florokinolon direnç oranları %90’ın üzerinde saptandı ve yoğun bakım ünitelerinde (YBÜ) servislere göre daha yüksek direnç izlendi.Sonuç: Özellikle YBÜ’lerinde yatan hatan hastalardan izole edilen alt solunum yolu enfeksiyonu etkenlerinde gözlenen artmış antibiyotik direnci dikkat çekicidir. Sonuç olarak solunum yolu enfeksiyonu etkenlerinin ve güncel direnç profillerinin bilinmesi özellikle YBÜ’leri gibi riskli birimlerde yatan hastalarda, hızla uygun ampirik tedaviye başlanması ve tedavi başarısının sağlanması açısından önemlidir. Çalışmamızda elde ettiğimiz verilerin ampirik tedavinin planlanmasında klinisyenlere yol gösterici olacağı düşünülmektedir., Objective: In this study, we aimed to guide clinicians in planning empirical treatment and contribute to regional data by presenting bacterial lower respiratory tract infection agents and antibiotic resistance profiles.Methods: All lower respiratory tract samples from patients admitted to our laboratory between January 01 and December 31, 2021 were evaluated retrospectively. Bacterial identification and susceptibility tests were performed using the VITEK 2 automated system and evaluated in accordance with the recommendations of the European Committee on Antimicrobial Susceptibility Testing (EUCAST).Results: Significant growth was detected in 461 of 923 lower respiratory tract samples. Antibiotic susceptibility test results of 340 samples were evaluated after excluding the repeated samples of same patient. Gram negative bacteria was isolated in 309 (90.9%), Staphylococcus aureus in 17 (5%) and Candida albicans in 14 (4.1%) of 340 samples. The most frequently isolated Gram-negative bacteria were Pseudomonas aeruginosa (121, 39.2%), Acinetobacter baumannii (88, 28.5%), Klebsiella pneumoniae (64, 20.7%) and Escherichia coli (36, 11.6%), respectively. Resistance rates of A. baumannii isolates to carbapenems, piperacillin/tazobactam, ceftazidime and fluoroquinolones were found to be over 90%, and higher resistance was observed in intensive care units (ICU) than in wards.Conclusion: The increased antibiotic resistance observed in lower respiratory tract infections from hospitalized patients in ICUs, is remarkable. As a result, knowing the causative agents and current resistance profiles is important, especially for hospitalized patients in ICUs, in order to initiate appropriate empirical treatment and to ensure treatment success. The data obtained from our study will guide clinicians in planning empirical treatment.

Objectives: Borderline ovarian tumors (BOTs) represent a heterogeneous group of ovarian epithelial neoplasms. Despite a favorable prognosis, 10–20% of BOTs exhibit progressively worsening clinic. Primary involvement of pelvic organs with echinococcus is very rare. Lymphoepithelioma-like gastric carcinoma is a rare neoplasm of the stomach. Case Report: A 58-year-old woman referred with abdominal swelling and gastric complaints. Imaging studies revealed a huge cystic mass with multiple septations and solid component, another cystic mass with an appearance of cyst hydatid in the pelvis, and thickening of the small curvature of stomach. Gastroscopy revealed an ulcer with a suspicious malignant appearance, and histology of the endoscopic specimen showed severe chronic inflammation and lymphocytic infiltration. No other involvement of hydatid cyst was detected. In the exploration, there was a 25 cm cystic lesion with solid components arising from right ovary, another 6 cm cyst over the former, 7 cm cystic lesion arising from left ovary, and 10 cm mass near the small curvature of the stomach. Excision of the masses; total gastrectomy with esophagojejunal anastomosis; total abdominal hysterectomy; bilateral salpingo-oophorectomy; omentectomy; appendectomy; splenectomy; and pelvic, paraaortic, and coeliac lympadenectomy were performed. Final pathology revealed lymphoepithelioma-like gastric carcinoma, bilateral serous BOT, and hydatid cyst. Discussion: Hydatid cyst should always be considered in the differential diagnosis of abdominopelvic masses in endemic regions of the world. Preoperative diagnosis of primary pelvic hydatid disease is difficult and awareness of its possibility is very important especially in patients residing in or coming from endemic areas.

- Abstract
Several major migrations and population turnover events during the later Stone Age (after c. 11,000 cal. BP) are believed to have shaped the contemporary population genetic diversity in Eurasia. While the genetic impacts of these migrations have been investigated on regional scales, a detailed understanding of their spatiotemporal dynamics both within and between major geographic regions across Northern Eurasia remains largely elusive. Here, we present the largest shotgun-sequenced genomic dataset from the Stone Age to date, representing 317 primarily Mesolithic and Neolithic individuals from across Eurasia, with associated radiocarbon dates, stable isotope data, and pollen records. Using recent advances, we imputed >1,600 ancient genomes to obtain accurate diploid genotypes, enabling previously unachievable fine-grained population structure inferences. We show that 1) Eurasian Mesolitic hunter-gatherers were more genetically diverse than previously known, and deeply divergent between the west and the east; 2) Hitherto genetically undescribed huntergatherers from the Middle Don region contributed significant ancestry to the later Yamnaya steppe pastoralists; 3) The genetic impact of the transition from Mesolithic hunter-gatherers to Neolithic farmers was highly distinct, east and west of a “Great Divide” boundary zone extending from the Black Sea to the Baltic, with large-scale shifts in genetic ancestry to the west. This include an almost complete replacement of hunter-gatherers in Denmark, but no substantial shifts during the same period further to the east; 4) Within-group relatedness changes substantially during the Neolithic transition in the west, where clusters of Neolithic farmer-associated individuals show overall reduced relatedness, while genetic relatedness remains high until ~4,000 BP in the east, consistent with a much longer persistence of smaller localised hunter-gatherer groups; 5) A fastpaced second major genetic transformation beginning around 5,0

- Abstract
The transitions from foraging to farming and later to pastoralism in Stone Age Eurasia (c. 11-3 thousand years before present, BP) represent some of the most dramatic lifestyle changes in human evolution. We sequenced 317 genomes of primarily Mesolithic and Neolithic individuals from across Eurasia combined with radiocarbon dates, stable isotope data, and pollen records. Genome imputation and co-analysis with previously published shotgun sequencing data resulted in >1600 complete ancient genome sequences offering fine-grained resolution into the Stone Age populations. We observe that: 1) Hunter-gatherer groups were more genetically diverse than previously known, and deeply divergent between western and eastern Eurasia. 2) We identify hitherto genetically undescribed hunter-gatherers from the Middle Don region that contributed ancestry to the later Yamnaya steppe pastoralists; 3) The genetic impact of the Neolithic transition was highly distinct, east and west of a boundary zone extending from the Black Sea to the Baltic. Large-scale shifts in genetic ancestry occurred to the west of this “Great Divide”, including an almost complete replacement of hunter-gatherers in Denmark, while no substantial ancestry shifts took place during the same period to the east. This difference is also reflected in genetic relatedness within the populations, decreasing substantially in the west but not in the east where it remained high until c. 4,000 BP; 4) The second major genetic transformation around 5,000 BP happened at a much faster pace with Steppe-related ancestry reaching most parts of Europe within 1,000-years. Local Neolithic farmers admixed with incoming pastoralists in eastern, western, and southern Europe whereas Scandinavia experienced another near-complete population replacement. Objective: Mastocytosis is a rare disorder characterized by abnormal accumulation of mast cells in various organs. Clinical complaints include pruritus, cutaneous flushing, dyspepsia, and episodes of anaphylaxis, and are usually the result of local and systemic mast cell mediator release. The triggers include a variety of factors including drugs, exercise, stress, anxiety, and temperature extremes. Case Report: A 26-year-old primigravida at 40 weeks' gestation with urticaria pigmentosa presented to our hospital. She was diagnosed with cutaneous mastocytosis based on pathologic examination of her skin biopsy. There were no complications during pregnancy, except for cutaneous manifestations with pruritus and premature uterine contractions at 27 weeks' gestation. After admission, antihistamine agents were administered during labor to treat the above symptoms, and antibiotic agents were given for prophylaxis of chorioamnionitis. Labor pain was successfully managed with warm showers, frequent position changes and massage, and therefore, epidural analgesia was not carried out. After 6 hours of labor, the patient gave birth to a healthy female infant via normal spontaneous vaginal delivery with right mediolateral episiotomy. Neither local anesthetic agents nor antibiotic agents caused any reaction. The postpartum period was uneventful. Conclusion: Pregnant women with mastocytosis should be treated symptomatically and should avoid factors that may exacerbate symptoms of disease. Clinicians should be aware of preterm labor during pregnancy. As a preventive measure, resuscitation equipment should be available during the labor, delivery and postpartum period to treat unanticipated hypotension and shock.

Objective: Malignant melanoma, which has a highly malignant potential, is a tumor of the skin and mucosal membranes. Malignant melanomas of the female genital tract, including the vulva and vagina, are rare. Their overall prognosis is poor and is worse than that for cutaneous melanomas. Case Reports: Clinical data for five patients with primary malignant melanoma are presented. Diagnosis was based on histologic and immunohistochemical examinations. Case 1 was a 58-year-old woman with mucosal lentiginous melanoma of the vulva. The other four patients, all of whom were over 40 years old, had vaginal melanomas. They were all treated with surgery, and three also received postoperative adjuvant therapy with interferon alpha-2b. Despite this, three of the patients died owing to widespread disease. Conclusion: Although malignant melanoma of the female genital tract is uncommon, elderly women should undergo regular gynecologic examinations and suspicious pigmented lesions should be biopsied. The use of immunohistochemical assays could markedly improve diagnosis. However, the prognosis for these tumors is poor, regardless of the treatment delivered, and they are associated with a high rate of recurrence and low long-term survival. Surgery is the best available treatment for controlling and potentially curing malignant melanomas.

Background and Aims: Acute pancreatitis is inflammation of the pancreas due to various causes. Insulin resistance is an important component of the metabolic syndrome and causes a chronic inflammatory condition, so it may affect disease severity in acute pancreatitis patients. In our study, we aimed to investigate the relationship of insulin resistance with acute pancreatitis severity and to compare it with other prognostic factors. Materials and Methods: In our study, 84 patients hospitalized in the clinic with a diagnosis of acute pancreatitis were evaluated. Our study was designed prospectively. To determine the severity of acute pancreatitis, the Atlanta Classification, Bedside Index of Severity in Acute Pancreatitis score, Imrie score, Ranson score, and Balthazar score were determined using the laboratory and imaging findings of the patients. The 'Homeostasis Model Assessment of Insulin Resistance' score was used to evaluate insulin resistance in patients. Accordingly, it was investigated whether there was a difference between the groups with and without insulin resistance in terms of disease severity. Results: 45.2% of the patients were in the group with insulin resistance. According to the Atlanta classification, 6 patients (7.1%) were evaluated as severe acute pancreatitis. 4 patients were evaluated as severe acute pancreatitis because of the development of prerenal acute kidney injury and 2 patients due to sepsis and need for intensive care. In the Receiver Operating Characteristic analysis performed in terms of disease severity, Balthazar and the 'Bedside Index of Severity in Acute Pancreatitis' score were found to be strong in determining the prognosis of the patients (Area Under the Curve; 0.955 and 0.979, respectively). When the 'Bedside Index of Severity in Acute Pancreatitis' and Balthazar scores of the group with insulin resistance were compared with the group without insulin resistance, a statistically significant difference was found (p = 0.019 and p = 0.013, respectively). Conclusion: The high 'Bedside Index of Severity in Acute Pancreatitis' and Balthazar scores in our study in acute pancreatitis patients with insulin resistance suggest that the presence of insulin resistance can be used as a prognostic factor for severe acute pancreatitis., Giriş ve Amaç: Akut pankreatit, pankreasın çeşitli nedenlere bağlı inflamasyonudur. Tüm dünyada önemli bir morbidite ve mortalite nedenidir. Metabolik sendromun önemli bir komponenti olan insülin direncinin inflamatuvar bir durum olması nedeniyle akut pankreatit hastalarında hastalık şiddetini etkileyebileceği düşünülmektedir. Çalışmamızda insülin direncinin akut pankreatit şiddeti ile ilişkisini araştırmayı ve diğer prognostik faktörlerle karşılaştırmayı amaçladık. Gereç ve Yöntem: Çalışmamızda klinikte akut pankreatit tanısı ile yatarak tedavi almış 84 hasta değerlendirmeye alınmıştır. Çalışmamız prospektif olarak dizayn edilmiştir. Akut pankreatit şiddetini belirlemek için hastaların laboratuvar ve görüntüleme bulguları kullanılarak Atlanta Sınıflandırması, 'Bedside Index of Severity in Acute Pancreatitis' skoru, Imrie skoru, Ranson skoru, Balthazar skoru belirlenmiştir. Hastalarda insülin direnci değerlendirilmesi için “İnsülin Direncinin Homeostatik Modeli Değerlendirmesi” skoru kullanılmıştır. Buna göre insülin direnci olan ve olmayan gruplar arasında hastalık şiddeti açısından fark olup olmadığı araştırılmıştır. Bulgular: Hastaların %45.2’si insülin direnci olan grupta yer aldı. Atlanta sınıflandırmasına göre 6 hasta (%7.1) şiddetli akut pankreatit olarak sınıflandırıldı. 4 hasta prerenal akut böbrek hasarı gelişmesi ve 2 hasta sepsis ve yoğun bakım gereksinimi olması nedeniyle şiddetli akut pankreatit olarak değerlendirildi. Hastalık şiddeti açısından yapılan Receiver Operating Characteristic analizinde Balthazar ve “Yatak başı akut pankreatit şiddet indeksi” skorunun hastaların prognozunu belirlemede güçlü oldukları görüldü (sırasıyla eğri altındaki alan; 0.955 ve 0.979). İnsülin direnci olan grubun “Yatak başı akut pankreatit şiddet indeksi” ve Balthazar skorları, insülin direnci olmayan grup ile karşılaştırıldığında istatistiksel olarak anlamlı farklılık saptandı (sırasıyla p = 0.019 ve p = 0.013). Sonuç: Çalışmamızda insülin direnci olan akut pankreatit hastalarında “Yatak başı akut pankreatit şiddet indeksi” ve Balthazar skorlarının yüksek saptanması, insülin direnci varlığının akut pankreatitin şiddetli seyretmesi yönünde prognostik bir faktör olarak kullanılabileceğini düşündürmektedir.

244. The effects of N-acetylcysteine (NAC) used in critical COVID-19 patients on clinical findings, inflammatory parameters and kidney functions

Objective: In this study, N-acetylcysteine (NAC) was used in critically ill patients who needed tertiary care due to COVID-19; The effects on clinical findings, inflammatory parameters and kidney functions were investigated.Materials and Methods: 190 adult patients with confirmed COVID-19 PCR positivity between April and September 2020 were included in this single-center retrospective study. The clinical and laboratory findings of the patients who received NAC (2400mg/day intravenously once a day) in addition to the standard treatment and the patients who were given the standard treatment alone were compared.Results: Age (p=0.423), gender (p=0.086), intubation (p=0.196), death (p=0.387), hospitalization (p=0.085), and intensive care unit stay (p=0.584) between groups There was no difference in terms of No significant difference was found between those who took NAC and those who did not, in the values of CRP (C-reactive protein), ferritin, fibrinogen, neutrophil, lymphocyte count, D-dimer, and creatinine measured on the first day, third day, seventh day, tenth day and fifteenth day (p> 0.05).Conclusion: NAC treatment did not affect the number of deaths, hospitalization and intensive care hospitalization days in patients admitted to the 3rd line intensive care unit due to COVID-19. At the same time, it did not show a positive or negative effect on inflammatory parameters and kidney functions., Amaç: Bu çalışmada N-asetilsistein (NAC)’in COVID-19 nedeniyle üçüncü basamak yoğun bakım ihtiyacı olan kritik hastalarda; klinik bulgulara, inflamatuar parametrelere, böbrek fonksiyonlarına olan etkileri araştırıldı.Materyal ve Metod: Bu tek merkezli retrospektif çalışmaya, Nisan- Eylül 2020 tarihleri arasında COVİD-19 PCR pozitifliği doğrulanmış 190 yetişkin hasta dahil edildi. Standart tedaviye ek olarak NAC (2400mg/gün dozda intravenöz olarak günde 1 defa) alan hastalar ile tek başına standart tedavi verilen hastaların klinik ve laboratuar bulguları karşılaştırıldı.Bulgular: Gruplar arasında yaş (p=0.423), cinsiyet (p=0.086), entübasyon (p=0,196), ölüm (p=0,387), hastane yatış (p=0,085) ve yoğun bakım yatış gün sayısı (p=0.584) açısından fark saptanmadı. Birinci gün, üçüncü gün, yedinci gün, onuncu gün ve onbeşinci gün bakılan CRP (C-reactive protein), ferritin, fibrinojen, nötrofil, lenfosit sayısı, D-dimer, kreatinin değerlerinde NAC alan ve almayanlar arasında anlamlı bir fark saptanmadı (p>0.05).Sonuç: COVID-19 nedeniyle 3. Basamak yoğun bakım yatışı olan hastalarda NAC tedavisi ölümü, hastane yatış ve yoğun bakım yatış gün sayısını etkilememiştir. Aynı zamanda inflamatuar parametrelere böbrek fonksiyonlarına olumlu veya olumsuz bir etki göstermemiştir.

246. Predictive Factors and Short-Term Fetal Outcomes of Breech Presentation: A Case-Control Study

Objective: This study evaluated the predictive factors and short-term fetal outcomes of breech presentation by comparing breech and cephalic pregnancies of ≥ 36 weeks' gestation. Materials and Methods: Two hundred and one breech and 149 cephalic pregnancies of ≥ 36 weeks' gestation, with no other maternal or fetal problems, were compared with regard to placental localization, fetal heart rate variability, smoking, body mass index, maternal weight gain, placental weight, birth weight, sex, Apgar scores, and umbilical cord length. Results: Maternal weight gain, body mass index at term, smoking and hemoglobin values were significantly higher in breech presentation than in cephalic pregnancies. The placenta was located in the cornu-fundal region in 63.2% of breech presentations and 26.8% of cephalic presentations (p < 0.001). Placental weights were 657 g and 597 g, respectively (p < 0.001). Umbilical cord length was shorter in breech than cephalic pregnancies (p < 0.001). Although breech pregnancies had significantly reduced fetal heart rate variability (p < 0.001), Apgar scores were much higher in breech fetuses than in cephalic fetuses. Ninety-five percent of breech pregnancies underwent cesarean sections. Conclusion: Cornu-fundal localization of the placenta, smoking, greater maternal weight gain, higher body mass index at term, greater placental weight, shorter umbilical cord, and lower estimated fetal weight may be predictive of persistent breech presentation. Reduced fetal heart rate variability did not have an adverse effect on Apgar scores after cesarean delivery in breech fetuses with no other problems at term.

In this study, the stress values obtained from commercial finite element analysis software ANSYS® and stress values measured by strain gauges applied on an axle housing in test environment were compared. The axle housing is a test sample from heavy duty commercial vehicles. Besides from stress values comparison, fatigue behavior of the housing was observed under gradually increased loading conditions via hydraulic loaded test benches. Load conditions, configuration change steps, test parameters and fatigue life results will be explained in detail. It was observed that the results of FEA and strain gauge are coherent to each other and as the load was increased, the fatigue life decreased. The stress values in specified points of housing increase linearly with the load increment. However, as predicted, the relationship between load change and fatigue life is not linear. For instance, fifty percent increase in load reduces life by about seventy-five percent. The aim of the study is firstly correlate the FEA results by comparing with the test measurements and then try to observe the effect of load increase on the fatigue life which will be a beneficial source for estimation of the life of the axle housing under diverse loadings in the further studies., Bu çalışmada, sonlu elemanlar analiz programı ANSYS®’ den elde edilen gerilme değerleri ile test ortamında diferansiyel kovanına bağlanan gerinim ölçerler ile ölçülen gerilme değerleri kıyaslanmıştır. Çalışmada kullanılan parça ağır ticari araçlarda kullanılan bir diferansiyel kovanıdır. Çalışmada gerilme değerleri karşılaştırmasının yanı sıra, hidrolik yüklemeli test tezgâhları ile kademeli artan yükleme koşullarında kovanın yorulma davranışı gözlemlenmiştir. Yük koşulları, test konfigürasyonları ve yorulma ömür sonuçları makalede detaylıca anlatılmıştır. Sonlu elemanlar analizi ve gerinim ölçer sonuçlarının birbiri ile uyumlu olduğu ve yük arttıkça yorulma ömrünün azaldığı gözlemlenmiştir. Gövdenin belirlenen noktalarında gerilim değerleri, yük artışı ile lineer olarak artmıştır. Ancak yük değişimi ile yorulma ömrü arasındaki ilişki doğrusal değildir. Yükteki yüzde elli artış, yorulma ömrünü yaklaşık yüzde yetmiş beş azaltmıştır. Çalışmanın amacı, öncelikle sonlu elemanlar çalışmalarının güvenilirliğinden testler ile karşılaştırma yaparak emin olmak, ilerleyen çalışmalar ile birlikte diferansiyel kovanının ömür tahmininde faydalı bir kaynak olacak şekilde ve daha fazla yük ile daha az çevrimde test gerçekleştirerek ürünün güvenilirliğinden emin olunabilecek alt yapıyı hazırlamaktır.

Amaç: Bu çalışmada amaç; diyafram kalınlığının weaning üzerine etkilerini ultrasonografi (USG) ile değerlendirmektir.Gereç ve Yöntem: Prospektif olarak yapılan bu çalışmada; hastaların demografik özellikleri, yoğun bakıma yatış ve mekanik ventilasyon endikasyonları, ek hastalıkları, diyafram kalınlığı, Sıralı Organ Yetmezliği Değerlendirmesi (SOFA) skoru ve Basitleştirilmiş Akut Fizyoloji Skoru (SAPS) II, mortalite oranı, periferik oksijen satürasyonu (SpO2), IMV süresi, dakika hacmi, solunum sayısı (RR), ekspiratuar hacimleri, RSBI değerleri kaydedildi.Hastalar invaziv mekanik ventilatör (IMV) basınç desteği (PS) modunda 30 dakika boyunca spontan olarak ventile edildi . Diyafram kalınlığı (DT), B moduna ayarlanmış 7-10 MHz lineer ultrason probu kullanılarak ölçüldü. Sağ hemidiyafram, 8. ve 10. interkostal boşluklar arasındaki midaksiller çizgide diyafram ve göğüs kafesinin birleştiği bölgede görüntülendi. Ölçümler sonrasında hastalar stabil hale geldikten sonra 1 saat boyunca T-tüpe alındı. T-tüpte spontan solunum denemeleri (SBT)'ni başarıyla tamamlayan hastalar ekstübe edildi. Hastalar başarılı ve başarısız weaning olmak üzere 2 gruba ayrıldı.Bulgular: Toplam 72 hasta çalışmaya dahil edilme kriterlerini sağlayarak değerlendirilmeye alındı. Hastaların 4 tanesi 24 saatten daha kısa süre IMV desteği aldığı için çalışma dışı bırakıldı. Diyafram kalınlık indeksi (DTI) ve diyafram kalınlaşma fraksiyonu (DTF) oranlarında gruplar arasında bir farklılık gözlenmedi. İnspirasyon sonunu diyafram kalınlığı (DTV) ve ekspirasyon sonunu diyafram kalınlığı (DFRV) olarak tanımlanan diyaframın kalınlık ölçümleri başarılı weaning grubunda daha yüksekti. Weaning başarısını tahmin etmek için DTV'nin en iyi eşik değeri 0.72, DFRV değeri 0.669 olarak bulundu.Sonuç: Weaning başarısı diyafram fonksiyonları, diyafram kalınlık parametreleri ve akciğer kapasiteleri ile yakından ilişkilidir. USG, diyaframı değerlendirmek için kullanılabilecek pratik, hasta başı uygulanabilen, noninvaziv bir yöntemdir. DTV ve DFRV weaning sürecinde başarıyı tahmin edebilecek parametreler arasındadır. DTF ve DTI weaning başarısını öngörmede yetersiz bulundu., Purpose: The aim of this study; to evaluate the effects of diaphragm thickness on weaning with ultrasonography (USG).Materials and Methods: In this prospective study; demographic characteristics of the patients, indications for hospitalization in the intensive care unit and mechanical ventilation, comorbidity, diaphragm thickness, Sequential Organ Failure Assessment (SOFA) score and Simplified Acute Physiology Score (SAPS) II, mortality rate, peripheral oxygen saturation (SpO2), invasive mechanical ventilator (IMV) duration, minute volume, respiratory rate (RR), expiratory volumes (VTE), RSBI values were recorded. The patients were ventilated spontaneously for 30 minutes in pressure support (PS) mode in IMV. Diaphragm thickness (DT) was measured using a 7-10 MHz linear ultrasound probe set to B mode. The right hemidiaphragm was visualized in the midaxillary line between the 8th and 10th intercostal spaces, at the junction of the diaphragm and rib cage. After the measurements were stabilized, the patients were taken to the T-tube for one hour. The patients who successfully completed T-tube spontaneous breathing trials (SBT) were extubated. The patients were divided into two groups as successful and unsuccessful weaning.Results: A total of 72 patients were evaluated by meeting the inclusion criteria. Four of the patients were excluded from the study because they received IMV support for less than 24 hours. There was no difference between the groups in the ratios of diaphragm thickness index (DTI) and diaphragmatic thickening fraction (DTF). The thickness measurements of the diaphragm, defined as diaphragm thickness at the end of inspiration (DTV) and diaphragm thickness at the end of expiration (DFRV), were higher in the successful weaning group. The best threshold value of DTV to predict weaning success was found to be 0.72 and DFRV value to be 0.669.Conclusion: Weaning success is closely related to diaphragm functions, diaphragm thickness parameters and lung capacities. USG is a practical, bedside, noninvasive method that can be used to evaluate the diaphragm. DTV and DFRV are among the parameters that can predict success in the weaning process. DTF and DTI were found to be insufficient in predicting weaning success.

Herein, we report a controllable pathway to accelerate the polymerization kinetics of dopamine using ultrasound as a trigger. The use of ultrasound was demonstrated to dramatically accelerate the slow liquid phase reaction kinetics and increase the deposition rate of the polydopamine coating on the surface of polymeric membranes.
