210. Validation of the Klinrisk chronic kidney disease progression model in the FIDELITY population

Abstract

BACKGROUND: Chronic kidney disease (CKD) affects >800 million individuals worldwide and is often underrecognized. Early detection, identification and treatment can delay disease progression. Klinrisk is a proprietary CKD progression risk prediction model based on common laboratory data to predict CKD progression. We aimed to externally validate the Klinrisk model for prediction of CKD progression in FIDELITY (a prespecified pooled analysis of two finerenone phase III trials in patients with CKD and type 2 diabetes). In addition, we sought to identify evidence of an interaction between treatment and risk.METHODS: The validation cohort included all participants in FIDELITY up to 4 years. The primary and secondary composite outcomes included a ≥40% decrease in estimated glomerular filtration rate (eGFR) or kidney failure, and a ≥57% decrease in eGFR or kidney failure. Prediction discrimination was calculated using area under the receiver operating characteristic curve (AUC). Calibration plots were calculated by decile comparing observed with predicted risk.RESULTS: At time horizons of 2 and 4 years, 993 and 1795 patients experienced a primary outcome event, respectively. The model predicted the primary outcome accurately with an AUC of 0.81 for 2 years and 0.86 for 4 years. Calibration was appropriate at both 2 and 4 years, with Brier scores of 0.067 and 0.115, respectively. No evidence of interaction between treatment and risk was identified for the primary composite outcome (P = .31).CONCLUSIONS: Our findings demonstrate the accuracy and utility of a laboratory-based prediction model for early identification of patients at the highest risk of CKD progression.

Objectives This study aimed to evaluate the efficacy and safety of finerenone, a selective, non-steroidal mineralocorticoid receptor antagonist, on cardiovascular and kidney outcomes by age and/or sex. Design FIDELITY post hoc analysis; median follow-up of 3 years. Setting FIDELITY: a prespecified analysis of the FIDELIO-DKD and FIGARO-DKD trials. Participants Adults with type 2 diabetes and chronic kidney disease receiving optimised renin-angiotensin system inhibitors (N=13 026). Interventions Randomised 1:1; finerenone or placebo. Primary and secondary outcome measures Cardiovascular (cardiovascular death, non-fatal myocardial infarction, non-fatal stroke or hospitalisation for heart failure (HHF)) and kidney (kidney failure, sustained ≥57% estimated glomerular filtration rate (eGFR) decline or renal death) composite outcomes. Results Mean age was 64.8 years; 45.2%, 40.1% and 14.7% were aged <65, 65-74 and ≥75 years, respectively; 69.8% were male. Cardiovascular benefits of finerenone versus placebo were consistent across age (HR 0.94 (95% CI 0.81 to 1.10) (<65 years), HR 0.84 (95% CI 0.73 to 0.98) (65-74 years), HR 0.80 (95% CI 0.65 to 0.99) (≥75 years); P interaction =0.42) and sex categories (HR 0.86 (95% CI 0.77 to 0.96) (male), HR 0.89 (95% CI 0.35 to 2.27) (premenopausal female), HR 0.87 (95% CI 0.73 to 1.05) (postmenopausal female); P interaction =0.99). Effects on HHF reduction were not modified by age (P interaction =0.70) but appeared more pronounced in males (P interaction =0.02). Kidney events were reduced with finerenone versus placebo in age groups <65 and 65-74 but not ≥75; no heterogeneity in treatment effect was observed (P interaction =0.51). In sex subgroups, finerenone consistently reduced kidney events (P interaction =0.85). Finerenone reduced albuminuria and eGFR decline regardless of age and sex. Hyperkalaemia increased with finerenone, but discontinuation rates were <3% across subgroups. 216. Factores de riesgo de hemorragia primaria posparto

Abstract

RESUMEN Introducción: La hemorragia primaria posparto es la principal causa de muerte materna en el Perú y se ha relacionado directamente con el parto domiciliario, por lo que podría reflejar la oportunidad y calidad de los servicios de salud cuando ocurre en un parto institucionalizado. Objetivo: Identificar los factores de riesgo de hemorragia primaria posparto producidos por desgarro del tracto genital. Métodos: Se realizó un estudio analítico de casos y controles en el Hospital "Augusto Hernández Mendoza" - EsSalud. La muestra estuvo conformada por 30 casos (con hemorragia primaria posparto por desgarro del tracto genital) y 120 controles (sin hemorragia primaria posparto). Se usaron las pruebas T de Student y U de Mann -Withney para comparar las variables numéricas y la prueba de Chi cuadrado para determinar la asociación entre las variables. La fuerza de asociación se determinó calculando Odds Ratio crudo y ajustado con sus intervalos de confianza (IC) al 95 % mediante regresión logística binaria con prueba de bondad de ajuste de Hosmer-Lemeshow. Resultados: El valor promedio de hemoglobina fue 8,85 ± 0,39 mg % en los casos y 11,90 ± 0,13 mg % en los controles (p = 0,000). En el análisis multivariado los predictores de riesgo fueron: Nulípara (ORa = 11,49; IC95 % = 3,11-42,42), soltera (Ora = 8,81; IC95 % = 1,83-42,41), < 8 controles pre-natales (Ora = 7,95; IC95 % = 2,60-24,31), y peso al nacer del recién nacido ≥ 3500 gramos (Ora = 7,34; IC95 % = 2,15-25,06). Conclusiones: El riesgo de hemorragia primaria posparto por desgarro del tracto genital fue mayor en solteras, nulíparas, con menos de ocho controles prenatales y recién nacidos con peso mayor de 3500 gramos.

En este trabajo se pronostica el patrón de dispersión de contaminantes para el quinquenio 1996-2000 en la estación de Casablanca, teniendo en cuenta la variabilidad natural, a partir del análisis de la tendencia de la matriz de frecuencia de rapidez vs rumbo del viento en dicha estación, mediante una variante de la teoría del análisis multivariado. Como modelo de pronóstico fue utilizado el modelo de Berlyand-Alvarez Modificado utilizado para el cálculo de las concentraciones de contaminantes emitidos por una fuente puntual. Los resultados obtenidos demuestran la validez de la metodología empleada y se concluye que el pronóstico de dispersión de contaminantes en la estación Casablanca, para el quinquenio 1996-2000, muestra poca variación con respecto al quinquenio anterior.

We consider the attractor $\Lambda $ of a piecewise contracting map f defined on a compact interval. If f is injective, we show that it is possible to estimate the topological entropy of f (according to Bowen's formula) and the Hausdorff dimension of $\Lambda $ via the complexity associated with the orbits of the system. Abstract

In this paper, we find some new exact solutions to the Einstein-Gauss-Bonnet equations. First, we prove a theorem which allows us to find a large family of solutions to the Einstein-Gauss-Bonnet gravity in $n$-dimensions. This family of solutions represents dynamic black holes and contains, as particular cases, not only the recently found Vaidya-Einstein-Gauss-Bonnet black hole, but also other physical solutions that we think are new, such as, the Gauss-Bonnet versions of the Bonnor-Vaidya(de Sitter/anti-de Sitter) solution, a global monopole and the Husain black holes. We also present a more general version of this theorem in which less restrictive conditions on the energy-momentum tensor are imposed. As an application of this theorem, we present the exact solution describing a black hole radiating a charged null fluid in a Born-Infeld nonlinear electrodynamics.

Abstract

Black hole perturbation theory, or more generally, perturbation theory on a Schwarzschild bockground, has been applied in several contexts, but usually under the simplifying assumption that the ADM momentum vanishes, namely, that the evolution is carried out and observed in the ``center of momentum frame''. In this paper we consider some consequences of the inclusion of a non vanishing ADM momentum in the initial data. We first provide a justification for the validity of the transformation of the initial data to the ``center of momentum frame'', and then analyze the effect of this transformation on the gravitational wave amplitude. The most significant result is the possibility of a type of gravitational memory effect that appears to have no simple relation with the well known Christodoulou effect., Comment: REVTexIV, 15 pages, 2 EPS figures

Abstract

We study the collision of two slowly rotating, initially non boosted, black holes in the close limit. A ``punctures'' modification of the Bowen - York method is used to construct conformally flat initial data appropriate to the problem. We keep only the lowest nontrivial orders capable of giving rise to radiation of both gravitational energy and angular momentum. We show that even with these simplifications an extension to higher orders of the linear Regge-Wheeler-Zerilli black hole perturbation theory, is required to deal with the evolution equations of the leading contributing multipoles. This extension is derived, together with appropriate extensions of the Regge-Wheeler and Zerilli equations. The data is numerically evolved using these equations, to obtain the asymptotic gravitational wave forms and amplitudes. Expressions for the radiated gravitational energy and angular momentum are derived and used together with the results of the numerical evolution to provide quantitative expressions for the relative contribution of different terms, and their significance is analyzed., Comment: revtex, 18 pages, 2 figures. Abstract

El presente artículo se centra en el análisis de las medidas cautelares y su efectividad dentro de los procesos de arbitraje internacional en materia comercial, tomando en cuenta aspectos fundamentales, como a quién corresponde el otorgamiento de éstas medidas cautelares, cuáles pueden ser dichas medidas, así como; su reconocimiento y ejecución. Para ello se utilizó en la elaboración del presente trabajo, el análisis documental y bibliográfico de una serie de fuentes, que permitieron conocer y entender el tema objeto de investigación. Para una mayor comprensión del lector, el trabajo se encuentra estructurado de cinco subtemas: el primero dirigido a una breve reseña histórica sobre el arbitraje comercial internacional, su desarrollo y crecimiento; el segundo dedicado a las generalidades de las medidas cautelares, pues como bien se sabe dichas medidas son necesarias en cualquier materia del derecho, para garantizar el proceso. Luego en el tercer subtema se abordan las medidas cautelares, pero ya en el ámbito específico del arbitraje comercial internacional. El cuarto está dedicado al otorgamiento de las medidas cautelares en el arbitraje y, por último, el reconocimiento y ejecución de medidas cautelares en el arbitraje internacional siempre en materia comercial.
