Abstract

Traffic optimization normally improves flow conditions at the expense of increased vehicles’ emissions. This paper proposes a bi-objective optimization approach to address this situation. In contrast to existing literature, this study considers environmental and congestion impacts of Network Design Problems (NDPs) using the Markov chain traffic assignment approach instead of user equilibrium. The NDP model selectively reverses roads’ directions to improve network performance. The model is optimized by simultaneously minimizing maximum traffic density and total vehicles’ emissions cost using non-dominated sorting genetic algorithm. A realistic city example was used to demonstrate the approach’s efficiency. Results showed that a compromise solution between the two objectives is more practical than single-objective optimization solutions.

Abstract

Gastrointestinal stromal tumors (GISTs) are rare mesenchymal tumors that almost always arise from the GI tract and account for 0.1-3% of GI tumors. During pregnancy, GIST is highly unusual given the predilection of this tumor to appear in the fifth to seventh decade of life. The finding of GISTs outside the GI tract is also rare as the cell of origin of these tumors is believed to be the interstitial cell of Cajal, found only in the GI tract. Extra intestinal GISTs have been reported with a few cases arising from the uterus or metastatic to the ovary; however; an asymptomatic uterine GIST occurring at pregnancy as an incidental finding during cesarean section has not been reported so far. We present a 32-year-old lady who underwent an emergency caesarean section and was found to have a GIST of the uterus. The tumor was excised in toto and she started imatinib therapy postoperatively. At the end of three years of close follow-up, she has done well with no evidence of recurrence.

225. Investigation of organic osmotic agents forward osmosis desalination process

This study investigated the effects of novel membrane-osmotic agent systems on forward osmosis process efficacy. Glucose, sucrose and NaCl were investigated as osmotic agents, and nano-filtration, reverse osmosis and hollow fibre flat sheet as membrane types. The effects of draw solution concentration, flow rate and temperature as well as feed water flow rate and temperature were investigated for the aforementioned membrane types. The efficacy of forward osmosis process was measured on the basis of water flux, water recovery rate, water permeability, specific energy consumption and solute flux where applicable. Single, binary and ternary systems were considered. Experimental results showed the NF membrane and the RO membrane performed better at low and high concentrations of osmotic agent. Higher water flux rates were achieved by using NF membrane for both types of osmotic agents with changing osmotic agent concentration rates. The best result was obtained through the combination of NF membrane and glucose at lower concentrations and with sucrose at higher concentrations. The NF membrane-sucrose system showed better results for all parameters when changing feed water flow rate at lower temperatures while the NF-glucose system showed better performance when increasing temperature of the solution. Specific energy consumption increased in all the combinations of membrane-osmotic agent with increasing osmotic agent concentration rates. However, the lowest energy requirements were noted for the combination of NF membrane - glucose as an osmotic agent. Overall, both NF and RO membranes showed better results at different osmotic agent flow rates, but glucose proved to be the superior osmotic agent. For the binary systems, higher FO process efficacy across almost all parameters was noted for the systems with deionised water (DW). However, systems also used more energy; because such systems also used more energy they are not necessarily superior to other systems. Water recovery rate and water flux were considerably higher in a ternary system involving sucrose + NaCl + DW compared to the ternary system using brackish water as feed solution in FO process. Overall, FO efficiency for a ternary mixture of glucose osmotic agent + NaCl salt + BW was higher in water recovery and water flux than the other ternary mixtures with DW as feed solution. Based on the study results, a number of recommendations for future work are provided. Manipulated Osmosis Desalination (MOD) is a promising desalination approach that should be further investigated by analysing the factors affecting the process’ efficacy. Such factors are likely to include various membrane parameters (thickness, porosity and different pore diameters) and draw solutions as well as the process parameters including temperature, flow rates and osmotic agent concentrations. Further investigations should be conducted for binary and especially ternary systems involving different types of membranes and solutes for refining and optimising the process of selection. The Close loop circulation system needs to be replaced by the no-circulation systems for better FO performance in future studies. Importantly, the experimental results should be compared with the developed mathematical models to further validate the results. Finally, future MOD studies should focus on identifying optimal regeneration approaches. The selection of osmotic agents based on their regeneration economics along with other important FO parameters could be undertaken in future research.

Abstract

To investigate a cluster of Middle East respiratory syndrome (MERS) cases in a women-only dormitory in Riyadh, Saudi Arabia, in October 2015, we collected epidemiologic information, nasopharyngeal/oropharyngeal swab samples, and blood samples from 828 residents during November 2015 and December 2015–January 2016. We found confirmed infection for 19 (8 by reverse transcription PCR and 11 by serologic testing). Infection attack rates varied (2.7%–32.3%) by dormitory building. No deaths occurred. Independent risk factors for infection were direct contact with a confirmed case-patient and sharing a room with a confirmed case-patient; a protective factor was having an air conditioner in the bedroom. For 9 women from whom a second serum sample was collected, antibodies remained detectable at titers >1:20 by pseudoparticle neutralization tests (n = 8) and 90% plaque-reduction neutralization tests (n = 2). In closed high-contact settings, MERS coronavirus was highly infectious and pathogenicity was relatively low.

The rising significance of the field of biopolymers has driven the rapid progress of this distinctive class of polymeric materials in the past decades. Biodegradable polymers have acquired much attention because they play an essential role in humans’ lives due to their specific tunable electrical conductivity and biodegradability characteristics, making them fascinating in many applications. Herein, we debated the recent progress in developing biodegradable polymers and their applications. Initially, we introduce the basics of conducting and biodegradable polymers, trailed by debates about the effective strategies currently used to develop biopolymers. Special importance will focus on the uses of biodegradable polymers in drug delivery and tissue engineering, as well as wound healing, demonstrating the recent findings, and uses of several biodegradable polymers in modern biological uses. Abstract

Although ultra-high-performance concrete (UHPC) presents superior mechanical properties and durability compared to conventional concrete; its spalling resistance to elevated temperatures is much lower compared to conventional concrete due to the high compactness and absence of capillary pores. This paper investigated the influence of lightweight aggregate (LWA) on the strength properties and microstructure of UHPC to enhance its resistance to elevated temperatures. UHPC specimens prepared with LWA as a partial replacement of silica sand were produced. The study evaluated the compressive and flexural strengths, failure mode, mass loss, and microstructure of the specimens, using SEM. The results showed that the compressive strength of the UHPC specimen was reduced with increasing the content of LWA at ambient temperature, but the compressive strength of the UHPC specimens prepared with LWA improved when exposed to elevated temperatures. The replacement of 10% of the silica sand with LWA led to an increase in the compressive strength from 100 MPa to 110 MPa after exposure to 200 °C; however, the flexural strength decreased from 23.6 MPa to 18.3 MPa. On the contrary, the flexural strength of UHPC increased with the inclusion of LWA at an ambient temperature but reduced with high-temperature exposure. The failure mode of UHPC was not significantly affected by the variation in LWA content and temperature. In addition, the SEM result confirms that LWA is an effective internal curing material for enhancing the microstructure and compressive strength of UHPC

Abstract

Aim:The study aims to analyze the frequency and extent of pneumoperitoneum after laparoscopic surgery and to differentiate routine post laparoscopy pneumoperitoneum from pneumoperitoneum due to possible bowel perforation.Method:Pneumoperitoneum after laparoscopic surgery persists for a variable number of days and can mask pathological pneumoperitoneum due to the complication of bowel perforation at laparoscopic surgery. This study aims to find a simple radiological solution to this issue, so that pathological bowel perforation can be detected at the earliest and corrective action taken.Results:We had four cases of bowel perforation after laparoscopic surgery, and, the clinical signs were subtle and not clearly indicating any peritonitis. Radiological tests on post operative day 1were inconclusive, as free gas was attributed to persisting carbon dioxide pneumoperitoneum. Hence a relook laparoscopy was delayed for 48 hrs ,when clinical signs were obvious. This scenario presented a challenge to distinguish persisting carbon dioxide pneumoperitoneum post laparoscopy from pathological pneumoperitoneum.Conclusion:We decided to measure by X-ray chest the width of gas under diaphragm in the usual laparoscopic procedures, on the first post operative day and compare this with the width of gas under diaphragm in the four cases of iatrogenic bowel perforation post laparoscopic surgery. It was found that iatrogenic bowel perforation at laparoscopy is characterized by a wider gas under diaphragm, with the width at widest point ranging from 1.5 cms to 2.0 cms. Hence a simple xray chest could raise suspicion of a bowel perforation complication , and dictate further investigations like computed tomography scan or relook laparoscopy at the earliest post operative period.

Abstract

This work presents a mathematical modelling of a proton-exchange membrane fuel cell (PEMFC) system integrated with a resistive variable load. The model was implemented using MATLAB Simulink software, and it was used to calculate the fuel cell electric current and voltage at various steady-state conditions. The electric current was determined by the intersection of its polarisation curve and applied as an input value for the simulation of the PEM fuel cell performance. The model was validated using a Horizon H-500xp model fuel cell stack system, with the following main components: a 500 W PEM fuel cell, a 12 V at 12 A battery for the start-up, a super-capacitor bank to supply peak loads and a 48 V DC-DC boost converter. The generated power was dissipated by a variable resistive load. The results from the model shows a qualitative agreement with test bench results, with similar trends for stack current and voltage in response to load and hydrogen flow rate variation. The discrepancies ranged from 2% to 6%, depending on the load resistance applied. A controlled current source was utilised to simulate the variation of fan power consumption with stack temperature, ranging from 36.5 W at 23 °C to 52 W at 65 °C. Both model and experiments showed an overall PEMFC system maximum efficiency of about 48%.

Abstract

The movement of water in concrete structures and buildings is a fundamental subject and therefore it is necessary to understand the mechanisms and processes involved. This paper presents a method to estimate the moisture diffusivity of concrete using gravimetric measurements, together with discretised electrical resistivity measurements. To improve the repeatability of the method, various concrete mixes with and without supplementary cementitious materials were tested. The samples were conditioned under two regimes representing poor and good curing; the samples were then exposed to a simulated hot environment with a diurnal temperature fluctuation of 20–40°C and 60% ambient relative humidity. The basis of the method was to measure the amount of water absorbed by the concrete sample at different depths as a function of time and then Boltzmann's transformation was used to analyse the experimental results obtained. Once a master curve using the Boltzmann transformation is formed, the moisture diffusivity can be obtained. The results showed that the relationship between moisture diffusivity and moisture content can be described by an exponential function.

Abstract

Background: Hernia surgeries are among the most frequently carried out surgical procedures. Minimal recurrence rates, great patient comfort, rapid return to work, and low cost should all be characteristics of the optimal hernia treatment method. Objective of this study was to investigate whether tacker and suture materials used for peritoneal closure after mesh placement in the laparoscopic TAPP inguinal hernia repair technique. Methods: Present study is a retrospective, observational & comparative study conducted in Department of General Surgery in a tertiary care hospital from January 2021 to December 2021. Institutional ethical committee clearance was obtained. Patient identity was kept confidential. According to the type of peritoneal closure, patients were separated into two groups: group S (which underwent closure with sutures) and group T (which received closure with tackers) 48 and 52 during the study period. SPSS was used for analysis. Results: The study was male preponderance comprising 94% of study participants, while the most common age group was found to be 61-70 years in both groups S and T (32%). most patients in the suture fixation group (39%) had left-sided hernias, but right-sided hernias (40%) were more typical in the tacker fixation group. Conclusions: Our research revealed no differences between the two peritoneal closure approaches in terms of VAS score at day 30, hernia recurrence rate, hospital stay, or complication rate. VAS score on day 1 and cost of operation were in favor of the suture group.

Abstract

Purpose: To investigate the anti-sickling activity of Senna alexandrina, Aerva javanica, and Ficus palmate extracts. Methods: The leaves were extracted with water and methanol. The anti-sickling effect of extracts was determined using blood from 20 sickle cell disease (SCD) patients by mixing it with 500 and 1000 μg/mL of extract. The treated blood samples were then examined under a light microscope. The osmotic fragility of erythrocytes was determined using 4 mL (0 – 0.9 %) buffered saline mixed with 1 mL extract (500 μg/mL) and 0.05 mL sickled red blood cells (RBCs). Results: Aerva javanica extract significantly (p < 0.0001) reduced the percentage of sickle cells compared to control. The sickle-shaped cells were 29.47 % when treated with 125 μg/mL concentration of methanol extract of A. javanica that was further decreased to 0.35 % on increasing the concentration to 500 μg/mL. A similar trend was also observed with extracts of F. palmata. Extracts had protective effect on erythrocyte osmotic fragility. Results of comet assay suggested that extracts did not damage DNA. Conclusion: These plant extracts have promising anti-sickling activity and protect erythrocyte osmotic fragility. However, in vivo studies are required to validate these results and for further development of the extracts for therapeutic use.

Abstract

The impact of a previous failure on procedural techniques and outcomes of chronic total occlusion (CTO) percutaneous coronary intervention (PCI) has received limited study. We examined the clinical and angiographic characteristics and procedural outcomes of 9,393 patients who underwent 9,560 CTO PCIs at 42 United States and non-United States centers between 2012 and 2022. A total of 1,904 CTO lesions (20%) had a previous failed PCI attempt. Patients who underwent reattempt CTO PCI were more likely to have a family history of coronary artery disease (37% vs 31%, p 30 CTO PCIs annually were more likely to achieve technical success in patients with previous failure. In conclusion, a previous failed CTO PCI attempt was associated with higher lesion complexity, longer procedure time, and lower technical success; however, the association with lower technical success did not remain significant in multivariable analysis.

Abstract

Background: During triage in the emergency room, observations of vital signs and identification of organ failure are increasingly frequently employed to diagnose systemic inflammatory response syndrome (SIRS) and sepsis. However, there is less emphasis on early detection technologies of systemic inflammatory response syndrome (SIRS) in the critical care unit. Method: This is A prospective research design study, A convenience sample technique utilized consists of (45) nurses working in the intensive care unit at King Khalid general hospital, KSA. Participants completed the electronic questionnaire sheet to assist knowledge of nursing staff about Systemic Inflammatory Response Syndrome by pre, immediate, and post educational program consisting of twenty questions. Results: The findings show a highly statistically significant increase in the mean score in a knowledge level of knowledge before and immediately after the educational program, then it slightly decreases one month after the educational program. There was no statistically significant relationship between the total knowledge of studied nurses and their Sociodemographic Characteristics. Conclusion: Criteria for systemic inflammatory response syndrome (SIRS) are very important to know and, unfortunately, they are not well used in ICU. It is recommended that educational programs be provided for intensive care nurses to help them with the detection of sepsis in the early stages.
