213. An Overview of the Statistical Techniques in Q Methodology: Is There a Better Way of Doing Q Analysis?

Abstract

This article critically reviews the statistical techniques used in Q methodology, such as methods of factor extraction and factor rotation. Different techniques of factor extraction including principal component analysis, principal axis factoring, centroid factor analysis and maximum likelihood method will be reviewed. Specifically, centroid factor analysis will be compared against other methods, and some common beliefs regarding this technique, including the concept of factor indeterminacy, will be reexamined. In addition, different techniques of factor rotation will be reviewed. In particular, the pros and cons of judgmental (manual) factor rotation will be discussed. Furthermore, limitations of current statistical packages available for Q methodology, PQMethod and PCQ, are highlighted. 215. Risk factors for and prediction of mortality in critically ill medical-surgical patients receiving heparin thromboprophylaxis.

Abstract

Background: Previous studies have suggested that prediction models for mortality should be adjusted for additional risk factors beyond the Acute Physiology and Chronic Health Evaluation (APACHE) score. Our objective was to identify risk factors independent of APACHE II score and construct a prediction model to improve the predictive accuracy for hospital and intensive care unit (ICU) mortality. Methods: We used data from a multicenter randomized controlled trial (PROTECT, Prophylaxis for Thromboembolism in Critical Care Trial) to build a new prediction model for hospital and ICU mortality. Our primary outcome was all-cause 60-day hospital mortality, and the secondary outcome was all-cause 60-day ICU mortality. Results: We included 3746 critically ill non-trauma medical-surgical patients receiving heparin thromboprophylaxis (43.3 % females) in this study. The new model predicting 60-day hospital mortality incorporated APACHE II score (main effect: hazard ratio (HR) = 0.97 for per-point increase), body mass index (BMI) (main effect: HR = 0.92 for per-point increase), medical admission versus surgical (HR = 1.67), use of inotropes or vasopressors (HR = 1.34), acetylsalicylic acid or clopidogrel (HR = 1.27) and the interaction term between APACHE II score and BMI (HR = 1.002 for per-point increase). This model had a good fit to the data and was well calibrated and internally validated. However, the discriminative ability of the prediction model was unsatisfactory ( C index < 0.65). Sensitivity analyses supported the robustness of these findings. Similar results were observed in the new prediction model for 60-day ICU mortality which included APACHE II score, BMI, medical admission and invasive mechanical ventilation. Conclusion: Compared with the APACHE II score alone, the new prediction model increases data collection, is more complex but does not substantially improve discriminative ability. Abstract

Background and Purpose: To assess the psychometrics of the French language Quality of Prenatal Care Questionnaire (QPCQ). Methods: Data from 302 women were used in a confirmatory factor analysis and in assessment of construct validity through hypothesis testing and internal consistency reliability using Cronbach's alpha. Results: The 6 factors (subscales) were verified and confirmed. Hypothesis testing further supported construct validity. The overall QPCQ had acceptable internal consistency reliability (Cronbach's alpha = .97) as did 5 subscales (Cronbach's alpha = .70—.92); the Sufficient Time subscale had poorer reliability (Cronbach's alpha = .61). Conclusions: The French language QPCQ is a valid and reliable self-report measure of prenatal care quality. Abstract

Aim This study aimed to assess the probability of developing a lipid test abnormality over time, among first-time users of antipsychotic medications with affective psychosis. Methods Survival analysis was used to analyse data from an early intervention in psychosis programme for the first 53 consecutive and eligible cases of patients between the ages of 14 and 40 years who had a diagnosis of affective psychosis. Data on initiation of antipsychotic medications and lipid laboratory test results were abstracted from chart reviews. Results Within the first 18 months of receiving antipsychotic medications, the probability of surviving without an abnormal lipid test was only 25% (confidence interval 95%: 13.1%, 40.4%). The median time to the development of an abnormal test was 8 months for males and 12 months for females ( P < 0.001). Conclusions Additional studies are needed to document the incidence over time of abnormal lipid tests to inform clinicians about the optimal frequency of monitoring. [ABSTRACT FROM AUTHOR]

250. The effect of surgeon volume on procedure selection in non–small cell lung cancer surgeries.

Abstract

Objectives Procedure selection by the surgeon can greatly affect patients' operative and long-term survival. This selection potentially reflects comfort with technically challenging surgeries. This study aims to examine surgeon choices for non–small cell lung cancer and whether surgeon volume predicts the type of procedure chosen, controlling for patient demographics, comorbidity, year of surgery, and institutional factors. Methods Data were abstracted from an Ontario population-based linked database from 2004 to 2011. Patient demographics, comorbidities, year of surgery, and institutional and surgical factors were evaluated. Three-level, random-effect, multilevel regression analyses were performed to examine factors influencing operative selection. Results Over the interval, 8070 patients (50.4% were male) underwent surgical resection, including pneumonectomy (n = 842), lobectomy (n = 6212), and wedge resection (n = 1002). Resections were performed by 124 unique physicians in 45 institutions. The proportion of patients undergoing pneumonectomy decreased from 14.8% in 2004 to 7.6% in 2011. Multilevel regression analysis showed physician volume, age, year of procedure, gender, and comorbidities were predictive of performing a pneumonectomy. By adjusting for these variables, the results indicated that for each 10-unit increase in physician volume, the relative risk of performing a pneumonectomy decreased by 9.1% (95% confidence interval, 8.2-10.0, P = .04). Conclusions Although patient and temporal factors influence the type of resection a patient receives for non–small cell lung cancer, surgeon volume also is a strong predictor. This study may be limited by minimal stage data, but the suggestion that a surgeon's total procedural volume for non–small cell lung cancer significantly influences procedure selection has implications on how we deliver care to this patient population.
