Böbrekapsesi çocukluk çağında nadir görülür ve özgün belirtileri olmadığından tanıdagüçlükler olabilir. Hastaların tanısında klinik, laboratuar ve görüntülemeyöntemleri yardımcıdır. Tanıyı takiben hızla antibiyotik tedavisibaşlanmalıdır. Bazı olgularda perkütan ve/veya cerrahi drenaj, ciddi olgulardaise nefrektomi gerekebilir. Bu çalışmada böbrek apsesi olan 5 hasta tartışılmışve literatür gözden geçirilmiştir. Çocuklardaüriner sistem taş hastalıkları son yıllarda çok daha fazla tanı almayabaşlamıştır. Erken tanı, ortaya çıkabilecek ciddi morbiditeleri önlemekaçısından önem taşır. Çocuklarda taş oluşumu altta yatan anatomik veyametabolik bir bozukluktan kaynaklanabilir. Klinik bulgular, çocuğun yaşı, taşınlokalizasyonu ve büyüklüğü ve altta yatan hastalığa bağlı olarak değişmektedir.Tedavide altta yatan nedenin ortadan kaldırılması önemlidir. Tedavi sonrasında yenitaş oluşumunun önlenmesi için yeterli sıvı alımının sağlanması, diyet ve yaşamtarzının düzenlenmesi gereklidir.

Urinarytract infections are very common in children. The most common pathogensisolated in urinary tract infections are Escherichia coli. Theincidence of Urinary tract infections in children 4-8 %in girls and 1-2 % in boys. Urinary tract infections in children is of concern because it can be associatedwith urosepsis, renal scarring,hypertension, and chronic renal insufficiency. In this reviewwe aim to describe the diagnosis and management of acute and recurrent urinarytract infections in the pediatric population., İdraryolu enfeksiyonları çocuklarda oldukça sık görülmektedir. En sık izole edilenajan Escherichia Coli’dir. İnsisdansı kız çocuklarda % 4-8 iken erkeklerde %1-2’dir.Çocuklarda ürosepsis, renal hasar, hipertansiyon, kronik renal yetmezlik ileilişkili olması sebebiyle endişe vericidir. Bu derlemede çocukluk çağı akut vekronik idrar yolu enfeksiyonunun tedavisini ve izlemi anlatılmasıamaçlanmıştır.

WOS: 000379324900013, PubMed: 26646776, The aim of this study was to compare the visual outcomes, intraocular pressure (IOP), and endothelial cell loss caused by trabeculectomy (TRAB) and Ahmed glaucoma valve (AGV) implantation in patients who had previously undergone penetrating keratoplasty (PKP). The data from all patients who underwent surgical treatment of glaucoma after PKP were reviewed at the Cornea Department of Haydarpasa Numune Education and Research Hospital. Eighteen patients who had undergone surgical treatment of glaucoma after PKP were included in this retrospective study. Time between PKP and glaucoma surgeries, visual acuity results, IOP results, endothelial cell counts (ECC) before the surgery, at 1st, 6th, and 12th month of surgery were recorded. Differences between two groups were evaluated. Mean loss of ECC was 315 cells/mm(2) in the AGV group and 197 cells/mm(2) in TRAB group at 12th month of glaucoma surgery. The difference between endothelial cell loss at 12th month of surgery was statistically significant and higher in AGV group (p < 0.001). The decrease in IOP was 64.2 % in AGV group and 46.9 % in TRAB group at 12th month of surgery. Both differences were statistically significant between 2 groups (p = 0.001, 0.001). TRAB successfully decreased both the IOP and endothelial cell loss in patients with post-PKP glaucoma. Ahmed glaucoma valve had a significantly better IOP lowering but higher endothelial cell loss effect.

Purpose: To investigate the influence of capsular tension ring (CTR) on prevention of intraoperative complications due to zonular weakness and increased capsular fragility during phacoemulsification of cataracts with pseudoexfo­liation. Materials and methods: 148 eyes of 148 patients who had cataract with pseudoexfoliation underwent pha­coemulsification surgery included. 74 eyes of the first group (study group) were implanted with CTR after hy­drodissection, while CTR was not performed in 74 eyes of control group (Group 2). In both groups, intraoperative complications such as zonular dialysis, conversion to ex­tracapsular cataract extraction (ECCE) or intracapsular cataract extraction (ICCE), posterior capsular rupture, vitreus loss, and early postoperative complications were recorded and compared between two groups. Results: In group 1, no complication occurred in 64 eyes (86.5%) and intraocular lens (IOL) implanted in the bag. In 10 eyes (13.5%) posterior capsule rupture occurred, in 4 of them converted to ECCE, IOL implanted in four eyes. In other 6 eyes, phacoemulsification completed un­derlay viscoelastic. In group 2, no complication occured in 42 eyes and IOL implanted in the bag. In 10 patients (13.5%) zonular dialysis occurred, in 4 of them IOL im­planted in the bag and in 6 of them converted to ICCE and IOL implanted in anterior chamber. Posterior capsule rup­ture occurred in 20 eyes (27.0%), all of them converted to ECCE and IOL was implanted. In 2 eyes capsullorrhexis occurred and converted to ECCE, IOL was implanted in sulcus. Conclusion: In patients with pseudoexfoliation, CTR im­plantation in phacoemulsification simplified the surgery, increased the rate of IOL implanted in the bag and pre­vented complications like IOL desantralization after sur­gery., Amaç: Psödoeksfolyasyonlu katarakt olgularında, fako­emülsifikasyon sırasında kapsül germe halkası (KGH) implantasyonunun, zonül zayıflığı ve artmış kapsül fraji­litesine bağlı gelişebilecek komplikasyonları önlemedeki başarısını değerlendirmek. Gereç ve Yöntem: Fakoemülsifikasyon cerrahisi yapılan psödoeksfolyasyonlu 148 hastanın 148 gözü çalışmaya alındı. Grup 1\'deki (çalışma grubu) 74 göze hidrodissek­siyonu takiben KGH uygulandı, kontrol grubundaki (Grup 2) 74 göze ise KGH uygulanmadı. Tüm olgularda ame­liyat sırasında gelişen zonül diyalizi, arka kapsül rüptü­rü, ekstrakapsüler katarakt ekstraksiyonu (EKKE) ya da intrakapsüler katarakt ekstraksiyonu (İKKE)\'ye dönme, vitreus kaybı gibi intraoperatif komplikasyonlarla, erken postoperatif komplikasyonlar kaydedilerek, sonuçların gruplar arası karşılaştırılması yapıldı. Bulgular: Grup 1\'de 74 gözün 64\'ünde (%86.5) kompli­kasyon gelişmedi ve kapsül içi göz içi lens (GİL) kondu, 10 gözde (%13.5) arka kapsül perfore oldu, bunların 4\'ünde EKKE\'ye geçilerek 2\'sine sulkus lensi, 2\'sine ön kamara lensi kondu. Diğer 6 gözde (%8.1) ise; fakoemülsifikasyon tamamlanarak 4 GİL sulkusa, 2 GİL ön kameraya kondu. Grup 2\'de ise 74 gözün 42\'sinde (%56.8) komplikasyon gelişmedi ve hepsine kapsül içi GİL kondu, 10 hastada (%13.5) zonül diyalizi oldu, bunların 4\'ünde hafif diyaliz vardı ve fakoemülsifikasyon tamamlanarak kapsül içi GİL kondu, 6 gözde ileri derecede zonül diyalizi görüldü ve hepsinde İKKE\'ye geçilerek, ön kamara GİL kondu. Yirmi gözde (%27) arka kapsül perforasyonu gelişti, hepsinde EKKE\'ye geçildi, EKKE\'ye geçilen 14 gözde (%18.9) sul­kusa, 6 gözde ön kameraya GİL implantasyonu yapıldı, 2 gözde kapsüloreksis perifere kaçtığı için EKKE\'ye geçildi ve GİL kondu. Sonuç: Psödoeksfolyasyonlu hastalarda katarakt cerra­hisinde KGH kullanımının; zonül hasarını engelleyip fako­emülsifikasyonu kolaylaştırdığı, kapsül içi GİL implantas­yonu oranını arttırdığı, cerrahi sonrası GİL desantralizas­yonu oluşumunu önemli ölçüde engellediği görüldü.

Bu tezde, Basınçlı Su Reaktörü tipi bir reaktör için reaktör sonrası yakıt çevrimi seçenekleri yüksek aktiviteli atık idaresi, özellikle de jeolojik bertaraf üzerindeki etkileri yönünden irdelenmiştir. Açık yakıt çevrimi ile alternatif kapalı yakıt çevrimleri, oluşan atıkların jeolojik bertaraf yoğunlukları (kullanılmış yakıt ve yüksek seviyeli atığın tasfiyesi için gereken alan) ve radyolojik toksisiteleri açısından karşılaştırılmıştır. Çalışmanın ilk bölümünde, seçilmiş yanma miktarları için, yakıt çevrimlerinde oluşan atıkların kompozisyonları ve bozunma ısıları MONTEBURNS kodu kullanılarak elde edilmiştir. Daha sonra, ANSYS kodu kullanılarak bir referans jeolojik bertaraf tesisi tasarımı için ısıl analizler yapılmış ve her bir atık tipi için gerekli bertaraf alanları belirlenmiştir. Referans bertaraf tasarımındaki bileşenlerin ısıl özelliklerindeki değişimlerin atık bertaraf yoğunluğu üzerindeki etkisini değerlendirmek amacıyla ısıl analizler farklı ısıl özellikler kullanılarak tekrarlanmıştır. Sonuçlar, yakıt çevrimlerinin karşılaştırılması için belirleyici bir parametre olan `birim atık bertaraf alanı (m2) başına üretilen toplam elektrik enerjisi (MWe-yr)` cinsinden ifade edilmiştir. Yakıt çevrimi seçeneklerinin atık yönetimi ve jeolojik bertaraf üzerindeki etkisini değerlendirmede alternatif bir parametre olarak oluşan radyoaktif atıkların radyolojik toksisiteleri de karşılaştırılmıştır. Atık bertarafı yoğunluğu ile ilgili analizlerin sonuçları 40000 MWd/t yanma oranına kadar açık yakıt çevriminin avantajlı olduğunu göstermiştir. Daha yüksek yanma oranlarında ise standart yeniden işleme uygulanan yakıt çevrimi diğer yakıt çevrimlerine göre daha avantajlı olmaktadır. Radyotoksisite analizlerinin sonuçlarına göre MOX yakıtının yeniden işlendiği kapalı yakıt çevrimi diğer kapalı yakıt çevrimleri ve açık çevrime göre daha avantajlıdır. In this thesis, back end fuel cycle options for Pressurized Water Reactor are discussed in terms of effects on high level waste management and, more specifically, on geological disposal. Once-through and alternative closed nuclear fuel cycles for a typical Pressurized Water Reactor are compared with respect to waste disposal densities (waste disposal area required for spent fuel and high level waste) in a permanent geological repository and radiological toxicities of resultant wastes. In the first part of the study, utilizing the code MONTEBURNS, relevant compositions and decay heats of wastes generated in the considered fuel cycles are obtained for several selected burnup values. Then, using the code ANSYS, thermal analyses are performed for a reference repository concept and disposal areas needed for waste types under consideration are determined. A sensitivity analysis is also performed for evaluating the effect of variations in thermal properties of reference repository components on waste disposal densities. Results are expressed in terms of `total electrical energy (MWe-yr) produced per unit waste disposal area (m2)`, which is taken as the decisive parameter to compare the cycles. As an alternative parameter to assess the effect of back end fuel cycle options on waste management and disposal, radiotoxicities of wastes generated in fuel cycles are also compared. The results of the disposal density analysis indicates that: the once-through cycle displays an advantage up to nearly a burnup of 40000 MWd/t with regard to waste (spent fuel and high-level waste) disposal density; however, at higher burnups, the closed cycle with standard reprocessing is better than once-through and other closed fuel cycles. According to results of radiotoxicity analysis, closed cycle with MOX recycling is more advantageous than once-through and other closed cycles. 139

Speech boundary detection contributes to performance of speech based applications such as speech recognition and speaker recognition. Speech boundary detector implemented in this study works on broadcast audio as a pre-processor module of a keyword spotter. Speech boundary detection is handled in 3 steps. At first step, audio data is segmented into homogeneous regions in an unsupervised manner. After an ACTIVITY/NON-ACTIVITY decision is made for each region, ACTIVITY regions are classified as Speech/Non-speech via Gaussian Mixture Model (GMM) based classification. GMM's are trained using a novel feature, Spectral Flow Direction (SFD), and an improved multi-band harmonicity feature in addition to widely used Mel Frequency Cepstral Coefficients (MFCC's).

TENMAK D.N.. 4780 Bu raporda, çevre radyoaktivitesinin izlenmesi programı kapsamında, Türkiye’nin 7 coğrafi bölgesinden 2007 yılında temin edilen çevresel örneklerde ve gıda örneklerindeki doğal ve yapay radyonüklitlerin aktivite derişimleri, toplam alfa/ beta aktivite sonuçları ve havadaki radyasyon doz hızı sonuçları sunulmuştur. Örneklerdeki doğal (238U, 232Th, 226Ra, 40K ve 7Be) ve yapay (l37Cs, l34Cs, 90Sr, 238-239+24opU( radyonüklitlerin aktivite derişimleri ve toplam alfa/beta aktivite derişimleri, gama spektrometresi, alfa spektronıetresi, sıvı sintilasyon spektrometresi ve toplam alfa/beta sayım sistemi kullanılarak ölçülmüştür. Sonuçlar, bazı örneklerde çok düşük düzeyde de olsa 1986 yılında meydana gelen Çernobil Nükleer Santrali kazasından kaynaklanan l37Cs ve90Sr radyonüklitlerinin var olduğunu göstermiştir. İncelenen yüzey toprağı örneklerindeki 238U, 232Th, 226Ra ve 40K doğal radyonüklitlerinin aktivite derişimlerinin ortalama değerleri sırasıyla, 32,1 Bq kg(-1), 35,0 Bq kg(-1), 29,0 Bq kg(-1) ve 446,7 Bq kg(-1) olarak bulunmuş iken, fisyon ürünü olan l37Cs radyonüklitinin aktivite derişiminin ortalama değeri ise 18,4 Bq kg(-1) olarak bulunmuştur. Analiz edilen gıda örneklerindeki 238U, 232Th ve 226Ra aktivite derişimleri, ölçülebilir en düşük aktivite (ÖEA) değerinden daha düşük iken, 7Be ve l34Cs radyonüklitleri gözlenmemiştir. Süt örneklerinde ölçülen l37Cs ve 90Sr radyonüklitlerinin ortalama aktivite derişimleri ise 0,24 Bq L(-1) ve 0,05 Bq L(-1)’dir. In this report, the activity concentrations of natural and artificial radionuclides, gross alpha/beta activities and air gamma dose rates in the environmental and food samples provided from Turkey’s seven geographical regions within the environmental radioactivity monitoring program in 2007 are presented, The activity concentrations of the natural (238U, 232Th, 226Ra, 40K and 7Be) and artificial (l37Cs, l34Cs, 90Sr, 238~239+24opu> fadjormdjcies and gross alpha/beta activities in the samples were measured by using the gamma spectrometry, the alpha spectrometry, the liquid scintillation counter and the gross alpha /beta counting system. Results show thatl37Cs and 90Sr radionuclides originating from the Chernobyl Nuclear Reactor accident in 1986 exist in some of samples even in low levels. The mean activity concentrations of 238U, 232Th, 226Ra and 40K in the studied surface soil samples were found as 32.1 Bq leg(-1), 35.0 Bq kg(-1), 29.0 Bq kg(-1) and 446.7 Bq kg(-1), respectively, while the mean activity concentrations of the fission product l37Cs was found as 18.4 Bq kg(-1). While the activity concentrations of 238U, 232Th and 226Ra in the analyzed food samples are lower than the minimum detectable activity (MDA), l34Cs and 7Be radionuclides are not observed. The mean activity concentrations of l37Cs and 90Sr radionuclides are 0.24 Bq L(-1) and 0.05 Bq L(-1), respectively.

Yapı malzemelerindeki radyonüklitlerin aktivite derişimleri normal değerin üzerinde olduğunda, radyo nü kİ iti erden yayınlanan gama ışınlarının sebep olduğu dış ışınlama ve bu tür malzemelerden serbest kalması sonucunda solunum yoluyla vücuda girebilen radon (222Rn), toron (220Rn) ve bunların yarılanma süreleri kısa olan bozunum ürünleri tarafından yayınlanan alfa ve beta ışınlarının sebep olduğu iç ışınlama önemli ölçüde artabilir. Yapı malzemelerindeki radyoaktivite seviyelerinin bilinmesi, yapı içi etkin dozun hesaplanması için çok önemli olmasının yanı sıra bu malzemelerin kullanılması ve ilgili standartların geliştirilmesine ilişkin yol gösterici bilgilerin oluşturulmasına da katkı sağlamaktadır. Bu raporda, Türkiye’nin farklı jeolojik bölgelerinden temin edilen ve 33 farklı yapı malzemesinden oluşan toplam 1033 numunede gama spektrometrik yöntem ile ölçülen 226Ra, 232Th ve 40K aktivite derişimlerinin sonuçları sunulmuştur. Söz konusu numunelerde ölçülen 226Ra, 232Th ve 40K aktivite derişimleri, sırasıyla yapısal malzemeler için, 4,3 ± 0,5 ile 258,0 ± 13,0 Bq kg1, 1,3 ±0,1 ile 124,9 ± 2,5 Bq kg1 ve 24,4 ± 2,5 ile 1092,0 ± 26,7 Bq kg'1 aralığında; kaplama malzemeleri için, 0,5 ± 0,1 ile 332,0 ± 18,1 Bq kg1, 0,6 ± 0,1 ile 226,0 ± I 1,0 Bq kg'1 ve 1,5 ± 0,6 ile 1060,0 ± 100,0 Bq kg1 aralığında; katkı ham maddeleri için, 1,6 ± 0,3 ile 2972,0 ± 148,6 Bq kg1, 1,1 ±0,2 ile 393,5 ± 4,2 Bq kg1 ve 4,0 ± 0,4 ile 3548,0 ± 75,0 Bq kg'1 aralığında değişmektedir. Bu raporda, yapı malzemelerinin kullanılmasından kaynaklanabilen radyolojik riskleri değerlendirmek amacıyla radyum eşdeğer (Ra(eq)) aktivite derişimi, aktivite derişim indisi (1γ), alfa indisi (Iα), dış ışınlamadan kaynaklanan soğurulmuş gama doz hızı (D(R)) ve yapı içi etkin doz hızı (H(R)) hesaplanmış ve ilgili ölçütler veya sınır değerler ile karşılaştırılmıştır. Yapısal malzemeler ve kaplama malzemeleri için hesaplanan Rae[] ortalama değerleri, sırasıyla 39,0 ± 4,4 ile 240,4 ± 15,7 Bq kg(-1) ve 2,4 ± 0,3 ile 276,0 ± 20,0 Bq kg(-1) aralığında değişmektedir. Yapısal malzemeler ve kaplama malzemelerine ilişkin ortalama Ra(eq) değerleri, ev ve işyeri binalarında kullanılan yapı malzemeleri için aşılmaması tavsiye edilen sınır değer olan 370 Bq kg(-1)’den daha küçüktür. Yapısal ve kaplama malzemeleri için hesaplanan aktivite derişim indisinin ortalama değerleri, yapısal ve kaplama malzemeleri için tavsiye edilen ölçüt değerlerden önemli ölçüde daha küçüktür. Yapısal malzemelerdeki radyonüklitlerin sebep olduğu dış ışınlamadan kaynaklanan yapı içi etkin doz hızının ortalama değerleri, briket için değerlendirilen ortalama değer (1,06 ± 0,13) hariç, aşılmaması tavsiye edilen sınır değer olan I mSv y(-1)’den, 1,5 ila 5 kat daha düşüktür. Yapılan çalışmalar sonucunda, yapısal ve kaplama malzemelerinin yapı sektöründe kullanılmalarında, herhangi bir radyolojik tehlikenin olmadığı görülmüştür. Katkı ham maddelerinin kullanımı sınırlıdır ve sadece belli bir kısmı, çimento, beton, tuğla gibi yapısal malzemelerde kullanılmaktadır. Özellikle endüstriyel yan ürün olarak elde edilen uçucu kül ve fosfojips gibi katkı maddeleri için hesaplanan indis değerleri, ilgili ölçüt veya sınır değerlere göre daha yüksek olduğundan, bu tür katkı ham maddelerinin kullanıldığı yapısal malzemelerin radyasyon kontrolüne tabi tutulmasının uygun olacağı değerlendirilmektedir. All building materials of terrestrial origin contain natural radionuclides in the series of uranium, actinium and thorium, and the radioactive isotope of potassium (40K). When the radioactivity concentrations of radionuclides in the building materials are much higher than the mean radioactivity level of earth’s crust, the external exposure caused by gamma rays emitted from these radionuclides and the internal exposure caused by the inhalation of radioactive inert gases radon (222Rn) and toron (220Rn), and alpha and beta rays emitted from these gases and its short-lived secondary decay products can be considerably increased. Thus, knowledge of the natural radioactivity level in building materials is important to evaluate the indoor effective dose and to develop the standards and guidelines for use and the management of these materials. In this report, the results of the radioactivity concentrations of 226Ra, 232Th and 40K in the total of 1033 samples representing 33 different building materials collected from various geological regions of Turkey measured by gamma spectrometric method are presented. The radioactivity concentrations of 226Ra, 232Th and 40K measured in the investigated samples range from 4,3 ± 0.5 to 258.0 ± 13.0 Bq kg(-1), 1.3 ± 0.1 to 124.9 ± 2.5 Bq kg(-1) and 24.4 ± 2.5 to 1092.0 ± 26.7 Bq kg(-1), respectively, for the structural building materials, and from 0.5 ±0.1 to 332.0 ± 18.1 Bq kg(-1), 0.6 ±0.1 to 226.0 ± 1 1.0 Bq kg(-1) and 1.5 ± 0.6 to 1060.0 ± 100.0 Bq kg(-1), respectively, for the covering building materials, and from 6.0 ± 0.3 to 2972.0 ± 148.6 Bq kg(-1), 1.1 ± 0.2 to 393.5 ± 4.2 Bq kg(-1) and 4.0 ± 0.4 to 3548.0 ± 75.0 Bq kg(-1), respectively, for the building raw materials. In this report, the radium equivalent activity (Ra(eq)), the activity concentration index (Iγ), the alpha index (Iα), the absorbed gamma dose rate (D(R)) from the external exposure and the indoor effective dose rate (H(R)) were calculated and compared to related criteria and limit values to assess the radiological risk from the utilizing of the building materials. The values of the Ra(eq) calculated for the structural and covering materials vary from 39.0 ± 4.4 to 240.4 ± 15.7 Bq kg(-1) and 2.4 ± 0.3 to 276.0 ± 20.0 Bq kg(-1), respectively. The mean values of the calculated Rae[] for the structural and covering materials are lower than the limit value of 370 Bq kg(-1) recommended for the building materials used in the construction of dwellings. The mean values of the calculated activity concentration index for the structural and covering materials are lower than the recommended criteria for these materials. The mean values of the indoor effective dose rate caused by the external exposure from the radionuclides in the structural materials, except brick samples (mean value of 1.06 ± 0.13), are 1.5-5 times lower than the recommended dose criterion of I mSv y(-1). According to the results of this study, it is seen that the structural and covering materials used in construction industry do not pose any radiological risk. The use of the building raw materials is limited and the only certain portion of these raw materials is used in the structural materials such as cement, concrete, brick, etc. Especially, as the values of index calculated for the building raw materials such as fly ash and phosphogypsum obtained as industrial by products are higher than the related criteria and limit values it is considered that the structural materials produced by using such building raw materials should be subjected to the radiological control.
