Objective: To compare post-operative pain of Bupivacaine vs placebo in patients undergoing percutaneous nephrolithotomy. Study Design: Quasi-experimental study. Place and Duration of Study: Armed Forces Institute of Urology, Rawalpindi Pakistan, from Aug 21 to Aug 22. Methodology: Eighty-eight patients undergoing percutaneous nephrolithotomy who fulfilled the inclusion criteria were included in study and randomized in two equal groups (44 patients each) by lottery method, Group-A received 20ml/50mg of 0.25% Bupivacaine and Group-B received 20ml normal saline around nephrostomy tube. Post-operatively, the visual analogue scale used to measure pain score at 6, 12 and 24 hours by an independent observer, blinded to randomization. After 24 hours mean of VAS calculated and noted. Results: Mean age of patient in Group-A was 39.40±8.42 years and in Group-B 38.00±8.67 years. There were 65 male (73.86%) and 23 female (26.13%) patients. Mean stone size was 3.00±0.68 cm and 3.006±0.63 cm in Group-A and B respectively. Median pain at 6, 12 and 24 hours was low in Group-A in comparison to Group-B with p-value <0.001. Similarly, overall pain score in 24 hours was also low in Group-A in comparison to Group-B with median (IQR) score being 4(4.33-3.67) vs 6.33(7.24-4), p<0.001. Demand of first post-operative analgesia was significantly prolonged in Group-A (256.68±23.70 minutes) than Group-B (168.72±30.86 minutes, p-value<0.001). Conclusion: The peritubal Bupivacaine infiltration is highly effective in reducing postoperative pain for patients undergoing PCNL as compared to placebo. [ABSTRACT FROM AUTHOR]

[Display omitted] CO 2 geo-storage is a technique, where millions of tonnes of CO 2 are stored in underground formations every year for permanent immobilization to reduce greenhouse gas emissions. Among promising geo-storage formations, basalt is attracting keen interest from researchers and industry. However, the literature severely lacks information on the wetting behaviour of basaltic rocks at geo-storage conditions. To enable a more general statement of basalt-scCO 2 -brine contact angles, the wettability of a basalt from Western Australia was compared with a similar rock type from Iceland. This study reports the advancing and receding contact angles for a basalt-scCO 2 -brine system at pressures ranging from 0.1 to 20 MPa and temperatures of 298 and 323 K, respectively. Based on the experimental data, the amount of CO 2 , expressed by the column height, which could be safely trapped beneath the basalt was then calculated. The basalt was initially water-wet but with increasing pressure, it was converted sequentially from a water-wet to an intermediate-wet and then finally into a completely CO 2 -wet template at pressures exceeding 15 MPa and 323 K. Under those experimental conditions, found in the field at depths below 1500 m, injected supercritical CO 2 into a porous basalt reservoir is assumed to flow freely in lateral and vertical directions and is less impeded by capillary/residual trapping, potentially leading to CO 2 leakage. It is suggested that the injection depth should not be chosen too deep to avoid increased free CO 2 plume mobility. It is found from CO 2 column height calculations that at 800 m depth (a minimum requirement to keep CO 2 supercritical), the height of the CO 2 column that can be safely trapped below the cap rock, was still 100 m but shrank to nil at ≥1500 m. Ordinary Portland cement (OPC) is proving to be hazardous to the environment. To replace the OPC, geopolymers (GPs) are introduced. However, to fully replace the OPC by GPs extensive laboratory tests are required to assess the long-term and short-term properties of GPs in different scenarios. Given the shortage of time for performing such extensive testing, artificial intelligence (AI) is used to analyze the properties of GPs. In this study, different AI techniques such as artificial neuro network (ANN), adaptive neuro-fuzzy inference system (ANFIS), and gene expression programming (GEP) are used to obtain the predictive models for estimating the compressive strength of fly ash and ground granulated blast furnace slag-based GP concrete. Different statistical parameters are used to evaluate the performance of predictive models. Similarly, sensitivity and parametric analysis are also conducted on the input parameters. Additionally, multiple linear regression was also performed on the whole database. After comparing all the results, it was concluded that GEP is the best AI technique to predict the compressive strength of GP-based concrete.

Carbon dioxide sequestration in geological formations has been proposed as a promising solution to reach net zero carbon emissions but the success of underground CO 2 storage in sandstone formations depends on the brine/CO 2 wettability of sandstone. Research evidence showed that natural geological formation is hydrophobic even in the presence of minute concentration of inherent organic acids. This study investigates the effect of methylene blue (MB) on CO 2 wettability of organic-acid contaminated quartz through the tilted plate contact angle measurement method. Pure quartz substrates were aged in a stearic acid/n-decane solution for one week and subsequently modified with different concentrations of MB (ranging from 10 to 100 mg/L) at a temperature of 60 °C. Advancing (θ a) and receding (θ r) contact angles were measured under varying conditions of temperature (25 °C and 50 °C), pressure (ranging from 10 to 20 MPa), and salinity (0–0.3 M). The experimental results indicate that pure quartz, when aged in a stearic acid solution, becomes CO 2 -wet at all temperature, pressure, and salinity conditions. However, at any physio-thermal condition, the wettability of the quartz surfaces was reversed when treated with different concentrations of MB, transitioning to a water-wet state. The findings of this research demonstrate the potential of MB to modify the wetting behaviour of quartz surfaces and enhance CO 2 residual trapping in sandstone formations. • Methylene blue was used to alter wettability of sandstone reservoirs. • The contact angle decreased with increasing methylene blue concentration. • Contact angle increased with temperature, pressure, and salinity. Millions of tons of CO 2 are stored in CO 2 geological storage (CGS) formations (depleted oil reservoirs and deep saline aquifers) every year. These CGS formations naturally contain small concentrations of water-soluble organic components in particular humic acid (HA), which may drastically affect the rock wettability - a significant factor determining storage capacities and containment security. Hence, it is essential to characterise the effect of humic acid concentration on CO 2 -wettability and its associated impact on storage capacity. To achieve this, we measured advancing and receding contact angles at reservoir conditions using the pendant drop tilted plate method for various humic acid concentrations (1, 10, and 100 mg/L) as a function of pressure (0.1–25 MPa), temperature (303–333 K), and brine salinity (0–0.3 M NaCl). Further, the influence of humic acid adsorption on the mineral's surface was examined by several independent techniques. Our results demonstrate that humic acid significantly changes rock wettability from water-wet (0–50°) towards CO 2 -wet (90–110°). An increase in pressure, temperature, and salinity had a similar effect. Humic acid adsorption also increased the surface roughness of the substrates. We conclude that even trace amounts of humic acid (i.e. 1 mg/L), which exist in storage aquifers, significantly increase CO 2 -wettability and thus reduce structural and residual trapping capacities. This study was conducted to isolate and diagnose the most important bacterial types causing urinary tract infection from the inpatients and outpatients to Salah El Din General Hospital using routine isolation and diagnosis methods. 70 samples were isolated, and the results showed two types of growth: Gram positive bacteria and Gram negative bacteria, where 10 bacterial types were isolated as follows: E. coli 42.85%, Kleibsiella. pneumonia 21.42% roteus mirabilis (11.42%), Staphylococcus aureus (7.14%), Enterobacter cloaca (5.71%), Serratia marcescens (2.85%), Pseudomonas aeruginosa (2.85%), Staphylococcus Saprophyticus. 2.85%, Micrococcus luteus 1.42%, and Citrobacter freundii 1.42%. The results of the susceptibility test for 9 types of antibiotics using the discs method showed a change in the level of resistance to different antibiotics. The most effective antibiotics were Imipenem, Ciprofloxacin and Sulfamethoxaole. Plant extracts (both aqueous and alcoholic) of Sambucus nigra and Tribulus terrestris fruits were used, and tested for their ability to inhibit the most isolated bacteria (E. coli, K. pneumonia P. mirabilis and S. aureus ). All alcoholic extracts were effective in the direction of Bacterial isolates, while aqueous extracts, have little or no effect. [ABSTRACT FROM AUTHOR]

231. Application of metaheuristic optimization algorithms in predicting the compressive strength of 3D-printed fiber-reinforced concrete

In recent years, the construction industry has been striving to make production faster and handle more complex architectural designs. Waste reduction, geometric freedom, lower construction costs, and speedy construction make the 3D-printed fiber-reinforced concrete (3DPFRC) alternative for future construction. However, achieving the optimum mixture composition for 3DPFRC remains a daunting task, entailing the consideration of multiple variables and necessitating an extensive trial-and-error experimental process. Therefore, this study investigated the application of different metaheuristic optimization algorithms to predict the compressive strength (CS) of 3DPFRC. A database of 299 data samples with 16 different input features was compiled from the experimental studies in the literature. Six metaheuristic algorithms, such as human felicity algorithm (HFA), differential evolution algorithm (DEA), nuclear reaction optimization (NRO), Harris hawks optimization (HHO), lightning search algorithm (LSA), and tunicate swarm algorithm (TSA) were applied to identify the optimal hyperparameter combination for the random forest (RF) model in predicting the CS of 3DPFRC. Different statistical metrics and 10-fold cross-validation were used to evaluate the accuracy of the models. The TSA-RF model exhibited superior performance compared to other models, achieving correlation (R), mean absolute error (MAE), and root mean square error (RMSE) values of 0.99, 2.10 MPa, and 3.59 MPa, respectively. The LSA-RF model also performed well, with R, MAE, and RMSE values of 0.99, 2.93 MPa, and 6.23 MPa, respectively. SHapley Additive exPlanation (SHAP) interpretability elucidates the intricate relationships between features and their effects on the CS, thereby offering invaluable insights for the performance-based mix proportion design of 3DPFRC.

In this study, we evaluate the effect of hydrolysis treatment on the properties of tar and soot produced during the pyrolysis of sewage sludge at different operating temperatures (500–800 °C). The pyrolysis experiments were performed in a two-stage tubular fixed-bed reactor at atmospheric pressure. Raw sludge and hydrolysis-treated sludge (at pressure of 0.4 MPa and temperature 200 °C) samples were collected from local municipal sewage sludge treatment plant. The tar and soot samples obtained from pyrolysis at different temperatures were characterized by gas chromatography–mass spectrometry and scanning electron microscopy with energy dispersive X-ray spectroscopy, respectively. The results showed that increase in the pyrolysis temperature decreases the tar and soot yield for both raw and hydrolysis-treated sludge. The minimum tar yield was achieved with hydrolysis-treated sludge at 800 °C. The soot from raw sludge pyrolysis was of greater yield at high temperature than hydrolysis-treated sludge pyrolysis. The tar from pyrolysis of hydrolysis-treated sludge had more aliphatic character as compared to raw sewage sludge. The formation of heavy polycyclic aromatic hydrocarbons during pyrolysis of hydrolysis-treated sludge was lower at high temperatures than that of raw sludge. Objectives: To compare the efficacy and complications of Haemorrhoidectomy alone and Haemorrhoidectomy plus lateral internal sphicterotomy (LIS) for the treatment of 3rd and 4th degree Haemorrhoids among middleaged patients. Study Design: Quasi experimental study. Place and Duration of Study: Combined Military Hospital, Quetta from Mar 2010 to Feb 2011. Methodology: A total of 60 patients of 3O and 4O Haemorrhoids, were randomly divided into two groups 'A' and 'B' with 30 patients in each group. Group 'A' and 'B' were subjected to Haemorrhoidectomy alone and Haemorrhoidectomy plus LIS respectively. The outcome measures were relief of symptoms, recurrence rate and complications. Results: Important immediate complication was postoperative pain in group 'A'. 33.3% had moderate pain and 26.7% had severe pain within 24 hours of procedure whereas in group 'B' 13.3% patients had moderate pain and 10% had severe pain (p-value 0.002). Postoperative bleeding and acute urinary retention were not major complications in both groups. Flatus and faecal incontinence was major problem in group 'B' at 1st postoperative day and 1st week with 30% and 16.7% cases respectively. After 2 weeks, there was significant improvement in flatus 10% and faecal incontinence 3.3% in group 'B'. At 6 months, only 3.3% in group 'B' had residual flatus incontinence. Anal stenosis was present in 13.3% patients in group 'A' as compared to none in group 'B' (p-value 0.043). On anoproctoscopy at 6 months, early recurrent/secondary haemorrhoids were seen in 23.3% patients in group 'A' and 3.3% in group 'B' (p-value 0.031). Ten percent patients from group 'A' presented with bleeding secondary haemorrhoids before 6 months (p-value 0.083). Conclusion: The Haemorrhoidectomy plus LIS was found a recommended procedure in relatively younger patients of 3O and/or 4O haemorrhoids with good anal tone on digital rectal examination. Incorporating selenium into high-surface-area carbon with hierarchical pores, derived from red kidney bean peels via simple carbonization/activation, yields a superior Li–Se battery cathode material. This method produces a carbon framework with 568 m2 g−1 surface area, significant pore volume, and improves the composite's electronic conductivity and stability by mitigating volume changes and reducing lithium polyselenide dissolution. The Se@ACRKB composite, containing 45 wt% selenium, shows high discharge capacities (609.13 mAh g−1 on the 2nd cycle, maintaining 470.76 mAh g−1 after 400 cycles at 0.2 C, and 387.58 mAh g−1 over 1000 cycles at 1 C). This demonstrates exceptional long-term stability and performance, also applicable to Na–Se batteries, with 421.36 mAh g−1 capacity after 200 cycles at 0.1 C. Our study showcases the potential of using sustainable materials for advanced battery technologies, emphasizing cost-effective and scalable solutions for energy storage. [ABSTRACT FROM AUTHOR]

The utilization of Self-compacting Concrete (SCC) has escalated worldwide due to its superior properties in comparison to normal concrete such as compaction without vibration, increased flowability and segregation resistance. Various other desirable properties like ductile behaviour, increased strain capacity and tensile strength etc. can be imparted to SCC by incorporation of fibres. Thus, this study presents a novel approach to predict 28-day compressive strength (C–S) of FR-SCC using Gene Expression Programming (GEP) and Multi Expression Programming (MEP) for fostering its widespread use in the industry. For this purpose, a dataset had been compiled from internationally published literature having six input parameters including water-to-cement ratio, silica fume, fine aggregate, coarse aggregate, fibre, and superplasticizer. The predictive abilities of developed algorithms were assessed using error metrices like mean absolute error (MAE), a20-index, and objective function (OF) etc. The comparison of MEP and GEP models indicated that GEP gave a simple equation having lesser errors than MEP. The OF value of GEP was 0.029 compared to 0.031 of MEP. Thus, sensitivity analysis was performed on GEP model. The models were also checked using some external validation checks which also verified that MEP and GEP equations can be used to forecast the strength of FR-SCC for practical uses. Purpose: In developing countries foreign lending becomes a problem now a day instead of spend this lending for the development purposes. Ultimately this problem causes poverty in these countries where usage of foreign lending is not in proper ways. The purpose of this study is to investigate the impact of IMF and World Bank lending on poverty in Pakistan, India and Bhutan. In this study corruption, GDP, unemployment, secondary enrolment, and external debt are used as independent variables and poverty headcount ratio as dependent variable. Study finds out the relationship of corruption, unemployment and external debts with poverty and showing the positive relationship while secondary enrolment and GDP showing negative relation with poverty. Moreover study finds out that lending of IMF and WORLD BANK mostly causes poverty in these developing countries instead of reducing poverty because of corrupt government's weak policies for the distribution of loans. It is examined that the countries with strong policies and non-corrupt government can take full advantage of these lending for poverty reduction. But it is noticed that the countries which are the members of IMF structural adjustment programs are facing more poverty problems as compare to those countries which are not involved in these programs or even have less numbers of lending. Those countries are much better than the countries involve in structural adjustment programs.

A mutated K-RAS gene is the originator of several characters of colorectal cancer among them tumor initiation, growth, survival, metastasis formation and even immune response. Thus; the aim of our experiments is to establish if the tumor samples express the K-RAS mutation before starting the monoclonal antibody-based therapy. Otherwise it makes no sense to block the receptor binding site if the signaling cascade is independent of the binding of the ligand to the receptor. Colorectal cancer, a cancer resulting from uncontrolled cell growth in the colon or rectum is the fourth most common cause of cancer death. The stage of its development affects the method of treatment of this disease. A prominent method to treat this type of cancer is the blockage of the EGFR by monoclonal antibodies. This kind of approach however seems only to be efficient as long as there is no mutation of K-Ras, a GTPase thought to play an important role at an early stage of the development of colorectal cancer. In case of a mutation in the RAS protein, the Ras GTPase is activated to such an extent that the subsequent over activation of downstream signaling pathways leads to tumor genesis.
