Abstract: Neutrinos in the cosmic ray flux with energies near 1 EeV and above are detectable with the Surface Detector array of the Pierre Auger Observatory. We report here on searches through Auger data from 1 January 2004 until 20 June 2013. No neutrino candidates were found, yielding a limit to the diffuse flux of ultra-high energy neutrinos that challenges the Waxman-Bahcall bound predictions. Neutrino identification is attempted using the broad time-structure of the signals expected in the SD stations, and is efficiently done for neutrinos of all flavors interacting in the atmosphere at large zenith angles, as well as for "Earth-skimming" neutrino interactions in the case of tau neutrinos. In this paper the searches for downward-going neutrinos in the zenith angle bins $60^\circ-75^\circ$ and $75^\circ-90^\circ$ as well as for upward-going neutrinos, are combined to give a single limit. The $90\%$ C.L. single-flavor limit to the diffuse flux of ultra-high energy neutrinos with an $E^{-2}$ spectrum in the energy range $1.0 \times 10^{17}$ eV - $2.5 \times 10^{19}$ eV is $E_\nu^2 dN_\nu/dE_\nu < 6.4 \times 10^{-9}~ {\rm GeV~ cm^{-2}~ s^{-1}~ sr^{-1}}$., Comment: Replaced with published version. Abstract: A measurement of the cosmic-ray spectrum for energies exceeding $4{\times}10^{18}$ eV is presented, which is based on the analysis of showers with zenith angles greater than $60^{\circ}$ detected with the Pierre Auger Observatory between 1 January 2004 and 31 December 2013. The measured spectrum confirms a flux suppression at the highest energies. Above $5.3{\times}10^{18}$ eV, the "ankle", the flux can be described by a power law $E^{-\gamma}$ with index $\gamma=2.70 \pm 0.02 \,\text{(stat)} \pm 0.1\,\text{(sys)}$ followed by a smooth suppression region. Abstract: Background: Protein-energy loss and inflammation are the main risk factors in the occurrence of complications in hemodialysis patients. The Prognostic Inflammatory and Nutritional Index (PINI) is a simple, inexpensive test to identify the early onset of inflammation and malnutrition in hemodialysis patients, critically ill subjects and those with malignancies. Methods: A systemic review of English literature was conducted on the topic published between 1985 and 2022. A focused and sensitive search strategy was applied to the PUBMED database to identify relevant scientific articles in English. Once articles were identified, a detailed quality and bias assessment was performed. Two independent researchers analyzed the detailed data extraction. Results: PINI proved to be a sensitive, powerful, low-cost and simple test. PINI has been useful in assessing evolution and prognostics in clinical care, with values above one being associated with a high risk of mortality and morbidity. It is useful in cases with surgical and postoperative complications, long hospitalization, as well as increased associated expenses. Conclusions: This is the first review of the literature on the above-mentioned topic (PINI) and is a valuable candidate for validating prognosis in patients with different pathologies.

Abstract: Sweet syndrome (SS) is a rare disease described as a febrile neutrophilic dermatosis with acute onset, the pathogenesis of which has not yet been elucidated. The syndrome is characterized by the sudden onset of erythematous infiltrated papules or plaques located on the upper body and is associated with fever, leukocytosis and neutrophilia. The lesions show a dense dermal infiltration with mature neutrophils. The condition is responsive to systemic steroids. The central nervous system, bones, muscles, eyes, ears, mouth, heart, lung, liver, kidneys, intestines, and spleen may be affected by SS as extracutaneous manifestations. More and more cases have been found to be associated with malignancies, particularly myelodysplastic syndrome, and, less frequently, other hematologic malignancies or solid tumors. Approximately 21% of patients with SS have an associated malignancy and up to 80% of MASS cases are associated with hematological diseases, predominantly myelodysplastic syndrome (MDS) or acute myeloid leukemia (AML). Myelodysplastic syndrome is a clonal disease of the bone marrow characterized by inefficient hematopoiesis, dysplasia of the bone marrow and peripheral cytopenias. Affected patients have a high risk of leukemic transformation. After analyzing later studies and current practical aspects regarding MDS-related SS, we suggest an algorithm for evaluating these patients.

Abstract: Infective endocarditis is a rare disease in children. The etiology is mainly bacterial. However, viral infective endocarditis, possibly related to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has also been reported. The pathophysiological principle of the connection between the two entities seems to be attributed to the transient immune deficiency of the body during the infection. Additionally, SARS-CoV-2 is reported in the literature as a direct cardiopathic virus. Therefore, the new coronavirus seems to have the ability to affect both the intact cardiac tissue and the previously damaged one both during the acute episode and at a distance from it. Consequently, we propose to review the main pathophysiological aspects of pediatric cardiac damage caused by SARS-CoV-2. The ultimate goal is to deepen existing knowledge, broaden the horizon of understanding and analysis regarding the systemic damage induced by viral infections, and strengthen an information base from which to start a meta-analysis. Next, we performed a non-systematized screening of the specialized literature with reference to cases of endocarditis in the pediatric population, reported in the period 2020-2023. From the total of articles found, we chose to include in the review a number of 6 case reports, including a number of 7 patients (5 children and 2 adolescents). Analysis of reports suggests that SARS-CoV-2 infection could play a role in the development of endocarditis, either directly through active infection or indirectly through a post-infectious immune response. Also, pre-existing conditions and complex medical history predispose to an increased risk of developing a severe, complicated form of endocarditis. Also, the lack of data on the vaccination history and the failure to categorize the infection depending on the type of antibodies (IgM or IgG) in some studies represent a major bias in the reports. Abstract: The paper highlights the constrains faced by the user when choosing the most appropriate solar collector from a set of collectors belonging to the same category. Two basic kinds of one porous and one non-porous with single flow/pass flat solar air heaters were tested. The collectors have the same size but different internal shapes and absorber materials. The two collectors were tested and a shading procedure was implemented in order to emphasize several aspects related to the thermal inertia of the collectors. Comparison has been made between the efficiency values obtained experimentally. The shade test shows a faster decrease of the outlet air temperature for V-porous absorber collector in comparison with the U-corrugated absorber collector i.e. 6 °C and -3.5 °C respectively in a time interval of 74 s. Exposing the collectors on a fresh heating cycle under a low stability radiative regime (450-750 W/m2) one observe that 60% of the first 180 s the corrugated collector outperforms the porous collector; after that interval of time the porous collector becomes more effective. The low thermal inertia of the solar air collectors yields high variation of the thermal efficiency in days with low stability of the radiative regime. In such days, using measured series of solar irradiance data with long sampling period between two successive measurements does not allow a proper choise of the most appropriate solar collector type. Abstract: Olea europaea L. is the most valuable species of the Olea type, and its products offer a wide range of therapeutical uses. The olive tree has been extensively studied for its nourishing qualities, and the "Mediterranean diet", which includes virgin olive oil as a key dietary component, is strongly associated with a reduced risk of cardiovascular disease and various malignancies. Olive leaves, a by-product in the olive harvesting process, are valued as a resource for developing novel phytomedicines. For this purpose, two ethanolic extracts obtained from Olivae folium from Spain (OFS) and Greece (OFG) were investigated. Our findings contribute to a wider characterization of olive leaves. Both extracts displayed important amounts of phenolic compounds and pentacyclic triterpenes, OFG having higher concentrations of both polyphenols, such as oleuropein and lutein, as well as triterpenes, such as oleanolic acid and maslinic acid. The antioxidant capacity is similar for the two extracts, albeit slightly higher for OFG, possibly due to metal polyphenol complexes with antioxidant activity. The extracts elicited an antimicrobial effect at higher doses, especially against Gram-positive bacteria, such as Streptococcus pyogenes. The extract with lower inorganic content and higher content of polyphenols and triterpenic acids induced a strong anti-radical capacity, a selective cytotoxic effect, as well as antimigratory potential on A375 melanoma cells and antiangiogenic potential on the CAM. No irritability and a good tolerability were noted after evaluating the extracts on the in vivo Hen's Egg Test−Chorioallantoic Membrane (HET-CAM). Therefore, the present data are suggestive for the possible use of the two types of olive leaf products as high-antioxidant extracts, potentially impacting the healthcare system through their use as antimicrobial agents and as anticancer and anti-invasion treatments for melanoma. Abstract: Protein-energy wasting and inflammation are major risk factors for complications in hemodialysis patients. As these risk factors are triggered by a pro-inflammatory state, oxidative stress and hemodynamic dysfunction, which overlap in hemodialyzed subjects, we aimed to assess the efficacy of a cost-effective and straightforward screening tool, the Prognostic Inflammatory and Nutritional Index (PINI), in regularly screening maintenance hemodialysis (MHD) patients, to detect early signs of inflammation and malnutrition. A 12-month follow-up was carried out on a cohort of 102 adult patients undergoing maintenance dialysis, during which the Prognostic Inflammatory and Nutritional Index (PINI) was calculated using the formula alpha1-Acid Glycoprotein (AGP) × C-reactive protein (CRP)/albumin (ALB) × transthyretin (TTR). A PINI score < 1 was considered normal. The patients were stratified based on their PINI score: 66 patients (64.70%) had a normal score, below 1, while 36 patients (35.30%) had a PINI score ≥ 1. Despite the absence of clinical evidence of inflammation at enrollment, the latter group exhibited higher levels of CRP. During the follow-up period, all patients with a PINI score ≥ 1 experienced at least one acute event, compared to only 6% of patients with a normal PINI score, which presented COVID-19 infection as an acute event. The evaluation of the PINI can effectively identify the silent malnutrition–inflammation syndrome and predict the risk of acute events. Abstract: The municipal wastewater treatment is the source of significant amounts of primary and secondary sludge which is under the present legislation referring to quality and management aspects. It is estimated that a half of wastewater treatment plant costs are due to the sludge management. Anaerobically sludge stabilization, capitalization as energy source, in order to diminish the costs and sludge volume decreasing, are the aims of the main operational steps of sludge treatment, as a part of wastewater treatment plant. The improvement of sludge anaerobically stabilization process must be possible by acting in the rate limiting step - hydrolysis in order to rise the organic carbon solubilization. The increase of soluble carbon can be possible by adding a pretreatment step of waste biological sludge, ultrasonic disintegration being one option. This paper emphasized the experimental results regarding anaerobically stabilization of the thickened waste biological sludge by ultrasonication taking into account the results of blank test, without ultrasonication. Experimental tests show that ultrasonic disintegration of the sludge having initial dried substances content (dry weight) 2.72% and soluble organic load COD of 598 mg O2/L led to soluble COD concentration of 4950-6710 mg O2/L after sonication with specific energy in the range of 3.06 - 14.24 kWh/kg dry weight. Anaerobically stabilization during 25 test days at 36 0C of the mixture 40% disintegrated biological sludge and 60% digested sludge (inoculum) mixture led to 30-38.6% increase of biogas production comparing with parallel test with non-sonicated sludge.

Abstract: Spherical harmonic moments are well-suited for capturing anisotropy at any scale in the flux of cosmic rays. An unambiguous measurement of the full set of spherical harmonic coefficients requires full-sky coverage. This can be achieved by combining data from observatories located in both the northern and southern hemispheres. To this end, a joint analysis using data recorded at the Telescope Array and the Pierre Auger Observatory above $10^{19}$ eV is presented in this work. The resulting multipolar expansion of the flux of cosmic rays allows us to perform a series of anisotropy searches, and in particular to report on the angular power spectrum of cosmic rays above $10^{19}$ eV. No significant deviation from isotropic expectations is found throughout the analyses performed. Upper limits on the amplitudes of the dipole and quadrupole moments are derived as a function of the direction in the sky, varying between 7% and 13% for the dipole and between 7% and 10% for a symmetric quadrupole., Comment: 33 pages, 13 figures, to appear in The Astrophysical Journal

Abstract: We describe the method devised to reconstruct inclined cosmic-ray air showers with zenith angles greater than $60^\circ$ detected with the surface array of the Pierre Auger Observatory. The measured signals at the ground level are fitted to muon density distributions predicted with atmospheric cascade models to obtain the relative shower size as an overall normalization parameter. The method is evaluated using simulated showers to test its performance. The energy of the cosmic rays is calibrated using a sub-sample of events reconstructed with both the fluorescence and surface array techniques. The reconstruction method described here provides the basis of complementary analyses including an independent measurement of the energy spectrum of ultra-high energy cosmic rays using very inclined events collected by the Pierre Auger Observatory., Comment: 27 pages, 19 figures, accepted for publication in Journal of Cosmology and Astroparticle Physics (JCAP)

Abstract: A flux of neutrons from an astrophysical source in the Galaxy can be detected in the Pierre Auger Observatory as an excess of cosmic-ray air showers arriving from the direction of the source. To avoid the statistical penalty for making many trials, classes of objects are tested in combinations as nine "target sets", in addition to the search for a neutron flux from the Galactic Center or from the Galactic Plane. Within a target set, each candidate source is weighted in proportion to its electromagnetic flux, its exposure to the Auger Observatory, and its flux attenuation factor due to neutron decay. These searches do not find evidence for a neutron flux from any class of candidate sources. Tabulated results give the combined p-value for each class, with and without the weights, and also the flux upper limit for the most significant candidate source within each class. These limits on fluxes of neutrons significantly constrain models of EeV proton emission from non-transient discrete sources in the Galaxy., Comment: 17 pages, accepted for publication in The Astrophysical Journal Letters

Abstract: Measurements of air showers made using the hybrid technique developed with the fluorescence and surface detectors of the Pierre Auger Observatory allow a sensitive search for point sources of EeV photons anywhere in the exposed sky. A multivariate analysis reduces the background of hadronic cosmic rays. The search is sensitive to a declination band from -85{\deg} to +20{\deg}, in an energy range from 10^17.3 eV to 10^18.5 eV. No photon point source has been detected. An upper limit on the photon flux has been derived for every direction. Abstract: We present IRAM PdBI observations of the CO(3-2) and CO(5-4) line transitions from a Ly-alpha blob at z~2.7 in order to investigate the gas kinematics, determine the location of the dominant energy source, and study the physical conditions of the molecular gas. CO line and dust continuum emission are detected at the location of a strong MIPS source that is offset by ~1.5" from the Ly-alpha peak. Neither of these emission components is resolved with the 1.7" beam, showing that the gas and dust are confined to within ~7kpc from this galaxy. No millimeter source is found at the location of the Ly-alpha peak, ruling out a central compact source of star formation as the power source for the Ly-alpha emission. Combined with a spatially-resolved spectrum of Ly-alpha and HeII, we constrain the kinematics of the extended gas using the CO emission as a tracer of the systemic redshift. Near the MIPS source, the Ly-alpha profile is symmetric and its line center agrees with that of CO line, implying that there are no significant bulk flows and that the photo-ionization from the MIPS source might be the dominant source of the Ly-alpha emission. In the region near the Ly-alpha peak, the gas is slowly receding (~100km/s) with respect to the MIPS source, thus making the hyper-/superwind hypothesis unlikely. We find a sub-thermal line ratio between two CO transitions, I_CO(5-4)/I_CO(3-2)=0.97+/-0.21. This line ratio is lower than the average values found in high-z SMGs and QSOs, but consistent with the value found in the Galactic center, suggesting that there is a large reservoir of low-density molecular gas that is spread over the MIPS source and its vicinity., Comment: 12 pages, 9 figures, accepted for publication in ApJ

Abstract: Abstract The hybrid design of the Pierre Auger Observatory allows for the measurement of the properties of extensive air showers initiated by ultra-high energy cosmic rays with unprecedented precision. By using an array of prototype underground muon detectors, we have performed the first direct measurement, by the Auger Collaboration, of the muon content of air showers between $$2\times 10^{17}$$ 2×1017 and $$2\times 10^{18}$$ 2×1018 eV. We have studied the energy evolution of the attenuation-corrected muon density, and compared it to predictions from air shower simulations. The observed densities are found to be larger than those predicted by models. We quantify this discrepancy by combining the measurements from the muon detector with those from the Auger fluorescence detector at $$10^{{17.5}}\, {\mathrm{eV}} $$ 1017.5eV and $$10^{{18}}\, {\mathrm{eV}} $$ 1018eV . We find that, for the models to explain the data, an increase in the muon density of $$38\%$$ 38% $$\pm 4\% (12\%)$$ ±4%(12%) $$\pm {}^{21\%}_{18\%}$$ ±18%21% for EPOS-LHC, and of $$50\% (53\%)$$ 50%(53%) $$\pm 4\% (13\%)$$ ±4%(13%) $$\pm {}^{23\%}_{20\%}$$ ±20%23% for QGSJetII-04, is respectively needed.

Abstract: Synthetic solutions of flutamide were subject to degradation using three advanced oxidation systems, namely UV/TiO2, UV/H2O2 and UV/H2O2/TiO2. Optimum conditions and degradation kinetics has been established for all three systems. The experimental results showed that all three systems can be successfully used for flutamide degradation with efficiencies higher than 99% and that advanced oxidation processes are showing good potential for degradation of organic pollutants that cannot be suitable removed/degraded using conventional wastewater treatment processes

Abstract: Mitocurcumin (a triphenylphosphonium curcumin derivative) was previously reported as a selective antitumoral compound on different cellular lines, as well as a potent bactericidal candidate. In this study, the same compound showed strong antimicrobial efficacy against different strains of methicillin-resistant Staphylococcus aureus (MRSA). The minimum inhibitory concentration was identical for all tested strains (four strains of MRSA and one strain of methicillin-sensitive Staphylococcus aureus), suggesting a new mechanism of action compared with usual antibacterial agents. All tested strains showed a significant sensitivity in the low micromolar range for the curcumin-triphenylphosphonium derivative. This susceptibility was modulated by the menadione/glutathione addition (the addition of glutathione resulted in a significant increase in minimal inhibitory concentration from 1.95 to 3.9 uM, whereas adding menadione resulted in a decrease of 0.49 uM). The fluorescence microscopy showed a better intrabacterial accumulation for the new curcumin-triphenylphosphonium derivative compared with simple curcumin. The MitoTracker staining showed an accumulation of reactive oxygen species (ROS) for a S. pombe superoxide dismutase deleted model. All results suggest a new mechanism of action which is not influenced by the acquired resistance of MRSA. The most plausible mechanism is reactive oxygen species (ROS) overproduction after a massive intracellular accumulation of the curcumin-triphenylphosphonium derivative.

Abstract: A search for time-directional coincidences of ultra-high-energy (UHE) photons above 10 EeV with gravitational wave (GW) events from the LIGO/Virgo runs O1 to O3 is conducted with the Pierre Auger Observatory. Due to the distinctive properties of photon interactions and to the background expected from hadronic showers, a subset of the most interesting GW events is selected based on their localization quality and distance. Time periods of 1000 s around and 1 day after the GW events are analyzed. No coincidences are observed. Upper limits on the UHE photon fluence from a GW event are derived that are typically at ∼7 MeV cm ^−2 (time period 1000 s) and ∼35 MeV cm ^−2 (time period 1 day). Due to the proximity of the binary neutron star merger GW170817, the energy of the source transferred into UHE photons above 40 EeV is constrained to be less than 20% of its total GW energy. These are the first limits on UHE photons from GW sources.

Abstract: A catalog containing details of the highest-energy cosmic rays recorded through the detection of extensive air showers at the Pierre Auger Observatory is presented with the aim of opening the data to detailed examination. Descriptions of the 100 showers created by the highest-energy particles recorded between 2004 January 1 and 2020 December 31 are given for cosmic rays that have energies in the range 78–166 EeV. Details are also given on a further nine very energetic events that have been used in the calibration procedure adopted to determine the energy of each primary. A sky plot of the arrival directions of the most energetic particles is shown. No interpretations of the data are offered.

Abstract: Rheumatoid arthritis (RA), one of the most common of the chronic inflammatory autoimmune diseases (CIADs), is recognized as an independent cardiovascular risk factor. Traditional risk factors such as smoking, arterial hypertension, dyslipidemia, insulin resistance, and obesity are frequently found in RA. Given the increased risk of mortality and morbidity associated with cardiovascular disease (CVD) in RA patients, screening for risk factors is important. Moreover, there is a need to identify potential predictors of subclinical atherosclerosis. Recent studies have shown that markers such as serum homocysteine, asymmetric dimethylarginine, or carotid intima–media thickness (cIMT) are correlated with cardiovascular risk. Although RA presents a cardiovascular risk comparable to that of diabetes, it is not managed as well in terms of acute cardiovascular events. The introduction of biological therapy has opened new perspectives in the understanding of this pathology, confirming the involvement and importance of the inflammatory markers, cytokines, and the immune system. In addition to effects in inducing remission and slowing disease progression, most biologics have demonstrated efficacy in reducing the risk of major cardiovascular events. Some studies have also been conducted in patients without RA, with similar results. However, early detection of atherosclerosis and the use of targeted therapies are the cornerstone for reducing cardiovascular risk in RA patients.

Abstract: Aortic aneurysms represent a very common pathology that can affect any segment of the aorta. These types of aneurysms can be localized on the thoracic segment or on the abdominal portion, with the latter being more frequent. Though there are similarities between thoracic and abdominal aortic aneurysms, these pathologies are distinct entities. In this article, we undertook a review regarding the different mechanisms that can lead to the development of aortic aneurysm, and we tried to identify the different manners of treatment. For a long time, aortic wall aneurysms may evolve in an asymptomatic manner, but this progressive dilatation of the aneurysm can lead to a potentially fatal complication consisting in aortic rupture. Because there are limited therapies that may delay or prevent the development of acute aortic syndromes, surgical management remains the most common manner of treatment. Even though, surgical management has improved much in the last years, thus becoming less invasive and sophisticated, the morbi-mortality linked to these therapies remains increased. The identification of the cellular and molecular networks triggering the formation of aneurysm would permit the discovery of modern therapeutic targets. Molecular and cellular mechanisms are gaining a bigger importance in the complex pathogenesis of aortic aneurysms. Future studies must be developed to compare the findings seen in human tissue and animal models of aortic aneurysm, so that clinically relevant conclusions about the aortic aneurysm formation and the pharmacological possibility of pathogenic pathways blockage can be drawn.

243. Effects of the Sex Chromosome Complement, XX, XO, or XY, on the Transcriptome and Development of Mouse Oocytes During Follicular Growth

Abstract: The sex chromosome complement, XX or XY, determines sexual differentiation of the gonadal primordium into a testis or an ovary, which in turn directs differentiation of the germ cells into sperm and oocytes, respectively, in eutherian mammals. When the X monosomy or XY sex reversal occurs, XO and XY females exhibit subfertility and infertility in the mouse on the C57BL/6J genetic background, suggesting that functional germ cell differentiation requires the proper sex chromosome complement. Using these mouse models, we asked how the sex chromosome complement affects gene transcription in the oocytes during follicular growth. An oocyte accumulates cytoplasmic components such as mRNAs and proteins during follicular growth to support subsequent meiotic progression, fertilization, and early embryonic development without de novo transcription. However, how gene transcription is regulated during oocyte growth is not well understood. Our results revealed that XY oocytes became abnormal in chromatin configuration, mitochondria distribution, and de novo transcription compared to XX or XO oocytes near the end of growth phase. Therefore, we compared transcriptomes by RNA-sequencing among the XX, XO, and XY oocytes of 50–60 µm in diameter, which were still morphologically comparable. The results showed that the X chromosome dosage limited the X-linked and autosomal gene transcript levels in XO oocytes whereas many genes were transcribed from the Y chromosome and made the transcriptome in XY oocytes closer to that in XX oocytes. We then compared the transcript levels of 3 X-linked, 3 Y-linked and 2 autosomal genes in the XX, XO, and XY oocytes during the entire growth phase as well as at the end of growth phase using quantitative RT-PCR. The results indicated that the transcript levels of most genes increased with oocyte growth while largely maintaining the X chromosome dosage dependence. Near the end of growth phase, however, transcript levels of some X-linked genes did not increase in XY oocytes as much as XX or XO oocytes, rendering their levels much lower than those in XX oocytes. Thus, XY oocytes established a distinct transcriptome at the end of growth phase, which may be associated with abnormal chromatin configuration and mitochondria distribution.

244. ECG and Biomarker Profile in Patients with Acute Heart Failure: A Pilot Study

Abstract: Background: Biomarkers, electrocardiogram (ECG) and Holter ECG are basic, accessible and feasible cardiac investigations. The combination of their results may lead to a more complex predictive model that may improve the clinical approach in acute heart failure (AHF). The main objective was to investigate which ECG parameters are correlated with the usual cardiac biomarkers (prohormone N-terminal proBNP, high-sensitive cardiac troponin I) in patients with acute heart failure, in a population from Romania. The relationship between certain ECG parameters and cardiac biomarkers may support future research on their combined prognostic value. Methods: In this prospective case-control study were included 49 patients with acute heart failure and 31 participants in the control group. For all patients we measured levels of prohormone N-terminal proBNP (NT-proBNP), high-sensitive cardiac troponin I (hs-cTnI) and MB isoenzyme of creatine phosphokinase (CK-MB) and evaluated the 12-lead ECG and 24 h Holter monitoring. Complete clinical and paraclinical evaluation was performed. Results: NT-proBNP level was significantly higher in patients with AHF (p < 0.001). In patients with AHF, NT-proBNP correlated with cQTi (p = 0.027), pathological Q wave (p = 0.029), complex premature ventricular contractions (PVCs) (p = 0.034) and ventricular tachycardia (p = 0.048). Hs-cTnI and CK-MB were correlated with ST-segment modification (p = 0.038; p = 0.018) and hs-cTnI alone with complex PVCs (p = 0.031). Conclusions: The statistical relationships found between cardiac biomarkers and ECG patterns support the added value of ECG in the diagnosis of AHF. We emphasize the importance of proper ECG analysis of more subtle parameters that can easily be missed. As a non-invasive technique, ECG can be used in the outpatient setting as a warning signal, announcing the acute decompensation of HF. In addition, the information provided by the ECG complements the biomarker results, supporting the diagnosis of AHF in cases of dyspnea of uncertain etiology. Further studies are needed to confirm long-term prognosis in a multi-marker approach.

245. Thromboembolic Disease in Haemophilic Patients Undergoing Major Orthopaedic Surgery: Is Thromboprophylaxis Mandatory?

Abstract: Haemophilia is a rare genetic disorder, that results from various degrees of deficiency of coagulation factor VIII (haemophilia A), or factor IX (haemophilia B), with an X-linked transmission. The patients affected are in the majority of cases males (who inherit the affected X-chromosome from the maternal side), with rare cases of females with haemophilia (FVIII or FIX < 40 IU/dL), situations in which both X-chromosomes are affected, or one is affected, and the other one is inactive (known as carrier). The hypocoagulable state due to the deficiency of clotting factors, manifests as an excessive, recurrent tendency to bleeding, which positively correlates with plasmatic levels. Severe haemophilia results in hemarthrosis, although recent data have shown that moderate or even mild disease can lead to joint bleeding. Recurrent episodes of haemorrhages, usually affecting large joints such as knees, elbows, or ankles, lead to joint remodelling and subsequent haemophilic arthropathy, which may require arthroplasty as a last therapeutic option. Orthopaedic patients have the highest risk among all for deep vein thrombosis (DVT) and venous thromboembolism (VTE) with morbid and potentially fatal consequences. While for the rest of the population thromboprophylaxis in orthopaedic surgery is efficient, relatively safe, and widely used, for patients with haemophilia who are considered to have a low thromboembolic risk, there is great controversy. The great heterogeneity of this particular population, and the lack of clinical trials, with only case reports or observational studies, makes thromboprophylaxis in major orthopaedic surgery a tool to be used by every clinician based on experience and case particularities. This review aims to briefly summarise the latest clinical data and to offer an insight into the current recommendations that readers would find useful in daily practice.
