BackgroundFemales with ASD tend to be under-recognized as they might present a different symptom manifestation, better social abilities, and masking behaviors. Since the main limitation of current literature on gender differences is represented by focusing on broad constructs, research needs to prioritize narrower constructs related to the subdomains of social abilities. Hence, the aim of this work was to explore gender differences in Interpersonal Synchrony of children with ASD.MethodN = 51 psychologist-child dyads, 25 females and 26 males participated in the study. An Observational Coding Scheme to study interaction features was applied to video-recorded sessions of the ADOS-2 administration.ResultsFemales presented more synchronous behaviors with shorter latencies. Their interplays were longer, more complex, more engaging and most frequently adequately concluded with respect to males. The complexity of interchanges, their total duration and the proportion of exchanges adequately terminated correlated with the Social Affect score, Personal-Social, and Language Quotients in females, but not in males. The success rate of psychologist proposals correlated with Language Quotient in both males and females. The number of exchanges positively correlated with the Performance Quotient in males. Despite females being significantly older than males, age-related differences did not emerge.ConclusionsOur findings suggest the importance of studying gender differences with respect to interaction variables. Females may present better IS abilities which, in turn, may promote social and language development. Further, our results suggested that successful interactions seem to rely more on social abilities in females, while males appeared to rely more on performance skills.

Infections due to carbapenem-resistant Enterobacterales (CRE) are increasingly prevalent in children and are associated with poor clinical outcomes, especially in critically ill patients. Novel beta lactam antibiotics, including ceftolozane-tazobactam, ceftazidime-avibactam, meropenem-vaborbactam, imipenem-cilastatin-relebactam, and cefiderocol, have been released in recent years to face the emerging challenge of multidrug-resistant (MDR) Gram-negative bacteria. Nonetheless, several novel agents lack pediatric indications approved by the Food and Drug Administration (FDA) and the European Medicine Agency (EMA), leading to uncertain pediatric-specific treatment strategies and uncertain dosing regimens in the pediatric population. In this narrative review we have summarized the available clinical and pharmacological data, current limitations and future prospects of novel beta lactam antibiotics in the pediatric population.

(1) Background: Ceftolozane/tazobactam (C/T) is a novel β-lactam/β-lactamase inhibitor with excellent activity against the multidrug-resistant (MDR) P. aeruginosa. Continuous infusion (CI) dosing allows the optimization of pharmacokinetic and pharmacodynamic (PK/PD) properties of β-lactam antibiotics and may support patients’ treatment as outpatients. (2) Methods: Adult patients receiving their entire course of C/T as a CI in the outpatient setting were retrospectively included in the study. The primary outcome evaluated was clinical resolution. The secondary outcomes evaluated were PK/PD target attainment (ƒT > 4 × MIC) and microbiologic clearance at the end of treatment. Therapeutic drug monitoring to assess C/T concentration was performed. (3) Results: Three patients were enrolled in the study and received 9 g of C/T in CI every 24 h. One patient received an additional course of antimicrobial therapy due to disease exacerbation six months after initial treatment, accounting for four evaluated treatments. The primary outcome was achieved in 3/4 treatments and the secondary outcome was achieved in 4/4 and 3/3, respectively. In all patients, free ceftolozane concentrations were >10 times higher than the EUCAST breakpoint (4 mg/L). (4) Conclusions: Elastomeric infusion of C/T delivered in CI can be an effective and convenient way to treat acute diseases caused by MDR-P. aeruginosa, avoid hospital admission, and contribute to infection control strategies. Despite the small number of enrolled patients, clinical and microbiological results support this strategy.

People in coastal cities around the world harvest seafood from local bays despite well-documented health risks. In cities such as San Diego, California, USA, much information about contaminants and human consumption patterns exists for finfish but is largely lacking for shellfish. This study sought to better understand shellfish contamination risks and human vulnerability to inform management and advisories. In summer 2018 and winter 2019, we sampled crustaceans for chemical contaminants and anthropogenic debris and throughout the year surveyed people harvesting from three public fishing piers around San Diego Bay. Of the emerging contaminants found, pyrethroids, benzylbutyl phthalate, PFOS and anthropogenic debris were in differing concentrations in the muscle and viscera of the California spiny lobster and two species of crabs. Combined with previous metal and organic contaminant data from the lobster, 22 contaminants were detected with 5 exceeding consumption thresholds and 8 lacking defined thresholds. California spiny lobster was the main crustacean harvested from piers, attracting shellfishers from a range of ages, incomes, home locations and self-identified racial/ethnic groups. Consumption preferences (e.g., muscle or viscera) were non-discriminant, making lobster contamination a community-wide risk. More monitoring of emerging contaminants and different shellfish species (and tissues) of interest is recommended to capture the spatial and temporal dynamics of health risks, especially because the use of bivalves as sentinels may not reveal the same risks (e.g., PFAS, phosphate flame retardants).

In recent years, bioactive compounds have been the focus of much interest in scientific research, due to their low toxicity and extraordinary properties. However, they possess poor solubility, low chemical stability, and unsustainable bioavailability. New drug delivery systems, and among them solid lipid nanoparticles (SLNs), could minimize these drawbacks. In this work, morin (MRN)-loaded SLNs (MRN-SLNs) were prepared using a solvent emulsification/diffusion method, using two different lipids, Compritol® 888 ATO (COM) or Phospholipon® 80H (PHO). SLNs were investigated for their physical–chemical, morphological, and technological (encapsulation parameters and in vitro release) properties. We obtained spherical and non-aggregated nanoparticles with hydrodynamic radii ranging from 60 to 70 nm and negative zeta potentials (about −30 mV and −22 mV for MRN-SLNs-COM and MRN-SLNs-PHO, respectively). The interaction of MRN with the lipids was demonstrated via μ-Raman spectroscopy, X-ray diffraction, and DSC analysis. High encapsulation efficiency was obtained for all formulations (about 99%, w/w), particularly for the SLNs prepared starting from a 10% (w/w) theoretical MRN amount. In vitro release studies showed that about 60% of MRN was released within 24 h and there was a subsequent sustained release within 10 days. Finally, ex vivo permeation studies with excised bovine nasal mucosa demonstrated the ability of SLNs to act as a penetration enhancer for MRN due to the intimate contact and interaction of the carrier with the mucosa.

We present some results, obtained using a multi-scale approach, based on the employment of different and complementary techniques, i.e., Optical Microscopy (OM), Scanning Electron Microscopy-Energy Dispersive X-ray Spectroscopy (SEM-EDS), X-ray diffraction (XRD), Raman and µ-Raman spectroscopy, Fourier transform infrared (FT-IR) spectroscopy equipped with Attenuated Total Reflectance (ATR) analyses, Inductively Coupled Plasma–Mass Spectrometry (ICP-MS), and Thermal Ionization Mass Spectrometry (TIMS), of an integrated activity focused on the characterization of micro-fragments of original and previously restored paintings of the pictorial cycle at the San Panfilo Church in Tornimparte, sampled from specific areas of interest. The study was aimed, on one hand, at the identification of the overlapping restoration materials used during previous conservation interventions (documented and not), and, on the other hand, at understanding the degradation phenomena (current or previous) of the painted surfaces and the architectural structures. The study of stratigraphy allowed us to evaluate the number of layers and the materials (pigments, minerals, and varnishes) present in each layer. As the main result, the identification of blue, black, yellow, and red pigments (both ancient and modern) was achieved. In the case of blue pigments, original (azurite and lazurite) and retouching (Prussian blue and phthalo blue) materials were recognized, together with alteration products (malachite and atacamite). Traces of yellow ochre were found in the yellow areas, and carbon black in the blue and brown areas. In the latter, hematite and red ochre pigments were also recognized. The obtained results are crucial to support the methodological choices during the restoration intervention of the site, and help to ensure the compatibility principles of the materials on which a correct conservative approach is based.

A growing number of studies is focusing on the pharmacology and feasibility of bioactive compounds as a novel valuable approach to target a variety of human diseases related to neurological degeneration. Among the group of the so-called medicinal mushrooms (MMs), Hericium erinaceus has become one of the most promising candidates. In fact, some of the bioactive compounds extracted from H. erinaceus have been shown to recover, or at least ameliorate, a wide range of pathological brain conditions such as Alzheimer’s disease, depression, Parkinson’s disease, and spinal cord injury. In a large body of in vitro and in vivo preclinical studies on the central nervous system (CNS), the effects of erinacines have been correlated with a significant increase in the production of neurotrophic factors. Despite the promising outcome of preclinical investigations, only a limited number of clinical trials have been carried out so far in different neurological conditions. In this survey, we summarized the current state of knowledge on H. erinaceus dietary supplementation and its therapeutic potential in clinical settings. The bulk collected evidence underlies the urgent need to carry out further/wider clinical trials to prove the safety and efficacy of H. erinaceus supplementation, offering significant neuroprotective applications in brain pathologies.

The European Centre for Medium-Range Weather Forecasts (ECMWF) mission is to deliver high-quality global medium-range numerical weather predictions and monitoring of the Earth system to its member states. The modelling and forecasting of natural hazards are an important part of this mission. Challenges in this area include the integration of innovative observations into the Earth system; realistic representations of water, energy and carbon cycles; coupling and initialisation of all Earth system components; adequate representation of uncertainties; supporting the development of user-specific products to enable optimal decision-making under uncertainties; and advances in software engineering. The new ECMWF strategy identified three pillars to sustain its future development (ECMWF, 2021a): science and technology (world-leading weather and Earth system science, cutting-edge technology and computational science), impact (high-quality products fit for purpose, efficient and easy access to products), and people (inspiring and hiring the best experts). Progress in all these areas will need enhanced collaboration with member states and partners across Europe and beyond.

The knowledge of tropospheric water vapor distribution can significantly improve the accuracy of Numerical Weather Prediction (NWP) models. The present work proposes an automatic and fast procedure for generating reliable water vapor products from the synergic use of Sentinel-1 Synthetic Aperture Radar (SAR) imagery and Global Navigation Satellite System (GNSS) observations. Moreover, a compression method able to drastically reduce, without significant accuracy loss, the water vapor dataset dimension has been implemented to facilitate the sharing through cloud services. The activities have been carried in the EU H2020 TWIGA project framework, aimed at providing water vapor maps at Technology Readiness Level 7.

To date SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2), a member of the Coronaviridae family, has infected more than 40 million people worldwide. A second wave of SARS-CoV-2 infection is aggressively surging. The clinical worsening of SARSCoV-2 infection appears to be strictly associated with comorbidities, which can be used to establish an intrinsic patient network whose molecular profile is pivotal for identifying and successfully treating populations at risk. Herein, we focus on the direct interaction between SARS-CoV-2 and virus-associated cancers, exploring the critical role of interleukin-6 (IL-6) as a mediator of this complex cross talk. IL-6 production is enhanced in diverse viral infections ranging from human papilloma virus (HPV) to hepatitis B virus (HBV), human immunodeficiency virus (HIV), and SARS-CoV-2 infection. High systemic levels of IL-6 are associated with viral persistence and poor clinical outcomes in SARS-CoV-2–infected patients. Blockade of IL-6/IL-6R, using specific molecules, is under investigation in active clinical trials for the treatment of patients with SARS-CoV-2. Although the data are as yet inconclusive, they pave the way for selective targeting of crucial cytokine-activated aberrant signaling in SARS-CoV-2 infection.

ImportanceObservational studies demonstrate links between patient-centered communication, quality of life (QOL), and aggressive treatments in advanced cancer, yet few randomized clinical trials (RCTs) of communication interventions have been reported.ObjectiveTo determine whether a combined intervention involving oncologists, patients with advanced cancer, and caregivers would promote patient-centered communication, and to estimate intervention effects on shared understanding, patient-physician relationships, QOL, and aggressive treatments in the last 30 days of life.Design, setting, and participantsCluster RCT at community- and hospital-based cancer clinics in Western New York and Northern California; 38 medical oncologists (mean age 44.6 years; 11 (29%) female) and 265 community-dwelling adult patients with advanced nonhematologic cancer participated (mean age, 64.4 years, 146 [55.0%] female, 235 [89%] white; enrolled August 2012 to June 2014; followed for 3 years); 194 patients had participating caregivers.InterventionsOncologists received individualized communication training using standardized patient instructors while patients received question prompt lists and individualized communication coaching to identify issues to address during an upcoming oncologist visit. Both interventions focused on engaging patients in consultations, responding to emotions, informing patients about prognosis and treatment choices, and balanced framing of information. Control participants received no training.Main outcomes and measuresThe prespecified primary outcome was a composite measure of patient-centered communication coded from audio recordings of the first oncologist visit following patient coaching (intervention group) or enrollment (control). Secondary outcomes included the patient-physician relationship, shared understanding of prognosis, QOL, and aggressive treatments and hospice use in the last 30 days of life.ResultsData from 38 oncologists (19 randomized to intervention) and 265 patients (130 intervention) were analyzed. In fully adjusted models, the intervention resulted in clinically and statistically significant improvements in the primary physician-patient communication end point (adjusted intervention effect, 0.34; 95% CI, 0.06-0.62; P = .02). Differences in secondary outcomes were not statistically significant.Conclusions and relevanceA combined intervention that included oncologist communication training and coaching for patients with advanced cancer was effective in improving patient-centered communication but did not affect secondary outcomes.Trial registrationclinicaltrials.gov Identifier: NCT01485627.
