Abstract

61 VI. ÖZET Bu çalışmanın amacı renovasküler hipertansiyonun bazı doku ve kan parametrelerine ve iki haftalık diltiazem tedavisinin bu parametrelerde oluşabilecek değişikliklere etkilerini incelemektir. Bu amaçla Wistar Albino türü 72 adet sıçan, kontrol (n=8), sham (n=16) ve hipertansiyon (n=48) olmak üzere üç gruba ayrıldı. Klip işleminden sonra hipertansiyon gelişmeyen 10 adet sıçan deney dışı bırakıldı. Sham ve hipertansiyon grupları akut ve kronik alt gruplara ayrıldı. Akut periyod cerrahi işlemden üç hafta, kronik periyod ise on beş hafta sonrası olarak belirlendi. Sıçanlarda Goldblatt 1 B1K (bir böbrek, bir klip) yön temi ile hipertansiyon geliştirildi. Diğerlerine sham operasyonu uygulandı. Yüksek kan basıncı ve kalp hızının etkilerini araştırmak için aşağıdaki paramet reler incelendi. Vücut ağırlığı, kalp ağırlığı, serum kreatinin, hematokrit, serum lipid peroksidasyonu, serum ve aort kolesterol ve fosfolipidleri, beyin Na+, K+-ATPaz. Akut hipertansiyon ve akut sham gruplarına ait serum lipid peroksidasyonu, se rum ve aort lipidleri ve beyin Na+, K+-ATPaz değerleri arasında anlamlı farklılık bulunmadı. Kronik hipertansiyon ve kronik sham gruplarına ait vücut ağırlığı, serum kreatinin değerleri, serum ve aort lipidleri arasında da anlamlı farklılık bulunmadı. Akut ve kronik hipertansiyon gruplarında kan basıncı, kalp hızı ve hematokrit düzeyleri artmış bulundu. Serum kreatinin düzeyleri sadece akut hipertansiyon grubun da yüksek bulundu. Kronik hipertansiyon grubuna ait diğer sonuçlar ise, serum lipid peroksidasyonunda artış, beyin Na+, K+-ATPaz'ında düşüş ve ventriküler hipertrofi göstergesi olarak bilinen kalp ağırlığında artış şeklindeydi. Diltiazem uygulanan gruplarda beklenen antihipertansif etki görüldü. Diltiazem akut hipertansiyon grubunda kan basıncı, kalp hızı ve serum kreatinin düzeylerini düşü rürken kronik hipertansiyon grubunda kan basıncı, kalp hızı serum lipid peroksidasyonunda düşüşe ve beyin Na+, FC-ATPaz'ında artışa yol açtı. Diltiazemin ventriküler hipertrofiyi düzeltmedeki yetersizliği olasılıkla tedavi süresinin kısa olmasına bağlıdır.62 Bu çalışmada ilginç olan bir bulgu da diltiazem uygulanan kronik hipertansiyon grubunda beyin Na+, K+-ATPaz'ındaki artıştır. Diltiazemin santral etkilerine ilişkin bilgi oldukça azdır. Diltiazem kan-beyin engelini kolay aşamamasına rağmen bu ilacın beyin deki düşük konsantrasyonlarının bazı psikiyatrik bozukluklarda tedavi edici etkileri olduğu bilinmektedir. Bu bulgular ışığında sonuçlarımız diltiazemin antihipertansif etkilerini oluştur mada periferik mekanizmalar yanında, beyin Na+, K+-ATPaz'ına etkisi nedeniyle santral mekanizmalarında rolü olduğunu düşündürmektedir. 63 VII. SUMMARY The purpose of this study was to investigate the effects of renovascular hypertension on some tissue and blood parameters and also the effects of two weeks of diltiazem treatment on any such changes. For this purpose 72 Wistar Albino rats were used. The rats were divided into control (n=8), sham (n=16) and hypertension (n=48) groups. Ten of the rats were excluded from the study because they remained normotensive after clip operations. The sham and hypertension groups were divided into acute and chronic subgroups. The acute and chronic periods were defined as the first three weeks and fifteen weeks after the surgical procedure respectively. The rats were rendered hypertensive using the Goldblatt 1K1C (one kidney, one clip) model, others underwent a sham operation. In order to investigate the effects of increased blood pressure and heart rate, the following were determined; body weight, heart weight, serum creatinine, hematocrit, serum lipid peroxidation, serum and aortic cholesterol and phospolipids and brain Na+, K+-ATPase. Between the acute sham and acute hypertension groups, there were no significant differences in body weight, heart weight, serum lipid peroxidation, serum and aortic lipids and brain Na+, K+-ATPase. Also between the chronic sham and chronic hypertension groups there were no significant differences in body weight, serum creatinine, serum and aortic lipids. For the following all differences were significant: In the acute and chronic hypertension groups blood pressure, heart rate, and hematocrit increased. Serum creatinine levels increased only in the acute hypertension group. Other results from the chronic hypertension group were; an increase in serum lipid peroxidation, a decrease in brain Na+, K+-ATPase and an increase in heart weight which indicating development of ventricular hypertrophy.64 In the diltiazem treated groups, following treatment, the diltiazem had as expected, an antihypertensive effect on the hypertensive rats. Diltiazem decreased blood pressure, heart rate and, serum creatinine levels in the acute hypertension group, and decreased blood pressure, heart rate, serum lipid peroxidation and increased brain Na+, K+-ATPase levels in the chronic hypertension group. There was no clear evidence however that diltiazem therapeutically improved ventricular hypertrophy but this may have been due to insufficient duration of treatment. The most interesting finding of this study was the increase in brain Na+, K+- ATPase in the diltiazem treated chronic hypertension group. There is little information available on the central effects of diltiazem. Although diltiazem cannot cross the blood- brain barrier easily, even small amounts of this agent in the brain are known to have a therapeutic effect on some psychiatric disorders, In the light of these findings the results of this study suggest that the antihypertensive action of diltiazem as well as being peripheral may also be central in that it affects brain Na+, K+-ATPase. 86

Abstract

Oxidative stress plays an important role both in spinal cord injury ( SCI) and erectile dysfunction ( ED). The present study investigated the effects of melatonin and tadalafil treatment alone or in combination on SCI-induced ED., Male Wistar albino rats ( n = 40) were divided into five groups: sham-operated control and SCI-injured rats given either vehicle, melatonin (10 mg/kg, i.p.), tadalafil (10 mg/kg, p.o.) or a combination of melatonin and tadalafil. Spinal cord injury was induced using a standard weight-drop method. On Day 7 after SCI, intracavernosal pressure ( ICP) was measured and all rats were decapitated. Cavernosal tissues were obtained to examine caspase 3, nitric oxide synthase ( NOS), myeloperoxidase ( MPO) and superoxide dismutase ( SOD) activities, as well as cGMP, nerve growth factor ( NGF), malondialdehyde ( MDA) and glutathione ( GSH) levels., Spinal cord injury caused oxidative damage, as evidenced by increases in MDA and cGMP levels. In addition, MPO and caspase 3 activites were increased after SCI, whereas GSH and NGF levels and SOD activity were reduced. Melatonin effectively reversed these oxidative changes. Furthermore, in rats treated with both melatonin and tadalafil, the recoveries were more pronounced than in rats given either melatonin or tadalafil alone. The ICP/mean arterial pressure value in vehicle-treated SCI rats was significantly higher than in the control group, whereas in the tadalafil- and tadalafil + melatonin-treated groups have returned this value had returned to control levels., As an individual treatment, and especially when combined with tadalafil, a well-known agent in the treatment of ED, melatonin prevented SCI-induced oxidative damage to cavernosal tissues and restored ED, most likely due to its anti-oxidant effects. Abstract

Ischemic injury, which occurs as a result of sympathetic hyperactivity, plays an important role in heart failure. Melatonin is thought to have antiatherogenic, antioxidant, and vasodilatory effects. In this study, we investigated whether melatonin protects against ischemic heart failure ( HF). In Wistar albino rats, HF was induced by left anterior descending ( LAD) coronary artery ligation and rats were treated with either vehicle or melatonin (10 mg/kg) for 4 weeks. At the end of this period, echocardiographic measurements were recorded and the rats were decapitated to obtain plasma and cardiac tissue samples. Lactate dehydrogenase, creatine kinase, aspartate aminotransferase, alanine aminotransferase, and lysosomal enzymes (β-D-glucuronidase, β-galactosidase, β-D-N-acetyl-glucosaminidase, acid phosphatase, and cathepsin-D) were studied in plasma samples, while malondialdehyde and glutathione levels and Na+, K+- ATPase, caspase-3 and myeloperoxidase activities were determined in the cardiac samples. Sarco/endoplasmic reticulum calcium ATPase ( SERCA) and caveolin-3 levels in cardiac tissues were evaluated using Western blot analyses. Furthermore, caveolin-3 levels were also determined by histological analyses. In the vehicle-treated HF group, cardiotoxicity resulted in decreased cardiac Na+, K+- ATPase and SERCA activities, GSH contents and caveolin-3 levels, while plasma LDH, CK, and lysosomal enzyme activities and cardiac MDA and Myeloperoxidase ( MPO) activities were found to be increased. On the other hand, melatonin treatment reversed all the functional and biochemical changes. The present results demonstrate that Mel ameliorates ischemic heart failure in rats. These observations highlight that melatonin is a promising supplement for improving defense mechanisms in the heart against oxidative stress caused by heart failure. [ABSTRACT FROM AUTHOR]
