Introduction Les cellulites cervico-faciales accroient la morbidité chez la femme gestante et met en jeu le pronostic vital materno-fœtal. Notre étude a rapporté sur 22 observations de cellulite cervico-faciale associée à une grossesse colligée sur une période de 3 ans. L’objectif était d’attirer l’attention du personnel de santé et la population sur cette pathologie chez la femme enceinte, d’évaluer le mode de traitement et déterminer le pronostic materno-fœtal. Méthodes : Il s’agit d’une étude rétrospective portant sur 22 cas de cellulites cervico-faciales chez la femme enceinte d’origine dentaire réalisée dans le service de stomatologie et de chirurgie maxillo-faciale du CHU-CNOS de Bamako du 1er janvier 2015 au 31 Décembre 2017.Résultats : L’âge moyen était de 26,5 ans avec des extrêmes de 19 à 33 ans. La porte d’entrée était dentaire dans tous les cas. A l’admission ,9 patientes 40,9% ont été adressées par les CSREF, et 13 patientes 59,1% sont venues d’elles- mêmes. Le siège anatomo-topographique le plus fréquent était la région périe mandibulaire 12 cas soit 54,5%. Le motif de consultation était l’odontalgie et la tuméfaction 72,7%. La dent causale était la 3ème molaire inférieure qui prédominait dans 9 cas soit 40,9%. Le traitement après admission était médico-chirurgical chez 18 cas soit 81,9%, le traitement médical a été effectué dans 4 cas soit 18,1%. L’évolution était favorable dans 17 cas soit 77,3%, 10 gestantes ont présentés des complications, dont 4 décès fœtal et un décès materno-fœtal, Un cas d’avortement a été retrouvé soit 4,5%, un cas d’extension cervicale plus dyspnée, un cas d’extension cervicale plus métrorragie, un cas de foyer d’encéphalite focal plus anémie, un cas d’extension cervicale plus perte de substance thoracique. Conclusion : La cellulite cervico-faciale chez la femme enceinte reste une pathologie fréquente et grave, sa prévention passe par une sensibilisation des femmes enceintes sur l’hygiène bucco-dentaire. Une consultation dentaire chez toutes gestantes suspecte de carie dentaire pourrait réduire le risque

Objective: To explore the relationship between the cycle threshold (Ct) value of ORF1ab gene and N gene in the first admission detected by novel coronavirus (Omicron) nucleic acid and the negative conversion cycle of infected patients. Methods: 5 212 patients with novel coronavirus omicron variant infection were admitted to the Northern Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from March 2022 to April 2022 were collected, according to the patient′s previous history and the tests of relevant indexes after admission, including 2 746 cases in non-basic disease group and 2 466 cases in the basic disease group. All subjects received the Ct value of the ORF1ab genes and the N genes tests. Statistical analysis of the difference of the negative conversion cycle of ORF1ab gene and N gene between non-basic disease group and basic disease group under different Ct values at the first admission. Pearson linear correlation analysis was used to assess the association of Ct value of ORF1ab gene and N gene with the negative conversion cycle, and multiple linear regression was used to analyze the factors affecting the negative conversion cycle. In late February 2021, a round of indigenous novel coronavirus (referred to as COVID-19) infection emerged in Shanghai, making the COVID-19 outbreak shift from regular prevention and control to drastic pandemic prevention and control measures. According to the requirements of the Guidelines for the Organization and Implementation of Regional Novel Coronavirus Nucleic Acid Testing (Third Edition) issued by the National Health and Wellness Commission, medical institutions at all levels with nucleic acid testing capabilities rapidly carried out novel coronavirus nucleic acid testing, which put forward higher requirements for testing and prevention and control for the emergency conversion of non-designated admission hospitals to designated hospitals. As a clinical department with close contact with novel coronavirus testing specimens, the department of clinical laboratory has long working hours and high work intensity, as well as the potential risk of aerosol and contact transmission of clinical routine specimens and nucleic acid specimens, which requires more improvement from human resource planning, laboratory nucleic acid testing, laboratory infection prevention and control, medical material security, and staff psychological guidance. In this paper, we hope to combine the practice of emergency transformation work of the laboratory department in the northern hospital area of Ruijin Hospital, including personnel allocation, planning of testing items for clinical samples, and establishment of emergency nucleic acid testing, to carry out relevant work in a scientific, standardized and orderly manner, and provide our associate organizations with valuable experience in the coordination and management of emergency testing in the laboratory department in response to the novel coronavirus outbreak.

A new hit of severe acute respiratory syndrome coronavirus 2 variant strain Omicron pandemic is spreading worldwide and gradually affecting China. Adrenal disease severely affects the balance of glucose, lipids, water and salts throughout the body, thus affecting the prognosis of patients with coronavirus disease 2019 (COVID-19). At the same time, it has become a key point on management of COVID-19 with adrenal disease, due to its specialty and lack of clinical understanding. The purpose of this expert suggestion is to standardize the clinical understanding and guide clinical practice. Especially in the context of complex epidemic prevention and control, it provides necessary reference basis and guidance for the diagnosis, treatment and comprehensive management of adrenal diseases during the current COVID-19 epidemic.

Objective: To explore the quality and safety of testing in the city′s regional medical laboratories for COVID-19 nucleic acid detection. Methods: To strengthen the quality control of the city′s medical laboratories for COVID-19 nucleic acid detection through monthly inspections, squatting inspections, emergency inspections, blind sample surveys, sample retention, and retesting. Results: Regional medical laboratories for COVID-19 nucleic acid detection were non-compliant in terms of personnel qualifications, business training, internal quality control, result judgement, retesting processes, report uploading, cleaning and disinfection, and medical waste disposal. The number of laboratories conducting COVID-19 nucleic acid testing gradually expanded from 29 at the beginning of the outbreak on March 18 to 98 on May 16, but the number of non-compliant laboratories did not increase significantly. The pass rate of the blind sample survey of the city′s medical testing laboratories ranged from 96.6% to 100.0%, and the concurrence rate between the results of the laboratory′s retained samples for retesting and the results of the retained testing facilities ranged from 98.3% to 100.0%. Conclusions: The quality of regional COVID-19 nucleic acid detection in the city is generally controllable, with no systematic quality problems. The quality control model with multiple measures is worthy of application in the quality control of regional COVID-19 nucleic acid detection.

Objective: To explore the related problems and solutions in nucleic acid detection of COVID-19 patients at the present stage, and to provide reference for clinical laboratories. Methods: A total of 462 patients admitted from April 1 to April 10 were selected and their basic Ct values and time changes were analyzed. The false-negative test results caused by sample collection from March 20 to March 31 and from April 1 to April 10 were statistically analyzed, respectively. A total of 213 clinical samples were selected for parallel detection using reagents A and B to compare the differences between them. According to relevant documents, make the reinspection standard operation procedure (SOP) document of suspicious samples. According to the quality control method of quantitative test, weak-positive control was prepared and tested 5 batches every day for 4 days, and 20 groups of data were obtained and used as the initial data to make quality control charts. Objective: To explore the epidemic spread and prevention strategy of COVID-19 by Omicron variant in a single office building. Methods: Retrospective data were collected for the period from March 19, 2022 to March 28, 2022. During this period, the infection rate, epidemic survey, transmission chain, population vaccination and Ct value of infected cases, as well as other data of 458 persons at risk in the single office building were collected. The response measures and outcomes were recorded, and the characteristics of this round of epidemic transmission were analyzed, and the prevention and control strategies were summarized. Results: The epidemic situation in a single office building lasted for 10 days from March 19 to March 28. Forty-two out of the 458 persons in the office building were infected, with an infection rate of 9.17%. After the large-scale single room isolation measure was put in place for key populations on March 21, the infection rate showed a significant downward trend, reversing the previous upward trend. The real-time reproduction number (Rt) value changed dynamically with time (from 10.8 to 2.2) and reached a value of lower than 1 on the third day of isolation. For patients with Ct value above 30, the positive conversion rate of close contacts was about 6.0%. For patients with Ct value between 20-30, the positive conversion rate of close contacts is higher, about 25.6%. Among patients with Ct value below 20, the conversion rate of close contacts was as high as 62.5%. Objective: To construct a training program on emergency response ability for all staff in emergency medical wards under COVID-19 epidemic, and to evaluate its clinical effect. Methods: A targeted training program for emergency response ability of all staff in the emergency medical ward was established through literature review, brain storming and online conference discussion. The scheme includes seven modules: training system, training plan, training outline, training course, teaching team, training materials and teaching evaluation. For medical staff, other staff (nursing workers, work attendance, cleaning), patients and family members of different categories of personnel, advanced version, basic version and popular science version are designed respectively, and the training content closely fitted the needs of epidemic prevention and skills required in different groups. During the period from April 5 to May 10, 2022, the training program was conducted to train the emergency response ability in all staff,and effect was evaluated using questionnaires invstigation before and after training, oral questions, operation assessment and on-site inspection, and the incidence of clinical actual infection was monitered. Hepatitis E virus (HEV) of the family Hepeviridae, is a major causative agent of acute hepatitis in developing countries. The Norway rat HEV genome is organized into six open reading frames (ORFs), i.e., ORF1, ORF2, ORF3, ORF4, ORF5 and ORF6. The additional reading frame encoded protein ORF6 is attributed to life cycle of rat HEV. As ORF6 protein's remains to be explored in terms of its structural and functional implications, the following study was conceptualized to explore the prospective role of this additional genomic component of rat HEV. The detailed computational investigation was carried out for the ORF6 protein to elucidate its physiochemical properties, primary structure, secondary structure, tertiary structure and post-translational modifications, motif prediction and other functional characteristics. The in silico analysis revealed ORF6 protein as unstable, highly thermostable, hydrophobic and basic in nature. The amino acid compositional analysis revealed higher abundance of Leu, Arg, Ile and Pro amino acids in the polypeptide chain of ORF6 protein. The secondary structural analysis revealed all the three major elements, i.e., 汐-helices, 汕-strands and coils. The generated 3D structural model of the ORF6 protein through homology modeling algorithm revealed mixed 汐/汕 structural fold of the ORF6 protein with abundance of coils. Additionally, the structural models revealed the presence of clefts and a tunnel. The identified binding functions and the presence of several clefts suggested the commitment of ORF6 protein towards interaction with other ligand molecules. This theoretical study will facilitate towards deciphering the role of unexplored ORF6 encoded protein, thereby providing better understanding towards the pathogenesis of Norway rat HEVs.

Hepatitis E virus (HEV) is a quasi-enveloped RNA virus of the family Hepeviridae. HEV is the chief cause of acute hepatitis worldwide, causing approximately 20 million infections annually, which results in 60,000 deaths. Due to insufficiency in appropriate HEV in vitro cell culture systems, our knowledge of its pathogenesis is inadequately understood. HEV encodes three open reading frames (ORFs): ORF1 (replicative machinery), ORF2 (viral capsid) essential for (infectious particles formation) and ORF3 (viral release). The presence of known and unknown coding and non-coding regions of HEV ORFs are still debated. Viral proteins entail disordered regions which are linked with the infectivity and pathogenicity of virus. Thus, we examined the dark proteome of HEV through analyzing intrinsically disordered protein regions (IDPRs) present in the ORFs by exploiting computational methodologies. Our findings suggested that ORF3 had the highest prevalence of disordered regions. The ORF3 region was followed by ORF2, which had comparatively lesser fraction of intrinsic disorder. The ORF1 had the least number of disordered residues in the HEV proteome. Our intrinsic disorder analysis results revealed that ORF1 polyprotein consists of mostly ordered domains, i.e., proteins having significant level of well-defined structures, with the exclusion of Pro and PCP domains. The analysis reveals Pro domain as a highly disordered protein while PCP domain as an intrinsic disordered protein. MoRF analysis revealed that HEV proteome contains multiple MoRFs across all ORFs. IDPRs are characterized by remarkable conformational flexibility and structural plasticity resulting in their engagement in several biological processes. Due to possession of MoRF in the HEV proteins, these regions can be used for protein-protein interactions due to the structural flexibility. Recently, a novel viral protein of HEV, named as open reading frame (ORF4), has been associated with its replication in genotype 1 isolates. However, much information regarding ORF4 has not been explored. Thus, the study was conceptualized to explore the structural and functional features of HEV ORF4 protein to better understand the possible molecular mechanisms. The detailed investigation of the ORF4 was carried out in terms of its physicochemical properties, secondary and tertiary structure predictions and functional analysis using different bioinformatics tools. The in silico analyses revealed that ORF4 sequences were enriched in Serine, Proline and Glycine amino acid residues suggesting the prevalence of disordered residues. The protein was found to be thermostable, unstable and highly hydrophobic. The structural analysis showed the presence of cleft, tunnel and pore suggesting their participation in interaction with other molecules. Moreover, identification of several modified sites in ORF4 sequences such as glycosylation, phosphorylation and myristoylation sequences suggest their involvement in cellular signaling pathways and biological processes. Thus, taken together, it can be interpreted that HEV ORF4 possesses significant enormous flexibility due to the presence of Serine, Glycine and Proline amino acids, which suggest its involvement in protein-protein interaction. Furthermore, the presence of motifs, clefts and tunnels also strengthens our analysis, suggesting the commitment of ORF4 towards interaction with other target molecules. Thus, it could be potent drug-targets.

Ultrasonic is one of the promising technological innovation to modify structure, inactivate enzymes and/or neutralize microorganisms in food products for enhancement of their quality and safety. Apart from pasteurization, sanitation, disinfection and cleaning procedures, in the area of food microbiology, ultrasound can facilitate recovery of microorganisms and their identification through cell lysis or detachment of microbe from food surface. Hence, study of effect of sound waves on microbial cells in suspended condition and food matrices has opened a new horizon of its application in the area of food microbiology. Use of ultrasound in microbial analysis is already in practice and expanding, but the physics of interaction of acoustic waves with microbial cells in presence of actual food matrices need further attention. Ultrasonic reactor (UR) design needs interdisciplinary approach to further exploit the promise of ultra sound in food microbiology. Substantial demand in recent years for bench-top ultrasound reactors for cleaning of food contact surfaces in common households has increased. This review deals with the state of art of the ultrasound technology, process development, and further scope of the technology specific for its applications in cleaning and sanitization, microbial inactivation and in microbiological analysis in food processing industries.

Objectif : Notre étude avait pour objectif d’évaluer les caractéristiques de la prescription et de la délivrance des antibiotiques dans les officines de Maradi. Méthodologie : C’est ainsi que nous avons mené une étude prospective transversale a visée descriptive allant du 1er Juillet au 17 Août qui avait concerné 265 individus dont 245 cas de prescription et 20 cas d’automédication. Résultats : Sur les 245 ordonnances collectées, aucune ne comportait la taille soit 0% et seulement 2 % mentionnaient le poids du patient. Le nom et prénom du prescripteur figuraient dans 42% des cas. Le cachet et/ou une signature figuraient dans 82,9%.La qualification du prescripteur était absente dans 54,3% des ordonnances et 15,5% provenaient des établissements non identifiés. La date de la prescription figurait sur 93% des ordonnances et la majorité des antibiotiques dispensés était à la suite d’une prescription médicale soit 92,5%. Les bétalactamines étaient les antibiotiques les plus prescrits avec 55,5%, les spécialités occupaient 66,6% contre 33,4% pour les produits génériques recensés dans notre étude. La durée du traitement était présente sur 12,4% des prescriptions, plus de 91% des ordonnances comportaient la forme galénique ainsi que la voie d’administration des antibiotiques prescrits. 0,4% des ordonnances révélaient une anomalie de prescription, 2% des interactions médicamenteuses. Conclusion : Les résultats de cette étude restent pertinents. Ils permettront d’améliorer les programmes de formation des agents de santé afin qu’ils maitrisent mieux les principes de prescription des antibiotiques.

Introduction : Les atteintes rénales sont fréquentes au cours de l’infection à virus de l'immunodéficience humaine (VIH). Objectif : Étudier les aspects épidémiologiques, cliniques et paracliniques de l’atteinte rénale chez les personnes vivant avec le VIH (PvVIH). Matériels et méthodes : Nous avons mené une étude rétrospective (du 1er Janvier 2013 au31 décembre 2017) et prospective (du 1er janvier 2018 au 31 décembre 2018) des données sur 5 ans, portant sur les dossiers des PvVIH hospitalisées en médecine interne au centre hospitalier universitaire (CHU) du Point G et présentant une atteinte rénale. Résultats: Parmi 336 PvVIH, 30 avaient une atteinte rénale soit uneprévalencede9%.L’âge moyen était de 36,8 ± 12,9 ans. L’échantillon était constitué de 16femmes et 14 hommes avec un sex-ratio à 0,9. Les signes d’appel urinaire étaient la dysurie 04 cas (13,3%), l’oligurie 04 cas (13,3%), et l’anurie 2cas (6,7%).Les principales atteintes rénales étaient : l’insuffisance rénale chronique (100%), la protéinurie (80%), la leucocyturie isolée 3 cas et une infection urinaire04 cas (36,37%). L’échographie (n=23) montrait des reins diminués de taille (05 cas), mal différenciés (14 cas), dilatés (04 cas). Le taux de létalité était de 53,33%. Conclusion : Il ressort de cette étude que la fréquence de l’atteinte rénale chez les personnes vivant avec le VIH est élevée. Une surveillance étroite de la fonction rénale chez ces patients s’avère donc indispensable.

Cette étude avait pour objectif d’analyser la prescription et la dispensation des céphalosporines dans le District de Bamako. L’étude a concerné les prescriptions de céphalosporine effectuées dans les centres de santé enrôlés ainsi que les ordonnances de céphalosporine reçues dans les officines de pharmacie choisies. Un total de 910 ordonnances comportant 9 molécules de céphalosporines différentes ont été collectées. Le groupe des céphalosporines de 3ème génération (C3G) était le plus prescrit avec un taux de prescription de 88,43%. Dans ce groupe, les molécules de Ceftriaxone et de Céfixime étaient les plus prescrites avec des taux respectifs de 44,00% et 32,65%. Des résultats satisfaisants ont été notés en ce qui concerne la dispensation des céphalosporines au cours de cette étude. En effet, dans les officines de pharmacies visitées, la dispensation des céphalosporines était faite dans 98,14% des cas sur la base d’une ordonnance médicale. Toutefois, quelques insuffisances ont été notées telles que la prescription de ces antibiotiques sans critère bactériologique dans 86% des cas. En vue de limiter l’émergence de résistances bactériennes et les risques d’échecs thérapeutiques, il urge de sensibiliser et de former les prescripteurs à la prescription rationnelle des antimicrobiens en général et des antibiotiques en particulier.

Introduction : selon l'Organisation Mondiale de la Santé (OMS), la varicelle est la plus contagieuse des pathologies éruptives. Si la maladie est le plus souvent bénigne, des complications peuvent survenir en cours d’évolution. Nous rapportons quatre cas de complications varicelleuses observés dans le service des maladies infectieuses du CHU Yalgado Ouédraogo, avec comme objectif de contribuer à la prise en charge précoce et adaptée. Observations : les patients étaient majoritairement de sexe masculin (3/4). Leurs âges étaient respectivement de 5 ans,14 ans, 37 ans et 39 ans. Ils résidaient tous dans la ville de Ouagadougou. Une notion de contage familiale a été retrouvée chez tous les patients. Les complications étaient représentées par l’encéphalite, les pneumopathies, les surinfections bactériennes et la thrombopénie. La maladie se manifestait par une éruption caractéristique chez tous les patients, faite de lésions vésiculo-papuleuses, des lésions ombiliquées et prurigineuses. L’éruption était précédée par une fièvre, des céphalées, un malaise, et une anorexie chez tous patients. Elle était profuse chez les patients adultes. Une toux grasse accompagnée de lésions pustuleuses et de dyspnée avait été notée chez deux patients. Sous traitement à base d’antiseptique, d’antihistaminique, d’antiviraux, et d’antibiotiques, l’évolution a été favorable chez tous les patients. Conclusion : la varicelle est encore responsable de flambées épidémiques dans la communauté comme le témoigne la notion de contage retrouvée chez tous les patients. Les complications de la varicelle ne sont pas rares. Devant tout cas de varicelle, il faut toujours y penser et prendre en charge une éventuelle complication. L’introduction du vaccin anti-varicelleuse dans le PEV contribuera à lutter contre cette maladie et ses complications

Introduction : le diagnostic précoce des déficits immunitaires primitifs (DIP) constitue un véritable défi dans le monde. Le but de ce travail était de rapporter les manifestations dermatologiques observées au cours des déficits immunitaires primitifs sur peau foncée à Dakar. Patients et Méthodes : il s’agissait d’une étude transversale descriptive de Janvier 2018 à Mars 2021, incluant tous les enfants âgés de 0 à 18 ans présentant des manifestations dermatologiques opportunistes sans infection à VIH. Le diagnostic du type de déficit immunitaire primitif était basé sur les 10 tableaux de la classification phénotypique de l’union internationale des sociétés d’immunologistes (UISI) 2019. Résultats : 64 patients ont été suivis dont 35 filles. L’âge moyen était de 6 ans (extrêmes : 1semaine-18 ans). La médecine traditionnelle était un des recours thérapeutiques dans 94% des cas soit en premier, soit au cours du suivi. Les manifestations dermatologiques étaient entre autre : virales dans 20 cas avec 21,87% de condylomes ano-génitaux ; bactérienne dans 18 cas ; fongiques dans 1 cas; allergique dans 14 cas; auto-immune et auto-inflammatoire dans 12 cas. Les 30 cas ayant bénéficié de bilan immunologique de base, ont révélé un déficit immunitaire primitif avec un syndrome d’Hyper Immunoglobulinémie E (IgE) dans 14 cas. Conclusion : Tous les cas, ayant un bilan immunologique de base, ont pu être classés selon les 10 tableaux, de la classification phénotypique de l’UISI. Ce résultat suggère une sensibilité des manifestations dermatologiques pour le diagnostic des DIP. Ce constat devra être confirmé par d’autres études.

Introduction : La disponibilité du sang et des produits dérivés du sang ainsi que la sécurité transfusionnelle constituent un problème majeur de santé publique. Ce travail avait pour but de déterminer la pertinence de l’introduction du dépistage génomique viral (DGV) des virus VHB et VIH dans les dons de sang au centre national de transfusion sanguine ( CNTS) de Dakar.Matériel et méthodes: L’étude s’est déroulée au CNTS de Dakar, centre de référence pour la collecte, le traitement et la distribution de produits sanguins au Sénégal. Il s’agissait d’une étude prospective et transversale qui s’est déroulée de Juillet 2016 au mois de février 2017. L’étude a concernée 1455 dons de sang pour dépistage des marqueurs viraux avec la technique par chimiluminescence. Parmi ces 1455 dons, 300 ont été sélectionnés pour le DGV. Résultats : Les prévalences au dépistage étaient de 8% pour le VHB et 0,7% pour le VIH par chimiluminescence. Tous les donneurs dépistés négatifs pour le VIH sont restés négatifs avec le DGV. De même, les 10 donneurs dépistés positifs pour le VIH et le VHB ont été confirmés positifs par DGV. Par contre, parmi les 58 donneurs dépistés négatifs, un donneur a été retrouvé positif avec le DGV pour le VHB.Conclusion : Ce travail a montré une concordance parfaite entre les deux techniques pour le VIH et une absence de risque résiduel de transmission du VIH. En revanche, il ya une concordance presque parfaite entre les deux techniques pour le VHB. Ces résultats devraient être confirmés sur un plus grand nombre d’échantillons et sur une longue période.

La néphrite tubulo-interstitielle (NTI) est un groupe de maladies inflammatoires à médiation immunitaire qui implique l’interstitium et les tubules rénaux, elle conduit à une insuffisance rénale et a de multiples causes : induites par les médicaments ; idiopathiques ; génétiques ; dysimmunitaires ; infectieuses (virales, bactériennes, parasitaires ou fongiques). Nous décrivons ici une étiologie fongique, alors que la fréquence de la néphrite interstitielle de 4,23% a été retrouvée en hospitalisation dans le service de néphrologie au Mali, aucune donnée disponible n’implique le rôle de candida dans la survenue de NTI. Il s’agit d’un patient de 66 ans, hypertendu connu mal suivi qui a présenté une infection urinaire avec isolement de Candida spp. Le diagnostic de néphrite tubulo- interstitielle à Candida associée à une insuffisance rénale aigue avait été retenu après avis des néphrologues. Le traitement par du fluconazole et la réhydration ont entrainé la stérilisation des urines et la normalisation du bilan biologique

Objectif : décrire les aspects diagnostiques et rappeler les grandes lignes thérapeutiques de la méningite post traumatique par brèche ostéoméningée en milieu précaire. Observation : Les auteurs rapportent le cas d’un patient de 30 ans chez qui l’exploration d’une rhinorrhée évoluant dans un état d’altération de l’état général et de crises convulsives tonico cloniques fébriles a conduit au diagnostic d’une méningite aigue bactérienne par brèche ostéoméningée un an après le traumatisme. Le patient a bénéficié d’un traitement médical avec une guérison sans séquelles. Vu les limites qu’impose notre plateau technique, La fermeture de sa brèche reste le problème thérapeutique que pose ce patient. Conclusion : la méningite post traumatique par brèche ostéoméningée est rare. Le premier pas vers le diagnostic est la surveillance des traumatisés crânio faciaux. La prise en charge reste multidisciplinaire.

Contexte : Les cellulites cervico-faciales d’origine dentaire sont des affections graves et redoutables qui posent un problème de prise en charge thérapeutique adaptée. L’objectif de ce travail est de présenter à travers un cas clinique, le cocktail explosif infection dentaire, sujet diabétique non équilibré et anti-inflammatoire non stéroïdien. Cas clinique : Patiente âgée de 55 ans, venue consulter pour des odontalgies localisées à la 36. La patiente présente des antécédents de diabète et d’hypertension artérielle. L’examen clinique et radiographique a permis de poser le diagnostic de parodontite apicale aigue. Un traitement médicamenteux (Amoxicilline, Paracétamol, Chlorexidine) a été mis en place et le patient a été adressé au diabétologue. Face à l’inflammation douloureuse d’origine dentaire,le diabétologue a prescrit des anti-inflammatoires non stéroïdiens. Deux semaines plus tard, le sujet revient avec un tableau clinique marqué par une nécrose tissulaire maxillo-faciale étendue. Résultats : La prise en charge s’est faite en urgence au bloc opératoire sous anesthésie générale. Les suites opératoires ont été simples. Cependant, on note des préjudices esthétiques, fonctionnels et un surcout financier non négligeable. Conclusion : La triade infection dentaire, diabète déséquilibré et prise d’anti-inflammatoire non stéroïdiens constitue un cocktail explosif susceptible de mettre en jeu le pronostic vital du patient.

La tuberculose neuro-méningée est une cause majeure de morbidité et de mortalité dans les pays en développement. Cette étude rapporte une présentation clinique atypique du tuberculome cérébral afin de faire une mise au point sur cette maladie infectieuse. L’étude porte sur ungarçon VIH négatif, âgé de 11 ans admis dans le service pour des céphalées et des vomissements en jet. L'examen clinique avait retrouvé un syndrome méningé (raideur de la nuque, signes de Kernig et de Brudzinski), des troubles sensoriels, des troubles de la conscience. L’examen avait aussi permis de palper des adénopathies cervicales et sous-maxillaires, indolores, mobiles, de consistance ferme; le plus gros mesurant 4 cm × 5 cm. Le scanner cérébral avait objectivé un processus expansif développé aux dépens du vermis cérébelleux en faveur d’un tuberculome cérébral. L’analyse du LCS avait permis d’objectiver une lymphocytose, une hyperprotéinorachie et une hypoglycorachie. L'analyse anatomopathologique avait permis de mettre en évidence une adénite tuberculeuse caséo-folliculaire. L’intradermoréaction à la tuberculine et la recherche de bacille acido-alcoolo-résistant dans les crachats étaient négatives. Le traitement antituberculeux et une corticothérapie avait permis une rémission complète. Le tuberculomecérébral est une maladie grave. Une prise en charge précoce est essentielle car les complications peuvent être sévères

It is the global voice of the library and information profession. In her address to delegates, IFLA President Barbara Lison expressed her delight at the atmosphere of the first in-person meeting after such a long period of isolation and travel restrictions. A number of members from the Chinese American Librarians Association (CALA) attended the Congress and spent several packed days of learning, reflection, discussion, and networking.
